The Pain Diaries: A Love Story

See the brief story below but in short we are launching a play, based on a real family here at the CPC, and we are still in fund raising mode. The play is both educational and fun; yes it is written in a way so that humour plays a significant role.... and we all how important that is in health care!

Future plans for the play include publication, development of CME's / CE's based on play excerpts and integration of excerpts into undergraduate curricula for chronic pain education.

So if you could have a look, forward to someone you might think would be interested in it, buy tickets, etc it would be greatly appreciated... and I think you will enjoy it as well.

My name is Carol Wei and I am a pharmacist/certified diabetes educator. I have been asked by the Academic Detailing group to update the current list of community pharmacists/CDEs who do insulin starts and diabetes management on the www.talksfordocs.com website

The academic detailers are a group of pharmacist who give presentations to physicians, and other allied professionals on various topics. Both members are pharmacists and their goal is to present unbiased information. Last fall their topic was insulin analogues in DM2

An extension, of their information detailing, is to provide tools and resources for the physicians. One resource list that has been included is a list of pharmacist/CDEs who would be willing and accessible, to collaborate with physicians, on insulin starts and management. The web site is www.talksfordocs.com and the list will be located in a diabetes toolkit. Technically, the site is on the public domain so it is accessible to anyone; however, the target audience is certainly family physicians in Calgary. A disclaimer of sorts indicating this list is not all-inclusive will be attached. e.g. "this list may not be inclusive.".

If you are interested in having your name on such a list, please fill out the listing authorization form and fax c/o Carol Wei at 403-948-6993 (dedicated pharmacy fax).

A response by November 30 is required, as I will be submitted the ‘updated’ list to the academic detailing group the first week in December.

Coordination of falls prevention for primary care patients

Pharmacist

As a Primary Care Network Clinical Pharmacist, you will have the unique opportunity to dramatically impact primary care reform in Calgary. Working collaboratively with patients, physicians and other allied health professionals you will

provide clinical services as required for challenging, complex and chronic care patients
conduct medication reviews and provide drug education
consult with other health care providers including family physicians and referring physicians for the identification and implementation of strategies and interventions directed by the treatment program
train, mentor and liaise with a group of Registered Nurses

Targeted patients are the elderly; those with complex medical conditions; those on multiple medications and those on medications requiring therapeutic drug monitoring.

You will work in the Access Geriatric Health program providing pharmacy consultation for family physicians and the multidisciplinary team, and will also play a key role in the Discharge Planning program providing post-discharge management and readmission intervention/prevention.

Qualifications:

Bachelor’s Degree in Pharmacy is required
Licensed with the Alberta College of Pharmacists as a practicing member
Additional credentialing or training in specialty programs (eg, Certified Geriatric Pharmacist, Certified Diabetes Educator, and Certified Asthma Educator) and research experience will be considered an asset.
Minimum 2 years expertise/experience in providing pharmaceutical care.

Responsibilities:
This position is responsible for assisting the Pharmacists with daily operations of the Pharmacy Department. Duties will include communications with patients and members of the health care team in the clinic, cashier duties, telephone, sending out prescriptions via courier, using the computer (prescriptions, ordering/receiving, courier etc.), assist in the processing of prescriptions and preparation of prescriptions, involvement of ordering and receiving drug orders and management of the pharmacy inventory.

Qualifications:
The successful candidate will have the following

Good interpersonal skills when dealing with customers, staff, & management.
Good communication skills, both written and verbal.
Self-starter with a high level of enthusiasm.
Ability to relate to all levels of customers and their needs.
Ability to work quickly and accurately under pressure.
Ability to work as a team player and have a team approach
Basic computer skills.
Experience preferable.
