The law includes numerous safeguards to ensure sound medical treatment and that decisions are made consistent with the patient’s wishes and best interests. The current law applies only to patients in general hospitals or residents in nursing homes; it does not apply to people at home, in assisted living facilities, or in hospice care. There is, however, a separate section on out-of-hospital do not resuscitate (DNR) orders.&lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;This act establishes a process for determining whether an adult patient without a health care proxy has decision-making capacity. If an attending physician and a consulting physician agree that the patient lacks the capacity to make health care decisions, the law establishes a priority list for choosing a person to be the surrogate decision maker. They boarded the plane in Binghamton never expecting that upon take off the plane would crash. Helen’s friend was among several people killed in the crash. After being rescued, she spent three months in a local hospital recovering from extensive injuries. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;Then at the age of 102, she survived the flood in 2006 that devastated the village of Sidney. She had to flee from her beautiful historical home to get to safety. After the flood, it took time but she was able to return to her restored home in its original splendor after suffering extensive damage. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;February of this year proved to be very special. It was the month that Hospice received a lovely gift. Helen became our patient. She was a composite of wisdom, wit and old fashioned charm. She was forthright, amusing and had a wonderful sense of humor. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;Helen attained professional goals at a time in history when most women did not pursue a college education. It did not take her long to decide that she wanted to follow in her mother’s footsteps. She was educated at Potsdam Teachers College. Her first teaching assignment was in a one room schoolhouse in Gouverneur, New York. Mrs. Weeden accepted a second teaching assignment in Sidney shortly after her position in Gouverneur. She remained in Sidney and taught 3rd grade for many, many years. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;She was a pioneer in managing the concept of a woman “Having it All”- career, home and family. She met her husband shortly after arriving in Sidney. She was married to her husband for 40 years before being widowed. Her daughter Esther lives in California and son Edward passed away on her 90th birthday. She was also a grandmother of two grandchildren and a great grandmother of three. A multi talented woman, her many gifts included writing poetry and painting. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;When I first met Helen, she was sitting in her favorite chair reading Ogden Nash in Latin. She shared with a smile that although Nash was interesting, her favorite book of all time was “Little Women” by Louisa May Alcott. Helen proved to be just as interesting on a personal level as was her reading material. I was curious to know whether or not she had some secret to being able to attain the wonderful age of 107. I was expecting a revelation about some magical herbs or special foods. She freely disclosed her recipe for life with a huge grin on her face, “Just be happy.” She added that it is important to do all things in moderation. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;Beloved by her family and as a friend to many, Helen would use her very special writing talents to honor an individual with a poem carefully written just for them. There is an old fashioned guest book that is carefully placed on an antique podium in her parlor. The yellowed pages reflect the many people who were guests in her home and who had left their names and messages over the years. The grace of this simple practice from days gone by defines the elegance and charm of this very special woman. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;The New York State Retired Teachers Association awarded her the honor of being the oldest living retired school teacher in New York State. She also received recognition from the New York State Assembly for her 107th birthday. &lt;/div&gt;&lt;div class=&quot;MsoNormal&quot;&gt;Helen was cared for by her beloved caregiver M&amp;amp;M. She had been with Mrs. Weeden for more than 25 years. They had a loving rapport that was carefully nurtured and a mutual respect that was truly enviable. Palliative Care is pleased to announce that on July 22, 2009, Dr. Patrick Dwyer was appointed as our first full-time Medical Director and Chief Medical Officer.  