Step-1

Place the Bobbi Baby Shield over the operation site, using the inspection port for positioning.

Step-2

Remove the release Liner from the Bobbi Shield to expose the adhesive which will stick to the diaper when closed.

A key principle employed by the BoBBi Baby Shield is to prevent the ingress of faecal matter onto the surgical site post Hypospadias repair or Circumcision

The BoBBi Baby Shield is designed for use primarily on male infants undergoing genital surgery, typically hypospadias repairs and circumcision.

The BoBBi Shield is designed to protect the surgical area providing a space for healing to take place. The key principles employed by the BoBBi Shield are to prevent the ingress of faecal matter onto the surgical site and to remove direct contact with urine from the diaper. Contact with a urine soaked diaper causes meteal ulceration. 30% of circumcision complications are due to meteal ulceration.

In addition to the above the BoBBi Shield provides protection and comfort to patients, keeping the area snug and dry during the healing process while allowing the children freedom of movement.

Circumcision care post circumcision surgery and recovery. BoBBI BABY SHIELD keeps your baby boys genitals comfortable, clean and infection free.

Hypospadias Repair or Circumcision care for infants

Any surgical intervention to an Infant boys penis, be it Circumcision, Epispadias repair or a Hypospadias repair, requires post surgery infection control. These procedures also leave some particularly sensitive tissues open to diaper contact, causing discomfort and pain to the boys penis.

Any infection to the penis can cause swelling, inflammatory reactions and the infant distress. This in turn delays the recovery period. In some cases this leaves the newborn’s penis open to further complications requiring further medical intervention.

The BoBBi Baby Shield was developed with the assistance of leading Urology and Paediatric specialists to address these areas of concern.

Bobbi Baby Shield forms a barrier to create distance between the genitals and the diaper helping alleviate the pain and irritation caused by diaper contact with the healing penis.

BoBBi Baby Shield,as a result of its anatomical shape, which includes covering the scrotum, prevents the entry of faecal matter onto the genital area.

Protect your newborn’s penis with the BoBBi Baby Shield?

If your child has under gone a Hypospadias Repair or Circumcision, the Bobbi Shield helps prevent penis infections and promotes comfort in post circumcision surgery and it ensures the comfort and Genital Hygiene of the child.

The moulded structure of the BoBBi Baby Shield which is “worn inside the diaper”, creates a physical barrier between the newborn boy’s genitals and the diaper. This create a space for healing to take place.

The Bobbi Shield is lightweight ensuring maximum comfort for the baby. The material is also flexible allowing for body position and movement from the infant.

Using the BoBBi Baby Shield during the post surgical recovery period allows for the baby to be ‘burped’ or winded either over the shoulder or on the lap, without causing discomfort and physical pressure to his circumcised penis.

Faecal contact is minimised from the surgical site and bandage, reducing the risk of Infection.

The Bobbi Shield hypoallergenic material allows for the transmission of fluids into the diaper.

The Bobbi Shield material is absorbent, attracting and holding significant fluids whilst still remaining in a shaped form.

Protecting thousands of baby boys worldwide

BoBBi Baby Shield is trusted by leading Paediatric Surgeons, Doctors, Nurses and Hospitals in the post operative care for Circumcision surgery or Hypospadias repair

Health benefits of newborn male circumcision outweigh the risks.

The medical benefits alone may not outweigh other considerations for individual families. The medical data show that the procedure is safest and offers the most health benefits if performed during the newborn period. The AAP policy recommends infant circumcision should be performed by trained and competent providers, using sterile techniques and effective pain management.

Parents who are considering newborn circumcision should speak with their child’s doctor about the benefits and risks of the procedure, and discuss who will perform the circumcision. “It’s a good idea to have this conversation during pregnancy, and to learn whether your insurance will cover the procedure, so you have time to make the decision,” said Dr. Blank.

The AAP believes the health benefits are great enough that infant male circumcision should be covered by insurance, which would increase access to the procedure for families who choose it.

Circumcision Policy Statement

ABSTRACT

Male circumcision is a common procedure, generally performed during the newborn period in the United States. In 2007, the American Academy of Pediatrics (AAP) formed a multidisciplinary task force of AAP members and other stakeholders to evaluate the recent evidence on male circumcision and update the Academy’s 1999 recommendations in this area. Evaluation of current evidence indicates that the health benefits of newborn male circumcision outweigh the risks and that the procedure’s benefits justify access to this procedure for families who choose it. Specific benefits identified included prevention of urinary tract infections, penile cancer, and transmission of some sexually transmitted infections, including HIV. The American College of Obstetricians and Gynecologists has endorsed this statement.

Post Circumcision or Hypospadias Infection Control

The Bobbi Baby Shield is placed over the circumcision or hypospadias site at conclusion of the procedure. The shield is correctly positioned with the apex of the triangle facing downwards between the infants legs and includes covering the scrotum as well as the newborn’s penis.

The inspection port or hole in the shield enables visual positioning of the shield ensuring the penis and scrotum are securely covered by the shield.

The Bobbi shield is a single use device which should be discarded at each diaper change. The shield has glue spots on the outer surface covered by a removable paper liner. This liner should be removed to expose the glue which causes the shield to adhere to the diaper. This holds the shield in position and facilitates removal of the soiled shield at each diaper change as the shield is automatically removed with the diaper. The shield is discarded in the same waste stream as the diaper.

