vulnerable to the dangers of heatwaves, Independa medical director warns

Press release Source: Independa Wednesday, August 3, 2011, 9: 00 am EDT
SAN DIEGO, the 3 August 2011/PRNewswire / - as the cities of the United States from the record-breaking heat wave this summer, is essential for those who care for the elderly to proactively protect their health, said Dr. Richard Della Penna, medical director of Independa ?, Inc. and an expert in attention. He adds that emerging technology makes it easier to take precautions and learn of potential problems.

"The elderly have increased risk for the population in general during extremely hot weather because our bodies do not respond to changes in temperature in the same way as we get older", says Della Penna. "More than cause discomfort, sustained heat and humidity are dangerous for the elderly".

By what age are you susceptible?

Factors, elders do not always feel so warm as young people in higher temperatures and do not necessarily feel thirst. Medical conditions can further decrease the ability of elderly people to deal with heat-related stress, and medications can interfere with the ability of cooling of their bodies, adds Della Penna.

Habits and lifestyles also play a role. Because they chill easily, older adults tend to dress warmly. They may not have air conditioning or fans, and even if they do, the people on fixed incomes often hesitate to use these electrical devices. In urban areas, older persons close often their Windows and doors for fear of crime.

"Isolation certainly is a risk factor for the elderly," said Della Penna. "Many of those who die during heat waves are the elderly who live alone and have no one to check on them."

During the hot spell that hit Chicago in July 1995, the death of 522 371 reported participants 65 years of age. This summer, the national weather service attributed as 64 deaths from the heat wave at the end of July as the Middle West, East and Southern States experienced temperatures in the 90s and 100s.

How can carers help

Della Penna requests for carers to act now, using technology of virtual communication and follow-up if they do not live near or far are on vacation.

"New technology enables remote caregiver proactive and be notified of possible danger signs," he said.

Della Penna recommends the following measures for carers:
Monitor weather reports and reach the recipients of care when the weather forecast calls for a heat wave, which can help them plan. Currently customers Independa can organize alerts linked to the outside temperature, for example, if the temperature reaches 90 degrees and can monitor local weather from the recipients attention from its desktop software.Encourage older to wear light clothing, loose.Assist older people to drink water or juice during the day. Reminder from Teleassistance for Independa platform can be programmed to send reminders to drink liquids at regular intervals of the recipients. To encourage recipients of care to use the air conditioning or fans in their homes, or to move environments more frescoes in homes of friends, cooling centres or other public places. If necessary, emergency transport. They suggest to minimize activities that generate heat in the home, including cooking with the stove or oven. You should avoid strenuous exercise.Talk to your doctor the recipient's attention about medications and ask about possible changes in the short term, for example, to protect against dehydration.
"Taking advantage of benefits of the technology and the medical staff care receivers", says Della Penna. "To provide advice for the elderly and helping them to plan for high temperatures, caregivers can leverage them take care to protect themselves." Caregivers can also use the technology to "see" their loved the or the patients houses and intervene if something doesn't seem quite right. "

Della Penna is guiding efforts to Independa to help the family and professional caregivers prolonging the independence of the elderly to promote their health, safety and social commitment of remotely. But the Beacon Falls, Conn., government worker was less impressed with the plan's execution: A parade of 26 home health aides in 10 months' time all seemed poorly screened and mismatched. One, she says, tried to get Debbie's father to invest in a gym venture; another left him unattended for hours while she Skyped loudly. "We had to learn the hard way," says Lanne.

Enter the health care industry's little-known middleman. Turns out, finding good care for today's elders is just the tip of the iceberg -- now there's the added step of finding a trustworthy "manager" for that care. (For her part, Lanne tried several before finding one through a national firm called SeniorBridge.) Indeed, as baby boomers hit age 65 in record numbers, the government estimates that 70 percent of them will require long-term care -- prompting predictions that the market for elder-care management has only begun to explode. With comforting names like Aging Gracefully or A Servant's Heart, most services are tiny local outfits, usually a licensed social worker or a nurse hanging out a shingle as a "geriatric-care manager" or "elder-care consultant." But there's also a handful of fast-growing national companies -- such as Los Angeles-based LivHome, which says its gross revenue nearly doubled from 2004 to 2010, to $48 million, and SeniorBridge, which boasts 44 offices nationwide, triple the number it had in 2008.

Trouble is, critics say, it's hard to sort out who's a pro, who's a well-meaning newbie and who's just rubber-stamping required insurance forms. Because states don't yet require licensing for these managers, their expertise varies greatly; they charge from $75 to $200 an hour for anything from crafting care plans and training home-care workers to attending doctors' appointments and managing a client's household -- and checkbook. The best, says longtime Washington, D.C.-based elder-law attorney Robert Bullock, orchestrate a team of experienced advocates from the financial, legal and medical fields. "You need someone who can go toe-to-toe with the doctors and facilities," he says.

