Oh my gosh! Another Filipina made us proud again. And this time she made it to the TOP. She is Danica Magpantay, 17 years old, a UP Fine Arts student, won the 2010 Ford Supermodel of the World competition, held in New York City yesterday January 22, 2011. She beat 70 other contestants from all over the globe including Lithuania, Poland, Brazil and Estonia who ended as runner ups. She is also the first ever FIlipina to won the title. Aside from the title Danica Magpantay also won $250,000 worth of modeling contract with Ford Models. Other Filipinas who made names on Ford Supermodel of the World Search are Charlene Almarvez (1st runner up, 2010 edition), Charo Ronquillo (3rd runner up, 2006 edition), and Melanie Marquez 1st runner , 1986) edition. Looks like there’s no stopping Filipina Supermodels from conquering the world. We're truly proud of you Danica! Keep up the good work and Mabuhay Philippines!

I made another video compilation with our pictures. This time with my all my college friends including my classmates. Last saturday, january 8 2010. my college classmates and I had our post christmas beach party at omandac beach resort. The party was supposed to be celebrated before 2010 Christmas Day but because of their very very busy schedule(as if). It was moved many times until we finally decided to have it January 8. Well not of us was able to attend. But still & honestly it was so fun. We played games, laugh, dance & pose! Below are some captured memorable moments.

Now I know, Life is really full of surprises. When I was college. I never thought my life would be like this, bored, tired, and unemployed. As the days passed I realized I am not getting younger anymore. at my age I should be working right now, just like my classmates but where am I? Here ,rotting. I studied for 4 years, strived hard to pass all all the subjects so i won't waste anytime taking it again for another semester, I burned myself out during the review just to pass the Licensure Examination so I won't retake it again, for me just to save time and I am so glad I made it. But with all those efforts, its still kinda useless. I may now have the letters R and N, but whats the use of this one when I was just at home, decaying. Whats the use of the thing I learned from nursing school?

Being a member of the "Unemployed Registered Nurses in the Philippines, not by choice but by chance" was not part of my plan and its so difficult . I really hate this because I wasn't prepared for this thing to happen. I already planned my life up to the last detail. Like at 21, I should be working and will be stable at age 25 , blah. blah.blah and stuff like that. I tried applying for three hospitals already and none of them replied to my application and its very very depressing. I think only miracle can help me now and some mercy from the hospitals I tried to applied for :( .That's the sad reality, I have to face it. Well for now all I can do is to wait because life must go on. Keep calm and carry on!

My training period at TDH as a Volunteer Nurse is almost over, It felt like its a relief because at last I am not waking up so early anymore everyday, just not to be late during endorsement and no more patients to worry about, but on the other side I am sad and worried because I don't know where to go after this and the friends that I gained, ughh. I will be missing them. :(. And the routines that I developed in my short stay, like the haggard look after the duty, the smiles during rounds, the laughter and above all the lunch together with the friendly staffs. Having my training there, is one of the great things that happened to me, since there where I first practice me chosen career. This will gonna be remembered forever. It was such a nice experience, working alone without a CI and relying only on your knowledge, i mean the one that you developed during your SN years and of course with some help of the senior nurses, in providing safe and quality nursing care. such an adventure. itchus! I am so thankful to the people who helped me to get there and have my training. And if I may not be one of the few lucky trainees to be absorbed, well I will just apply again, that's life! dawat-dawat ti. But the important thing is I now developed some independence and I was able to win some friends even in just a short period of time. wink!

Oh! It’s quite a long time since the last time I visited my blog and Yes! You’ve heard it right, I am really back, and I am so back. After many months of contemplating whether to continue or cancel this account. I already made up my mind. Well, that is to continue! Yehey! I really missed writing stuff so much, and I miss you! Dear friends, echus! It’s already June 2010 and almost a year since my last post and its back to school time. Good luck student, I hope students you will enjoy your school days just like mine. Chos!

As of now I am not kinda busy, you know! I was just staying home doing nothing and most of my time is spent on tv and computers, surfing the net and facebook. I am also into playing my all time favorite game, The Sims 3. It was so fun, so cute and so lovely. The part that I love most of it, is that because of its lovely beautiful suburban neighborhood, it was just like in the movies The Stepford Wives and now its seamless, very human like almost perfect! I think that’s one of the good things of being unemployed though you don’t have much money staying at home well at least you have lot of time playing your favorite computer games. Hehe (^_-)v.

