By virtue of a single protease molecule able to degrade thousands of the plaque molecules, these proteases should be capable of delivering a catalytic benefit even if only small amounts pass through the cell barrier surrounding the brain.&lt;br /&gt;

&quot;Digestive enzymes are promiscuous,&quot; says Heinzelman. &quot;We can create catalytic proteases that attack the beta-amyloid plaque that cause neurons in the brain to die. Current therapies use amyloid-binding antibodies that are created by the body or injected to get rid of the plaque, but these antibodies used to attack the problematic Abeta molecules can only bind one time and clear one Abeta molecule, then they are done.&quot;&lt;/blockquote&gt;
The delivery system is problematic, too. Heinzelman suggests an approach that addresses both therapeutic efficacy and delivery.&lt;br /&gt;
&lt;ul&gt;
&lt;li&gt;He wants to re-engineer an existing technology to link proteases with &quot;ferrying&quot; antibodies that can encourage passage of the proteases from the circulation side across the brain cell barrier and into the brain tissue.&amp;nbsp;&lt;/li&gt;

This approach has been demonstrated in the laboratory.&lt;br /&gt;

In a report published in the June issue of the journal Critical Care Medicine, the investigators said that in more than one-third of the ICU days evaluated, mechanically ventilated patients with acute lung injury were given systemic corticosteroids at a range of doses during their ICU stay.&lt;br /&gt;

&lt;li&gt;For the study, researchers looked at the medical records of 330 acute lung injury patients with a collective 2,286 days in an ICU at four Baltimore hospitals between October 2004 and October 2007.&amp;nbsp;&lt;/li&gt;
&lt;li&gt;They found that patients were 52 percent more likely to newly develop delirium if they had been treated with steroids on the prior day.&amp;nbsp;&lt;/li&gt;
&lt;li&gt;Older age was also associated with an increased risk of developing delirium. Delirium can be marked by poor memory and thinking, sometimes with disorientation, hallucinations and agitation.&lt;/li&gt;

“We need to be cautious in our use of steroids in critically ill patients and weigh the risks and benefits of using them,” says study leader Dale M. Needham says there are a wide variety of reasons why an ICU patient would receive steroids. They may be ordered to respond to inflammation in the lung or elsewhere in the body.&lt;br /&gt;

Steroids also were used frequently to treat septic shock in the ICU, but more recent studies have shown fewer benefits from steroids in this condition.&lt;br /&gt;

Using both animal models and postmortem human retinas from donors with Alzheimer’s disease, researchers found changes in the retinal pigment epithelial layer, which harbors the supportive cells located in the back of the eye, and in the thickness of the choroidal layer that has blood vessels providing nutrients to the retina. Changes in these two regions were detected using sophisticated, state-of-the-art imaging and immunological techniques.&lt;br /&gt;
&lt;br /&gt;
With high-resolution, microscopic imaging and visual acuity measurements, investigators were able to monitor tissue degeneration in the cell layer and vascular layer at the back of the eye, as well as decline in visual function, that were strongly associated with Alzheimer&#39;s disease.&lt;br /&gt;

“Greater magnitude in these eye abnormalities may mean a greater chance of a patient having Alzheimer’s disease,” said Alexander Ljubimov, PhD, director of the Eye Program within the Regenerative Medicine Institute and co-author of the study. “We found that a rat model showed similar signs to the human ailment in the eye. If true in a larger number of humans, these findings may be used to study Alzheimer’s disease mechanisms and test potential drugs.”&lt;/blockquote&gt;
Though additional research is needed to investigate the mechanisms of these ocular changes in relation to changes in the brain, investigators hope to ultimately aid early diagnosis of Alzheimer’s disease by studying the most approachable part of the central nervous system: the eye. Cedars-Sinai has been at the cutting edge of studies on the eye and Alzheimer’s disease with a previous report showing amyloid plaques, a hallmark of Alzheimer’s disease, also build up in the eye using a similar animal model of the disease.&lt;br /&gt;

“It is fascinating that the eye may provide such a window to the brain and eventually predict diseases such as Alzheimer’s, although more human studies are now needed to confirm this animal work,” said Clive Svendsen, PhD, director of the Cedars-Sinai Regenerative Medicine Institute and a co-author on the study.&lt;br /&gt;

The book helped me to &lt;b&gt;prepare&lt;/b&gt; for the likely future and to develop strategies I would use when the time came.&lt;br /&gt;