And someone, say health pros, who won't take advantage of vulnerable clients. Critics say the bigger companies often send out a salesperson, not a care manager, to initially meet families at fraught moments, like when an elder family member is being discharged from a hospital. (Mike Nicholson, CEO of LivHome, says, "We provide a high-quality service, and we sell it.") And while experts say the vast majority of care managers operate ethically, there have been criminal abuses. Several cases in Florida garnered attention a few years ago, when managers used their access to clients' financial records to defraud them of thousands of dollars.

Experts suggest doing background checks and looking for managers with longstanding professional credentials. Certified members of the National Association of Professional Geriatric Care Managers, for example, must have a degree in social work, psychology, nursing or gerontology and follow a code of ethics that prohibits practices like taking fees from other service providers. "It's an accountable screening," says Cathy Cress, author of Care Managers: Working With the Aging Family.

View the original article here

Suppliers of services to older adults have concerned past

A former partner of Carson City with a turbulent past in the children and senior care industry is in the business in the area.
Mike and Norma Childers obtained a license in November to operate a home for individual residential care by the Division of health in Nevada, Office of health care quality & compliance. Childerses, operating as a sincere care LLC, will take in people at his home in Washoe Valley, according to the license application.

In 2006, Norma Childers not declared decision to a charge of disorderly conduct after he was jailed and accused of abuse for allegedly placing tape on the mouth of the children in their care in the sugar n' Spice nursery. She was forced the charges after a preliminary hearing, but the case was settled prior to trial.

As part of the agreement of plea submitted in December 2006, he was forced to undergo an assessment of the management of anger, to avoid allegations of violence and disorderly conduct and serve 100 hours of community service. The Childerses decided not to operate a facility in California for 10 years.

Norma Childers refused to comment on Tuesday, as did the counter Randy Kuckenmeister, who wrote a letter to judge Todd Russell claiming that he and the children of a woman of Carson City for 86 years of age which was appointed special master agreed Norma Childers to care for his mother in the mother's House.

The plan, according to a letter filed in that case may 2 by Kuckenmeister, was to ensure that Childers ensure "appropriate license" of the State in which it could bring two others "in the long term care beds" the home of the woman. In exchange for the trust of the women he would pay only utilities and insurance at home and for the purchase of the women, save the estate of $ 12,000 per month, according to Kuckenmeister. The State of that agreement was not immediately clear.

Mike Childers said Tuesday he and his wife does not think to have problems in their new effort of attention.

"Just because someone makes a complaint, do not stop." We would love to do it.
After she was caught, Lisa M. Blair told police that her criminal behavior at a South St. Paul assisted-living facility "just got out of hand."

For months, the Cerenity Residence worker stole thousands of dollars from elderly residents who relied on her for day-to-day help, including paying their bills. At least two residents were bilked for nearly $63,000 in a scheme that ended when an 88-year-old raised suspicions in December about Blair, police records show.

State health investigators found no fault with Cerenity Residence for the breakdown, which lasted for more than 18 months. Blair pleaded guilty this month to four felony counts of financial exploitation of a vulnerable adult. "I maybe took advantage," she told police. "I knew it was the wrong thing to do."

The case is part of a broad problem of elder abuse in Minnesota that raises questions about the state's oversight of a rapidly growing industry.

In at least a handful of cases, state records show, breakdowns left vulnerable adults exposed to abuse. Either facilities missed warning signs or applicants with known criminal histories were approved to work because regulators granted exceptions to rules aimed at protecting vulnerable people.

Since 2005, state investigators have reviewed 171 cases of alleged financial abuse or exploitation in nursing homes, assisted-living facilities and in-home care settings. In 77 percent of those cases, investigators substantiated those charges, almost all of which involved staff members stealing from residents, according to a Star Tribune review of state investigative files.

Officials with the Minnesota Health Department declined to be interviewed for this story. The agency does not track the number of people who have been allowed to work in health care settings despite their criminal records, but in an e-mail officials estimated that the agency grants "very few" such variances each year.

Employing care workers with criminal histories or other problems is an outgrowth of a national shortage of people available to work in a field with low wages and demanding conditions, according to Deb Holtz, Minnesota's ombudsman for long-term care.

Holtz said the presence of background checks may give some clients a false sense of security, not knowing some offenses may be forgiven.

"The general public isn't aware of everything that goes into a background check," Holtz said. "The information should be public, and it should be easily accessible on all the state agency websites. There should be information given to consumers when they are considering employing people."

Nationally, seniors lost at least $2.9 billion in 2010 through financial abuse, with about one in six cases involving nursing homes, assisted living facilities or paid caregivers, according to a recent study by a research group affiliated with insurance giant MetLife.