And speaking of unemployment, I find it very difficult, I spent my days just staying home and got very boring. Looking for a job is such a tut, very tut. Oh, by the way, I forgot to share what happened to me in the last 12 months, but I think that will be on my future posts. Keep posted. Wink~

Whoa..It seems it's just only yesterday when my blog is just starting. I have very few entries then which also provides very few visitors [tsk..tsk..poor blog]. And now I really can't believe that I am celebrating its 1000 visits. I've been blogging for almost a year now, but I'm not been so active to it due to my busy schedule, but because of my 2009 new years resolution #3 which states that "be more active in blogging", slowly I am gaining more interest to it. Well as you can see, I am trying to write an entry at least once or twice a week, and not bad I was able to make 12 blog entries fro the month of April..wee..It's a sign that I should really exert more effort in my blogging career (char!, career jud?). I am also inspired by the visitors who came from different parts of the world who reads my entry. Wow, of course that is something nice, I didn't expect this much that my blog would go this far, considering that I am just only a rookie in world of blogging that's why I am very thankful to everyone who supported me, the visitors, the followers, the friends, the advertisers, my sponsors, who accepted my applications, thanks you so much for the trust, I promise that I will try all my very best to entertain everyone.

Last week I had a 3 day absent with my online ka-chorvahan it's just because I've been a bit busy with, again, my completion duty. And guess what, I am still lacking two OR cases..grr..I had my duty at D.O. Plaza Memorial Hospital at Brgy. Patin-ay, Prosperiad, Agusan del Sur. It was a three hour roller coaster ride (due to countless zigzag road) from our town which I really had a super to the max ever motion sickness going there. hahay!..We left Tagum at exactly 8 pm, riding a non-airconditioned, super ordinary bus which causes me so much stress, and thank God I only vomited once [and thanks to NCCC cellophane too! hehe]. We arrive in Agusan at around 11:30 pm, we stayed in Aries Dorm. The place was nice and peaceful. And the people of Agusan were nice ang friendly. I love the moment of waking up every morning seeing the simplicity of the whole Agusan, and enjoying the beauty of nature. [sigh] I really enjoyed my stay there, except for that roller coaster road you will encounter..hehe..can't wait to go back!..wee!

pacquiao won against hatton

Manny Pacquiao did another great fight in the "Battle of the East and West". On the first round of their fight, Hatton already drop but the fight still continues. Manny defeated Ricky Hatton in the second round with his left hook punch, making the Hitman in Total Knock Out. Even the fight is stopped only at 2:59, it was still a total entertainment. Of course, Filipinos from all over Philippines were very happy with Pacman's victory. Everyone from rich to poor celebrated his win against Hatton. Even Filipinos abroad didn't miss the chance to celebrate Pacmans's victory. It was one of the greatest fight of the year. I can't wait to watch again the fight of Manny Pacquiao with another great boxing champion Mayweather, wee..Go Manny go!., good luck for you're next fight!.

Best and worst nursing schools bared
June 9, 2005

Rep. Joseph Santiago has released a list of the 20 best performing nursing schools in the country, with 80 to 99 percent of their graduates passing the annual licensure examinations from 1999 to 2003, and of the 20 worst performing schools, with only nine to 25 percent of their graduates making the grade over the same period.

"We are making this list available to the public so that parents may be guided accordingly. They can avoid sending their children to deficient schools whose graduates have at best 25 percent chance of passing the nursing licensure tests," Santiago said.

He said the list was culled from the records of the Professional Regulation Commission and the Commission on Higher Education (CHED). The list is limited to nursing schools with over 100 examinees.

Santiago had earlier prodded CHED to grade all schools nationwide based on the performance of their graduates in professional licensure and eligibility tests, and to put out the ratings in the newspapers for all to see.

THE BEST AND THE WORST

Based on the list released by Santiago, the 20 best-performing nursing schools and the percent of their graduates passing the licensure tests were: The University of the Philippines-Manila, 99 percent; St. Paul College Iloilo and Silliman University, 98 percent; University of Santo Tomas, 97 percent; West Visayas State University, 96 percent; St. Louis University-Baguio and Pamantasan ng Lungsod ng Manila, 95 percent; Southern Luzon Polytechnic College, 91 percent; Trinity College of QC and Cebu Doctors College, 89 percent; Mountain View College, 88 percent; Remedios Romualdez Memorial School-Manila and UE-Ramon Magsaysay Memorial Medical Center, 87 percent; St. Paul University-Tuguegarao, 84 percent; Velez College-Cebu, San Pedro College-Davao and Cebu State College, 82 percent; De La Salle University-Dasmari?as and University of La Salette-Santiago, 81 percent; and Romualdez Medical Foundation, 80 percent.

