They often wind up in the hospital and get expensive surgeries that they can&#39;t afford to pay for. These surgeries may prolong their lives, but may or may not allow them to work again. An individual who gets medical care when the problem is a small one may be able to work and pay taxes much longer than the individual who only gets care when the situation is critical. He informed me that I could not transfer the funds and they had to cancel my policy first and start all over with a new one. I would receive a check between five to 7 weeks. The claim will now go forward in the proper way and our client will be seen by a Consultant who will make a proper report on his injuries. We will then value those and ensure that the full amount of the client?s damages is paid to him. &lt;/p&gt; &lt;p&gt; The public needs to be aware of these sorts of devious tactics from Defendants. If you have a personal injury claim then never settle it with the Defendant directly or with his insurer. Always insist that the negotiations be done through a solicitor who is willing to operate on a no-win no-fee basis. What follows is a basic illustration of these stakeholders as identified by Teitbaum &amp;amp; Wilensky (2007) in their textbook, &lt;em&gt;Essentials of Health Policy and Law&lt;/em&gt;.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;&lt;em&gt;Patient&lt;/em&gt;&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;The most important stakeholder in the healthcare policy making, in this writer&#39;s opinion, is the patient. There are several factors that are of primary importance to patients regarding healthcare policy. These factors include:&lt;/p&gt; &lt;ul&gt; &lt;li&gt;Access&lt;/li&gt; &lt;li&gt;Affordability&lt;/li&gt; &lt;li&gt;Quality &lt;/li&gt; &lt;/ul&gt; &lt;p&gt;Affordable access to healthcare remains a challenge for many in the U.S. according to a 2008 United States Census report with government programs cover approximately 27% (83 million) of the entire population (U.S. Census Bureau, 2008). This same report suggests that that approximately 50 million people in this country have no healthcare insurance at all. There currently exists huge, voluntary membership organizations which represent these various stakeholders. For example, the American Medical Association is &quot;the voice&quot; of physician providers in the United States.&lt;/p&gt; &lt;p&gt;Formulation of healthcare policy, which in many cases for providers is focused on payment, is important to this stakeholder group. Medicare, which is a U.S. government operated healthcare program, is often criticized for its low payment per procedure ratio by this group (Macgillis, 2009).&lt;/p&gt; &lt;p&gt;&lt;strong&gt;&lt;em&gt;Government &amp;amp; Insurance Providers&lt;/em&gt;&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;The government, both state and federal, can be considered major stakeholders in healthcare policy making. As discussed earlier in this paper, some 83 million Americans receive some type of health coverage from governmental entities. An example of this can be found in Medicare, which is a joint health insurance program that is paid for by the Federal government and state governments. Primarily, Medicare covers people who are 65 years of age or older. According to Medicare.gov, the program currently covers over 40 million Americans (Medicare, 2010).  It makes sense that the government would be a major stakeholder in healthcare policy decision making as the government is one of the larger players.&lt;/p&gt; &lt;p&gt;Insurance providers, such as Blue Cross and Blue Shield. Aetna, Cigna and many others are also &quot;major&quot; stakeholders in healthcare policy and decision making. This writer would also like to point out that insurance providers are also very influencial in the healthcare policy and law decision making process. These corporations have billions of dollars to donate to political campains and efforts that are designed to help shape public health policy debate among the public and lawmakers.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;&lt;em&gt;Public&lt;/em&gt;&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;As mentioned at the beginning of this paper, everyone can be considered a stakeholder in healthcare policy decision making. Everyone means the general public. Much of the law regarding public health is conerned with health adminstration, wellness promotion and disease prevention. Collectively, it is the public that most major healthcare initiatives are aimed at. An example of this might be the Centers for Disease Control and Prevention urging people to recieve their annual influenza immunmization shot. Laws and policies, in general terms. are designed to promote public health and prevent illness.