"This has been happening a lot, and we probably haven't gotten a handle on it," said Sandra Timmermann, executive director of MetLife Mature Market Institute. "How do you get your arms around this hidden problem that is becoming more and more prevalent?"
Thief gets second chance

State law is designed to prevent people who commit significant crimes from working in nursing homes or other care settings. Health care workers undergo criminal background checks, but their new employers can ask the Health Department to overlook previous wrongdoing and allow them to work. There's nothing in the law that requires regulators or caregivers to disclose such variances, leaving seniors or their loved ones largely in the dark about who may be caring for them. The state Department of Human Services can also grant exceptions to individual workers.

Kilolo Claiborne had a well-worn criminal past when she went to work as a home health aide in Ramsey County. Claiborne was a suspect in separate incidents of theft, credit card fraud and providing false information to police, according to law enforcement records. In 2003, she was convicted of felony theft.

That didn't stop the state Health Department from granting a waiver in 2008 that allowed Claiborne to work with vulnerable adults. Less than a year later, she stole $1,575 from a terminal cancer patient who lived in a group home where she worked. She was convicted of felony theft in 2009.

"There is nothing in the law which prohibits granting a variance to a convicted felon," Health Department spokesman John Stieger said in a written response to questions about the case.

In Claiborne's case, the waiver came with conditions the facility was supposed to follow, such as not allowing Claiborne to work without a supervisor present or have access to client funds. However, the Health Department's investigation showed Claiborne was left alone for a period of time on the morning the theft occurred. State records do not show that the facility was disciplined.

In the case of Michelle Ann Jensen, the state flagged her criminal history in June 2009, shortly after she went to work at Whispering Pines Ranch in Austin. A previous felony conviction for prescription fraud disqualified her from direct contact with residents. She requested reconsideration and was supposed to be under continuous supervision until her appeal was resolved.

In August 2009, Jensen stole about $900 intended for the facility's locked cash box, which included residents' cash and rent payments, according to state records. In April 2010, she was convicted of misdemeanor theft.

Jensen blamed her problems on an addiction to prescription drugs. She said she wishes she'd never gotten the job at Whispering Pines.

"I take fault for my actions," said Jensen, adding that she regrets what she did and has been drug-free for nearly two years. "But they could have all prevented it if they would have followed state guidelines and not let me work alone."

A Health Department investigation did not cite the facility for failing to provide continuous supervision of Jensen. Regulators noted that Whispering Pines addressed the problem by requiring two workers to count cash at the beginning of each shift and log disbursements. The facility management did not return phone calls seeking comment.
Signs of past problems

Betty Frye was victimized twice.

In 2007, her bank discovered her checking account was overdrawn. In a period of two weeks, about $3,100 in fraudulent checks were written against her family's accounts. Investigators traced the fraud to a personal care assistant who came to Frye's south Minneapolis home to help the 88-year-old with her personal needs. The aide, who was not identified in police records, passed the checks to acquaintances who cashed them.

A nurse who worked with the woman had expressed concerns about the worker, according to Dennis Frye, Betty's son. "She was quite adamant about not wanting her to come here," Frye said. "Obviously there was something in her background."

The family also had problems with a previous aide who ran up long-distance charges by calling a dating service in California. "It's a betrayal," said Dennis Frye, whose mother has since died. "They are supposed to take care of you. When you have nurses, bath aides, other people coming and going in your home, you have to be careful."

Blair also figured in two different cases, according to police records. Blair was investigated in 2007 over a case involving her grandfather, who was a resident at Cerenity's facility in White Bear Lake. Blair told police she reimbursed her grandfather for his losses and no criminal charges were filed for the incident, police records show.

Cerenity officials said they had no reason to suspect Blair, the tenant services coordinator, was stealing from residents last year, according to Chuck Heidbrink, president of Cerenity Senior Care, which has five locations in Minnesota.

"The tenants loved her," Heidbrink said. "She was a tremendous employee. It was a shock to us and the tenants."

As a result of the Blair case, Cerenity began requiring the presence of two employees on any matters where clients need financial assistance.

Blair, who will be sentenced Sept. 12, did not return phone messages seeking comment.

State Health Department investigators failed to uncover the full scope of Blair's wrongdoing. The agency's report identified only one victim, but police investigators noted that Blair stole from two seniors at the South St. Paul facility. The first victim lost nearly $47,000, while a second victim lost about $16,000, police records show.

In each of the past four years, federal officials have faulted the Minnesota Health Department for failing to properly investigate complaints of abuse and neglect.

Health officials defend their investigation of Blair, saying its work was "adequate to substantiate financial exploitation" under Minnesota's Vulnerable Adults Act. "The finding would remain the same no matter how many victims there were," Health Department spokesman Stieger said. The department interviewed about a dozen staff members and residents. Said Stieger: "We do consider this a thorough investigation."

Brad Schrade ‧ 612-673-4777

<a href="http://www.startribune.com/local/126459753.html" U.S. Attorney Andre Birotte Jr. said in the statement.

The Paso Robles couple pleaded guilty in May to conspiracy to harbor illegal immigrants.

The Moraleses, who were born in the Philippines and are U.S. citizens, operated four elder-care facilities. They recruited Filipinos to work as live-in caregivers. Phony visas were arranged so they could come to the U.S., prosecutors said.