Listed as the 20 worst performing schools were Mapandi Memorial College, nine percent; M.V. Gallego Foundation Colleges, 15 percent; Galang Medical Center, 17 percent; Unciano Colleges, 18 percent; Good Samaritan Colleges, 19 percent; Lipa City Colleges, Immaculate Conception College-Albay, Mabini Colleges and St. Gabriel College-Kalibo, 20 percent; Notre Dame Jolo College and Philippine College of Health and Sciences, 21 percent; Golden Gate Colleges, Northern Christian College and Tecarro College Foundation, 22 percent; Dr. P. Ocampo Colleges, De Ocampo Colleges and World Citi Colleges-QC, 23 percent; San Pablo Colleges and Medina College, 24 percent.

Santiago said a total of 55,744 nursing school graduates took licensure tests from 1999-2003. Only 27,345 of them, or 49 percent, passed the tests.

MEDIUM-SIZED SCHOOLS RATING

Santiago has also bared a new list of some of the country’s highest and lowest rated nursing schools, this time focusing on medium-sized institutions.

The new list enumerated the 20 highest rated medium-sized nursing schools, with 61 to 98 percent of their graduates passing the annual licensure examinations from 1999 to 2003, as well as 20 lowest rated schools, with zero to just 31 percent of their graduates making the grade over the same period under review.

The fresh list, based on records of the PRC and the CHED, covers nursing schools with less than 100 examinees. The previous list covered nursing schools with over 100 examinees.

The 20 highest rated medium-sized nursing schools and the percent of their graduates passing the licensure tests are: Mindanao State University-Marawi City, 98 percent; Mary Johnston College-NCR, 96 percent; St. Paul College-Dumaguete City and Philippine Christian University-Manila, 95 percent; Xavier University Cagayan de Oro City, 94 percent; St. Dominic Savio College-Central Luzon, 93 percent; University of the Philippines-Leyte, 92 percent; St. Mary’s University-Bayombong, 89 percent; St. Dominic Savio College-Manila and Dr. Gloria Lacson Colleges-Central Luzon, 88 percent; Sacred Heart College-Lucena City City, 83 percent; St. Mary’s College-Tagum, 74 percent; University of St. La Salle-Western Visayas, 71 percent; Philippine Women’s University-Manila, 69 percent; Concordia College-Manila, 68 percent; Mindanao Sanitarium and Hospital College of Medical Arts-Soccksargen, 67 percent; Holy Trinity College-Puerto Princesa City, 64 percent; Baliuag Colleges, 63 percent; Philippine Women’s University-Quezon City, 62 percent; and Surigao Education Center, 61 percent.

The 20 lowest rated medium-sized nursing schools are: Isabelo Calingasan Memorial Institute Southern Tagalog, 0 percent; Perpetual Help Paramedical School-Bicol, five percent; Lyceum of Northern Luzon-Ilocos, seven percent; Jamiatu Marawi Al-Islamia Foundation-Soccsksargen, eight percent; Immaculate Conception College-Manila, nine percent; Urdaneta College of Technology and Perpetual Help College-Pangasinan, 17 percent; Our Lady of Lourdes College Foundation-Bicol, 19 percent; Pamantasan ng Araullo-Central Luzon, 20 percent; Medina College-Pagadian City, 21 percent; Carthel Science Educational Foundation-Central Luzon, 24 percent; St. Luke’s College Foundation-Central Luzon, 25 percent; Pangasinan College of Science and Technology, 26 percent; Southern Bicol Colleges, 27 percent; University of Batangas, 28 percent; Laguna Northwestern College, 29 percent; Bacarra Medical Center-Ilocos and Abra Valley College, 30 percent; and J.P. June is fast approaching and again it's back to school. Maybe many of those who graduated from high school is already deciding what to take up for their college degree. Well, even me after graduating high school also had difficulty choosing one. When suddenly it strucked in my head to take up nursing which I never regreted and still loving it until now..wee luv to be a nurse..i mean student nurse. I find nursing as one of those cool prefessions worth to be in, what makes it cool is that, it's not only all about paper works and computations, though it is still part of nursing life (e.g. Case Presentaiotn & IVF Computation), but it focuses more about on caring for people, taking care of the sick and everything that is helping. I have found a very cool nursing recruitment video at Youtube from Johnson and Johsnson health channel entitled "Dare to Care". Watch it for you to atleast get an idea what nursing really is and what it is all about. I really love the ad and it was very enlighthing, very cute, inspiring, and so lovely. The moment I saw it, I feel very proud to be one of them, even I'm just only a student..at least!..duh..Though I admit that nursing is very exhausting and challenging (in other word cancer-causing..joke!) but it's all worth taking it. Watch the video above and we don't know, maybe in the near future, you'll be the next to proudly say "I'm a Nurse!".

