The 'wet' techniques, and the superwet technique, referring to the amount of fluids injected, are variations of the tumescent technique. Tumescent anesthesia has had perhaps the most significant impact of all developments in liposuction. Warmed tumescent liquid, a diluted solution containing lidocaine, epinephrine and intravenous fluid, is injected into the area to be treated. As the liquid enters the fat, it becomes swollen, firm and blanched. The expanded fat compartments allow the liposuction cannula to travel smoothly beneath the skin as the fat is removed. The saline softens the fat, the adrenaline decreases the blood loss and bruising, and the anesthesia provides relief from discomfort.

Ultrasound Assisted Liposuction (UAL)
Ultrasonic sound waves, like shock waves, are transmitted into the fatty tissues from the tip of the cannula probe. The fat cells are melted or liquefied and then removed by low-pressure vacuum through a suction tube. Ultrasonic liposuction is often reserved for more difficult areas to contour where the deep fat is thicker, and thus harder to extract; i.e. back rolls, upper abdomen, and flanks. What is gynecomastia?

Gynecomastia is enlargement of the gland tissue of the male breast. During infancy, puberty, and in middle-aged to older men, gynecomastia can be common. Gynecomastia must be distinguished from pseudogynecomastia, which refers to the presence of fat deposits in the breast area of obese men. True gynecomastia results from growth of the glandular, or breast tissue, which is present in very small amounts in men.

What causes gynecomastia?

Gynecomastia results from an imbalance in hormone levels in which levels of estrogen (female hormones) are increased relative to levels of androgens (male hormones). Gynecomastia that occurs in normally-growing infant and pubertal boys that resolves on its own with time is known as physiologic gynecomastia.

All individuals, whether male or female, possess both female hormones (estrogens) and male hormones (androgens). During puberty, levels of these hormones may fluctuate and rise at different levels, resulting in a temporary state in which estrogen concentration is relatively high. Studies regarding the prevalence of gynecomastia in normal adolescents have yielded widely varying results, with prevalence estimates as low as 4% and as high as 69% of adolescent boys. These differences probably result from variations in what is perceived to be normal and the different ages of boys examined in the studies.

Gynecomastia caused by transient changes in hormone levels with growth usually disappears on its own within six months to two years. Occasionally, gynecomastia that develops in puberty persists beyond two years and is referred to as persistent pubertal gynecomastia

Gynecomastia can also be a side effect of a number of medications. Examples of drugs that can be associated with gynecomastia are listed below:
spironolactone (Aldactone), a diuretic that has anti-androgenic activity;

Calcium channel blockers used to treat hypertension [such as nifedipine (Procardia and others)];

ACE inhibitor drugs for hypertension [captopril (Capoten), enalapril (Vasotec)];

some antibiotics [for example, isoniazid, ketoconazole (Nizoral, Extina, Xolegel, Kuric), and metronidazole (Flagyl)];

anti-ulcer drugs [such as ranitidine (Zantac), cimetidine (Tagamet), and omeprazole (Prilosec)];

anti-androgen or estrogen therapies for prostate cancer;

methyldopa (Aldomet);

highly active anti-retroviral therapy (HAART) for HIV disease, which may cause fat redistribution leading to pseudogynecomastia or, in some cases, true gynecomastia;

digitoxin;

diazepam (Valium);

drugs of abuse (for example, alcohol, marijuana, heroin); and

lavender oil and tea tree oil, when used in skin-care products, have been associated with gynecomastia.

How do we limit the risk of complications in Liposuction.

Our procedures and protocols have been in place, and used without question for almost 30 years. Some people might think that our measures are extreme, however that is why our clinic can boast about our success rate. It is not inconvenient to do Liposuction in several sessions over alternating days, it should always be the rule. Many Doctors and clinics are just too eager to take a patients money, and only tell them what they want to hear, or let the patients make their own decisions based on their personal schedules. That will only lead to problems, and sometimes dire consequences.

Safety of the patient is our first priority, that is why we do not accept all patients for surgery. If a patient is at risk, first an assessment will be done with a Doctor who specializes in that particular area( such as a Cardiologist for a heart related condition, etc) If all is approved, surgery will be done in accordance to any specialized instructions.

The greatest risks of liposuction are associated with the tendency of some surgeons to do too much surgery on the same day. The three types of excessive surgery include 1) removal of an excessive volume of fat by liposuction on a single day, 2) liposuction of an excessive number of body areas on the same day, 3) combination of liposuction and other unrelated surgical procedures which involves excessive surgical trauma and prolonged exposure to general anesthesia.

For those reasons as stated above, Only local anesthesia will be used, no more than 4 flasks of lidocaine or other anesthetic will be used at one time, no combination of surgeries will be done on the same day, Liposuction will be limited to particular zones.

