Many students attending CF Patton Middle School have food allergies, especially to peanuts, tree nuts (like walnuts, pecans, and almonds) milk, eggs, soy, wheat, fish, and shellfish. Sometimes if they eat, touch, or even breathe a tiny amount of the food they're allergic to, they can become ill. That's why students who have food allergies need all of us to help keep them safe. Please read the attached information sheet, and share it with your child, in order to Protect A Life from food allergies, or you can read more at www.faankids.org.

Physical and Dental Forms Due

6th grade physical forms and 7th grade dental forms are due! If your child had an examination last spring or over the summer, remember to send the form in to the school nurse. If your child’s examination is in the near future, please return the form as soon as the examination is complete. Forms are available from the “Nurse” section of this website. These examinations are required by PA school law.New students to the State, regardless of grade, are required to submit both the physical and dental forms.

Immunization Updates – Provisional Enrollment of Students. PA State regulations dictate that by the beginning of the 2011/2012 school year, all students, in all grades, must have received: Two properly-spaced doses of varicella (chicken pox) vaccine, or documentation of the history of chicken pox disease. And 7th grade students must have received : Tetanus and diphtheria toxoid and acellular pertussis vaccine (Tdap) and Meningococcal conjugate vaccine (MCV) Students whose school immunization records are not complete will be receiving a letter by the end of September, stating that they are PROVISIONALLY ENROLLED in school until this vaccine information is received. If you have received a letter, please provide the school nurse’s office with the dates of the vaccines as soon as possible.
