What is Mean By This?

Menstruation is the periodical loss of blood from the uterus or 
from any mucous membrane, occurring for the reason that no 
fecundated ovum is present in uterus or tube, and may be divided 
into three periods: the premenstrual, the menstrual, and the 
postmeristrual.

During the premenstrual period, the ten days 
immediately preceding the appearance of blood, the following 
changes take place :



What Happens In Superficial Capillaries?
The superficial capillaries become greatly dilated, and serous 
infiltration of the endometrium takes place, which separates the 
meshes of the stroma, accompanied by a gradual but decided 
dilatation of all the blood vessels and lymph channels. There 
occur a growth of round cells in the interglandular tissue, and an 
entrance of leucocytes into the mucous membrane. The glands 
become larger and wider, being often filled with secretion.


How Does this effect the Layers Around ?
This swelling of the mucous membrane, the dilatation of the 
blood vessels, the production of round cells, and the growth of 
the superficial layer of the endometrium produce the so-called 
decidua menstrualis. Although in the connective tissue large 
cells, not to be distinguished from young stages of decidua cells, 
are found, it is to be noted that typical decidua cells do not, as a 
rule, develop at this time in the superficial layer. The endo- 
metrium is at this period from 6 to 7 millimetres in thickness.


The period during which blood is thrown out is the next, or 
menstrual period. The superficial capillaries are greatly dilated, 
and an exit of blood elements, not dependent on a bursting of the 
capillaries, goes on for several days. The bleeding occurs partly 
through diapedesis, and, in strong bleedings, through rhexis. 
There is little or no destruction of the mucosa, only a very slight 
fatty degeneration of the epithelium of the uppermost layer, so 
that in the excreted blood relatively few epithelial cells are 
found.


After This , What Changes Happen?

After menstruation, the uterus shows an almost continuous covering of epithelium, interrupted in spots, especially if pathological processes are present.

Most of the ciliated epithelium 
is preserved. The first stimulus to bleeding is due to contraction 
of the uterus, which at the height of congestion is possibly ac- 
companied by contractions of the tube.

The uterus is larger and in the first few days following likewise soft and flabby. 
The flabbiness lasts longer than the bleeding. 
A spontaneous dilatation of the cervix canal takes place, and reaches its height on the third or fourth day. 
This dilatation  takes place without regard to the amount of blood discharged, whether the menstruation be painful or painless. 
The cervix is hyperemic, the glands showing an increased secretion of mucus. 
The blood thrown off is mixed with the mucus of the uterus and 
cervix, and later with the acid secretion of the vagina, for this 
reason coagulating less easily than other blood.

CONVULSIONS IN PREGNANCY , TYPES AND TREATMENT

Convulsions are occured during pregnancy in Women due to variety of reasons and causes .

Based on the causes ,
These are generally supposed to be of three kinds,
1st : the nervous or hysteric ;
2d, the epileptic ;
and 3d, the apoplectic.

HYSTERIC CONVULSIONS

The Hysteric convulsions are most common during the early months of pregnancy ; they may be brought on by want of sleep, excessive fatigue, disordered digestion, or in fact by any irritation of body or mind.

EPILEPTIC CONVULSIONS

The Epileptic convulsions are rare; seventy-nine cases only occurred in 38,306 of labor, or 1 in about 485.
The majority of these cases are connected with albuminuria ; they are apt to be preceded by a peculiar and intense pain in the forehead, severe pain in the stomach, and by a tumid state of the face and hands. Under allopathic treatment forty-two mothers were lost out of one hundred and fifty-two cases, or more than one-fourth.

APOPLECTIC CONVULSIONS
Apoplectic convulsions seldom or never occur except towards the termination, or after the conclusion of labor, they are gene- rally caused by the stress upon the blood vessels of the brain during the severe forcing labor-pains; they differ from the other varieties by there being but little convulsion ; the. body and limbs are thrown about for a short time, and then the patient lies in a comatose state ; there is little or no distortion of the face, and no frothing at the mouth ; the muscles are flaccid and powerless ; the breathing stertorous ; there is no return of intelligence, and rarely any repetition of the paroxysm.

