Is there any risk to pregnancy? Our doctor told us that there will be risk for baby's life due to thyroid level. Eltroxin 50 mcg is enough to bring down the thyroid level from 10.33 to normal level. My 29 years old wife is into her eleventh week of pregnancy. She is suffering from some yellowish fishy odour discharge from vagina. We consulted a doctor who suggested Ceftum 500 twice a day, Flazyl 200 thrice a day and Azithral 500 once a day for 5 days. But still now she is discharging the same but the fishy odour is mild. Urine culture report is normal. Why is she suffering from yellowish discharge? She has protein deficiency. Now she is taking Beparin 25000 IU (1 ml per day) and Ovidac 5000 IU twice in a week. A recent scan report showed gravid uterus reveals a well-formed gestational sac and good decidual echoes. A live embrayo and yolk sac are seen. Embroyonic cardiac pulsation is present. No evidence of perigestational bleed is seen. Please guide. ]]></description>

I am a 39 years old woman suffering from pain in my left elbow for the last 18 months. I consulted a doctor who told me that it is tennis elbow. I have a lot of pain and unable to straighten my elbow out. I have had several cortisone shots in it over the past year and a half. I have even done a few months of physical therapy on it. Once the cortisone wears off, the pain recurred. My doctor suggested a surgery for my problem, but on consulting a surgeon, I was asked to wear a wrist splint for 3 to 6 months. I have worn the splint now for a week, and my elbow seems to be worse. It becomes extremely stiff and also my wrist is starting to hurt now. Are stiffness and the new wrist pain normal after wearing a wrist splint? ]]></description>

 <title><![CDATA[How can abdominal pain be treated? ]]></title>

My 24 years old wife is suffering from pain in the abdomen. She underwent an X-ray, ultrasound, endoscopy and CT-scan of her abdomen but all these reports came normal. They are terming it as functional pain. The pain started a month back but it stopped after we stayed in the hospital for two nights. The doctors gave her painkillers and antibiotics. But the pain recurred after 15 days. This time it was more severe and she became unconscious with pain and was again readmitted to the hospital for 5 days. She was fine for another 10-12 days and now the pain has started again four days back, but this time it is bearable. The doctor found some erosion inside her stomach on endoscopy. She was given these medicines – Clonotril - 0.5, Razo 20 and Paracetamol (650 mg). Normally the pain starts in the abdomen then spreads to back as well. She is having her periods regularly but they are usually painful. Four days back, when pain started, her menses had started. But now the bleeding has stopped but not the pain. ]]></description>

 <title><![CDATA[Why am I suffering from itching and skin rashes? ]]></title>

I am a 28 years old male suffering from skin itching, rashes and redness for the last one year. Although the symptoms are allergy like, I've not been able to associate them with any particular trigger. I was on a daily dose of Allegra 180 sometime back but have learnt that even an alternate day dose of Allegra 120 works fine. I got some blood tests done. My IgE level was 97.5 iu/ml and routine stool test showed the presence of E. histolytica ova/cyst. I followed a 10 day course of Metrogyl (400 mg) but that brought no respite either during or after the course. What could be the possible cause of my condition? How safe is it to take Allegra in the long term? Also, my wife underwent a miscarriage sometime back. Could that have any connection to my condition? ]]></description>

 <title><![CDATA[How can pain due to systemic lupus erythematosus be treated? ]]></title>

I am a 22 years old student suffering from systemic lupus erythematosus (SLE) for the last one year. I have been taking Medrol 4 mg. I consulted a doctor because I had severe pain in my joints and swelling in my fingers and wrist. I also had nodules in my neck region. On taking pain-killer steroids, my pain subsides and I am able to walk but if I skip a dosage the pain becomes too severe. My knees hurt a lot and I feel warm also. I can neither stretch them nor fold them. I have pain in my back too. It is difficult for me to type or read with my head bent for even 10 - 15 minutes. My aunt (mother's sister) is suffering from rheumatoid arthritis and has got her joints operated and now her spine is bent. Could this be hereditary? My height is 5 feet and 2 inches and weight is 62 kg. I have been putting on weight continuously from the time I started taking steroids. Whenever I go to the doctor for a monthly check-up, he gives me the same tablet again. He says my ESR is normal and I am fine. Why are my nodules spreading and my pain not subsiding? ]]></description>