Once they arrived, some immigrants worked 24-hour shifts alone, according to plea agreements filed in the case.

They lived in the care facilities, some sleeping in a closet, on a sofa or in a walled-off portion of an unheated garage, according to the plea agreements.

Their pay was credited against their smuggling debts, and the workers were told that authorities would be called if they tried to leave, prosecutors said.

Authorities eventually were tipped off to the working and living conditions, and the FBI Advertisementarrested the couple in March 2010.

Copyright 2012 The Associated Press. Elder mistreatment encompasses a range of behaviors including emotional, financial, physical, and sexual abuse, neglect by other individuals, and self-neglect. This article discusses the range of elder mistreatment in community-living older adults, associated factors, and consequences. Although self-neglect is not considered a type of abuse in many research definitions, it is the most commonly reported form of elder mistreatment and is associated with increased morbidity and mortality. The case on which this article is based describes a 70-year-old woman who neglects herself and dies despite multiple contacts with the medical community. Despite significant gaps in research, enough is known to guide clinical practice. This article presents the practical approaches a health care professional can take when a reasonable suspicion of elder mistreatment arises. Public health and interdisciplinary team approaches are needed to manage what is becoming an increasing problem as the number of older adults around the world increases.

View the original article here

Disturbing report on elder care could make Liberals squirm

AppId is over the quota
AppId is over the quota

During a tour of the province in 2008, B.C. ombudsman Kim Carter encountered a frustrated, angry and often scared faction of the population: seniors living in residential care.

The shocking numbers of those appealing for her help prompted Ms. Carter to launch a fulsome investigation into the system that was spawning the complaints. On Tuesday, the most extensive – and disturbing – examination of the state of seniors’ care in the province landed in the B.C. Legislature with the kind of thud that often portends trouble for a sitting government.

Three years in the making and more than 400 pages in length, the report is an often uncomplimentary indictment of efforts by the Liberal government to address issues that have been known about and discussed for years. In fact, two years before Ms. Carter launched her probe, the government had received a similarly themed report from a blue-ribbon council on aging and seniors’ care that made several recommendations that the ombudsman would revisit in her review.

To that extent, it’s not that the ombudsman’s findings will strike a person as terribly revelatory, but they are certain to incite head-shaking among a citizenry that has heard the B.C. government say before that is addressing age-old problems among the province’s old agers.

For instance, there remains no minimum standard of care in many facilities, it seems. On that front, Ms. Carter’s report is loaded with anecdotal evidence: residential home beds soaked in urine; feces caked on bed railings; residents agreeing to go to bed at 4 o’clock in the afternoon because of a shortage of staff.

Clearly not enough is being done to crack down on homes that continue to offer what would charitably be described as sub-standard care.

Ms. Carter’s report includes 176 recommendations, many of which the government said on Tuesday it would be implementing. However, the ombudsman let it be known she was disappointed the government wasn’t going far enough with many of her suggestions.

Among the areas the government’s watchdog is demanding action on are:

-An internet venue from which seniors can navigate the home and community care system. Among the information that seniors should be able to access is space availability at residences around the province as well as important background information on the homes;

-Consistent reporting and tracking of elder abuse and neglect;

-The protection from retaliatory action for seniors who complain in good faith about the home and community care services they receive;

-Objective and enforceable standards of care for home support services. This would include legally binding minimum requirements for assisted-living residences on staffing, residents’ rights, food safety and nutrition, emergencies and record management;

-Ensuring equal treatment, benefits and protection of seniors in residential care by establishing one legislative framework that applies to all residential care facilities instead of the two that currently exist;

-Enhancing dementia and end-of-life care services in residential care;

-Helping seniors stay in their homes by assessing the adequacy of current support programs and analyzing the benefits and costs of expanding those services.

As we say, many of these recommendations have been made before, either formally or informally by people inside and outside the seniors’ health care system. And while the B.C. government should be applauded for many moves it has made over the years to help an increasingly vulnerable part of the population, it clearly has not done enough.

And it is not a good sign that the ombudsman, who gave her report to the government some time ago to consider, already seems discouraged by the Health Ministry’s response to many of her recommendations.

B.C. has 22,000 people in residential care. Ninety-five per cent are 65 years or older – 52 per cent are past the age of 85. The number in residential care is only going to increase as the baby boomers get older and require greater care.

“Our goal is for there to be consistent, province-wide standards and processes that treat seniors across B.C. in a fair and equitable manner,” Ms. Carter said in her report.

That doesn’t sound like too much to ask.

View the original article here

Disturbing report on elder care could make the Liberals squirm

AppId is over the quota
AppId is over the quota

During a tour of the province in 2008, B.C. ombudsman Kim Carter encountered a frustrated, angry and often scared faction of the population: seniors living in residential care.

The shocking numbers of those appealing for her help prompted Ms. Carter to launch a fulsome investigation into the system that was spawning the complaints.