Treatment for Convulsions — Clinic reports following herbs for different conditions and depending on the convulsion classification

Aconite, if the skin be hot and dry ;
Bryonia, if the abdomen be tender, with full pulse and perspiration ; if there be flatulence, and diarrhoea with tenesmus,
Chamomilla, Mercurius and Hyosciamus ; if there be difficulty of urination, with coldness and pallor of the face,
Pulsatilla ; if the face be livid, purple and warm, Belladonna ; if there be a tendency to stupor, with stertorous breathing, or a state of incoherent wandering,
Opium, which will render the mind clear and calm, with a corresponding improvement of the other symptoms. Hyosciamus, when there is an agitated or ruffled state of the nervous system, with an over-active state of the vascular ; it acts like a charm, soothing and stilling the nervous system, while the labor goes on progressively.

A few strong doses of Opium will often postpone the attacks, and thus afford time for the vital powers to react effectually against the disease. It acts very promptly after bloodletting, and must be repeated not only after every attack of convulsion, but also when there is the least indication of a return.

Why Exactly Does Bleeding from Womb Occur and What is the diet?

This is an entirely different and much more serious affection than menstruation during pregnancy.

Arnica should be given, either in the tincture or 3d dilution, and repeated every five, ten or fifteen minutes, according to the urgency of the case.

Ipecac, should be used if the flow of blood is copious, and comes in a continued stream, attended with pains at the navel, bearing down efforts, arid pressure upon the womb, and lower bowel, with chills, or general coldness, but heat of the head and face, general lassitude and inclination to lie down. Dose.

Either sufficient of the tincture or powder should be given produce slight nausea, or else the 3d or 6th dilution may be used every ten or fifteen minutes until relief ensues.

Chamomilla may be used, when nearly the same symptoms are present as those mentioned as indicating the use of Ipecac. ; especially if the pains are like labor-pains.

Platina, when the blood is black and thick, but not coagulated or grumous ; when there is a dragging sensation from the back to the groins, and the internal genitals are exceedingly sore and tender to the touch.

HYDRORRHEA AND PREGNANCY

Pregnant females are occasionally attacked with a fluid dis- charge from the vagina, quite distinct from the leucorrkoea, which has been described. It may occur, once or several times during pregnancy, and continue for a week or two, or it may persist for several months.

Excessive Discharges due to Hydrorrhea These discharges are neither preceded or followed by any pains or contractions of the womb ; their nature is such as to interfere but slightly with the pregnancy, the latter advancing as usual to its natural term, and at the time of delivery the true bag of waters is regularly formed.

Most generally the female enjoys her usual health before the discharge comes on, when she unexpectedly finds herself wet, the fluid escaping drop by drop, or else she hears the peculiar sound caused by the sudden discharge of a considerable quantity of waters.

In most cases she suffers no pain either during or after the discharge, though slight uterine contractions may set in ; but if the patient keeps perfectly quiet, the pains soon cease and everything resumes its natural order again.

The discharged waters are usually a little yellowish, very limpid, and at times tinged with blood, leaving stains upon the linen, and having a well marked spermatic odor.

The discharge is much influenced and increased by mental emotions and bodily excitement ; on the other hand it augments in quantity during the most perfect quietude, as for instance, at night during sleep.

In one case, observed and treated by myself, the discharges only took place at night, during rest or sleep, and then often amounted to one or several pints.

TREATMENT FOR HYDRORRHEA :

Although this is generally not a serious affection, still it is a very annoying one. Doctor thinks, that nothing can be done except to keep the patient dry, quiet and clean.

Arsenicum and Digitalis deserve attention in which there were sudden, frequent and profuse discharges of waters from the vagina.

PROLAPSED UTERUS DIAGNOSIS & A PHYSICIAN'S GUIDE TO TREAT HER

PROLAPSED UTERUS Diagnosis :

There is no difificulty in making a diagnosis for prolapsed uterus except in the early stage. An inverted uterus might be mistaken, or a polypus hanging down from the uterus, or a cyst or other tumour in the vagina. The presence or absence of the external os will determine whether the swelling is the uterus or not.