Beverley Allitt: The Angel of Death

Nurses are well known to be the "angels in the sick room" but not for Beverley Allitt tagged as the Angel of Death, known to be one of Britain's most notorious serial killer in ealry 1990s, for killing 4 children and injuring 5 others. Due to scarcity of nurses during her time and despite of her poor academic performance she had been granted a temporary six-month position at the understaffed hospital. She worked at the Children's Ward 4 at England's Grantham and Kesteven Hospital in Lincolnshire as an State Enrolled Nurse (SEN) in 1991. Acting as a caring nurse as she attempted to murder her young victims by injecting and overdosing potassium and insulin to the patients in order to have arrest. Among the victims who died due to Allitt's violence includes Liam Taylor, Timothy Hardwick, Becky Phillips and Claire Peck. During the examination of the body of one of the victim, a discovery of Lignocaine found in its system, a drug used as a local anaesthetic and for patients with cardiac problems (eg. post MI, digitalis toxicity, post cardiac arrest). After a series of hearings, Allitt was charged with four counts of murder, 11 counts of attempted murder, and 11 counts of causing grievous bodily harm. Later on, Allit was found out to exhibited symptoms of both Munchausen’s syndrome, a mental disorder in which a person feign disease, illness, or psychological trauma in order to draw attention or sympathy to themselves and Munchausen’s syndrome by Proxy, which injury is deliberately and gradually inflicted upon another person usually for gaining attention or some other benefit. Both disorders are characterized by gaining attention through illness. In 2006, Allitt launched an appeal on the length of her sentence, this is a review of her sentence which led the Probation Service to contact victims' families about the process but the review remains pending.

Politicians are so busy having their ads on tv now, seems like an early campaign, and it calls for...what else? an early voter's registration (early daw?, late na gani), why not!?. My whole week has been so busy, completion here, registration there. I took a lot of time just to have my voter's registration done, since I am plannig to use the ID for opening new bank account or to be used as PRC requirement for the incoming NLE. Registering takes only a short time, but it takes days for me to get one. What really took me so long it's because of that Purok Clearance thing, I really had a hard time getting one because everytime I go to the Purok Chairman she's always not present (though i am not blaming her, of course she has also her life to manage), so I always wait, first attemp, she was on shopping for groceries, then second she was on the dentist for tooth repair and on third at last she was there and I was able to get one, what a journey huh?!. And just this afternoon I finished registering, I went to COMELEC fill-up the form, pose in front of the camera (pose jud as in), signed and all, then end, that's it!. Though I don't still have the Voter's ID right now because it is still on process but at least I have the stub and I can claim it anytime.

So, when registering all you have to do is go to the COMELEC (Commission on Election) office just bring black ballpen, purok clearance or if none because you're living in an exclusive subdivsion or somewhere and there's no purok there, just present 2 valid ID's, bring also photocopy of your birth certificate or marraige contract, and YOURSELF. So this coming May 2010 election i'm ready to vote na. wee. Register now 'cause your vote counts!

I really can't wait for the BIG day to come, my graduation of course!. Well, another chapter of my oh so beautiful life again will close and will open. This May 17, 2009. mark your calendar (char!..i mean not your calendar, mark my phone calendar! lang pala). Where I and the rest of my batchmates and friends together with our parents will experience one of the greatest day of our lives ever.. woohoo.., our [drumroll...janjararaaaaan] College Graduation!. Well, of course earning a degree is not that easy, I mean you worked hard for it and so I deserved it, this is something that I can boast, and be proud of, the burning of my eyelashes (nagsusunog ng kilay) everynight just for me to pass the quizzes and exam, is a great effort that I made just to finish my study. That's why when I heared that I am a candidate for our graduation of course I feel so excited to receieve my diploma, to wear the graduaiton cap, my toga, sing our graduation song, and everything, wee..and that day would be the very best day of my life. It will be just like a dream come true, our graduaiton was supposidly last March 22, together with the other courses but was moved because of our OR/DR completion. As of now, we are so busy with our completion but still manages to attend for the graduation practices. And I hope and praying so much that I will pass the NLE this November 2009 and I think that would be the greatest graduation gift I will receieve.