An easy solution for positional vertigo

Benign positional vertigo

I did about 10 years ago. I lowered my head down to sleep after a normal day and WHAM! I had the combat violent and disturbing the Vertigo that I had. My eyes rolled out of control as the room revolved around and with a sufficiently intense dizziness like that make me vomit. After this initial episode I kept having episodes of dizziness while they are on my right side or suddenly move my head.

If this experience sound familiar then probably you have a condition called benign positional vertigo (BPV). It is a condition that I see often in my practice. While it is often distressing for those affected it is not serious and is fairly easy to treat.

BPV is distinguished primarily by two points.

1. Vertigo associated BPV is always related to the movement of the head or body position changing.

2 Dizziness BPV has a fatiguing quality to it. Dizziness is more intense immediately after the move, and then vanishes.

The inner ear contains structures that give us a sense of position in relation to gravity. These structures are the semicircular canals. There are three channels for each ear placed perpendicular to one another. This gives us three dimensions of sense of position. The canals are filled with fluid and are lined with sensitive hair cells that detect movement of particles suspended in fluid. Movement is picked up by the cells of hair as the flow of particles, beyond them. Head movements are detected this way and we are aware of direction within the space even when our eyes are closed.

The theory is that the intense dizziness of BPV occurs when particles stick together and form clusters. Groups act as boulders in the semicircular canals and more to stimulate hair cells as they move beyond them. For some, it is not known what triggered the formation of groups, but it often correlates recent colds or allergies.

Epley maneuver is the treatment that I recommend to my patients because it is easy to make and generally clears the dizziness in a matter of days without drugs.

The first thing one must do is to assess what affected ear. Usually severe dizziness and eye rolling have a quality of repeated rotation. I tell my patients to pay attention to address the Chamber appears to spin when comes the dizziness. If it seems to turn to the right, the right ear is the problem. On the other hand, if the turn on the left, then assumed the ear left it is groups. Once it detects the problem side a few simple moves in the correct order is all that is required.

EPLEY MANEUVER

The steps outlined here will be for someone with Vertigo of the right ear.

Step one. (Extension-derecho of head rotation) Lie on your back and let that head hanging off the edge of the bed that is tilted with the head turned to the right so that the right ear is pointing to the ground.

Step two. (Rotation of head left extension) Turn the head to the left of position one to the left ear is pointing to the ground. Remember that the head still extended back a bit.

Three step. (Body of rotation to the left) Rotate the entire body to the left up lying on left side with the nose pointing to the ground.

Four step. (Sit in an upright position) In a continuous motion rise of up sat three position at the foot of the bed.

Make this series of movements of one or two times a day for a week.

This is what there is to it.

Useful tips:

Normally the movements will induce dizziness at every turn. Do not rush through the steps. Wait for the dizziness to disappear completely in each step before moving on. It is important to not lie down immediately after making this maneuver. Normally I recommend doing this procedure in the early morning, perhaps again at noon and then not again until the next day.

If performed correctly only should take three or four days until symptoms subside.

For an illustrated guide to click in the West End of chiropractic Epley manoeuvre link below.

http://www.westendchiropractic.ca/

Robert Vari degree, D.C.

Langford, British Columbia

Robert Vari is a chiropractor in private practice who specializes in non-invasive, hands in the treatment of musculoskeletal disorders.
Robert is the owner of West End chiropractic, massage & acupuncture that serves the residents of the region of the West coast of Victoria, BC.

Auto care goes a long way in preventing keratoconjunctivitis Whitlow

Finger herpetic whitlow is a contagious infection of the finger that is considered to be very painful. Caused by herpes simplex, herpetic whitlow - the virus can be spread easily through direct physical contact, like any other viral infection. Finger Herpes affects people of all ages and time of all backgrounds. This is especially common among children and the professionals of the medicine. The new draft law extends the scope to include any person who works in facilities, regardless of the degree of accuracy that interact with the residents.

"Extending the statute can provide greater protection for a population of elderly who cannot speak for themselves when they abused them," said lawyer of personal injury from Bowling Green Marshall j. Hughes, whose law firm has extensive experience in handling cases of elderly abuse and increased oversight in Bowling Green, Louisville and parts of Tennessee.

"Any employee, not only those who provide direct care to nursing home residents, still have many opportunities to exploit them only because they are in such close proximity", added.

The new draft law comes on the heels of another movement of Kentucky to resolve the cases of elderly abuse. In June, the Centers for Medicaid and Medicare services with the State a grant of $ 3 million to upgrade its system of verification of background to one that seeks only his name to one that displays to potential employees by fingerprint.

"Could be a preventive measure excellent," said lawyer Lee Coleman, who also handles trials of death arising from major abuses and personal injury.

"And because fingerprints are sent to the police of the State and the FBI for criminal records, the selection process will be wider and hopefully reduce the instances of more abuse," said the lawyer for personal injury of Bowling Green.