Three conditions have to be distinguished :

Prolapse Uterus must be diagnosed from

Giving way of the uterine ligaments ;

From laceration or relaxation of the pelvic floor ;

A combination of both these ;

Elongation of the supravaginal cervix, in which prolapse is more apparent than real.

If a patient with symptoms of prolapse is found on examination to have the uterus in position and an intact perineum, she is probably suffering from descent of the pelvic floor. When lying on her side the perineum can be observed as a groove between the buttocks descending slightly during inspiration. Under normal conditions a strong bearing-down effort will cause a descent of ^ to 1 inch, but when there is undue relaxation of the pelvic floor this may extend to two or more inches.

This condition is not nearly so common as the other forms of prolapse. The cases most likely to be overlooked are those of incipient prolapse, where the descent of the uterus is slight, unless the patient is examined while standing, and told to bear down as much as possible.

Success in Diagnosis will depend upon an intelligent appreciation of the actual condition of the patient. If, for instance, the prolapse is due to a giving way of the uterine ligaments, a tear in the perineum, sewing up the latter will not cure the patient, althoiigh it will do good. There are three principal methods of treating patients, viz., medical, mechanical, and surgical.

NATURALLY DIAGNOSING
UTERINE PROLAPSE

A cold hip-bath every morning, followed by a brisk rubbing with a rough towel, is an excellent tonic. Fresh air, plain food, and early hours should be enjoined. Any tendency to constipation must be most carefully corrected.

The choice of aperient will depend on the type of case, but for most patients there is nothing to equal cascara sagrada combined with a little hyoscyamus and oil of peppermint, to prevent griping.

A glass of good burgundy, port, or stout in non-gouty patients does much good.

Astringent vaginal douches help matters.

That which I prefer consists of two parts of alum to one of sulphate of zinc ; a drachm of this is added to a quart of tepid or cold water and used morning and evening.

During menstruation the water must be hot instead of cold, and the patient should also keep her feet up. In the intermenstrual period she can go out in a bath-chair or a carriage, but must not stand or walk much. On account of the general relaxation of the tissues pessaries are not of much service at first.

Many of the symptoms before described are common to other forms of disease in the pelvis and an exact diagnosis can only be made by a careful bimanual examination.

PATIENT'S TREATMENT FOR PROLAPSED CONDITION

TREATMENT :

The patient should have everything constricting the waist loosened or removed.
Lying on her back with the knees flexed and the legs drawn up in an easy and unconstrained position, so as to relax the abdominal muscles, the physician will find the cervix pointing up- wards and forwards, and from this point will be able to trace the cervix into the body of the uterus to the fundus as it lies behind the posterior vaginal wall.
With the left hand over the abdominal wall he will be able to assure himself of the absence of the fundus from its usual position behind the pubes or further back.
The swelling in Douglas' pouch will also be very tender if it is the fundus; but it is also tender when due to perimetritis

WHAT IS PROLAPSED UTERUS SYMPTOMS AND ANGLE ?

It might be asked, to what extent, then, is the mobilty of the uterus normal? From clinical observation, I should say that any descent of the uterus, except what has been already indicated, is abnormal. But the antero- posterior position of the uterus can vary in health through fairly wide limits.

It moves on its axis from a position of anteversion with an empty bladder, through an angle of 45°, so that in the standing position the cervix looks slightly forwards and downwards, and the fundus backwards a little upwards .

PROLAPSED UTERUS SYMPTOMS AND SIGNS

The symptoms of prolapsed uterus are more marked in the early than the later stages.

As soon as the uterine ligaments begin to stretch the patient suffers from pain in the back low down at the junction of the sacrum to lumbar spine.

She usually feels a bearing-down pain within the pelvis, which is worse on walking or standing, and relieved by lying down.

Many complain of pain in the left ovarian region as well.

The bladder is sometimes irritable, with frequent micturition.

Young unmarried women may have slight metrorrhagia.

It is in the early stage that prolapse is most likely to be overlooked.