I am a 45 years old male who experienced mild pain in palm and feet without burning sensation two years back. The doctor prescribed me Methycobalamin 1500 mcg one tablet daily for two - three months along with Superactive capsule. I got temporary relief after taking these medicines. Now again I feel mild pain in palm and feet but this time with burning sensation. I consulted my doctor again who has now prescribed me Gabapentin + Methycobalamin 600 SR and diagnosed it as peripheral neuropathy. I have taken the medicine for three weeks and only a little improvement. In the past two years, I also had a severe back pain with strain in lower spine but only once. I am taking medicine for high blood pressure regularly for the last eleven years. I take Amlopress 5 mg and Losartan potassium 25 mg daily and my blood pressure is under control with these medicines. Do I need any further treatment for peripheral neuropathy? ]]></description>

My three years old son has been suffering from occasional throat infections, fever and viral infections. I have observed small swelling nodules in the occipital region behind his head, where the hair line ends two and a half years back. The glands about 9-10 in number (I assume are lymph nodes) are oval and freely moveable and palpable in various size, starting from 1/2 cm to 1 inch and after a while all get subsided. A month back, I have noticed small rubbery glands on the lateral side on his neck. There are about 4 of them in a row with about an inch to 2 inches gap between them. The glands close to the clavicle have decreased in size. However, two close to the ear have grown. They were about half a centimetre and now have grown to one centimetre. Also I have noticed 2-3 new glands on the occipital area about an inch in size. We consulted a doctor who has been examining the baby for a year now. He has asked us to ignore them and told that they will go away and prescribed an antibiotic Clamp-kid for 5 days. The problem still persists and has not subsided. After that we have taken him to a general physician, who has suggested a lot of tests including blood tests and FNAC, we have got a lateral X-ray of skull and full chest done. The report has come out to be normal. Should we really get the blood tests and FNAC done? What should be done to get rid of extra glands? ]]></description>

I am experiencing nausea and heaviness in gall-bladder region after stopping the medicines. Recently I am taking Liv52 twice and glucose every four hours. ]]></description>

I am a 43 years old man, has and I have pain in the right lower back side of the lung with a sore throat since 20 days. I took some homeopathic medicines which reduced the pain. After that, I saw blood in my sputum and consulted a doctor who prescribed antibiotics and asked for a blood test and chest x-ray. The blood TLC – 11,800, eosinophil - 46, ESR - average 20.6 and rest all parameters were normal. Chest x-ray shows haziness in the right lower side of lung (pneumonic patch), sputum examination showed no tuberculosis germs. After that, I travelled by road for about 900 km in two days and the pain recurred and the blood test showed TLC- 8,800, eosinophil - 28, ESR average - 12.5 and X-ray showed no change in the patch. Now CT scan of chest also shows haziness. Have I been correctly diagnosed? I have small children in the house, what precautions do I need to take? Can I travel? ]]></description>

Foetal Maturity - L 32 weeks 3 days +/- 1 week, Approx Weight: 1770 gm +/- 258 gm, cardiac activity and foetal movements are normal. Foetal heart rate is 132 bmp, liquor amnii is adequate. AVI is 11.9, placenta is posterior and in upper segment grade I mature. NST is reactive. Bio-physical score is 10/10. Colour Doppler showed umbilical artery: PI=1.19, RI=0.69, S/D=3.2 (increased impedance), middle cerebral artery: PI=1.32, RI=0.70 S/D=3.3. The doctor says that S/D is high, and there is less blood flowing to the baby from umbilical cord. What could be the reason for this? What should we do for a full term delivery and a healthy baby? ]]></description>