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;Reflective Conclusion&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;There are many stakeholders involved in public health policy formulation and decision making in the United States. These include patients, healthcare providers, the government and insurance providers and of course, the general public. It is noteworthy to point out that money has a major influence on shaping the debate of healthcare policy and law in the United States as witnessed in the recent healthcare reform that became law in 2010. There are of course other stakeholders that are on a smaller scale, such as medical equipment providers, healthcare advertisers and so forth. Subsistence allowance expenses broadly include additional travel expenses necessarily incurred but not for example fares or car allowances.&lt;/p&gt; &lt;p&gt;The cost of a business journey and meal cost is a claimable travel expense from one workplace to another including travelling between your main permanent workplace and a temporary workplace or travel to or from a certain workplace because the job demands it. Business journeys do not include ordinary commuting, home to a permanent workplace or private journeys which are not travel expenses.&lt;/p&gt; &lt;p&gt;You can claim travel and subsistence costs on the necessary costs of business journeys like public transport fares, car allowances, hotel accommodation, lodging allowances, meal receipts, tolls, congestion charges, parking fees and business phone calls. You cannot claim meal allowance or travel allowances for costs not directly related to the business journey. A daily meal allowance for a non business partner in the travel expenses cannot be claimed.&lt;/p&gt; &lt;p&gt;The entertainment expenses in travel and entertainment costs are not tax deductible travel and subsistence expenses. Entertainment expenses would be allowable expenses if for advertising and promotional purposes as genuine business expenses but not as a subsistence expense.&lt;/p&gt; &lt;p&gt;Car Mileage Allowances Tax Allowances&lt;/p&gt; &lt;p&gt;When an employee uses their own car, van, motorbike or cycle to make business journeys the employer normally pays car mileage rates payments to cover the travel expenses which are tax allowances. Provided the car allowances paid is below the maximum amount per mile travel and subsistence costs are not taxable. If your employer doesn&#39;t pay you the maximum, you are entitled to tax relief on the difference between what your employer actually pays you for your business journeys and the maximum tax free amount that your employer could have paid you for those journeys&lt;/p&gt; &lt;p&gt;Maintain accurate records of the dates, mileage and details of all work journeys incurring business mileage and the car mileage allowances payments your employer gives you. You cannot get relief for your actual expenses if they are greater than the allowed Inland Revenue mileage rate maximum per mile. The Inland Revenue mileage rate is currently 40 pence per mile for the first 10,000 miles and 25 pence per mile thereafter in each financial tax year. The 40 pence per mile car mileage rates is increased to 45 pence if another employee travels in the same vehicle on the business journey. The Inland Revenue mileage rate when using a bicycle is 25 pence per mile which is recorded the same way as the car allowances.&lt;/p&gt; &lt;p&gt;You may be able to get tax relief for business mileage when you use your own vehicle on business, or for fuel you buy when you use a company car. You can go back several years to get the relief depending on whether you have previously sent in a self assessment tax return.&lt;/p&gt; &lt;p&gt;As an employee you are entitled to vehicle mileage allowances if the employer pays less than the approved Inland Revenue mileage allowance rate. If your employer pays you more than the approved mileage rate, the excess is taxable. .The same rule on this car mileage rate travel expense tax allowance is also applicable to self employed business expenses.&lt;/p&gt; &lt;p&gt;Tax relief for fuel when using a company vehicle&lt;/p&gt; &lt;p&gt;If you pay for fuel when using a company vehicle for business travel you can get tax relief on fuel costs, less any payments repaid by your employer and covered by a dispensation as an alternative to car mileage rates. You must keep a record of your business mileage to calculate your expenses. Where relief for business travel is given at statutory mileage allowance relief rates and not for actual costs this does not prevent relief for subsistence and accommodation costs. Mileage allowance relief applies to the costs of using the employees own vehicle and does not prevent relief for additional costs attributable to business travel that are not costs of using that vehicle.