Although breast augmentation surgery leaves many patients enthusiastic about their new body, this initial enthusiasm may shift over time as patients’ perception of their body image changes. For those who have received breast augmentation surgery and are experiencing dissatisfaction, pain, or complications as a result of implants, Dr. Asaadi offers breast implant removal and replacement.

What is Breast Implant Removal Surgery?

Breast implants may be removed for various reasons. In certain cases, a silicone implant can break, causing silicone to leak. When scar tissue hardens and contracts around the implant, this is known as capsular contracture. Although a common complication of breast augmentation,

Dr. Asaadi believes capsular contracture can be avoided and is solely a symptom of poor surgical techniques. Having performed breast augmentations with a 100% success rate of no capsular contracture or implant leakage, Dr. Asaadi swears by the method of using saline implants and utilizing the inframammary incision to accomplish successful breast augmentation surgeries at his NJ practice.

Although a ruptured implant can be painful, it may also go unnoticed, in a phenomenon known as “silent ruptures”. To avoid “silent ruptures” and the development of capsular contracture, Dr. Asaadi only uses saline breast implants.

Saline-filled implants are made out of a sterile saltwater solution, in which a silicone shell is inserted into the breast and then filled to its desired volume with saline-fluid. Having the consistency of water, saline implants provide a more natural, safe, and symmetrical result. For this reason, some patients also opt to remove their silicone implants and replace their breast implants with saline implants.

Who Can Get Breast Implant Removal Surgery?

Breast implant removal surgery is commonly performed if the patient is no longer satisfied with their implants or wishes to exchange them. It is natural for tastes to change over time, and life events such as pregnancy can alter the appearance of the original implants. Although breast implants do not expire, patients often remove or exchange their implants every 10 to 20 years due to complications such as a ruptures or hardening of scars, transformations in their physical body leading to discomfort, or changes in personal preferences. Breast implant removal surgery is recommended if the patient is experiencing capsular contracture, pain caused by the implants, or if breast imaging has shown a leak in the implant.

What Will My Breasts Look Like After Breast Implant Removal Surgery?

Results from breast implant removal are highly individualized, dependent upon age, overall health, and implant size, but patients can expect natural-looking results. Patients with very large implants may require further surgery to address excess skin.

Capsular contracture is often thought to be one of the most common complications of breast augmentation. Capsular contraction occurs when scar tissue forms around the implant; it is usually induced by a low-skilled plastic surgeon or from certain surgical techniques. While many plastic surgeons believe capsular contracture is a risk that comes with breast implants post procedure, Dr. Asaadi does not believe this is true and that capsular contracture is completely avoidable when breast augmentation is performed correctly.

Capsular contracture does not have to do with implants, but how breast augmentation is performed. In fact, Dr. Asaadi has performed breast augmentations with a 100% success rate of no capsular contracture or implant leakage by using saline implants and the inframammary incision to accomplish successful breast augmentation surgeries at his NJ practice.

What Is Capsular Contracture?

Migraine headaches are extremely painful. Symptoms like sudden pain, intense nausea, and vomiting can last for hours during an attack. Not only do these headaches cause physical pain, but they also affect your daily life—disrupting your plans at any given moment.

Dr. Asaadi’s pressure test is designed to properly diagnose the cause of your migraine so an individualized and effective treatment plan can be applied.

During the pressure test, Dr. Asaadi will test four trigger points on your face: your two temples, the area above your eyebrow, and your nose. In order to do so, a saline solution will be injected into these pressure points. If you develop a headache after the injection, this determines that pressure to the surrounding nerves is causing your migraine.

After diagnosing the cause of your migraine, Dr. Asaadi can then treat the surrounding muscles to release pressure on that nerve and prevent future migraines from occurring. One important factor to consider when undergoing a facelift is the presence of malar mounds or festoons. Malar mounds and festoons are areas of puffiness above the cheeks and nose and below the eyelids. Though they are not extremely well-known conditions, they are very common in patients requesting facelifts and can create problems post-surgery if not properly addressed beforehand. The other possible cause is due to fat deposits around the eyes. One of our patients came to Dr. Asaadi after suffering from nasal cancer and malar bags. The procedure utilized a section of tissue between her eyebrows and hairline to reconstruct her nose. In combination with the forehead flap, the patient received a short-scar foreheadplasty to remove sagging and wrinkles from her forehead region. Your brows express a variety of emotions, including anger, depression, surprise, and tiredness. If these expression lines have become a permanent fixture on your face, you may be experiencing premature signs of aging caused by eyebrow descent. These lines may appear between the eyebrows, horizontally on the forehead, or at the bridge of the nose. Oftentimes, these lines are mistaken for excess skin. To treat plunging eyebrows, our plastic surgeon recommends a foreheadplasty.

