A full mouth reconstruction is a major decision and patients should feel completely confident that this is the right procedure for you as well as our center being the right implant center to have the procedure performed.

Just as a heads up, we are just entering our busy season which generally lasts through April. This dynamic creates such demand for the All-On-4 system in Houston that the makers of the All-On-4 rarely provide favorable payment terms. One last thing to look out for (not just with our center but any other center that performs the All-On-4) is the annual price increase that Nobel Biocare occasionally assesses in December / January. Most centers pass this price increase on to the patient to maintain dollar margins. It's not a huge difference maker but every penny counts in my book. I hope this helps.

Now the "All On 4" Dental Implant System or Mini Denture Implants can permanently secure teeth with the look, feel and functionality of naturally beautiful teeth. All-On-4 Dental Implants

The "All On" procedures incorporates full arch extractions, avleoplasty (bone re-contouring) bone grafts and the surgical placement of 4 to 6 dental implants that will permanently secure a non-removable bridge comprised of 12 to 14 teeth (per arch).

DENTIQ MATERIALS:

As a comprehensive center, we plan and perform implant procedures for single-site implants to full arch "immediate load" reconstructions using Nobel Biocare, Zimmer, Straumann and BioHorizons implant systems. When bone grafting is indicated, our office incorporates GEM 21S®, which is a completely synthetic grafting system for bone and periodontal regeneration composed of purified recombinant growth factors.

LIFETIME GUARANTEE:

We proudly honor the work that we do for the life of the patient. These are the terms: the patient should complete, and maintain, all clinically necessary treatment; he or she should maintain their regularly scheduled recare visits (3 to 4 x's a yr); and, it should go without saying that the patient should be in good financial standing with our office.

PAYMENTS:

Depending on the case, we ask for a 25 to 50% deposit, or the full prepayment (b) amount at scheduling. In the event that you choose not proceed with treatment, and you make an effort in good faith to give adequate notice, the full case prepayment amount (less 5%, and any treatment that has been rendered) will be refunded within 30 days.

Many patients who have compromised teeth or that wear dentures want to know if they are indeed a candidate for the All-on-4 procedure. The good news is that most people do meet the requirements to efficiently, effectively and safely perform the All On 4 procedure — in fact, 96% of our patients are candidates.  In  order to definitively determine who is an appropriate candidate for this  procedure, (or a treatment alternative) we utilize various diagnostic technologies including 3D imaging and pressure mapping.    

At the first visit, hi-resolution photos, digital  x-rays, a 3D CAT scan, pressure mapping and a thorough clinical evaluation will take place  followed by expert explanation of the case diagnosis and treatment options inclusive of the "All On 4" and alternative treatments which can range from single-site implants to replace each missing tooth space, implant retained and implant supported dentures, mini implant solutions and non-implant options.  During this visit, impressions are taken for diagnostic modeling and diagnostic design (which will also be used to make the  replacement teeth), and a clear financial overview is presented.

THE SURGERY PHASE: ALL ON 4™ (FOUR) DENTAL IMPLANTS 

For most cases, this phase involves the extractions, bone grafts (if necessary), bone re-contouring and the surgical placement of the 4 dental implants per upper and / or lower arch. The "All On 4" patient receives implant-secured teeth immediately following surgery — ONE-visit!    

RECOVERY & FOLLOW-UP VISITS 

Patients typically resume a full schedule of activities within 24 to 72 hours. Generally speaking, follow-up visits are 1, 4 and 12 weeks after surgery.  Specific post operative instructions will vary on a case by case basis. A modified diet is required until patients receive the final teeth, which incorporate concealed titanium reinforced framework OR an all-zirconia substructure. In the mean time, patients can eat anything that can be cut with a fork, and does not involve biting or tearing with your front teeth (e.g. hamburgers, salmon, pasta, cooked vegetables, etc). Hard foods and foods that require tearing such as hard breads, pizza, carrots, corn on the cob, steak, and crunchy and sticky snacks treats such as nuts, hard candy and gum should be avoided until otherwise instructed by the principal dentist.    