But friends and family members should still keep a watchful eye for signs of abuse of older people, both caution of lawyers in Kentucky.

"Background checks only can go so far in the prevention of cases of abuse of elderly people," says Coleman. "For example, is what happens to offenders who have a clean record, but start abusing elderly later hired?" "Attentive to the signs of abuse or neglect is everyone's responsibility".

Possible signs of further abuse may include:

"Please note that better meets your loved" advises Coleman. "May not necessarily see the typical symptoms, but other behaviors or physical changes that are incompatible with their status". "If the suspicions are housed, it is best to investigate or to contact one lawyer experienced more abuse to make sure that you get the answers you deserve".

In fiscal year 2010, the Kentucky Department of community services was 13,491 reports of alleged abuse of people older than 60, the highest number of complaints it has received since the beginning of the communication of data in 2006, according to the State Cabinet for the annual report of health and family services.

"With strict enforcement and the intensive campaign of promotion, this new legislation can reduce that number even more," says Hughes. "The largest population in Kentucky nursing home deserves nothing less than the best quality of life in their golden years".

About Hughes & Coleman injury lawyers

Hughes & Coleman lawyers of injury, with offices in Bowling Green Kentucky, Elizabethtown and Louisville, is dedicated to the protection of the rights and interests of abuse of elderly people and victims of neglect, as well as families who cares deeply for his beloved elders.

Partners Marshall j. Scratch, that - we have scarcity, as a report by the Institute of Medicine of course back in 2008. In fact, the majority of the counties in the United States does not geriatricians practice within their borders.

The real challenge, according to Nancy Lundebjerg, director of the American Geriatrics Society and co-organizer of a coalition of the Alliance of nursing workforce: "we can keep the shortage of people with this formation get worse?"

The problem arises in part from the reluctance of the students specialize in geriatrics or Gerontology. Only 2% of nurses and social workers do, said Christopher Langston, program director at the John Hartford Foundation., which helped to finance the Institute of medicine's report.

And less than 400 geriatricians year out programmes of academic training, in part because perceptual (students see it as a depressing field, although its practitioners it is very satisfying) but in part by financial. With virtually all Medicare patients, they are the least paid in all specialist doctors geriatricians. They are actually penalized for its decision to the care of the elderly. After three-year internal medicine residents, their income falls by $15,000 if he completes the fourth year of the training needed to become a geriatrician, says Mr. Langston. More training is even less money.

The body of the national health service offers a modest solution, which should be in small scale: an agenda for cancellation of loans to help pay off the staggering debt that many health professionals correspond to them as students. Average physician, for example, owes more than $150,000 after completing his education. Doctors, nurses, dentists, social workers and other professionals who agree to work in areas like rural Texas or the center of the Baltimore City are eligible for reimbursement of loans of up to $30,000 a year for two years or $170,000 for five years. Reimbursement may not always delete the debt of the members of the body, but can make a substantial difference and perhaps a more attractive alternative process geriatrics.

But specialists in geriatrics, incredibly, are not included in the federal law on eligibility for the national health service corps. Created to increase the supply of primary care practitioners, the body allows specialists in geriatrics to participate at the moment, thanks to the intervention of Secretary of health and human services, Kathleen Sibelius. But unlike professionals in family and in internal medicine and Pediatrics, geriatrics specialists is recent and impermanent. A future Secretary could bounce them.

"Do you think that our health system is facing a crisis of kid," said Mr. Langston, with some frustration. "It is not, it faces a crisis of age person."

The Alliance of labour force for the elderly, whose 28 member organizations include a series of consumer, professional and governmental groups concerned with ageing, from the Alzheimer's Association to the federal Department of Veterans Affairs - care has proposed an amendment to add specialists in geriatric law governing body. It is a simple Bill, only 29 words for a long time as the title, the care of an Aging America Act. It does not require additional costs, but if it is successful in the development of more people in service, Congress could decide to eventually increase the appropriation of the body.

But the legislation probably not going anywhere for now. An earlier version died of neglect in the last Congress. This time, two Democratic senators (Barbara Boxer of California and the outgoing Herb Kohl of Wisconsin), a Republican (Susan Collins of Maine) and an independent (Bernie Sanders of Vermont) co-sponsoring the revised legislation. Sponsors are still looking for sponsors in the House. But the current financial and political climate makes it unlikely that the Bill will pass this session. The best hope is to attach to any piece of legislation must pass, according to Mr. Langston.

So here we are with 6,000 geriatricians, a number that is actually declining. Is equivalent to a medical specialist for every 2,000 Americans over the age of 75, a group to obtain much more quickly. "It is not a good relationship," said Mr. Langston.

Useful Paula is the author of "when the time comes: families with aging parents share their struggles and solutions."