The shield should be utilised until there is a closure of the wound with intact skin in place. This will vary in time duration but such healing should be complete within a day or two following a circumcision surgery. The type of procedure utilised by the physician will also determine the duration of healing and such advice should be requested from the attending doctor.

Hypospadias surgery is generally more complex and the consequent healing period is significantly longer than that of a circumcision. The shield could be utilised for a five to seven day period.

The shield is presented in packs of five allowing for flexibility in purchases to match the surgery that has been conducted.

The Bobbi Shield is designed as a single use device to be used and discarded with each diaper change.

Foreskin removal caused significant changes in the bacterial colonization or microbiome on the penis, suggesting circumcision offers some protection against HIV

Study examines Bacteria Changes Following Circumcision

Excerpt from – Genomics Research Institute

PHOENIX, Ariz. – April 16, 2013 – Male circumcision reduces the abundance of bacteria living on the penis and might help explain why circumcision offers men some protection against HIV, according to a study led by the Translational Genomics Research Institute.

Removing the foreskin caused a significant shift in the bacterial community or microbiome of the penis, according to a study published today by the online journal mBio.

This international collaboration focused on 156 men in Rakai, Uganda – part of the world’s largest randomized-controlled trial on male circumcision. Researchers showed that men who were circumcised as part of the study had 33.3 percent less bacteria on their penis than those who remained uncircumcised one year after the study began.

Researchers further showed that the decrease was primarily found in 12 types of bacteria, most of which were intolerant to oxygen.

Past studies have shown that circumcision reduces female-to-male HIV transmission, among other benefits. This study suggests a possible mechanism for HIV protection – the shift in the number and type of bacteria living on the penis. Further studies will have to be done to demonstrate that a change in the penis microbiome can help reduce the risk of HIV transmission, according to the authors.

ABSTRACT for Bacteria Changes Following Circumcision.

Male circumcision reduces female-to-male HIV transmission. Hypothesized mechanisms for this protective effect include decreased HIV target cell recruitment and activation due to changes in the penis microbiome. To quantify the effects of male circumcision, we compared the coronal sulcus microbiota of men from the control (n = 77) and intervention (n = 79) groups at enrollment and year-1 from a randomized-controlled trial in Rakai, Uganda. We characterized microbiota using16S rRNA gene-based qPCR and pyrosequencing; log response ratio (LRR); Bayesian classification; non-metric multidimensional scaling (nMDS); and permutational multivariate analysis of variance (PerMANOVA). These analyses revealed that circumcision profoundly altered the male genital microbiota. At baseline, men in both groups had comparable coronal sulcus microbiota; however, by year-1, circumcision decreased total bacterial loads and produced less diverse microbiota with fewer dominant taxa. This loss in biodiversity was characterized by reduced prevalence and absolute abundance of 12 anaerobic bacterial taxa. Although circumcision also increased the aerobic bacterial taxa, these changes were minor compared to the decreases in anaerobes. Characterizing microbial communities in terms of absolute abundance revealed important changes that would have been obscured by a simple relative abundance approach.

Hypospadias is a birth defect of the penis that may require surgical intervention. Hypospadias repair involves relocating the urethra to the tip of the penis.

Hypospadias Explained

Hypospadias is a birth defect of the penis which is increasing in frequency. The condition involves the urethra, the tube carrying urine from the bladder to the tip of the penis. In hypospadias, a problem occurs during the development of the penis which results in the urethra opening on the under surface of the penis rather than at the tip.

Untreated, this condition can cause significant penile problems for the growing boy.In the case of major hypospadias, he cannot urinate normally, often having to sit in order to get the urine into the toilet. Comparing with peers can lead to psychological problems.

Types of Hypospadias

Subcoronal: The urethral opening is located on the underside of the penis below the glans.

Midshaft: The urethral opening is located on the underside of the penis about the middle of shaft.

Penoscrotal: The urethral opening is located at the bottom of the penis shaft, near the testes.

A condition called epispadias is similar to hypospadias, except that the opening of the urethra is on the top surface of the penis. Hypospadias is more common that epispadias.

Epispadias is a rare condition, whereas hypospadias is the most common birth defect 1:250 male infants.

Clinical motivation for a Genital shield was stimulated by incidence of sepsis post Hypospadias repair, brought on by the colonisation Staphylococca neonates

The attached medical publications indicate the following:-

There is an incidence of sepsis post Hypospadias repair and circumcision.

Most septic cases are minor but there are cases of more severe infection resulting in significant morbidity and in some cases mortality.

Staphylococcal colonisation in neonates.

This staphylococcus colonisation takes place in either the nursery or outside of the hospital. There is a link between physical hand contact and staphylococcus transmission.

Male infants experience a higher level of staphylococcal colonisation than female infants.

Circumcised infants have a higher incidence of staphylococcal colonisation than uncircumcised infants. This may be due to the additional physical contact required before, during and after the procedure.

Whilst the Hypospadias repair or circumcision procedure is carried out under sterile conditions, subsequent handling and care of the penis may be carried out under non-sterile conditions.

There is general agreement that a concerted effort to minimise infection risk is required.