Following surgery, the first 3 follow-up visits are simple thirty minute post-op / follow-up visits to ensure that everything looks, feels and heals as indicated. For traveling patients, these follow-ups will take place within a 2 day to 2 week span.

THE RESTORATIVE PHASE: OVERVIEW: ALL ON 4™ (FOUR) DENTAL IMPLANTS

The restorative phase can start 16 weeks after the initial surgery when the implants have fully integrated with the jawbone. At "The Prep" visit, the implants are prepared for master impressions so that the final teeth can be made. Depending on the smile design and revision requests, this final fabrication process takes 2 to 4 weeks at which point, the “final” teeth are secured. A standard adjustment visit is scheduled 1 to 2 weeks later to ensure that everything looks, feels and functions exactly as it should.

[How Much Do Dental Implants Cost]

Dr. Malo found that if he placed all the implants in the anterior of the jaw, where there is usually better bone density, and if those implants were highly angled, they could provide stable support for a complete denture.

You can see above that the implants are highly angled. But notice that the exposed tips of the dental implants, the part sticking out of the bone, are all pointed upward and are relatively parallel.

On the right you can see that when the denture is screwed on, the screws are all placed normally, because the angulation all occurs under the surface of the tissue.

But this technique isn't without its critics. Renowned implant dentist Dr. Arthur Chal of Phoenix who has pioneered other techniques, after thoroughly studying the all-on-four technique, is concerned about its risks. If one of the implants fails, the entire mouth has to be re-done, and he feels that this is an unacceptable level of risk. COST OF MULTIPLE DENTAL IMPLANTS FROM $1,600ea 

Extractions range in cost from $175 to $275ea.  The surgical placement of the titanium post (the dental implant) varies from $1,600 to $1,800ea with multiple sites.  Bone grafts (if necessary) are $250 to $450ea.  Restoring the dental implant with the implant crown is $975 to $1,500ea. Extractions range in cost from $175 to $275ea.  The surgical placement of the titanium post (the dental implant) varies from $1,600 to $1,800ea with multiple sites.  Bone grafts (if necessary) are $250 to $450ea.  Restoring the dental implant with the implant crown is $975 to $1,500ea. In certain cases, additional procedures may be required such as guided tissue regenerations or sinus elevations.  Our office accepts all PPO insurance plans and offers affordable financing options for credit qualified patients.

Mini Dental Implants

A recent strategy used by dentists is early intervention, meaning that implants are inserted into locations in which teeth have freshly been extracted. When successful, the procedure can take months off treatment because the osseointegration process is sped up. However, the candidacy for such treatment depends upon the anatomical factors within the extraction site. If the tooth site ends up being broader than the implant, the implant cannot be placed immediately after the extraction. In this case dental work must first be performed in order to ensure an ideal fit for the implant. As for small teeth and incisors with narrow spaces, the placement of mini implants is often used.

ALL ON FOUR DENTAL IMPLANTS, HOUSTON

With the "ALL ON FOUR DENTAL IMPLANTS" procedure, a full upper and/or lower arch reconstruction can be comfortably performed under IV sedation in ONE visit.  This procedure uses four implants with the back implants tilted to increase the bone-to-implant contact, which enables the "immediate load" of the replacement teeth.

The All-on-4 procedure was developed in the 1990s as a way to best restore the full arches of the upper and lower jaws. The procedure uses only four implants to secure all teeth in place hence, the name All-on-4. All-on-4 was developed by Nobel Biocare and European dentist Paulo Malo using bio-mechanics, computer simulation and clinical research. Their results provided patients with high quality full teeth restoration without the use of bone grafting in the shortest time possible. The All-on-4 procedure provides especially effective restoration of the upper jaw because longer and stronger implants can be secured in the back part of the jaw.  Previous treatments required patients to receive a total of six or more, with treatment extending throughout 18 months of treatment. The patients were then forced to leave the office without any teeth or a poor fitting temporary denture for over a year.
