The Central Coast Local Health District has recently released its “ Caring for our Community Plan 2021-2031”. The Plan sets out the blueprint and roadmap for community health services across the Central Coast over the next ten years. It was developed in consultation with over 200 community members, staff, general practitioners, community services and external partners over the past 12 months.

The Plan acknowledges the important trends and shifts occurring towards community based health care and supports the health and well-being of the community. Community health services often work in partnership with other health and related services such as non-government organisations, general practice and private providers.

Community health services are an integral part of health care on the Coast and play a significant role in keeping people healthy and well in the community. This lessens the need for impatient care and supports early discharge from hospital.

A copy of the plan can be viewed here