View the original article here

Care of most renowned community service agency

TAMPA PALMS-
The Agency in the area of Central West Florida on Aging, a non-profit organization, was presented superiority pending Award for community service recently for greater care Florida promotion.

austin Curry, director Executive for the promotion of greater attention, presented the award to maureen kelly, President and CEO, at a lunch in the Office of the Agency.

The Agency on aging operates a help line for older adults and provides information and resources for people seeking help on major issues, Kelly said.

It also administers more than 28 million dollars of public and private funding agreements with agencies to provide services such as day-care centres for adults, he said.

"Our goal is to provide all the support necessary to allow people to stay at home as long as possible," said Kelly.

Care more promotion had planned a lunch of lasagne 46 officers and Curry thanked them for their service.

Curry said at the luncheon of planning, "the idea is to push these dedicated individuals, shot in the arm so to speak, for his efforts of budget cuts and increase in the demand for services." As many say, ' are truly angels "to assist the elderly who need".

Library hosts rain barrel workshop

lynn barber, a master gardener with the Hillsborough County Cooperative Extension Service, will lead a rain barrel workshop on Saturday.

During the 10 a.m. event temple terrace public library, 202 Bullard Parkway, Barber will talk about the usefulness and the importance of using a rain barrel at home, especially during times of drought.

She also teaches how to make your own barrel of rain with a 55 gallon drum.

Seating at the event is limited to 50. In the last decade, minorities have poured into nursing homes at a time when whites have left in greater numbers.

At first glance, the analysis suggests that black, Hispanic and Asian elders are gaining greater access to care for the elderly. But the growing proportion of minorities in nursing homes comes in part because they don't have the same access to more desirable forms of care as wealthier whites, said the study's main author Zhanlian Feng, Assistant Professor of health of the community at the Warren Alpert Medical School of Brown University.

"Apparently, we are closing the gap in terms of access of minorities to beds for the elderly, but I do not think that it is something to celebrate," Feng, said. "They are really the last resort". "Most elderly people prefer to stay in their homes, or some place like home, but not an asylum for the elderly unless they have to."

New analysis shows between 1999 and 2008 population of elderly people in the country was contracted by 6.1 percent to slightly more than 1.2 million people. In this period decline in the number of whites in nursing homes by 10.2 per cent at the national level, while the number of blacks increased to 10.8 per cent, the number of Hispanics increased by 54.9% and the number of Asians increased by 54.1%. The study also examined changes in population of elderly people in the top 10 metropolitan areas of each minority.

Previous research has shown that in areas predominantly minority nursing homes tend to be of lower quality and are more likely to close, while assisted living facilities are more likely to be built in areas where residents are high-income. The result, which is reflected in the figures in the new book from Health Affairs, is a gap that plays not only economic and geographically, but also racial, said Feng.

"We know that these alternatives are not equally available, accessible or affordable to everyone, certainly not to many minority elders", said.

As politicians seek to "rebalance" attention to other forms of care homes for the elderly, for example with changes in the financing of Medicaid to support household and community, services should represent these disparities, said Feng. As it is, the targets are clearly more likely to use more convenient alternatives; more concerted efforts are necessary to promote the use of elderly minority of them too, he said.

"Rebalancing is a recognition of the preferences of the majority of people in long-term care," said Feng. "This effort to be successful you have to bear in mind that is so".

To determine the figures, Feng and his co-authors used the minimum data set laid down by the Federal Government, which keeps track of the users of the elderly population and assess their care needs routinely, receiving important Medicare and Medicaid funding. There is no similar for other types of more attention data, which is predominantly paid with private insurance dollars.

One way that Feng and his coauthors have quantified the disparity in the elderly is the grouping of metropolitan areas in quartiles based on the proportion of whites, blacks, Hispanics or Asians who were 65 years or more in each area. The researchers found that the representation of blacks, Hispanics and Asians in nursing homes increased as its share of elderly in the population increased. That correlation is not maintained for whites, which do not seem to be forced to choose nursing homes as they age.

In New York, a metropolitan area 10 higher for Blacks, Hispanics and Asians, the residents of the home of elderly people in these groups increased 22 per cent and 84 per cent and 40 per cent, respectively. Los Angeles / Long Beach, the increases were 1 per cent for Blacks, 41 percent of Hispanics and 56 percent among Asians.

Different rates in different cities, the challenge faced by the authorities is not only national, but also local, the researchers said.

"Efforts to reduce disparities should target communities and facilities with a high concentration of minority residents," they wrote in health issues.

This article was originally published in the print edition of July 25, 2011 in the Louisiana weekly newspaper

By admin

ZhanResidents of a home for the elderly in Nanjing, China.
In any debate here about the decision to move to one older person in some sort of focus, can virtually have a complaint in the comments section, often someone citing estate immigrants. Americans are too self-centered, too careerist, going to the criticism.

"Originally come from Argentina and in my culture we respect and honor for the elderly and that is shameful and selfish to put a parent in a home for the elderly", Maria Gonzalez of Cleveland wrote in the spring.

