DRINK WATER ON EMPTY STOMACH~~

It is popular in Japan today to drink water immediately after waking up every morning.

OLEANDER POISONING & MANAGEMENT

Ø Oleanders you almost have seen it but have never thought of it. Being a medico what I knew is that every particle is a poison until and unless it exceeds it normal limit.

Ø Yes oleanders are also poisonous and are fatal if consumed a little more.

Ø Usually not all oleanders are poisonous, only yellow oleander is fatal. It is also known as thevetia Peruvian.

Ø The oleander plant contains glycosides which resemble digitoxin, a cardiac glycoside.

Ø Hence it poisonous effects mainly over the heart.

Symptoms or clinical features

v Within 3 hours the symptoms start causing mainly gastro intestinal symptoms.

v Symptoms include: nausea, vomiting, diarrhoea and abdominal pains.

v Others symptoms belong to cardiac origin which includes: heart rates reduce, and the patient may become anxious.

v Some other features include: dizziness, numbness, tingling, restlessness, hypotension and also abdominal cramps may also occur.

Lab investigations

v Hyperkalaemia and acidosis develops.

v Hyperkalaemia means increased concentration of free potassium responsible for cardiac symptoms.

v Digoxin is estimated by radioimmunoassay.

treatment

Management of the oleander poisoning mainly includes supportive management as there is no any antidote available.

Fluid, electrolyte balance & acid base disturbances need to be corrected.

Gastric emptying is done immediately.

Activated charcoal is given repeatedly.

Atropine corrects all the cardiac defects including bradycardia & arrhythmias.

If ventricular tachy- arrhythmias are present then a drug named lidocaine is given.

If the poisoning is very severe then digoxin specific Fab antibody fragments is provided.

v Only a few types of mushrooms are poisonous among the many types.

v If the onset symptoms occur within 3 hours of ingestion of mushroom, then most likely it’s not deadly mushroom so you can be relax.

v “Amanita phalloides” is one of the mushrooms responsible for majority of deaths. Also called as death cab.

v The two types of poisons which it contains is “ phallotoxins” and “amatoxins”.

v The former cause’s gastritis while later is responsible for liver & renal damage.

Ø Let us tell you the fact that most cases are due to unknowingly ingesting an unknown poisonous mushroom so be careful eating any unknown mushroom type of diet.

Ø 2ndly small kids while playing in the grounds ingests them thinking to be some frit like thing.

Management

Management of scorpion bite cases?

v Scorpions are of different types in the world varying in their colours and poisons.

v In India two types are poisonous snakes are present.

v Red scorpion also called mesobuthus.

v Black scorpion also known as palamnaeus.

Clinical features

v Features usually develop about 2 to 12 hours after sting.

Local features

v Intence pain at the site.

v Pain may be elicited by gentle palpation or tapping.

v Pain and numbness may extend upto the extremity.

General or systemic features

v Patient may become or feel restlessness, anxiety, vomiting, and dysphagia, Bradycardia, salivation and sweating.

v Sensation of tongue thickening.

v Heart rate may be reduced.

v Hypotension and priapism may occur.

v Subsequently after the initial features, the patient may develop hypertension, increased heart rate, hands and feet become cold, chest discomfort and heart attack may occur.

v Late stages may develop hypotension and shock.

Management :

v Analgesics like paracetamol or ibuprofen may be given.

v T.T vaccine should be provided if patient is not immunized.

v Heart rate, cardiac disturbances and oxygen saturation be monitored.

v Intubation may be required if patient shows signs of respiratory failure.

v Avoid large fluid administration.

v Prazosin is given to control hypotension.it is also an antidote for scorpion bite.

v Alternatively nifedipin may be given. But should be avoided in presence of tachycardia.

v Dopamine may also be given for hypotension.

v Glucose –insulin administration may be helpful in systemic poisoning.

Some precautions regarding Facebook and Identity Theft

Friends this question might be more meaningful if I ask how many of you know the social networking sites like… Facebook, twitter, LinkedIn, MySpace, pinterest and many more…because today everyone knows them…not only educated people but also many uneducated having just basuc knowledge about computers and education are now on these sites and enjoying. But they doesn’t know what are the precautions to be taken on being online…my todays blog is regarding ths safety precautions for the Facebook users…this dosent mean that all other social networking sites are safe… all are having the same risks as well as benefits too.

I am not trying to be a killjoy about Facebook and all the benefits of social media and networking but you need to be much more circumspect about whom you choose to trust online.

Years ago it would take several weeks for identity thieves to gather such kind of information about a single person or any group of persons . Their interests friends , grops residents and many more details….. Social networks have made it easier for the bad guys to scoop up information about innocent members of the public. Everyone must learn to be more careful about how they share information online, or risk becoming the victims of identity thieves.

Remember, when you were getting warnings on being careful of opening emails or attachments from people you don't know. Even opening attachments from people you do know can be hazardous to your PC's health. Social Media is one in the same but you have control over what you post and how much information you put out there.