Many patients, if examined lying down and told to force down, show little or no sign of prolapse, but if the examination is made while the patient is standing, the uterus is found to come down a little lower than normal.

MORE SYMPTOMS OF PROLAPSE

She usually feels a bearing-down pain within the pelvis, which is worse on walking or standing, and relieved by lying down.

The cervix is found to be enlarged and usually covered with erosion, while profuse leucorrhoea is present.

The lower the uterus and the longer it has been down the more marked are these symptoms.

Added to this there may be actual ulceration of the cervix in such a condition of neglect that it might be mistaken for an epithelioma.

The vagina undergoes certain changes in chronic pro- cidentia. It is usually much dilated and larger than normal, the rugse are lost, the epithelial surface loses its mucous character and becomes hard and horny.

In all doubtful cases, when the patient has symptoms of prolapse she should be examined standing, and if she is told to bear down as much as possible, whatever the degree of prolapse it will be revealed.

And, secondly, were no such consequence to be apprehended, every operation hitherto devised fails, in the majority of instances, in accomplishing the object for which it was performed.

WHAT IS THIS HYDATID DISEASE ?

The disease called hydatid is not a peculiar production of the uterus, as it is often found in other parts of the body ; neither is it confined to the human subject alone, some animals, the sheep especially, are frequently affected by it. 

Hydatids, according to the Linnean system, constitute a tribe of 
the genus tenia, and belong to the class and order of intestinal

worms. They are characterised by being famished with a vesicle, 
which is sometimes attached to them posteriorly, or in which some 
of them are altogether enclosed. They occasionally exist singly 
in the uterus, but more generally hang together in clusters, each 
little vesicle being attached to its fellow by a very delicate filamen- 
tous thread, the whole presenting an appearance very similar to a 
bunch of grapes ; they are connected with the lining membrane of 
the womb by a thickish substance, resembling fibrin, having, 
nevertheless, a bloody appearance when first expelled from the 
uterus. The size of these vesicular bodies varies, some are very 
small, whilst others are as large as the egg of a thrush ; when 
macerated and perfectly freed from blood, they are generally colour
less. Dr. Baillie, however, states that he has, in many cases, seen 
hydatids of the liver of a pale amber colour. 

Various opinions have been entertained with regard to the forr 
mation of uterine hydatids. As they so commonly exist in 
connection with a dead ovum, some have attributed their origin 
to this circumstance, whilst others believed that the death of the 
ovum has been the effect of the previous existence of hydatids in 
the cavity of the womb ; that, in truth, these animalcule were 
the actual devourers of the foetus. Dr. Blundell, late of Guy's 
Hospital, has preparations showing this work of destruction in 
progress. I have never seen a case of uterine hydatids in an 
unmarried female, and am not aware that it has been observed by 
others ; still it would be wrong to assert that these bodies cannot 
possibly form without sexual intercourse ; and, therefore, if called 
to an unmarried woman, from whose uterus hydatids were growing, 
I should not feel justified in expressing an opinion which might 
prove injurious to her reputation. Wherever there exists a reason- 
able doubt, it is but an act of common justice to give the patient 
the benefit of such doubt. 

I have stated to you that married women are very generally the 
subjects of this disease, and not only married, but in a state of 
pregnancy ; the symptoms, therefore, at first are those which are 
attendant upon that condition, but after the death of the ovum the 
signs of pregnancy disappear. The uterus, instead of diminishing, 
increases in size, in consequence of the enlargement of the hydatids
within its cavity ; and, therefore, it often happens that the medical 
man, as well as the patient, is deceived, as they imagine pregnancy 
to be still advancing. There is this marked difference, however, 
between the two : the enlargement which is the result of concep- 
tion is slow, whilst that from hydatids is very rapid. Again, there 
is the absence of the sensation called « quickening," although there 
may be a kind of indistinct flutter perceived by the patient herself, 
probably caused by flatus ; or, perhaps, it may depend upon that 
power of contraction which the hydatids themselves possess. Sir 
Charles Clarke states, that although hydatids in other parts have 
this power, yet it has never been"observed in those of the womb ; 
it however, by no means follows that because they have not been 


