Diabetics getting younger and younger...

The number of diabetics is continuing to rise and the more worrying statistic is that the disease is claiming more younger Malaysians as among its victims.

The most recent National Health and Morbidity Survey (NHMS) shows that almost double those in the 30 to 39 age group were afflicted with the disease compared to five years earlier in 2006.

With one-in-five adult Malaysians over 30 already a diabetic, Health Minister Datuk Seri Dr S. Subramaniam said this situation could impact the economy and lower quality of life, particularly when complications of the disease set in.
“The fact that it is affecting younger people, which in turn has an effect on productivity as well as the economy, is very worrying. It also leads to a lower quality of life,” he said.

Some 2.6 million adult Malaysians aged 18 and above are diabetics and this figure is projected to hit 4.5 million by 2020, according to the 2011 NHMS.

The survey also shows an increasing number of diabetic patients across all age groups – almost doubling from 4.9% (in the 2006 survey) to 9.4% for those aged between 30 and 34 years, and from 6.4% to 10.9% for those between 35 and 39. For those in the 40 to 44 age group, the increase is from 10.3% to 17.6%; and those between 45 and 49 – from 15% to 20.6%.

“Besides genetic predisposition, diabetes is a lifestyle disease brought on by unhealthy eating habits. We have also had a change in the nature of our jobs from one that was labour-intensive to a sedentary one where there is little physical activity,” said Dr Subramaniam. He also said there were an estimated 53% of Malaysians who were still undiagnosed. “What this means is that patients are likely to seek treatment late and it will be costly to treat diabetes-related complications when they seek treatment,” he said.

Dr Subramaniam said the rising number of diabetics and late diagnosis continued to strain the public healthcare system.He cited a 2010 study which estimated the cost of treatment for diabetes to account for 16% of the national healthcare budget or RM2.4bil. “This is a substantial amount,” he said. He said the impact was already being seen with an increasing number of diabetic patients at public health clinics and rising number of hospital admissions due to diabetic complications.

“This has cost implications to the Malaysian Government in continuing to provide quality care to its population. It will also affect productivity and ultimately negatively impact our economic development,” he said.

1. Psychiatric disorders
Diabetics may experience many different emotions such as anxiety, anger and depression. 2. Nerve damage
Uncontrolled diabetes may result in nerve damage. The high blood glucose damages nerves in the arms, hands, legs and feet. Patients should watch out for the loss of feeling in their feet, numbness (pins and needles), tingling and burning sensations, and muscle weakness. Gastroparesis is when the muscles of the stomach stop functioning properly. It is a condition that reduces the ability of the stomach to empty its contents, and it does not involve a blockage or obstruction.

Diabetes can damage the blood vessels in the retina, which may lead to blindness. People with diabetes may also develop eye problems like glaucoma, cataracts, and diabetic retinopathy. 6. Kidney disease
High blood glucose levels due to poor control can lead to your kidneys being overworked, which can lead to kidney disease. Severe cases can lead to kidney failure or irreversible end-stage kidney disease, requiring patients to undergo dialysis or a kidney transplant. Signs of kidney problems can be increasing amounts of protein in urine; water retention in ankles, abdomen and chest; foul taste in the mouth; vomiting; and tiredness throughout the day.

7. Hypertension (High blood pressure)
Due to the hardening and narrowing of the arteries, a diabetic's heart has to pump harder than normal and this builds up pressure against the artery walls. Symptoms of this are frequent headaches, blurring vision and shortness of breath. To reduce the effects of this complication, you should eat a low-fat and high-fibre diet, exercise regularly, quit smoking and keep a healthy weight.

8. Heart complications
Diabetes causes an increased narrowing of the arteries and also the hardening of the arteries which slows down blood flow. Diabetics are at risk of heart problems such as coronary artery disease (when the blood supply to the heart muscle is diminished), which is the main cause of heart attacks.

9. Stroke
Diabetics can suffer from strokes when the blood flow to the brain is blocked. This happens due to the hardening and narrowing of the arteries in the brain. If one's blood pressure, cholesterol levels, and blood glucose levels are not controlled, the risk of stroke increases significantly.

10. Hyperosmolar Hyperglycemic Nonketotic Syndrome (HHNS)
HHNS is where blood glucose levels shoot up too high and your body tries to get rid of the excess sugar by passing it through your urine. The symptoms include high blood sugar levels over 600mg/dl; dry and parched mouth; extreme thirst; high fever; warm and dry skin; sleepiness or confusion; loss of vision; hallucinations; dehydration; and weakness on one side of the body.

It is often an onset of a serious illness or infection. If HHNS continues, it can lead to severe dehydration, causing seizures, coma and eventually death.

Haze leads to heart attacks, strokes, lung and kidney problems

PETALING JAYA: It's not just skin irritation and breathing problems. Prolonged hazy conditions can lead to heart attacks, strokes and lung and kidney problems.

Consultant cardiologist Dr David Quek said vulnerable groups would be at higher risk of experiencing these harmful effects if the haze were to go on for weeks or months.

“This applies especially to older people and those with kidney or diabetic problems, as well as heavy smokers with impaired and chronic lung conditions,” he said on reports that the transboundary haze from Indonesia could last until August.

Dr Quek noted that it was also possible that deaths could be recorded from complications brought about by the haze and smog.

He added that airborne chemicals and certain particulate matter (PM) could “penetrate deep down through the lungs” and get into the body's system.

There are studies linking air pollutants to heart disease and increased mortality risks, which reinforce the understanding that any kind of poisonous air pollutant could have harmful effects.

However, Dr Quek cautioned that it was difficult to quantify the individual effects of these chemicals.
“Nevertheless, the effects they can have include constricting the blood vessels in the heart and brain, as well as interfering with normal functioning of multiple organs,” he said, adding that long-term exposure to such air pollutants could lead to higher cancer risks in the long term.