Here are some tips for users who want to protect themselves from identity thieves on Facebook:

1) Don't blindly accept friends. Treat a friend as the dictionary does, namely "someone whom you know, like and trust." A friend is not merely a button you click on. You don't need, and can't realistically claim to have, 815 true friends.

2) Learn the privacy system of any social networking site you join. Use restrictive settings by default. You can open up to true friends later. Don't give away too much too soon.

3) Assume that everything you reveal on a social networking site will be visible on the Internet for ever. Once it has been searched, and indexed, and cached, it may later turn upon line no matter what steps you take to delete it.

4) Don't let people know that yournot home or are away for an extended period of time -- it is an open invitation for someone to visit your home and problems.

Our honeymoon period with social networking sites ought tube over by now -- but many users still have a 'couldn't care less' attitude to their personal data.

Hence be protected… don’t give unnecessary information on your Facebook accounts… nor to your friend…and of Corse keep your Facebook passwords safe…so that no one can see your details without your permission.

Management of a snake bite case

Friends snake bites are very common in the entire world. But you might be unknown of some of the basic facts about snake. …..Which might help you to survive an attack of snake bites? Hence I recommend you all to read this article and share with your friends… as this much information might save the life of any one may be of your loved ones.

Have you ever wondered why these snakes hs been provided by this poison by the nature… yes you are right, it is just defence mechanism of it…so 1s precaution for you is never never irritate a snake by intentionally because it will endanger your life.

As per the data available more than 60 % cases are because of human involvement with the snakes…

Snakes esp. poisonous snakes are nocturnal means they usually come out of their pits only at night for the purpose of hunting so always be careful at nights or wherever there is more darkness.

In India, only 4 species of poisonous snakes are found.. They are cobras, kraits, vipers, hump nosed viper and sea snakes.

Some other poisonous snakes which are ffound in the world are: pit vipers, Russell viper, bird snakes, and coral snakes, bush master etc…

Hence 2^nd precautions what you should remember that in only 5-6% of snakes are poisonous and the rest 90-95% snakes are non-poisonous so never be nervous or afraid that you wil die after a snake bite. Because there are chances of being a non-poisonous snake as they are more numerous…so always have a positive attitude that it was a non-poisonous snake…because facts reveal that many peoples bitten by the snakes die not because of the snake but due the cardiac failure due to excessive anxiety.

So this doesn’t mean that you need not to take any precautions. Do take all necessary precautions such as immediate visit a hospital and get an anti-venom injection.

So this also doesn’t mean that you are free to disturb any snake…ant venom also acts when it is administered immediately after bite… further when neurotoxin snakes such as cobra bites, person may die within 5 minutes.

Symptoms after snake bite ( cobra and kraits )

1) Neurotoxic snake bites:

a) Local swelling and pain.

b) Krait bites are painless without any selling.

c) Heaviness of eyelids.

d) Blurred vision and diplopia (things appear double).

d) Paraesthesia around mouth.

e) Headache, dizziness, vertigo, hyper salivation and red eyes.

f) Respiratory failure is the major cause responsible for death.

g) Hypotension and ECG changes may occur.

2) bites by vipers

a) They mainly have local and hemotoxic effects.

b) Local swelling, bruising, blistering, and necrosis.

c) Bleeding in the gingivae.

d) Hypotension and shock.

Heart failure and kidney failure may also occur.

They usually occur within 5 days of bite.

3) bites by sea snakes

a) Muscle pains, muscle weakness.

b) Cardiac involvement also occurs here.

Management in simple steps.

1. Reassure the patient.

2. Immobilise the bitten limb.

3. Get to the nearest hospital.

4. Apply tourniquet just proximal to the wound if it takes more than an hour to reach the hospital.

5. Tourniquet should be loose enough to allow a single finger.

6. Don’t do any local incision as it may allow poison to enter even more easily.

7. Do not apply ice over wound.

8. Try to identify the snake but doesn’t go behind it.

9. Maintain airway and breathing and circulation (in hospital).

10. Clean the bite site with soap water and elevate to reduce oedema.

11. Don’t allow the patient to sleep. Or keep him conscious.

12. Administer monovalent or polyvalent anti-snake venom whichever available.

13. Antisnake venom is added in normal saline and infused at the rate of 1ml/ minute. If reaction occurs, it may be given rapidly over 1 hour.

14. Dose of anti-venom should be 60 to 100 ml for both children’s and adults.

15. Antivenom may cause some life threatening side effects such as hypotension, bronchospasm and angionerotic oedema. Hence it should be managed

16. These reactions are treated with adrenaline 0.5 to 1ml subcutaneously, chlorpheniramine maleate 10 mg I.V and hydrocortisone 100 mg is given I.V.

17. In addition neostigmine in a dose of.5 mg is given every 30 min for cholinergic side effects.

Note:

Friends this much information is enough for anyone to save anyones life but all these activities require experts to carry out. But some of the steps such as giving first aid and precautions can be taken by any individual. Hello friends, recently we came across certain devastating and life threatening natural disasters such as tsunami, earthquakes, radiations, volcano's etc. and many more. today I am going to tell you all about tsunami , many of do not know what it actually means, what are the causes for its generations, its spread, where it occurs, what are the precautions we need to take to survive during it and lastly how to track it.