&lt;/p&gt; &lt;p&gt;Travel and Subsistence Allowance Expenses&lt;/p&gt; &lt;p&gt;Travel expenses include the actual travel expenses, meal allowance, subsistence expenses and associated costs incurred as part of the cost of making the business journey. The cost of business travel includes the cost of any necessary subsistence costs incurred in the course of the journey and the cost of meals necessarily purchased whilst at a temporary workplace. Meal cost should be supported by the meal receipt to avoid problems claiming the tax allowances. Daily allowances or per diem allowance can be claimed if approved as an Inland Revenue rate&lt;/p&gt; &lt;p&gt;If an overnight stay is needed then the cost of the accommodation and any necessary meals is part of the cost of business travel. If the overnight stay is in a hotel, claim the hotel bill including the meal cost, or claim the lodging allowance of 25 pounds if the overnight stay was with family or friends, in effect daily allowances.&lt;/p&gt; &lt;p&gt;Subsistence expenses must be attributable to the business travel and incurred in the course of the journey being additional to costs if it were not for the business travel. Consequential costs not incurred in travelling but due to circumstances, for example the cost of a child minder while away from home is not an allowable travel expense. Subsistence costs do not need to take into account the costs saved as a result of the business travel. After you receive this document, you will have several days (even a few months) to gather your supporting documentation. I strongly recommend following the steps on this section to prepare your supporting documents even before they are requested by the Visa Officer.&lt;/p&gt; &lt;p&gt; However, even if you finish gathering your documents, don&#39;t send them to the Visa Office until the ask you to do so! How long can I actually stay in the US: 6 months or 10 years? &lt;/p&gt; &lt;p&gt; I cant tell you how often we get asked this question, but it has to be at least a dozen times a month. The lack of knowledge or misunderstanding about the role of a visa and the I-94 card has been a major factor for many foreign nationals that have fallen out of status in the United States. Why is this? And what is the difference between the visa and the I-94 card. &lt;/p&gt; &lt;p&gt; The simple answer: A visa allows a person to appear at the gate (a port of entry) and ask to be let in, and the I-94 card is your actual admission ticket, telling you how long you can stay and what you can do while you are here. &lt;/p&gt; &lt;p&gt; The long answer: Very few foreign nationals are aware that their stay in the US is controlled by the I-94 card issued to them at the time of entry, and not by their visa. This lack of understanding has landed many in trouble. It is the I-94 card that says how long you can stay in the US, and in what status you were admitted. Just to make sure you clearly understand this, lets look at another example: a citizen of China who wishes to enter the US as an H-1B professional can get a visa that is only valid for 3 months, however, they can be admitted for the length of the approved petition, typically 3 years. This means that the individual must travel to the US within 3 months of being issued the visa, but once he or she enters the US they would be able to stay for up to 3 years. He or she would only need to depart the US at the end of the specified period or the date specified in his I-94 card.&lt;/p&gt; &lt;p&gt; Now that we have a general overview of the I-94 and the visa lets look at the specifics of the process.&lt;/p&gt; &lt;p&gt; What is an I-94 card?&lt;/p&gt; &lt;p&gt; An I-94 card, also known as the Arrival/Departure Record, is a small white or green colored card given to all nonimmigrants when they enter the US. The I-94 card serves as evidence that a nonimmigrant has entered the country legally. It is stamped with a date indicating how long the nonimmigrant may stay for that particular trip. It is this date and not the expiration date of the visa that controls how long a nonimmigrant may legally remain in the US. A new I-94 card with a new date is issued each time the nonimmigrant legally enters the US. Note that because of special rules in place between the US and Canada, Canadian visitors are not normally issued I-94 cards.