Happy Easter Sunday everyone, what did you do today?..Did you attend the mass? Me? no.. because as usual we had our completion duty for our DR cases today, I was able to cater two cases, one is assist and the other is handle. I am sad because my patient, had an immediate delivery, she arrived at the DR from ER fully dilated and considering she was just 5 months pregnant, the baby was so tiny and so little, he (since it is a boy) was so thin, huhuhu. But on the other side I am very glad because I am almost done with my OR and DR cases, and it was such a nice feeling to almost finish the cases because everytime I had completion it was just like pasakit, because we have to pay for our completion fee, I am hoping that I will finish this by the end of April because I want to start reviewing first for the board exam after our Graduation. Wish me luck guys!. Please pray for me..please!

This song was entitled "I Promise" sang by Jaci Velasquez, and was also written by her and Johny Ramirez, the track (#8) was from her first album "Heavenly Place", that was released in 1996. The album ranked #142 in US music charts and sold 10,800 Units nationwide. FYI, she was just sixteen then.

i got supended..

So sad, my person.com account was suspended . Huhuhu, I cannot access anymore my contacts. It's just because I made an advertisement of my blogger blogsite, very nonsense. I send each one of my contacts with my blogger URL, then voila!..instant suspension, very sad. I've been a person.com member for almost a year, but been inactive since no one wants to chat with me..grr... And just recently I opened my account again to check for any offline messages, but I found nothing, see arisgada lagi..that's why i decided to give away my blogsite address. And the next day I found out, I got suspended and my account was blocked (is that spamming?) and cannot access anymore my contacts, so pathetic. I've already watched Xmen Origins: Wolverine leaked version

XMEN ORIGINS: Wolverine movie will be shown in theaters this May 1 starring Hugh Jackman as Wolverine/Logan. This Upcoming movie was stolen and hacked last month and the copy was spread over the internet, FBI and MPAA were now investigating to prosecute the source of the leaked movie. Last night I was able to watch the leaked version, bought from pirated dvd vendor around town. The movie seems nice but it is very obvious that it is still unfinished because some of the special effects are still missing, some of the scenes were still unedited, ropes that were used for flying stunts are still visible. The story revolves around the violent past of the mutant comic book superhero with retractable claws Wolverine and his quest for revenge against a villain Sabretooth portrayed by Liev Schreiber whom he blames for the death of his girlfriend Kayla Silver Fox played by Lynn Collins and eventually became the weapon X project. Here are some the details from the unfinished movie I watched, the story started with a young wolverine, who kills his father, then run away with his brother Victor (Sabertooth), the two became soldiers and fought in wars, captured by Major William Stryker, became his allies and sent to a special mission in Africa together with other mutants to locate the source of the Adamantium, after that is up for you to know. Of course I don't want to suspend my account or be sued because of copyright infringement or something, duh!. You should really watch the original movie showing this May 1st in theathers near you.

Boys Before Flowers ver. Korea

First the Taiwan, then the Japan, and now the Korea, where's next, Philippines? Fine, fine, fine, well I know it is already too late to post this one but I always believe in saying that better late than never. This post is all bout my favorite TV show of all time, Hana Yori Dango (HYD) & Meteor Garden (MG) which is now has its own Korean version the...tideng!!.. "Boys Before Flowers". I was able two watch the first two episodes in mysoju.com but then, gets tired after awhile of watching sincemost of the scenes were already familiar, same as in the previous versions, as well as the story line except for some changes and a little twist. The main character's name here is Jan Di, which is San Cai in MG and Makino Tsukushi in HYD, came from an average family who owns a dry cleaning shop which is located near the Shin Hwa College, the school for rich and well-known. Which eventually became her school after saving a boy from jumping off the roof. She is admitted into the school on a swimming scholarship where she meets the four richest and most spoiled boys in school known as the F4. Jan Di tries to avoid confrontation with the F4 at all cost because she knows that these boys can cause trouble to those who speak out against them and will get a red notice and is bullied and driven out of school. However, when Jan Di's friend, Oh Min Ji, accidentally gets ice cream on the leader of the F4's shoes, in defense of her friend she's forced to declare war on the leader of the F4, Goo Joon Pyo. Her resolve gets the attention of her crush Yoon Ji Hoo and oddly enough, rouses romantic feelings against her worst enemy, Goo Joon Pyo. The series is based on the Japanese shoujo manga "Hana Yori Dango" (Boys Before Flowers) by Kamio Yoko. Plus the F4's here were cute. WeE.. I am very excited to watch this version on Philippine TV, I heared also some chismis that ABS-CBN will be making a Filipino version of this story, it seems Sarah g. or Kim c. or ann c. will take the main role, but we'll see. Still I am looking forward on watching this on Philippine TV. xx