TSUNAMI

* Tsunami is a name given to abnormally and excessive large tidal waves generated over a oceans occurring as result of cosmic waves generated under the water as r result of underwater earthquakes.

* when earthquake occurs under the ocean .it leads to production of very strong tidal waves over the surface which ultimately leads to large and strong water tides travelling towards the earth surfaces and when they ultimately reach earth surface causes severe devastations to many kilomts. inside the earth surface.

*The water waves may reach up to 20-40 metres over the earth surface hence making surely impossible for anyone to survive. Secondly they being very strong, they usually break each and everything which comes under its way.

*Tsunami is usually more devastating when it reaches the surface and hits it by its full speed and one may not survive if it’s nothing in its way before it hits you like any big building or a tree.

*Tsunami though unknown earlier but changing environmental conditions such as increasing automobiles, increasing industrialisations have ultimately lead to increased pollution and carbon content which have the capacity holding heat have raised the temperature of the earth as a whole leading to ice being melt at the polar levels which has increased the water levels of the oceans. Hence if this is not controlled now only it may cause many more tsunami in the future taking many lives.

Some facts about tsunamis

1)A tsunami is a series of very strong destructive and very dangerous water waves that result from earthquake activity or some other type of underwater disturbances ( which includes meteorite , landslides , underwater volcanic activity etc.).

2) Although tsunamis may be referred as tidal waves but it is not true because tsunamis have nothing to do with the moons gravitational pull on the tides, this term is a misnomer; although a tsunami's impact upon a coastline is dependent upon the tidal level at the time a tsunami strikes, tsunamis are unrelated to the tides.

3) As we all know that Tides occur due to imbalanced, extra-terrestrial and gravitational influences of the moon, sun, and planets hence their presence may only cause the already initiated waves to raise high over the surface of water . The term “seismic sea wave" is also misleading. because “Seismic" implies an earthquake like generation mechanism, but a tsunami can also be caused by a non-seismic event, such as a landslide or meteorite impact hence both causes may act or may not act here.

4) In recent years as we all came across, tsunamis have caused an incredible amount of damage in many coastal countries including India, srilanka, etc. hence In order to survive a tsunami, you must be prepared, vigilant, and calm. My blog sets out the ways that can help you to survive a tsunami, provided you learn and act upon these steps in advance.

Precautions and ways to survive a tsunami

Steps are as follows:-

1) Keep regular track over the underwater activities. Keep scientific instruments to get information’s about these. Learn about the tsunami from medias such as televisions, radios etc.

2) If you home, school, or workplace lies in the coastal regions then you have to keep information’s about these regularly.

3) Keep a track of sea elevations over the normal with the help of local government or technical authorities.

There are warning signs indicating that your area is prone to tsunamis.

4) Your local authorities may have issued information about the potential for tsunamis.

5) Tsunamis may have struck your coastal region in the past. Hence search for it through library or any other informatics centre’s for you rule out any future recurrences.

6) FEMA has a website which enables online flood risk searches. So you may search for this on the internet.

7) Also check whether your home, school, or workplace etc. buildings are tsunami resistant or not, if not take necessary precautions to develop them and make them tsunami resistant with the help of engineering techniques.

8) Prepare in advance. If your research shows you that you are at risk site, and then prepare yourself and your family both an evacuation plan and a safety pack.

9) Prepare a safety pack which Include food, water, and a first aid kit are the basics required. Keep the safety pack somewhere obvious, well-known to everyone in the building or house and easy to grab in an emergency. It can also help to leave a raincoat or other coat for each person near the safety pack.

10) Assemble a personal survival pack for each person in the family, and family survival pack with common items for everyone. Also don’t forget survival items for your pets.

11) Develop and be trained a personal evacuation plan. An evacuation plan must be prepared in advance to be of use. In developing one, consider your family, your workplace, your school and your wider community.

12) If necessary, begin to develop a community-wide evacuation plan if nothing is being done in your community. Or if you don’t know anything about this then you may take help of a professionally trained officer or worker at the government authorities.

13) Take the initiative to start developing the plan, and involve local authorities and other residents. Lack of evacuation plans and local warning systems increase the vulnerabilities for you, your family and community. 14) These are the things that should be part of a successful evacuation plan:

15) Discuss with family and colleagues on the options for evacuation.

16) Practice drills to ensure that all members of the community are clear about what they need to do and where they need to go during a safety evacuation.

17) Include a plan that can ensure a head count of every single member of the community; ensure that assistance for disabled or ill persons can be provided.

18) You may find all these facilities for training at government disaster management authorities. They also provide training for other disasters such as earthquakes and also manmade disasters too.

19) Lastly some tips which may help ypu to survive during earthquakes are when you see a large tidal waves heading towards yourself rather than running do simple things such as wear water coats, rise to the top of a building or immediately hold certain strong things such as a tree may help you and your family to survive.
