Hi, your script is exactly what I need but unfortunatley is not quite working. The form is there but when I click submit ti then goes to an error page advising that the url could not be found. Any help would be appreciated. Alternatively if you need other info then please contact me. Cheers

I'm having the same problem, I believe (but maybe wrong) that it has something to do with the class_phpmailer.php. This has a number mailing protocols, one of which is smtp which I use for e-mail. I think these need to be set-up inside the file. If somebody can confirm this is right and how to do do it I would appreciate a response, thanks.

I'm a newbie at creating web pages, php, and mySQL, but I believe you have to create the database yourself, the membersite_config.php only creates the table.

If you have set-up a localhost server (xampp for example) you can create a database through the phpMyAdmin function. If you have a hosting package then this should be done through the your host control panel. The host needs to be compatible with php and
mySQL.

You don't need to call the database testdb, you can call it anything you like, within reason and the mySQL parameters. All you then need to do is put the database name in the appropriate line of the membersite_config.php file.