&lt;/p&gt; &lt;p&gt; When you enter the US as a nonimmigrant, a US Immigration Inspector will examine your passport and visa, question you on the reason for your visit, and, if satisfied that you qualify for the nonimmigrant status you are seeking and will abide by the US immigration rules, give you a small white card, the Form I-94 (Arrival/Departure Record). As we stated above, it is this record -- not your visa that tells you (in the lower right-hand corner) when you must leave the United States. &lt;/p&gt; &lt;p&gt; It is on this card that the Immigration Inspector will write either a date or &quot;D/S&quot; (duration of status). If you have &quot;duration of status&quot; you may remain in the U.S. as long as you are in the same job or attending the same school. You will complete this form and submit it to the Immigration Inspector when you arrive at the airport, seaport, land border crossing point, or other port of entry. Citizens of the countries on the US Visa Waiver Program list, who are entering the United States for a short stay and who are not US citizens or Permanent US Residents, must fill out form I-94W instead of the I-94. &lt;/p&gt; &lt;p&gt; You have to be very certain that all the information in your I-94 Card is correct, and that your name, date of birth, and country of citizenship match the information in your passport. The I-94 card will be stapled in your passport, normally on the page opposite your visa.&lt;/p&gt; &lt;p&gt; What is the purpose of I-94 card: &lt;/p&gt; &lt;p&gt; The I-94 Card serves as the registration form for foreign nationals admitted to the US as nonimmigrants. This document is created by US Immigration when the foreign national is inspected upon arrival at a US port of entry. The Immigration Inspector will endorse the I-94 with the date of arrival, place of arrival, status granted (i.e., F, J, H, L, etc.), and length of authorized stay. The foreign national keeps the I-94 Card as their official record of admission and permission to remain in the US.&lt;/p&gt; &lt;p&gt; Difference between I-94 and Visa:&lt;/p&gt; &lt;p&gt; It is extremely important that you clearly understand the difference between your visa and I-94 card. At the time of this writing you will find that processing for visits take between 2 and 3 hours. This is totally unacceptable especially when this prison is less than 30 miles from theCanadianborder.&lt;/p&gt; &lt;p&gt;Processing starts at 7AM - visting ends at 3PM - however the hospitality centers opens at 6AM you cannot be on the property before this.&lt;/p&gt; &lt;p&gt;New York State has 3 large prisons in Malone, NY - Upstate Correctional, Bare Hill Correctional and Franklin Correctional. Within 1.5 hours of Malone there is an additional 3 large prisons. Clinton Correctional, Altona Correctional, Chateaugay Correctional. There maybe more I do not even know about. I could write an entire article about the ignorance of why New York State would even put this many prisons in this area when the population of inmates in these prisons comes primarily from New York City and other large upstate cities like Albany and Buffalo. the families all have to travel to this far off town of Malone in order to visit loved ones who are incarcerated. The cost of this trip is considerable. Many children do not get to see there fathers or mothers for months at a time. In theseeconomictimes when gas is over 3.00 a gallon the cost isenormous for families of inmates who have to travel 8 or 9 or 10 hours.&lt;/p&gt; &lt;p&gt;Bare Hill is considered one of the cookie cutter prisons in NYS - it was built during the 1980&#39;s. The visiting room is half the size it ought to be. At some point they doubled the size of this prison but never doubled the size of the visiting room. At busy times visits will be terminated to let more visitors in. After driving all night this is not acceptable!&lt;/p&gt; &lt;p&gt;These cookie cutter dorms hold 60 men. Many prisons across NY state are empty like Fishkill with entire dorms with no one living in them but yet they are heated. Why is it that we need to ship people to theCanadianBorder? When so many prisons downstate go empty? New York State Corrections needs to look at why this is. If they don&#39;t I am certain our newGovernorCuomo will be!&lt;/p&gt; &lt;p&gt;Bare Hill is a transient prison. People come and go in this prison. Some are there for a bit but the majority will only be there for short times. You will find that most people either are coming out of S Block, parole Violators or Sex offenders but there are new people to the system. This prison seems to be adisciplinaryprison for the most part. I am seeing that the majority of visitors to this prison are first time visitors to a New York Correctional Facility.