TREATMENT OF HYDATID :
Treatment of Uterine Hydatid — There are no known reme- 
dies which will prevent either the formation or growth of uterine 
hydatids, nor do we possess any means whereby we can procure 
their expulsion in the more early months ; all that can be done is to 
explain to the female the nature of her disorder, and to attend to 
any symptoms that may arise, patiently waiting until, from the 
distended state of the womb, its extraordinary action is excited ; 
and this, as before stated, will occur, sooner or later, varying 
greatly in different individuals ; the sooner this contraction takes 
place, the better will it be for the patient, as the blood-vessels 
increase in proportion to the enlargement of the uterus, and, 
consequently, are of smaller size in the earlier months.


 The os uteri under these circumstances, is usually considerably dilated. 
so that the finger can, with ease, be introduced into the cavity, and 
the hydatids distinctly felt. If, however, the hemorrhage be not 
alarming, no manual interference of this kind is cither necessary or 
proper : the main object of the practitioner is to control the 
hemorrhage : to this purpose all his efforts must be directed. The 
flow of blood, be it remembered, is in some instances so excessive, 
that the female's life is in immediate peril, the gushes being as 
profuse as during the puerperal condition ; indeed, they appear 
much more so in consequence of the large quantity of watery 
fluid with which they are mixed. In the attempt to restrain these 
bleedings, we must not forget that the only effectual method 
consists in exciting a general, perfect, and permanent action of the 
muscular fibres of the uterus, without which its renewal is certain. 

Confinement to the recumbent position is imperatively required, 
and every circumstance carefully avoided, which would be likely 
to produce either bodily or mental excitement. In some cases 
(that they are rare must be admitted) this plan of treatment, 
combined with great attention to diet, is all that is required, 
especially where the action of the uterus is powerful ; of course 
no nutriment, of a stimulating kind, should be allowed, and 
everything should be received into the stomach nearly or quite cold,
caloric, as you know, having a great tendency to excite the action 
of the heart and arteries. Next, in the list of remedial means, I 
would place the application of cold : a bladder, containing pounded 
ice, as recommended, in a previous lecture, should be placed upon 
the lower part of the abdomen ; a piece of ice has by some been 
recommended to be introduced within the vagina, by which a 
sudden state of contraction is often procured. 


After hydatid disease, there is sometimes a tolerably copious 
secretion of milk, which soon subsides after a laxative has been 
administered. Lastly, it is often necessary, in consequence of 
extreme debility resulting from this affection, to employ tonics and 
cardiacs for a long space of time, a return to perfect health and 
strength being often very protracted and tedious. Everything 
likely to increase tone in the general system is here required, 
avoiding, of course, all local stimuli.

Pelvic Floor Dysfunction in Women

What is Pelvic Floor?

Prior to discussing Pelvic floor dysfunction, let us have a quick look at what actually is the Pelvic Floor.

The Pelvic Floor or pelvic diaphragm is the base of the pelvic region that is made up of muscle fibres of the levator ani (which is a broad thin muscle situated on the side of the pelvis), the coccygeus (the muscle of the pelvic wall located behind the levator ani and in front of the sacrospinous ligament) and some associated connective tissues that cover the entire area below the pelvis. Pelvic Floor are the muscles that contract during orgasm and assist in childbirth. If become weakened they can also be the cause of incontinence, lack of libido and difficulty climaxing during intercourse.

What is Pelvic Floor Dysfunction (PFD)?

When we speak of Pelvic Floor Dysfunction, it covers a wide array of disorders that take place when muscles of the pelvic floor become feeble or stiff and stop functioning properly. It also occurs when there is some kind of an injury to the coccyx, sacroiliac joint, hip joints or lower back. Pelvic Floor Dysfunction also can take place when the sensitivity of the tissues surrounding the pelvic organs increases or decreases, resulting in severe pain in the pelvic region. On a number of occasions the fundamental cause of pelvic pain cannot be determined. PFD can be experienced by both women and men.

PFD often includes other clinical conditions such as Urinary Incontinence, Anal Incontinence and Pelvic Organ Prolapse amongst the most popular ones. Sexual dysfunction, loss of sexual appetite and difficulty to orgasm can be by-products of these conditions.