At last we are through with our headnursing concept, since it was a whole sem exposure we were rotated into different roles such as being a headnurse, table nurse, medicating nruse and so on. So below are some of the roles and fuctions of the nurse in the ward.

ROLES AND FUNCTION OF A CHIEF NURSE, HEAD NURSE, AND STAFF NURSE

CHIEF NURSE

Ø Organizes and administers the department of nursing.

Establishes objectives for the department of nursing and the organizational structure for achieving these of objectives. Interprets and puts into effect administrative policies established by the governing authority. Assists in preparing and administering budget for the department. Selects and recommends appointment of nursing staff.

Directs and delegates management of professional and ancillary nursing personnel. Plans and conducts conferences and discussions with administrative and professional nursing staff to encourage participation in formulating departmental policies and procedures, promote initiative, solve problems, and interpret new policies and procedures. Coordinates activities of various nursing units, promoting and maintaining harmonious relationships among nursing personnel and with medical staff. Analyzes and evaluates nursing related services rendered to improve quality of patient care and plan better utilization of staff time and activities. Participates in community educational health programs.

The Chief Nurse must be a nurse who:

· Understands both patient care and nurses; has demonstrated stature in the nursing profession; has had some influence in the nursing field; has kept abreast of changes in the profession.

· Plans for the department; establishes goals and directions for the department and helps the hospital establish its own goals; ensures that nursing has proper influence on the total institutional goals and that nursing goals and directions complement and support the total.

· Has integrity and can be relied on to speak frankly and not waste time playing games; is respected for his or her opinions.

· Is a leader who can measure, evaluate, act, motivate, and deal with people; can calculate trouble in advance and steady the organization; excludes unnecessary interference in the internal operations of the department by being a strong and effective manager

· Is an interpreter for the nursing and hospital, both intramurally and extramurally; speaks for quality and can motivate people in general; understands elements of human behavior.

· Thinks independently; is knowledgeable in the field and willing to draw upon that knowledge to establish a position; can say no and then find a productive alternative.

· Is a member of the management team with a business mind and attitude; is able to delegate responsibility so that the available resources are utilized; keeps decision making close to the patient and thus responds to patient needs and not professional resentment; demonstrates warmth and concern for the people as individual and has an interest in total patient care; above all, has a sense of humor and is able to supply a light touch when the going gets rough.

DUTIES OF A CHIEF NURSE

· Establishes and implements standards which assure safe and therapeutically effective nursing care of patients.

· Provides for the participation of nursing staff members in the formulation of the nursing care of patients.

· Make provisions for all members of Nursing Service Department to know the standards and their personal responsibilities in upholding them.

· Establishes a plan by which each patient’s nursing care is planned, given or supervised, and evaluate by a professional nurse.

· Develops nursing service policies, which focus on the care of the patients and high quality nursing care.

· Participates in the establishment and promotion of administrative policies and the practice which pertain to the practice of nursing.

· Introduces Nursing Service applicants and recommend to the Administration appointments, promotions, transfers, disciplinary actions, termination of nursing service department personnel.

· Establishes the function and qualifications of each nursing position.

· Determines a staffing plan which will accomplish the stated objectives and standards of nursing service and promote the maximum utilization of nursing personnel.

· Participates in reviewing and recommending personnel policies as related to nursing personnel.

· Directs the personnel and the activities of the nursing department. Conducts investigation of complaints or irregularities involving the nursing service staff and submit findings and recommendations to the administrator.

· Organizes the nursing service department to delineate authority, functional responsibilities, lines of relationship and communication to provide the safe and therapeutically effective Nursing Care.

