Very good report, but more detailed information about researches methodologies, especially about statistical analysis of study results are needed.

I would like to share some my ideas:

Ideas for TB researches

Basic/Fundamental researches:

•	To create specific primers and molecular probes for detection of Isoniazid (and/or Z, E, IA, Fq..) resistance conferring mutations by automated molecular diagnostic (Gene Xpert MTB) technology.

•	To identify the M. tuberculosis genes which are responsible to induce synthesis of enzymes for all components of Mtb's cell wall; To find difference in cell wall structures of Latent and Active forms of M. tuberculosis.
•	Observation on changes in quantity of specific immune cells (e.g. Th1, INF-µ, IL or TNFα factors) during the treatment of TB/HIV(-) and TB/HIV(+) cases.
Clinical Trials:
•	Participation in Clinical Trials of New TB drugs (e.g. Delamanid (OPC-67683), PA-824, TMC-207 or SQ-109) and Novel treatment regimens.
Operational Researches:

•	To define reasons of DST profile changes in time of M/XDR-TB standard treatment.

•	To compare treatment outcomes in M/XDR-TB New and Previuously treated cases (including pediatric M/XDR-TB cases, with accent on all Bacteriological, Clinical and Epidemiological aspects of M/XDR-TB management).

•	To compare results of all used standard TB diagnostic tests (routine and new/rapid) in any specimens of PTB and EPTB cases (including pediatric TB cases).

Ready to discuss all aspects. In my opinion, realization of web based international network between research institutions will help.

sincerely,

Nelly Slomonia

I'm at a real loss for words. Profoundly sad. He was one of the first people I met, back in the dark days when AIDS had Zambia firmly in its grip, when only a few had the courage to speak up. Winstone patiently and candidly articulated the experience of HIV for me (and so many others) who were floundering, grasping at straws, trying to understand HIV well enough to create programming that would respond... And he continued to do that over the years, first with promoting testing, then treatment, then TB/HIV which he represented from his heart as no one else could. The world is a better place because of his efforts. As we grieve, we must also rededicate ourselves to honoring his legacy, by reaching more people with better services. Rest in peace Winstone -- we'll take it from here!

How is it possible to have accurate data in countries where the health systems are not working properly? WHO should really focus its energy in strengthening health system and especially the reporting systems.

The WHO report may be true but remember what Ben Franklin said about statistics? The total number of TB cases may be declining; however, the number of multi-drug resistant and extensively drug resistant cases continues to rise each year. There are also geographical and lack of accurate reporting issues to be considered in the interpretation of the findings.

Good news. Early bacteriocidal activity of Delamanid is absolutely logical. This drug is very hopeful by affecting of Mtb cell wall (Mycolic acid) and inhibition of cell wall synthesis/respiration. I think it's will be critical to include Delamanid in novel combinations/regimens (as PA-824).

Sincerely,

Nelly Solomonia