The statistics are actually shocking! Women do not like to discuss this issue and yet it is such a common occurrence. About 1 in 3 women are affected by at least one of these conditions. Besides, statistics show that 30 to 40 percent of women suffer from some degree of incontinence in their lifetime, most often after childbirth and post menopause, and that almost 10 percent of women will undergo surgery for urinary incontinence or vaginal or pelvic organ prolapse. Keep in mind that it is NOT natural to have it, and just because you are older or have it as a result of pregnancy, you do not have to suffer from it. Taking action is key, preventing the disorder is even better and it is not difficult. All it takes is just developing a few new habits, such as incorporating Kegels into your workout, using a Kegel device as an aid and doing regular Pelvic Core exercises.

The cause of Pelvic Floor Dysfunction:

The first thing that needs to be taken into account while discussing the causes of PFD is that a wayward lifestyle is often responsible for PFD, obesity being one of these instances. Rigorous exercising and a commitment to a diet is the way to deal with that issue. Obesity puts too much pressure on the pelvis and can even lead to vaginal and uterine prolapse and surgery in the worst case.
Nevertheless, PFD in females can result from a number of issues. They may range from surgeries in the pelvic region to complications during pregnancy to genetics.

Menopause and several common chronic diseases like Irritable Bowel Syndrome, Interstitial Cystitis or Endometriosis can also result in PFD. Inherited deficiency of collagen can be the cause of loss of elasticity of the Pelvic Muscles leading to weakness and dysfunction.

Some other common reasons of PFD are trauma, accidents, fracture to the tail bone and other reasons such as persistent poor postures, fragile core muscles etc. To correct posture and strengthen the core it is recommended to do Yoga, Pilates and Kama workouts.

Postpartum Pelvic Floor Dysfunction is the most common cause and affects women who have given birth. However it is believed that the pregnancy itself and not the delivery that is responsible for Incontinence, which results from PFD. A study was done to compare the after effects on pelvic floor of pregnancy and birth and compared mothers who had a C- Section with those who gave birth naturally. The results did not differ, suggesting that pregnancy is the cause.

Some conditions are reversible with pelvic floor exercises, or Kegel exercises recommended to strengthen the muscles. Kegel devices are also recommended to increase pelvic floor tone by stimulating muscle contractions with electrical impulses as well as aiding in properly performing Kegels. Treating Pelvic Floor Dysfunction

One of the most popular ways of treating PFD is biofeedback, which is a specific physical therapy. With various biofeedback methods, a therapist can enhance the patient’s rectal sensation and coordination of pelvic floor muscles. Some therapists use a tiny dildo-like probe that is placed in the rectum or vagina, other use a number of electrodes that are stuck to the skin surface around the opening to the anus or around perineum and on the wall of the abdomen.

These gadgets can sense the contraction and relaxation of particular muscles and give doctor an idea of the severity of the PFD as well as improve the muscles function.

In other cases, the therapist massages the perineum or vaginal area internally, looking for trigger points which contract or shudder unusually. Upon location of the trigger-spots, the therapist gently exerts pressure to that particular area by massaging the spot. This method is called “Thiele stripping”.
Many women find having another person performing this procedure invasive and inconvenient. An alternative is performing a similar procedure yourself in the comfort on your own home. OVIDUCT - ITS TYPES & THEIR ROLE IN PREGNANCY

In the pregnant oviducts did not exceed 30 microns In the ovarian 
end and 60 microns In the mid-reglon.

These can be classified into both


MATURE 
IMMATURE

In the mature ducts the musculature showed a maximum measurement of 75 microns In the mid-reglon and 45 microns in the ovarian end. This showed that there were cyclic changes in the tunica muscularis of mature ducts, and that the musculature was limited to its minimum during pregnancy. As the growth cycle in the oviducts and the uterus of pregnant animals was not similar, it may also be presumed that the hormonal action which brought the changes in the two organs may be different and unrelated.

WHAT DO YOU MEAN BY IMMATURE OVIDUCTS ?