· Evaluates Nursing Care and the Climate in which it is practice to identify the achievements and problems to provide data for forecasting and planning.

· Develops and maintains an effective system of nursing record and report.

· Collaborates with the administrative staff, and representatives of allied group in planning for coordinative services to patients.

· Determines and recommends a department’s budget to implement stated objectives and control and evaluate same.

· Plans with administration and other appropriate groups for the development of new and effective use of existing facilities needed to attain service and educational objectives.

· Initiates, promotes and participates when necessary in studies and research design to assess nursing administration practices, and nursing care, and promotes their utilization.

· Participates in activities which promote professional growth and development.

HEAD NURSE

Job Duties

· Head nurses direct and supervise nursing staff in provision of nursing care and ensure the availability of support services which facilitate this care. The first-line nurse administrator serves as a resource to staff , interpreting philosophy, goals, standards, policies and procedures. The head nurse participates in varying degrees in policy formation and decision making with other member of nursing administration. These administrators are the vital link between the nursing management and the staff that delivers care to the patient.

· Head nurses are responsible for delivering care that is therapeutically effective and safe as well as cost-effective. They accomplish this by effective utilization of resources through the administrative process. Usually, a head nurse is responsible for a nursing unit.

· Activities that received the most votes by head nurses as most representative of their responsibilities were identified in one survey.

· Supervises and coordinates all patient care on the unit, including communication with physician.

· Assumes 24-hour responsibility of the unit.

· Confronts and resolves conflicts involving staff, physician, patients, and personnel.

· Schedules daily activities of the unit, including assignment planning, scheduled meal and break time, and conferences.

· Provides feedback to each employee on a planned basis, including a yearly observation.

· Meets regularly with nursing administration f0r feedback and problem solving.

· Aids in the implementation of new policies and procedures.

· Implements, evaluates, and revises plans to meet the unit goals.

STAFF NURSE

Job Duties

· Renders professional nursing care to patients within an assigned unit of a hospital, in support of medical care as directed by medic!l staff and pursuant to objectives and policies of the Hospital.

· Performs nursing techniques for the co-fort and well being of the patient. Prepares equipment and assist physician during trEatments and examinations of patients. AdministeRsprescribed medications, orally, aNd by inJections; Prov des treatment using therapeutic equipment; observes patients’ reactions to medications and treatments; observes progress of intravenous infusions ant subcutaneouS in&iltra4ions; chaNges or as3ist physician in changing dressings and cleaning wounds or incisions; takes temperature, pulse, respiration rate, blood pressure, and heart beat to detect deviations from normal and gauge progress of patient, following physician’s orders and approved nursing care plan. Observes, records, and reports to supervisor or physician patients’ condition and reaction to drugs, treatments, and significant incidents.

· Maintains patients’ medical records on nursing observations and actions taken, such as medications and treatments given, reaction, tests, intake and emission of liquids and solids, temperature, pulse and respiration rate. Records nursing needs of patients on nursing care plan to ensure the continuity of care.

· Observes emotional stability of patients, expresses interest in their progress, and prepares them for continuing care after discharged. Explains procedures and treatments ordered to gain patients’ cooperation and allay apprehension.

· Rotates on day, evening, and night tours of duty may asked rotate among the various clinical and nursing services of institution. Each service will have specialized duties, and staff nurse may be known by the section to which assigned, such as Staff Nurse, Obstetrics or Staff Nurse, Pediatrics. May serve as a team for a group of personnel rendering nursing care to a number of patients.

· Assist in planning, supervising, and instructing midwives, nursing aides and students. Demonstrates nursing techniques and procedures, and assist non-professional nursing care personnel in rendering nursing care unit.

· May assist with operations and deliveries by preparing rooms, sterilizing instruments, equipments, and supplies, and handling them, in order of use, to surgeon or other medical specialist.

Nursing Responsibilities in Drug Administration

Last Dec. 2008, we had our PCI (Practicing Clinical Instructor) exposure, it is a three day exposure at SMC-NLR where we handled 2^nd year BSN students. We are given with topics by our Clinical Instructor to be discussed regarding this concept; I am tasked to discuss the different Nursing Responsibilities in Drug Administration. This topic was already been discussed to us back when we were still in 2^nd year by one of our CI and my notes regarding this topic was lost and I don't know where did I placed that. I ‘d searched the internet and all but unable to find a copy of this thing. Luckily, I was rescued by the clinical instructors; they let me borrow their acetate film about the assigned topic for me to use during my discussion. I decided to write this entry for you to not experience what I had experience & for those who need this topic, I hope this will help especially to student nurses and as well as nurses, since we are very prone in committing medication error and observing these responsibilites will promote our patients' safety. So here it is!