A foreheadplasty, commonly referred to as a brow lift, elevates sagging brows into an ideal position, regaining your youthful appearance and removing predated expressions. Advances in the medical field have allowed plastic surgeons to perform this procedure endoscopically. This technique, also known as a short-scar foreheadplasty, allows for an invisible scar recovery. The procedure uses small incisions within the hair to reposition the brow. The new positioning is held in place with stitches, and after a few weeks the skin will naturally reattach itself to a lifted position. When Dr. Asaadi performs the procedure, he often cuts the corrugator muscle (located at the medial end of the eyebrow) to relax the apperance of frowning. An endoscopic procedure is ideal for correcting a very low brow and can also improve deep lines between the eyebrows and above the nose.

The differences between bicoronal (traditional) foreheadplasty and an endoscopic (short-scar) foreheadplasty:

These folds can disturb the natural contour of your face, and in severe cases, even impair your vision. Blepharoplasty, also known as cosmetic eyelid surgery, can correct these issues, giving you a younger, more alert, and refreshed appearance.

Lower Eyelid Blepharoplasty

Lower eyelid surgery is most often used to correct displaced fat pads and hollows beneath the eyes. This procedure can also correct malar bags, or severe bags known as festoons underneath your eyes. Fat grafting for breast augmentation holds both benefits and risks that women should be aware of before considering their options for breast surgery. In the past, fat grafting to the breasts was cautioned, as it made it harder for mammograms to read. With today’s advances in radiology however, physicians can pay more attention to suspicious changes in the breasts, even after breast augmentation surgery. So what should you know when considering fat grafting for breast augmentation? The American Society for Aesthetic Plastic Surgery shares the benefits and risks. Here’s what you should know:

Fat cells during grafting do not always survive

What this means: fat that is injected into the breast carries the risk of being absorbed by the body, becoming liquid, forming a cyst, calcifying, or producing scarring.

Multiple sessions for desired results

What this means: Because a vast amount of fat cells may not survive during fat grafting to the breast, multiple sessions may be needed to help women achieve a bigger breast size. Additionally, there might be a limit to the breast size obtained and it can take up to 6 months for your result to take shape.

Further testing may be needed

What this means: The American Society for Aesthetic Plastic Surgery stresses that just “as with any other surgical procedure, it is possible for fat grafting to produce changes in breast that may be deemed suspicious on examination by a physician or on mammography, and may lead to further testing to determine if the findings are related to breast cancer.” While testing for breast cancer continues to become more advanced, testing beyond your traditional mammography may be necessary if there are any suspicious signs of breast cancer.

Fat Grafting is not only used to increase breast size

What this means: While fat grafting is commonly used for breast enhancement, harvesting fat cells to the breasts can also be used to improve implants already in place, adding volume to rippling areas or softening the appearance for a more natural look. The procedure may also be used during traditional breast augmentation too enhance the shape of the breasts.

“Last week I saw a patient in the office who was interested to have breast enlargement but she was afraid of having breast implants. She read somewhere that implants grow mold & cause arthritis & so on & so forth, and this is not true,” says Dr. Asaadi. While many sites can offer information about popular surgical procedures, such as breast implants, it is important to understand where your information is coming from.

So how do you know how safe your implant will be or how long it will last? For starters, meet with a board certified plastic surgeon who is experienced and understands your individual anatomy. A surgeon who is upfront and hones the proper education and experience will play a key factor in the longevity of your implant. He/she should understand your body type and help you determine what implant is right for you based on your desires, health, and most importantly, your shape and size. Depending on your individual anatomy and which method of implant augmentation you are undergoing will help determine the size of your implant. During implant augmentation, breast size can be increased 1 or more sizes, but during fat grafting breast augmentation, breast size can be increased usually only by 1 size. Just like a shoe size however, your implant should fit the size of your body.

When undergoing breast reconstruction, women who do not have enough skin to support an implant may require skin expansion. During skin expansion, a balloon expander is inserted beneath the skin and chest muscle. A salt-water solution is then injected through a tiny valve through the course of several weeks to months, allowing the skin to stretch and creating sufficient room for a permanent breast implant.