Dr. Kat Lieu's New York sounded just a little less dismissive. "Maybe is because I am an asiatico-estadounidense, but I never see very far from my parents," he wrote. "They were always there for me." "I will always be there for them." It would never consider the possibility of a nursing home for the elderly, she promised.

Of course, it is manifestly false that the Americans are in general somehow leaving their parents. The vast majority of our elders do not live in any type of institution and does not receive any aid paid, but rely, as always, in their families. Most members of the family are not cavalier about the decision to place a member of the family in a nursing home or assisted living, nor do walk away having done so.

I have often wondered: these attitudes really derive from the innate national or cultural differences? Or are a function of the form of operating in different societies, in which case may modify these values as societies change?

I thought it was fascinating, therefore, to find homes for the elderly are emerging in China, where the Confucian principle of "filial piety" held sway for thousands of years. Zhanlian Feng, researcher who only reported its findings on the growing of minority populations in U.S. nursing homes, emigrated from China in 1997 and graduate of Brown University, where he now teaches.

At the time he left his native land, the old ways still held. "Do if he spoke with an adult child and asked, ' If you get too sick to care at home for his mother or father, would consider some sort of House or installation?," surprises, "said in an interview.""It was a shameful idea." It was expected that adult children, especially children, to take in their parents. End of the story.

But when Dr. Feng returned to China in 2006 with a grant from national institutes of health research, "attitudes of the people have changed a lot in a few years," he said, "even in rural areas where you think traditions have a stronger suspension."

Contemporary China is experiencing many of the same demographic and socio-economic pressures such as United States, it was recently reported in the journal of the American Geriatrics Society. People are living much longer; family structures are changing; women have entered the labour force. With no national Medicare health insurance program and the policy of the only child that requires greater attention to shoulder responsibilities less Chinese families also face some challenges larger than ours.

There is no safety net, said Dr. Feng, and "has created a growing need to care for aging people." (A lot: now people approximately 112 million people aged 65 and a projected 329 million by 2040.)

Surveying seven Chinese cities, he found a proliferation homes for older persons, the vast majority of private number built and operate with negligible Government subsidies. For example, the ancient capital of Nanjing, had 27 houses in 1990 and 52 a decade later. In 2009, when Dr. Feng and his team began to investigate, the city had 148. Beijing and Tianjin showed a similar growth. Shanghai had 552 facilities.

It is probably not accurate to call the homes for the elderly. Residents younger and healthier than found in U.S. nursing homes, these facilities seem to be more similar to the independent or assisted living. The majority does not employ physicians or nurses.

Even so, they represent a seismic shift in the way that China is concerned about their elderly. "We are talking about changes in two generations," said Dr. Feng. "The younger generation wants more privacy" and prefer to live in nuclear families.

"One could imagine that the elderly would be annoying," was Dr. Feng. "But they will say," it would be rather alive by myself. I am very happy here. Do I chat with people, I can do activities with colleagues rather than spend each day with the family of my son.? "" Not sure that they are all so happy as they sound, but most people age have small pensions and "you can buy autonomy".

(A similar change took place in the United States in the 1940s with the advent of Social Security).

If attitudes on increased attention can change so quickly in China, it is difficult to believe that any industrialization society is immune. I hope that we will continue to hear from some quarters about how other cultures compared to the Americans without a heart, but not how Dr. Feng sees.

"Immigrants tend to beautify and mystify things in their countries of origin," he said. But when analyzing the way of life of older persons and families in the United States and China, "I see striking similarities".

Useful Paula is the author of "when the time comes: families with aging parents share their struggles and solutions."

View the original article here

Judie Rappaport: Elder law attorney could answer questions of health subrogation

Dear Judie: I've been substitute for health (88) of my aunt for several years. I am available for emergency needs and visits for routine doctor as to cancel two trips over the last few years, so it could act in a responsible manner for their health care needs. I would like to take this opportunity to a two week trip that took me out of the country. There is no family member to assume. Can I temporarily transfer powers of alternate health to the Department of nursing at the Centre of assisted living where you live? There is a body which has that kind of service? Where can I find what can and cannot do?

: Jenny, Vero Beach

Dear Jenny: It is clear that you want to do the right thing for the aunt; She is lucky to have you as their health care. The answer depends on whether his powers of subrogation of health care are delegated and whether the Fund will accept a delegated agent to make decisions during his absence. If it isn't, we see two options:

Retain a lawyer of more law to revise the document to appoint a substitute in the event that it is not available and to find a professional to be the alternative.

Yourself by making available by phone or email may be the simplest solution.

OPEN POP MAIL I MEDICO

Dear Judie: I live in Detroit and I am visiting Pop, 84, for the first time since Christmas. It used to be a neatnik and now the House is filled with piles of paper and stacks of mail unopened. When I tried to pick up the mail and open it, Pop, said, "I have looked at them, are all unwanted." "I'm going to throw them when you leave." do you think that you will do?

-Pete, Stuart

Dear pete: No, he does not.