The immature oviducts, considered as a whole, resembled 
closely the mature duct in structure but exhibited the following 
differences:

1) The folds or plicae were fewer and simpler in their pattern.

2) The epithelium was simple, ciliated columnar without pseudostratification and contained a large number of spherical lymphocytes.

 3) The epithelial cells were loosely ar- ranged and there was no cellular activity as would be indicated by protoplasmic protrusions or nuclear extrusions.

 4) The elastic fibers in the tunica muscularis were less numerous than in the mature ducts.

 5) Because the folds were few and simple.

HOW MATURE OVIDUCTS ARE AFFECTED DURING PREGNANCY ?

The mature ducts were collected at different stages of the estrous cycle. The ducts from pregnant animals were 
collected during the early part of the pregnancy suid immature 
ducts were collected from calves varying from three weeks to nine 
months of age. 


Serial sections were made from each duct at three 
different regions, namely the ovarian end, the mid-region and the 
uterine end. The sections were stained by hematoxylin and eosin 
and also special connective tissue stains. The three coats of the 
duct, the tunica mucosa, muscularis and serosa were described in 
detail. 


The distinct characteristics of various regions of the 
oviduct in regard to the number of mucosal folds, the pseudo- 
stratified condition of the epithelium, the cellular activity 
and the variations in the thickness of the muscular coats were 
noted. The differences in structure exhibited by the ducts from 
pregnant animals as compared to the mature ducts, which were con- 
fined mostly to cytoplasmic protrusions, were reported.

WHAT IS PARTURITION & HOW DOES IT RELATE WITH INFLAMMATION AND ULCERATION OF CERVIX 
Parturition is a very frequent cause of inflammation and ulceration of the cervix, as might be presumed a priori. 


Not only is parturition 
frequently followed by inflamation of the uterus involving the cervix, which may perpetuate itself in the latter region even when it has beesubdued in the body of the organ, but it often occasions inflammation of the cervix alone, other parts of the uterine system not being simultaneously affected. This is owing to the cervix being the part of the uterus the most exposed to laceration and contusion during parturition. 

The cervix may be lacerated more or less extensively during the most natural labour. In a rapid confinement, a strong contraction, or a succession of strong contractions propelling the child with great force against the imperfectly dilated os, will, as I have repeatedly witnessed, thus lacerate the cervix, under circumstances otherwise the most favourable. 

DILATION OF OS UTERI DURING PARTURITION
Morever, this dilatation of the os uteri is very slight until parturition actually commences, and is not consequently calculated to interfere 
with the integrity of the mucuous membrane with which the cervical 
canal is lined. As soon, however, as the pains which precede and ac- 
company the expulsion of the foetus commence, the dilatation of the os uteri progresses rapidly, and in the course of a few hours is carried to such an extent as to admit of the passage of the foetus. 
A necessary consequence of this rapid dilatation of a canal lined by a mucous membrane in an entire state is, that it must, in many cases, be accompanied by contusion, erosion, and laceration of the membrane.

The peritoneal folds themselves are seldom compromised in non-

puerperal inflammation of the uterine appendages. When inflam- 
mation occurs in ' this region after parturition, there is a great 
tendency in the peritoneal membrane to take on the inflammatory 
action, as is the case when the uterus itself is the seat of inflamma- 
tion. In the unimpregnated non-puerperal condition, on the contrary,

There is very little tendency to inflammation in the peritoneum, and 
the organs contained between its folds mav remain inflamed during 
months or years without its being much affected. This is a singular 
pathological fact, but one which is equally true when applied to 
inflammatory affections external to the peritoneum in other points 
of the pelvic cavity. Even when peritonitis does complicate the 
attack in the non-puerperal state, it seems to have a greater tendency 
to localize than to extend its action — the contrary of what obtains 
in the puerperal condition.

In the form of the disease which I am now more especially describing, that which is unconnected with this condition, the pus escaping internally and the abdominal walls not being involved, the secondary symptoms are not very severe, except during exacerbations and relapses. The patient is able to get about, and to follow more or less her usual avocations. She is merely in delicate or bad health, has unusual pelvic pains and sensations, and menstruation is disturbed and laborious ; the real cause of this condition being nearly always a mystery both to herself and her medical attendant.