&lt;/p&gt; &lt;p&gt;After traveling the majority of an overnight trip you want to make the very most of your time allowed to visit. There are many places where this can be done. Educating first time visitors would be extremely helpful. Having an area where children can play or watch tv during these long waits. Children should not be expected to be quiet and well behaved during this time. These are children who are waiting to see there Dads, Uncles,Friends,Grandfathersthese kids are excited by this!&lt;/p&gt; &lt;p&gt;Please do what you can and read below so the Bare Hill CorrectionalVisitingprocess can be spead up. Waiting for hours on end is grueling after a long long car or bus ride up there and we all want to see our loved ones for as long a visit as possible!&lt;/p&gt; &lt;p&gt;&lt;strong&gt;Bare Hill Correctional Information for Visitors&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;Make sure you know which day you are allowed to visit. Ask your loved one for a schedule before you go up there.This changes from week to week according to the inmates DIN&lt;/p&gt; &lt;p&gt;When you arrive at Bare Hill Correctional check in at the Hospitality Center which is located as you pull in to the facility it is on your right.&lt;/p&gt; &lt;p&gt;You will sign in and the staff will give you a pass with a number on the corner make a notation when you sign in if you have a package - Fill out the pass completely&lt;/p&gt; &lt;p&gt;--------------------------------------------------------------------------------------------------------------------&lt;strong&gt;If you are a first time visitor&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;You will need 2 forms of ID  one must be a government type ID with picture (license, passport, DMV ID etc)&lt;/p&gt; &lt;p&gt;&lt;strong&gt;You will need to fill out the first time visit form&lt;/strong&gt; &lt;strong&gt;which you must get in the hospitality center prior to going to the Administration building &lt;/strong&gt; have this completely filled out&lt;/p&gt; &lt;p&gt;--------------------------------------------------------------------------------------------------------------------&lt;strong&gt;If you have a package&lt;/strong&gt; wait for your number to be called then bring your package up to the officers in the hospitality center for an initial check.&lt;/p&gt; &lt;p&gt;You will wait once again till your number is called in the hospitality center&lt;/p&gt; &lt;p&gt;Numbers are called in groups of five - when called you will proceed to the Administration building&lt;/p&gt; &lt;p&gt;&lt;strong&gt;TO SPEED UP PROCESSING FOR EVERYONE&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;&lt;strong&gt;***Please get to the Admin Building as fast as possible&lt;/strong&gt; otherwise you are holding everyone up behind you.&lt;/p&gt; &lt;p&gt;When your number is called if you came by bus (only) and you need a locker please inform the officer they will get it for you.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;Remove all jewelry, belts, anything you can remove that is metal and empty your pockets in the hospitality center prior to going to Administration.&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;Once your pass number is called you will proceed to the Administration Building where you will be processed - this building is located across the parking lot from the hospitality center.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;*****You will not be allowed entry to the facility if the metal detector goes off*****&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;If you are wearing an underwire bra or any metal you will be required to remove it if the detector rings.&lt;/p&gt; &lt;p&gt;You will be provided with clothing to go through the detector then change again back to your clothing.&lt;/p&gt; &lt;p&gt;This holds us all up it is suggested you wear a sports bra with no hooks and no metal on any clothing.&lt;/p&gt; &lt;p&gt;When visiting any NYS Correctional Facility bring a change of clothes just in case!&lt;/p&gt; &lt;p&gt;If you came by car bring your keys to the administration building and you will be provided with a bag to put them in.&lt;/p&gt; &lt;p&gt;When you arrive at the Administration Building you will be asked to be seated till your number is called.&lt;/p&gt; &lt;p&gt;&lt;strong&gt;Have your ID and pass ready&lt;/strong&gt;&lt;/p&gt; &lt;p&gt;When you are called you will sign into the log book hand your pass and ID to the officer at the desk. You will wait till he checks the computer and enters your information.&lt;/p&gt; &lt;p&gt;The officer will then tell you when you can go to the metal detector.&lt;/p&gt; &lt;p&gt;Before going through the detector ask for a bag for your keys if needed.&lt;/p&gt; &lt;p&gt;Remove everything from your pockets, jewelry money etc and place it in the wooden box provided.
