Hi, I recently finished grad school in counseling... I'm trying to find a symptom checklist like the one you mentioned, so I can better organize my notes during the day. Would you mind posting the one you use? Or letting me know where else to look?

Thanks!

Bob,
Some would say the opposite of OCD is ADHD: can't focus on anything and nothing gets finished.

Rick,
Your wife's a funny lady!

My wife says that if you have an OCD, you can't leave it at that. It needs to be CDO. (alphabetical order)

I've never argued with taking penicillin for an infection, so I've never argued against taking meds for other sorts of ailments.

I'm cautioned to go easy on the Afrin for nasal congestion, so why shouldn't I go easy on ALL meds, and not take more, or for longer than, the duration of the condition?

I enjoy the different kinds of notes.

We use PIE notes.
P - Problem (addressing with therapy)
I - Intervention (used by therapist, more objective)
E- Evaluation (of response of client- more subjective )