A moment about local anesthesia and the safety of Tumescent Liposuction.

Lidocaine is the safest of local anesthetics available for liposuction. At lidocaine concentrations in the blood exceeding 6 milligrams/liter, patients might experience some toxicity. Blood concentrations of lidocaine in excess of 12 milligrams/liter can produce serious cardiac toxicity. The maximum recommended dosage of lidocaine for tumescent liposuction is 50 mg/kg (50 milligrams/kilogram of patient weight). The most extraordinary aspect of the tumescent technique is its unprecedented safety record when used as directed. Most liposuction surgeons know that it can be dangerous to give a patient a dosage of tumescent lidocaine that exceeds 50 mg/kg. All liposuction-related deaths have been associated with either the use of general anesthesia, or IV sedation, or lidocaine doses in far in excess of 75 mg/kg. The tumescent technique is dangerous in the hands of surgeons or anesthesiologists who have not had specific training in the technique. There has never been a reported death associated with tumescent liposuction totally by local anesthesia.

Preventing complications should be the most important concern of both patients and liposuction surgeons. Surgeons can reduce the risk of liposuction complications by proper selection of patients. Surgeons should avoid patients who have 1) unrealistic medical expectations, 2) a history of significant medical problems. Patients can reduce the risk of complications by 1) resisting the temptation to save time by attempting to do multiple unrelated surgical procedures along with liposuction, and 2) resisting the temptation to maximize the volume of fat removed on a single day. It is much safer to divide one very large liposuction procedure into two or more separate procedures done on separate days. Liposuction by general anesthesia can be considered safe, however, liposuction totally by local anesthesia is safe.

Breast Implant Sales, Coupons, Deals Specials?

What is wrong with this picture?? Now we are shopping for surgery sales by coupon websites?? has the world gone crazy?? Who knows.

I received an e-mail this morning with my daily junk mail, from the world famous Groupon website. HALF PRICE BOOBS?????
How can this be?
People are now shopping for the CHEAPEST surgeons by coupon?
Is this normal for most educated consumers?? I think not.

The first thing I did, as any educated consumer would, was to "analyze" the deal(not that I am shopping for implants).
Upon inspection of the price differences (half versus full price), I then proceeded to the terms of the deal. First of all, was the actual product(implant) being used. Starting off strong with a five year warranty on the implants themselves, but with a recommendation that implants be changed every 10 years.
Shouldn't the implants last at least as long as the recommended change period? I certainly would think so. I know there are other implants available that have a Lifetime or until changed policy of warranty. I found that out by a simple internet search.

Then we move on to a mandatory insurance policy only at that clinic, for an additional $200. (eating into the savings part).
The other "not includeds" don't bother me as much. So, now I wonder. If I am paying half as much , but they are only guaranteed for half the term, did I save money?
Did I put myself at risk for a second operation at my own cost somewhere down the line? Did I save anything at all?

Probably the most important thing missing here is....... How to shop for surgery.
By coupon? or reputation. I was so busy checking the deal I completely forgot about checking qualifications of the surgeon, the clinic, or patient testimonials.

Liposuction Surgery Procedures For Different Body Types

We all have our own idea of what our bodies (and those of others around us) should look like. Some of us have frames that allow more "curves" (bigger hips, buttocks, stomachs, etc.), some of us are rail thin but have a little bit of stubborn fat in one or two places.

Gender is a factor in fat distribution. Men tend to have what is referred to as an android body type. They have thin legs and hips and carry their weight around their waists in an apple shape. Women generally have what are called gynoid body types. They tend to carry weight in the thighs and hips, and are pear-shaped. These are not hard and fast rules. There are men out there who are pear-shaped and women who are apples on sticks. There are also variations on these themes.

Many body types that can benefit from liposuction are called "deformities." That is such a harsh term. I don't care to use this terminology unless there is simply no way around it. I don't consider any of us to be deformed. The word deformity should be reserved for a condition that impairs function, not to describe a different silhouette. Everyone has a different body type and we all aren't Barbie or Ken and it would be boring if we were. We're just trying to balance our bodies a little and get a better silhouette.

Although the diagrams depicted in the following sections are just examples, individuals can have any combination of any of these problems and problems in additional areas. We are just covering the most common complaints in these sections.

Female Body Types and Complaints
Most women with excess body fat tend to carry their weight in areas in the thighs, hips, abdomen and general lower torso, buttocks and knees. There are several female body types that can benefit from liposuction. As I said, the medical community calls these deformities, but doesnt that sound harsh? Although the diagrams below are just examples, individuals can have any combination of these problems.

