<div xmlns="http://www.w3.org/1999/xhtml"><p>We have available for you the Performing Arts Series Parent Donation Letters for the 2007-2008 school year.&nbsp; this year we will be distributing letters to all grade levels on 9/12/07.</p>

<p><strong>Permission form</strong> must be completed for <strong>each child</strong> regardless of contribution.&nbsp; Please retain all completed permission forms received for your 2007/2008 Performing Arts record.</p>

<p>All donations should be turned in by September 28, 2007, in compliance with Judith Lindsay, Supervisor of Visual, Performing and Theater Arts.</p>

<p>Parent Letter printed in Spanish will be available.&nbsp; Due to limited quantity, it is available upon request from front office.</p>

<p>Please remind students and parents to deposit donations into two <strong>black boxes</strong> located in main hallway.&nbsp; <strong>Do not</strong> forward payment envelopes with <strong>attendance folder</strong> to the office.&nbsp; Any permission slips enclosed in payment envelopes will be returned to respective teachers.&nbsp; <strong>School staff must not handle these envelopes to obtain a permission slip.</strong></p>

<p>Teacher will indicate that the permission slip has been received on a Class roster List or equivalent.</p>

<div xmlns="http://www.w3.org/1999/xhtml"><p>This year CCSD has installed new financial program for school accounting.&nbsp; This new program no longer will provide information which were available through our previous program.</p>

<p>I have been informed by the county financial services dept that contribution information regarding individual student cannot be released for the purpose to send 2nd&nbsp; and subsequent notices to collect donation. When second notice is required, entire class must receive 2nd and subsequent notices of donation for the field trip. I am permitted to provide teacher with total collected funds. You may notify parent regarding missing permission slip. </p>

<p>In accordance with county mandate, student statements will be provided at least twice each school year.&nbsp; Any time during the school year,&nbsp; when a parent has inquiry or requests information regarding their child/ren's contribution status, copy of student statement can be provided for the current year.</p>

<p>I apologize for the inconvenience.&nbsp; Changes in the new financial program were not informed and as I continue to learn about the program, I will inform you of any changes which may affect you.</p></div>

<p>&nbsp; &nbsp; a.&nbsp; Field Trip Request<br />&nbsp; &nbsp; b.&nbsp; Field Trip Checklist<br />&nbsp; &nbsp; c.&nbsp; Cobb County Permission to Participate in <br />&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp;One Day Fieldtrips</p>

<p>2.&nbsp; Coordinator will complete the Field Trip Request and <br />&nbsp; &nbsp;&nbsp; Checklist (a &amp; b above) and return to Assistant Principal.<br />&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp; <strong><u>Packet must be turned in at least<br /></u>&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp; <u>20 days before the trip.</u></strong></p>

<p>3.&nbsp; Assistant Principal will:<br />&nbsp; &nbsp; a.&nbsp; Put Field Trip on school calendar or notify contact person<br />&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp;if there is a conflict.<br />&nbsp; &nbsp; b.&nbsp; Send packet to Bookkeeper</p>

<p>4.&nbsp; Bookkeeper will notify teacher when it is OK to send home <br />&nbsp; &nbsp;&nbsp; permission slip and request for money.&nbsp; <br />&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp; <strong><u>No notice should be sent home until <br /></u>&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp;&nbsp; &nbsp; <u>OK is given by bookkeeper</u></strong>.</p>

<p>5.&nbsp; Grade level field trip coordinator will contact Cafeteria <br />&nbsp; &nbsp;&nbsp; Manager if trip will involve sack lunches.&nbsp; This needs to <br />&nbsp; &nbsp;&nbsp; be done as soon as trip is put on the school calendar,<br />&nbsp; &nbsp;&nbsp; at least 20 days before the trip.</p>

<p>Please remember to check GPS/QCC's for your grade level to make sure your trips relate to the standards for your grade level.&nbsp; With the changes in the standards, some topics are moving to different grade levels.</p></div>
