According to Ayurvedic physiology, Agni - the elemental principle of fire, heat, and light- has a crucial role in vision. This study explores the structural and functional association between Tej Mahabhoota, and ocular physiology and pathologies, as described by classical Ayurvedic texts. The Agni Mahabhoota is predominant component in vascular structures of eye, supporting visual function through Alochaka Pitta. The Drishti- functional unit of vision- is enriched by Avyaya Tej, reflecting the integral role of agni in visual perception. Tejojalashrit layer among the ocular Patalas underscores the structural dependence of the eye on Tej. Disturbances in Agni balance, particularly due to increased vitiated Kapha & kleda (associated with Jala Mahabhoota) which further affects the Rakta contributes to ocular pathology. This manifests into increased srava in urdhwajatrugat region, perticularly in eyes leading to Abhishyanda. When this pathology exagerates, generates Adhimantha and other eye diseases. Therapeutic measures like Anjana Karma and Langhana restore Agni equilibrium and prevent disease progression. The findings emphasize that maintaining the functional integrity of Agni is fundamental to preserving ocular health and preventing degenerative visual disorders within Ayurvedic framework.

Keywords: Agni, Ayurved, ocular physiology, ocular pathology, Tej, Alochak pitta, Drushti, Tejojalashrit pata

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

The methodology involves diverse techniques (Svedana types) and consideration of benefits (Svedana *ke guṇa), potential complications, and specific contraindications (upayogiyo), as discussed primarily in the fourteenth chapter of the classical text. The application of Swedana is aimed at balancing the bodily humors (doṣas) and treating conditions arising from their vitiation, such as pain or urinary issues.

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

Within Ayurveda, a system called Arishta Lakshana offers detailed warning signals - bodily changes, behaviors, or sensations - that suggest disease cannot be cured or death draws close. Ancient writings including the Charaka Samhita, Sushruta Samhita, Bhela Samhita, and Ashtanga Samgraha record these signs using inputs from sight, sound, smell, touch, and mental state. Today’s hospice approaches also depend on clues like irregular breathing, skin patterns, confusion at journey's end, and failing organs to detect when someone is dying. Several contemporary prognostic indicators demonstrate notable conceptual parallels with traditional Ayurvedic descriptions. Close study shows many similarities between old Ayurvedic markers and today’s clinical findings during final stages of illness. While examining this overlap, the discussion touches upon moral questions tied to predicting outcomes, how traditional knowledge could fit within supportive care models, and whether evaluating the five senses - Panchendriya Pariksha - holds value now.

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

The formulation consists of Yashtimadhu, Triphala (Amalaki, Haritaki, Vibhitaki), and Lauha Bhasma, administered with Madhu and Ghrita as adjuvants. These ingredients collectively exhibit Tridoshahara, Rasayana, and Chakshushya properties, contributing to ocular nourishment, enhancement of visual function, and correction of underlying Dosha and Dhatu imbalance. A comprehensive review of classical texts and standard Ayurvedic references reveals that the synergistic pharmacodynamic actions of the formulation support its effectiveness in ocular disorders and associated systemic conditions. The fine processing of Lauha Bhasma enhances bioavailability, while Madhu and Ghrita act as Yogavahi, facilitating deeper tissue penetration and improved therapeutic efficacy. Although Saptamrita Lauha has been extensively used in clinical practice since ancient times, scientific validation through standardization, pharmacological evaluation, and well-designed clinical studies is necessary to establish its safety and efficacy in contemporary Ayurvedic and integrative ophthalmic care.

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

It is consistent with diabetes mellitus, a metabolic disease characterized by hyperglycemia brought on by either insufficient or ineffective insulin, resulting in symptoms such as polyuria, polyphagia, and polydipsia. The prevalence of this chronic illness is quickly rising globally and affects almost every bodily system. A 2023 study by the Indian Council of Medical Research found that 101 million people in India have diabetes expected to increase to 783 million by 2045 and 643 million worldwide by 2030. In order to improve therapy efficacy and recognize therapeutic limitations, a thorough understanding of Madhumeha's pathophysiology from both modern and Ayurvedic viewpoints is necessary. Type 1 and Type 2 diabetes are the two main types; Type 1 is caused by beta cell loss, while Type 2 is caused by insulin resistance, with age playing a major role. Prameha's crucial importance in traditional medicine is highlighted by its inclusion among Ayurvedic "Ashtho mahagadas."

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

The methodology of Shaman is rooted in the principle of employing substances (dravya) possessing qualities (guṇa) antagonistic to the vitiated doṣa, thereby pacifying the disease state (rog śānti or śānti). Shaman substances are one of the three broad classifications of medicinal dravya. This article reviews the systematic classification of Dravya as presented in classical texts and its pivotal role in the science of Dravyaguna Vijnana. By categorizing substances based on their origin, composition, and pharmacological action, Ayurveda provides a structured framework for the judicial use of medicines (Aushadha) and diet (Ahara),. The review highlights how these classifications enable a physician to achieve precise clinical outcomes by understanding the inherent properties (Guna) and actions (Karma) of a substance.

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

Bacterial, fungal, or viral infections can cause pharyngitis, or inflammation of the throat; viral infections frequently go away on their own. Throat cultures and quick antigen tests are used to accurately diagnose bacterial infections, which require antimicrobial therapy. Since needless prescriptions do not speed recovery or reduce the risk of reinfection, the article highlights the significance of having a complete medical history and using antibiotics responsibly. This summary covers new pathogens of concern in clinical practice and lays the foundation for later sections that concentrate on diagnostic tests for acute pharyngitis.

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">

It has been mentioned in various Ayurvedic classical texts like Sushrutha Samhita, Chakradatta etc. Ksaharsutra as a standard treatment method has been extended to various types of sinus or fistulas with encouraging results. This case has been taken up evaluate the role of Ksharsutra as a minimal invasive method in management of recurrent anterior perineal sinus.

Clinical Findings- A 28 yr old male with history of sinus in perineum who has already been operated twice for same disease with an active pus discharge, discomfort and itching.

Diagnosis/Methodology/Therapeutic Intervention-The case had already been diagnosed for perineal sinus by MRI sonogram.Under local anesthesia with Lignocaine 2% was given in perineum. Copper malleable probes were introduced through two different external openings. The front end of the probe was pulled through an artificially made openings using an electric cautery. Two Ksharasutras were placed in 2 tracks with the help of these probes. Both Ksharsutra were knotted and changed weekly once.

Results/ Outcome- Cut through for smaller tack was achieved in 4 weeks and longer sinus track took 8 weeks to cut through. No recurrence was observed during 6 months and 1 year follow ups.

Conclusion/Takeaway- Ksharsutra as a minimally invasive procedure is very effective and convenient method for recurrent perineal fistulas and sinuses which gives tremendous and lasting results without interfering day to day work schedule of the patient.

<dc:rights xmlns:dc="http://purl.org/dc/elements/1.1/">
