It covers many different types of services including a day center, personal care in the home, Lifeline emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.&amp;nbsp;&lt;BR&gt;&amp;nbsp;&lt;BR&gt; &amp;nbsp;&lt;BR&gt;&amp;nbsp;&lt;BR&gt;JOB SUMMARY:&amp;nbsp;&lt;BR&gt;The clinic manager must be a Registered Nurse and possess a current valid Virginia nursing license. The clinic manager works under the general supervision of the PACE Site Director.  Responsibilities include completion of the initial, six month and annual nursing assessments of participants and the development of the nursing care plan and ensuring the plan is initiated.  The clinic nurse is responsible for monitoring and evaluating the health needs of PACE participants and recognizing changes in participant status and initiating appropriate actions.  Evaluates participant’s response to care and updates the plan of care as appropriate by documenting in the electronic medical record.  Engages in participant and caregiver teaching as appropriate and effectively communicates with participant, caregivers and the PACE interdisciplinary care team.   Responsible for management and oversight of PACE nursing staff and universal care aides (PCA) at the day center. The clinic manager is responsible for monitoring medication administration to participants at the facility.   Responsible for the daily operation of the PACE clinic to include maintaining adequate supplies and overseeing physician schedules.  The clinic manager is responsible for ensuring referrals to outside medical appointments for PACE participants are coordinated and approved for payment.&amp;nbsp;&lt;BR&gt;&amp;nbsp;&lt;BR&gt;HOURS: Days, with evenings and weekends as needed; 1 week administrative call rotation </description><pubDate>Thu, 12 Jan 2012 15:00:15 GMT</pubDate><guid /></item></channel></rss>