Saddlebags, Fatty Inner Thighs, and Knees
When I gained an extra 20 lb at one point, I had saddlebags, fatty thighs, and knees. I also had a small waist, which made the lower half of me look even larger. What joy. The clinical name for saddlebags is trochanteric excess. But who goes around saying that except doctors?? Say saddlebags and people know what you mean. Thigh liposuction takes care of this problem very well, if performed correctly. The body shape below can be sculpted to appear more balanced and shapely, as opposed to looking like a pear. Additionally, women often wonder how to get rid of love handles. Often, liposuction can take care of this problem area.

Excess Medial Thigh Deformity
This shape is not as common as saddlebags, but is common enough to be a regular reason for liposuction. Excess medial thigh deformity is when the middle points of the thighs are larger than the upper. The thighs appear to start lower than they should, but the hips, upper thigh and waist are usually normal size. No saddlebags are present, just exceptionally curvy medial thigh. This diagram also shows excess fat on inner thighs and knees.

Excess Flank, Waist, and Abdominal Fat
This diagram shows a figure with a thicker waist due to excess lateral abdominal fat and excess fat on the flanks. Even a small-framed person can have the appearance of having a larger waist or a wide body when the fat is distributed this way. Liposuction can improve this body type and create a thinner waist and remove the flank fat that hangs over the top of your jeans. This diagram also depicts excess inner thigh fat.

Violin Deformity and Fatty Calves
This is when you have a fuller, upper hip area and saddlebags, but a normal-sized or smaller mid-hip area. Your hips and thighs mimic the outline of a violin, with the indent at hip level. This diagram also shows some excess fat on the calves and inner thighs.

Upper Arm Fat
Liposuction to fat on the upper arms can be very beneficial, but it is also risky if not performed correctly. It can result in lymphedema, or temporary or chronic swelling of the arm and hand. Lymphedema can occur if the surgeon disrupts the ductwork and lymph nodes of the arm that drain fluids.

Submental (Under Chin) Fat
Having a deposit of fat under the chin is very common. Liposuction to remove this fat deposit is often done by itself or with additional facial cosmetic procedures such as a face lift or chin augmentation. Submental liposuction by itself can make a person look like she has lost a lot of weight. Neck and under chin liposuction may be performed in conjunction with a neck lift if the skin is too lax afterwards or with a platysmaplasty (a neck lift with muscle tightening) if the muscles of the throat and under the chin have also relaxed and separated.

Male Body Types and Complaints
Men tend to carry their extra body fat in the torso area, the flanks, abdomen and back. When the fat circles the torso, it is often called a spare tire or love handles. The pubic mound may have fatty deposits. Men can have fatty deposits on the upper chest, which can look like breasts, or they can develop actual breast tissue, which is a condition called gynecomastia, or they can have both. Of course men can have leg and arm fat as well, but the common liposuction areas for men tend to be on the torso.

Abdomen and Flanks
Many men have their flanks and lower abdomen slimmed down with liposuction. These areas may be the most common complaint in male patients.

Gynecomastia and Spare Tire
Gynecomastia is excess tissue in the male breast. I have heard gynecomastia rudely referred to as "bitch tits." It can be a side effect of steroid use or can be due to hormonal imbalances. In many men who are carrying extra pounds, these pseudobreasts are just fat. However, it can be actually breast tissue in some men. Liposuction can remove the fat deposits. If breast tissue is present, it can be removed surgically.

The spare tire is a common and much loathed male fat deposit. Fat deposits in the waist area tend to spread to the back as well. When excess fat collects along this area many men think about getting liposuction. For men, liposuction here is a very routine procedure.

Abdominal and Pubic Mound Protuberances
There are two areas of fat that can expand considerably to extend the abdomen, subcutaneous fat, which is routinely removed, and visceral (intra-abdominal) fat, which is rarely removed with liposuction. The fat that you can pinch that sits above your abdominal muscles is what can be safely removed by liposuction. However, the fat that is underneath your abdominal muscles and that surrounds your organs is dangerous to remove. Please see our Anatomy Section for more information on visceral fat.

Usually, as the fat around the front of a mans abdomen expands, so do the fat deposits on the flanks and back. Liposuction can really make a difference for those men who have a spare tire and love handles.

On a man, the pubic mound also collects fat and expands with weight gain. A mans body may put down a pad of fat right over the pubic bone, just above the genitals. This can cause the pubic mound to protrude. The fat can even surround the base of the penis. Liposuction can help this problem.

Submental (Under Chin) Fat
Many men develop a double chin. Liposuction can remove the fat that collects under the chin and expands the neck and this is done commonly. What is the process of facial rejuvenation by ACR- PRP?

Treatment procedure