Dr. Dwyer will be responsible for overseeing the medical aspects of our patient’s care and will serve as liaison with members of our local medical community.&lt;br /&gt;&lt;br /&gt;Dr. Dwyer has been affiliated with Catskill Area Hospice and Palliative Care since 1989 when he became an Associate Medical Director and volunteered to attend interdisciplinary team meetings and perform patient consultations as needed.  Dr. Dwyer cited that the people at hospice attracted him to joining our team.  He stated, “We have a unique combination--a talented group of people that not only offer a great service but that have a lot of love and a lot of heart.”&lt;br /&gt;&lt;br /&gt;Dr. Dwyer is Board Certified in Internal Medicine, Medical Oncology, and Hematology.  He earned his undergraduate degree from the University of Notre Dame, and his medical degree from Washington University (St. Louis).  Dr. Dwyer completed his medical residency at The Johns Hopkins Hospital.  Dr. Dwyer also trained as a Clinical Associate at the National Institutes of Health in the National Cancer Institute.  He completed his medical oncology training and hematology fellowship at The Johns Hopkins Medical Institutions.  After his training, Dr. Dwyer also served with distinction as a hematologist-oncologist for 25 years at Bassett Healthcare.&lt;br /&gt;&lt;br /&gt;Both of Dr. Dwyer’s parents were served by hospice and he has had a long commitment to the philosophy and mission of hospice. Certainly the concept of informed choice is not a partisan principle. All Americans would agree that they want to have the freedom to make their own decisions regarding their health care, especially at the end of their lives.&lt;br /&gt;&lt;br /&gt;The problem with the present system is that often information about end of life options is withheld, for a variety of reasons, until the person is literally days from drawing their final breath.  At that moment the person usually is not able to make his/her own decisions and desperate grieving family members are forced to decide what to do.&lt;br /&gt;&lt;br /&gt;Hospice is based on the philosophy and belief that people with a serious life threatening illness deserve an informed choice regarding treatment options as their illness progresses.&lt;br /&gt;&lt;br /&gt;In my past 18 years of Hospice work I have had literally thousands of letters and satisfaction surveys from hospice family members stating, “We wish we had known about Hospice sooner.”  I am pleased to report that this has improved as more and more, people are becoming better educated about the benefits of hospice and as a result, are more comfortable and confident as they reach out to us earlier.&lt;br /&gt;&lt;br /&gt;We are finding that the truth about end of life care is that when people are educated about the benefits of hospice before they actually need it, and if they are referred to hospice early enough to enjoy the many services offered, they often live longer, fuller lives than those not receiving hospice.&lt;br /&gt;&lt;br /&gt;Sadly, those not receiving hospice services spend much of their last precious year of life between doctor’s office visits, trips to the ER, and hospitalizations where they not only expend their waning energies but may also be exposed to infectious diseases that target the frail and weak.&lt;br /&gt;&lt;br /&gt;The truth is that advance care planning puts you in charge of what happens to you in case you become ill.  Advanced care planning gives us this “Freedom of choice” opportunity that we all have, regardless of political debate. For five years he had been on oxygen. He had COPD and heart problems. The doctor informed us that his vital organs were shutting down, so as a family we gathered in the IC Unit of Fox Hospital and said our teary goodbyes. But Dad was not ready to die. After ten days in the hospital, he was not better, but stable. There really wasn’t anything more that could be done for him except keep him as comfortable as possible. Over the years, my dad confided in my mother and best friend of 57 years, that if possible he would prefer not to go to a nursing home. Yet, his biggest concern was how much of a burden his day to day care would be to my mom.&lt;br /&gt;&lt;br /&gt;Before being discharged from the hospital, someone from hospice came to speak to us. She explained the type of services that Hospice could offer my father and our family. We left the hospital with peace of mind and a plan of action. My dad would be able to stay at home, watch TV and read his papers in his own chair. At night he could sleep in his own bed. The Hospice staff and nurses were incredible. They made regularly scheduled visits to my parent’s home. These visits were crucial to my dad’s physical and mental well being and equally important to my mom, his primary caretaker. It allowed her time to run an errand without worrying about dad.  