		<description>Hello, thank you for posting this blog.  I am first year grad.  During my internship, I had experience terrible anxieties for completing DAP notes.  I finally made it through the skin on my back.  However, I too become confused as to what exactly goes where and and how much!  Nevertheless, I am very happy to have found your website.  I wrote everything about all of the different types of notes and its definitions.  For me, this is a blessing because, I fully comprehend exactly what I need to do a new employee in writing an effective note that will be clear and readable and keep me working in the public.  Oh, in addition, thanks for reminding (most probably have forgotten) us about the notes can be subjective to court review.  Thanks so much.  Terrilynn</description>