WHAT HAPPENS IN PARTURITION

In parturition,in which very extensive pelvic suppurations take place, giving rise to external perforations. In this form of the disease, death occasionally occurs ; but even under such circumstances it is rare, unless inflammation assume an extreme and exceptional degree of intensity.

METRITIS - TYPES, CAUSES AND SYMPTOMS OF THEM

By Metritis is generally understood inflammation of the entire 
uterus, both of the body and of the neck, in the puerperal as

well as in the non-puerperal state.

It is not, however, my intention to enter into the consideration of puerperal metritis — the form of acute inflammation which immediately follows parturition. I shall not either, as I have stated in the preceding chapter, include in the same description the history of inflammation of the body and of the neck of the uterus, confining myself for the present to the study of the disease in the former region.
Thus limited, inflammation may occupy the substance of the unim-
pregnated uterus, and be either acute or chronic ; or it may attack
the mucous membrane lining the cavity of the uterus, constituting
internal metritis. We will successively examine each of these forms
of uterine inflammation.

ACUTE METRITIS.
Acute inflammation of the non-impregnated, non-developed uterus, is a rare disease. This is a fact which is generally admitted by ute-
rine pathologists. I believe, however, that acute metritis will be
found of even less frequent occurence than it is supposed to be when it is no longer confounded with inflammation of the lateral ligaments — a mistake at present frequently made, even by experienced practitioners.

The rarity of acute metritis is the natural result of the peculiar
dense, fibro-muscular, non-cellular structure of the body of the
uterus. Tissues of this nature being but slightly susceptible to inflamation as a necessary consequence of their peculiar structure, if the
uterine system is exposed to the causes of inflammation, its peri-
phery the mucous surfaces, the cervix, or lateral ligaments,which are so much more highly vitalized, are generally the regions attacked.

Acute metritis is always accompanied by considerable febrile reaction. The skin is hot, the pulse quick, but not small and thready, as when the peritoneum is compromised. The tongue is covered with a white fur, and continued nausea is nearly invariably experienced, but it is seldom carried so far as to produce vomiting, as in metro-peritonitis. Thirst, headache, and want of rest are also present, as in all febrile diseases ; and the bowels are constipated. The breasts are often sympathetically affected, one or both becoming swollen and painful.

CHRONIC METRITIS
In describing chronic metritis, I shall likewise confine myself to the consideration of the disease in the body of the uterus. Although the distinction is not made by writers on uterine diseases, it is of extreme practical importance. It is in a great measure because it has not been adopted, that there is not to be found a correct description of this form of uterine inflammation. Some of the leading symptoms of chronic metritis are erroneously attributed by many uterine pathologists to the displacements of the uterus which it occasions ; and this has likewise much contributed to obscure its history, especially of late years.
Causes.Chronic metritis sometimes occurs as the termination of acute metritis, whether puerperal or non-puerperal ; but it is, generally speaking, as I have just stated, the result of the gradual extension of chronic inflammation of the neck to the body of the uterus, and the product of years of uterine disease.

Symptoms. Chronic metritis is a malady, the symptoms of whicvary considerably in intensity, according as the patient is examineduring the quiescent state of the uterus — that is, in the interval of
menstruation — or during the presence of the menstrual flux, and
for a few days before and after. Although a most distressing and
wearing affection, it is not altogether incompatible with what a
superficial observer might consider tolerable health, especially
during the interval of menstruation. At that time, indeed, there is
scarcely ever any febrile reaction, and the local uterine symptoms
are much mitigated. The general symptoms are then not unfre-
quently confined to functional derangement of the stomach, of the
nervous system, and of the general nutrition ; the result of the
sympathetic reaction of the diseased uterus on the economy at
large.
A very different state of things, however, is observed when
the molimen hemorrhagicum that precedes menstruation sets in.
The uterine inflammation, previously latent, again becomes evident ,both the local and general indications of its existence reappearing
with renewed intensity.