She knew there was someone who would be able to give her a break when needed; someone who understood not only what my dad was going through but the anxiety, fear, weariness and even the occasional guilt mom would feel. There were days when my father became irritable. This was hard on my mother. The Hospice staff would reassure her that this was normal. It helped my parents to talk to someone outside of the immediate family.&lt;br /&gt;&lt;br /&gt;These (Hospice) people have an uncanny insight that the average person does not. Without interfering during such emotional times, they were able to be present in the way that we needed them to be.  My father said he appreciated their candor.  No one ever sugar coated the truth. He understood that he would not get better, but would be kept safe and as comfortable as possible. Their honesty and objectivity prepared him for this journey in a way that the family could not.&lt;br /&gt;&lt;br /&gt;My mom was forever asking, “What would we do without Hospice?”  My dad would reply, “That we would probably be making many scary trips to the emergency room in the middle of the night. With hospice, we can call and speak to someone with expertise who knows my condition.”  The hospice phone number was taped strategically throughout the home. We didn’t hesitate to call.  The comfort of knowing that someone with expertise and sensitivity was always available to us made a world of difference.&lt;br /&gt;&lt;br /&gt;Dad was continually amazed by the computer system that the workers used and he trusted that whoever spoke to him was looking at his records and knew about what was going on with his personal situation. He found this coordinated effort on his behalf priceless. The medication management service Hospice provides is phenomenal, especially for the elderly. Our family task of ordering, renewing and picking up prescriptions, equipment and supplies was eliminated.  Hospice did all of that for us, allowing us to focus on spending quality time together as a family.&lt;br /&gt;&lt;br /&gt;Hospice eliminated what would have been major obstacles in my parent’s daily lives. My Mom would have been terrified if she had to transport Dad to doctor visits or labs for the bi-weekly blood work.  With hospice, Dad’s care was handled in the comfort of their own living room.  Most of all, Dad would remind us that he probably would have had to be placed in a nursing home if not for Hospice.&lt;br /&gt;&lt;br /&gt;One of the nurses told me something that will forever echo in my thoughts.  We would often ask, “How do you do this?”  She said, “When I get to a door, there is no way of knowing what I am walking into, so, I look up to heaven and say, I’m not doing this alone, you’re coming in with me.” She said she always tells people, “I cannot spare you the inevitable, just make you all feel safer as it comes.”&lt;br /&gt;&lt;br /&gt;We will be forever grateful for the experience of being right by Dad’s side in his own living room when we convinced him it was OK to go.  We let him know that we were all safe and would take care of one another. On that morning my Dad took his last breath in his own home, in his favorite chair, surrounded by family.  The plan Hospice laid out for us in that hospital room in April 2008 had brought us all through safely and together to September 20, 2009, Dad’s last day home. Camp Forget-Me-Not is not just for hospice families. Camps are open to any school-aged child or teen in Otsego, Delaware or Schoharie counties who has suffered a personal loss. Even a friend who has lost a friend is welcome. We recognize that loss outside the “family” can be just as profound as a loss within the family. &lt;br /&gt;&lt;br /&gt;In any family system, if one benefits, all benefit. Parenting today is already challenging. When a loss occurs, parents are now confronted with the challenge of effectively parenting their grieving children. Many parents aren’t prepared for this and haven’t gained the knowledge to confidently deal with this significant issue.&lt;br /&gt;&lt;br /&gt;It’s comforting to know that their children are in the care of grief specialists and in most cases, relieves them of some of the pressure. If a child/teen is better able to deal with their loss, so will the parents and family members. During camp, education for parents or guardians about child/teen grief is provided to better equip them to face the challenges ahead. “Healing Circles” are each run by a professional with training and experience in child and adolescent grief. &lt;br /&gt;&lt;br /&gt;Summer camp will take place on June 23-24 in Oneonta with the fall camp on October 3-4 in Delhi. There is no cost for the camp or for anything provided during the camp. Why aren’t the birds singing this morning? How do I build a house? What is that bright light in the evening sky? How do I build a bridge so that it stays up? The ways of the physical world, the natural and built environment make sense to him.  