NURSING RESPONSIBILITIES IN DRUG ADMINISTRATION

The following are responsibilities which the nurses must be aware of whenever administering drugs. As professional, nurses are dutybound to carry out these responsibilities.

· Medications must be kept in an orderly manner in a place where they are not freely accessible to patients or to the public and where they are protected from air, moisture & light. Only medicines that are properly labeled must be retained. Instructions regarding storage, refrigeration & expiration dates should be carefully observed.

· Controlled substances (narcotics) should be kept under lock and keys and appropriate records should be kept regarding their use.

· No medications should be given without a proper medical order from a physician clearly indication the medication, doe, route and schedule.

· A medication card is filled out for each drug indicating the date, the patients room number, drug name, dose, route schedule, and duration of administration. The nurse who transcribes the order to the medication card would initial it. The identity of the medication should be carefully checked when it is taken from the storage, when it is prepared and when the container is returned to the shelf.

· Medication in the solution or suspension should be shaken well to ensure homogeneity before a quantity is removed from the container in filling a drug order. Care should be taken to wipe the rim of container in taking solutions.

· Know the drug to be given in terms of nature, action, usual dosage, indication and contraindications, mode of action, side effects, physical characteristics and why it is particularly given to the patient.

· Be careful with the same sound. Know abbreviations of the drug.

· Gather all necessary equipment before starting preparation of medication.

· Do not converse with anyone (as much as possible) while preparing the medication. Allow no interruptions.

· Read and check label on medication three times.

a) Before taking from the shelf.

b) Before pouring or measuring.

c) Before returning to the shelf.

· Avoid touching medications especially tablets or pills.

· Do not return excess medicine to the bottle to avoid contamination.

· Always keep medicine card and medicine tray together.

· Arrange prepared medications according to dispensing and other factors.

· Never leave medicine cupboard or shelf unlocked.

· When preparing the medications, do not use the following:

a) Medications from unmarked container with illegible labels.

b) Medications that are cloudy or have changed of color.

c) Medications that have sediment at the bottom unless the medication requires shaking before use.

· Identify the patient carefully using all precautions. Check the bed cards; look at the identification bands, call the patient by name or ask the patient to state his name. Remember the five rights of medications, right dose, right patient, right time, and right route of administration.

· With rare exceptions, patients have the right to know the name and action of the drug they are taking, and they have the right to refuse a medication, medications that are refused must be changed and the reason for refusal must be recorded.

· Provide the correct adjunctive nursing measure.

· Give medications within 30 minutes of the time ordered except for preoperative medications, which must be given exact time ordered or medications that are ordered to be given hourly or every 2 hours (e.g. eye medication, prior to surgery).

· If a patient vomits after taking an oral medication report the fact to the responsible nurse and state the names of all medications. With-hold the medication. Often the physician re-orders the same drug by a different route, for example subcutaneously or intramuscularly.

· Special precautions must be observed for certain drugs. Most agencies requires that two qualified nurse double-check the dosages of anticoagulants, insulin, digitalis preparations and certain IV medications. Check agency policies.

· After medication has been administered, record it on the patient’s chat. The recording should include the time, the name of the drug, the dosage, the method of administration and any related data. Some agencies require that the method of administration be specified if it is other than oral, the oral route is usually not specified in the record.

· Evaluate the effectiveness of a medication of suitable time after its administration. For example, the effectiveness of an intramuscularly injected analgesic can be evaluated 10-20 minutes after administration.

· Medications are usually discontinued before surgery, and the physician writes new orders after the surgery. New orders are generally given drugs to a newly admitted patient, takes at home or when a patient is transferred to another service within an agency. Check agency policies.

· When medications are intentionally omitted; e.g. before surgery or a diagnostic test, record the omission and the reason on the patient’s chart. It may also be necessary to notify the prescriber.

· Medication error sometimes occurs. When an error is made, report it immediately to the responsible person so that corrective measure can be implemented promptly. Errors are usually documented on an unusual incident form that becomes a part of the agency’s file.

The nurse who prepared the medicine should be the one to administer and record. Never allow a patient to carry or bring medicine to another.