In some cases, your plastic surgeon may harvest fat from other areas of the body, improving contours with a more natural look. While this method can cause more noticeable scarring, research shows this method to regenerate stem cells of the breast that may have been lost from radiation.

Questions to consider when visiting your plastic surgeon for breast augmentation surgery include:

What is the size implant suitable for me?

The size of the implant patients receive should be fit the patient’s chest, base of the breast, thickness of the breast, and other anatomy of the patient. Just like a shoe size, patients should choose the size implant that fits the size of their chest. If you have a larger chest for example, you may opt for a larger implant. Patients with a smaller, tighter chest however, should not choose an overly large implant to ensure safety and to avoid complications with the implant following breast augmentation.

What type of implant fits my individual anatomy?

The most famous questions patients ask before their breast augmentation remains, should I use silicone or saline implants? Both silicone and saline implants have advantages and disadvantages, however, Dr. Asaadi prefers to use the saline implants due to their natural feel, look, and shape.

Your plastic surgeon can insert your implant through four different areas. These areas include the armpit, around the areola, the stomach, and under the fold of the breast. The culprit of these unwanted pains and symptoms is a migraine, and for those who experience its life halting effects, finding immediate relief is often insurmountable. When this type of extreme head pain strikes, sufferers know that the rest of the day must be spent in bed without any light or sound. If you’re one of the 25-35 million Americans affected by migraines each year, then you know the struggle of finding a cure. Sometimes choosing between these options and deciding which treatment is right for you can be difficult, but Dr. Asaadi can discuss the options that best fit your individual needs and expectations. 4 Components of Successful Rhinoplasty in NYC

If you’re considering rhinoplasty, or cosmetic nose surgery, you’re most likely concerned with the current appearance of your nose. What many people fail to realize is that changing the form of your nose can also affect the way it functions. While the promise of a cosmetic improvement to the way your nose looks is perhaps the most motivating component in the decision to have rhinoplasty performed, it can not come at the expense of the proper function of your nose. Leading New York plastic surgeon Dr. Mokhtar Asaadi has developed a checklist of four essential components to preserving the function of your nose during a cosmetic rhinoplasty procedure.

1. Deviated Septum

A deviated septum is the term applied to a condition that occurs when the nasal septum (the thin wall between the nostrils) is bent to either side. Ideally, the nasal septum is positioned in the center of the nose. When performing rhinoplasty in NYC, it is important to make sure that the nasal passage is not altered, or deviated, which can restrict airflow. Changing the shape of the nose during rhinoplasty can affect the position of the nasal septum. Dr. Asaadi is very careful to ensure that the septum is in the correct position following rhinoplasty. If the patient has a deviated septum before the procedure, the condition is repaired.

2. Enlargement of Turbinates

Nasal turbinates are small projections of bone inside the breathing passage on each side of your nose. Each side has an inferior, middle, and superior turbinate. Turbinates serve a very important function in the nose because they help to clean and filter the air you breathe before it enters the lungs. They also help you feel the air flow through your nasal passages. Like a deviated septum, enlarged nasal turbinates can cause nasal obstruction. Changes to the shape of the nose can alter the nasal turbinates. If a person has a curved nasal septum, the inferior nasal turbinate will become enlarged to compensate for that concavity. This is generally the result of a nasal fracture but can also result from changing the structure of the nose during rhinoplasty surgery.

3. Internal Nasal Valve

The internal nasal valve is the area within the nasal airway that is responsible for controlling the flow of air. The collapse of the internal valve, which can occur naturally, as a result of aging, following an injury, or after altering the shape of the nose, can cause nasal obstruction and difficulty breathing through the nose. Problems with the internal nasal valve often arise when rhinoplasty is being performed to correct a large nasal bump. One of the most effective ways of repairing a collapsed internal nasal valve is to place small grafts of cartilage within the nose to support the space and ensure that the internal valve does not close.

4. External Nasal Valve

The external nasal valve is situated on the sidewalls of your nostrils. Like the internal nasal valve, obstruction of the external nasal valve can lead to difficulty breathing through the nose. In addition to aging and trauma, external valve collapse can occur following rhinoplasty surgery where too much cartilage has been removed from the nasal tip. When performing any septal surgery, Dr. Asaadi saves the cartilage that is removed until he is sure that no problems will occur that will limit the function of the external nasal valve. If the reshaping of the nasal tip does lead to complications with the external nasal valve, Dr. Asaadi can add cartilage under the skin to support the nasal structure.