If he doesn’t know the answer to your question he will figure it out.&lt;br /&gt;&lt;br /&gt;Richard Nichols, better known as Rigger, even has a hunch as to why he developed ALS, or Lou Gehrig’s disease. For the last five years, Rigger has been dealing with the symptoms of ALS. &lt;br /&gt;&lt;br /&gt;His entire life has been one of seeing and doing with his hands; applying innovative solutions in a design-and-do-it-yourself way. He collects and loves tools. His body has been his major tool and his appropriate nickname attests to that.&lt;br /&gt;&lt;br /&gt;Now he cannot use his arms and hands. Lifting his head is difficult and the prospects are not optimistic. Mentally he is quick and bright. His home is filled with innovative devices that he has designed, enabling him to live as normally as possible: pulleys, supports, levers, lifts, high and low stools, chairs that turn into couches and installations for massage.  Even though he can no longer assemble them himself, he designs and supervises the work performed by one of his many handy neighbors. &lt;br /&gt;&lt;br /&gt;Working since he was 14 years old, Rigger, now 75, would still be working if he had the physical capacity to do so. During his 60-year-long career he has been a farmer, house and silo builder, rigger, welder, electrician, real estate agent, fisherman, organizer of outreach services to homeless people, lover of nature and people.&lt;br /&gt;&lt;br /&gt;Rigger lives at home and is supported by Shirley Grandsbury, his live-in caregiver. Rigger has been receiving hospice care since February 2008 by a team of caregivers and volunteers, who just love him to pieces. And he loves &lt;br /&gt;them!  Hospice support services have made a significant difference in the quality of his life. &lt;br /&gt;&lt;br /&gt;Rigger doesn’t respond to conventional medications very well and takes as little as possible. Therefore, hospice looked for solutions that would enhance his comfort. Touch therapy has proven to be one that he finds very beneficial in reducing the pain, cramping and stiffness that are his constant companions. &lt;br /&gt;&lt;br /&gt;When Rigger first began to receive massage therapy he could not lift his head. His jaw was so tight that he could not chew food. He drank his meals. Following a few massage sessions he could lean his head back. He could open his mouth wide enough to receive food, chew and swallow. Once again he was able to go to summer picnics and share a meal of beans, potato salad and hot dogs with his friends.  Beats Ensure!&lt;br /&gt;&lt;br /&gt;For some hospice patients, movement can be difficult and minimal.  With effective touch, the blood circulation is enhanced, muscles relax and warm, joints become more flexible, movement is eased and comfortable, anxiety and stress diminish.  In order to expand hospice’s capacity to bring touch therapies to their patients, a special program had to be developed. &lt;br /&gt;&lt;br /&gt;It has been my privilege to develop our “Palliative Touch Program” which gives massage therapy to hospice patients. This program will also provide training to hospice Home Health Aides and LPNs so they can incorporate this welcomed service with their patients on a needed basis. We were very fortunate to have received a $10,000 grant from The Community Foundation for South Central New York to help fund this valuable program.  &lt;br /&gt;&lt;br /&gt;You may be wondering about Rigger’s hunch as to why he got ALS. He was hit by lightning and jolted 15 feet to the ground off a pyling rig. There is some evidence, although not definitive, linking ALS with lightning. In the meantime, hospice will continue to care for and love Rigger. Their every touch, word, and action is focused on providing comfort and support to their patients and families. This kind of excellence reflects the tremendous growth in the number of hospice families we serve on a daily and annual basis. &lt;br /&gt;&lt;br /&gt;Last year, in caring for 819 hospice patients, 52,000 staff visits were provided, plus an additional 3,000 visits by our volunteers.  We have become the most successful hospice organization in New York State by serving 40% of the patients who are eligible for hospice services in our service area, far outreaching the average NYS hospice at 22%.  &lt;br /&gt;&lt;br /&gt;We also have the most successful Nursing Home Hospice Program in the state. Working as a partner with 5 area nursing homes, an average of 55 nursing home residents per day receive the very best care and compassionate one-on-one attention. &lt;br /&gt;Over the past 25 years, we have grown from a small volunteer agency (the Love Committee) to the 11th largest employer in our area.