The current stone was left untouched inside the kidney and I was put on Potrate MB6 for 3 months. On taking a second opinion I was prescribed these drugs for a week - Lasix (40 mg) once in the morning, Veltam 0.4 once at night and Didanzen Forte 3 times a day. Can the stone pass out on its own? I do not have any pain at the moment. I had a similar episode 10 years back as well with a 9 mm stone, which passed out while urinating. ]]></description>

With this medicine his pleural fluid got reduced but pain is still there. We consulted another doctor for second opinion who totally changed the medicine saying my husband's ADA is normal. He then prescribed Rcincx (300 Sonaizid +600 Rifampicin) 1 tab, PZA 750 - 2 tabs and Combutal 1000 mg - 1 tab daily empty stomach to be continued for 3 months. But after starting these drugs his pain has increased and now we are in dilemma. What should we do? Doctors are having all different opinion. My husband has lost his appetite and gradually losing his weight. His current weight is 60 kg and height is 5.8 feet. Please advise. ]]></description>

I am a 31 years old male suffering from azoospermia since long. In childhood, I had a severe pain in my left testis and after consultation; I was operated by a surgeon. Left testicle measured as 29 x 20 mm, but situated in left inguinal canal. Right testicle measured as 46 x 34 mm enlarged with altered echo pattern, multiple hypoechoeic area inside, along with this, hernia also removed same time. Four years back, I got married and underwent certain tests, which showed no sperms, fructose – positive, left testis – 31 x 18 mm with hypoechoeic pattern, undescended at opening of the internal inguinal ring with 8 mm cyst and right testis – 15 x 10, small atrophic with altered echo pattern with 3.5 mm cyst. After one year, I again underwent same test, which showed no sperms, fructose – negative. Same result showed for another two years. Last year, I again underwent same surgery for my left testis and now it is in its proper place but right testis is found non homogeneous with echogenic periphery and echogenic areas in its substance measured 1.8 x 1.3 x 1.4 cm (volume 1.7 cc). We have also undergone IUI with donor's sperm thrice followed by IVF but still not get positive result. Can I father a child? ]]></description>

My 73 years old father is suffering from anaemia for the last two years and his present haemoglobin level is 8.9. His PCV is 29.4, WBC 8840/cu.mm, ESR 28mm/hr, MCV 81.0fl, MCH 24.5pg, MCHC 30.3, RBC 3.63, Platelets 113,000/cu.mm and LDH 824 U/L. His peripheral smear showed RBCs - hypochromic, microcytic cells and polychromasia cells seen. WBCs were normal. Platelets - thrombocytopaenia. He had cardiac bypass surgery three years back and is on the medicines – anti-diabetic medicines, diltiazem, simvastatin, ACE inhibitor and fish oil capsules. He had a few episodes of bleeding from his recurrent piles recently. Since he had bypass surgery his haemoglobin has dropped and not improving in spite of iron and multi-vitamin preparations. He recently developed high fever with diarrhoea and vomiting and was hospitalised and found to have haemoglobin of 7.4 and was given a unit of blood transfusion. What is the cause of his anaemia? He has few gall stones on abdominal ultrasound and is asymptomatic. Colonoscopy was done a year back. ]]></description>

Then I underwent endoscopy and biopsy in which the endoscopy was positive but the biopsy was negative. I was again given triple therapy HP kit for 3 weeks. I was feeling better but was continuously on proton pump inhibitor (PPI) for the last four years. Two years back, I underwent another endoscopy with biopsy, which showed antral gastritis but H. pylori negative. The doctor gave me the HP kit for one week. Last week, I got a stool antigen test done for the first time, which came out positive for H. pylori again. I have been prescribed Amoxicillin (1000 mg) twice daily for 10 days, Levoflox (250 mg) twice daily for 10 days and Rebeprazole (20 mg) twice daily for 10 days. Despite taking HP kit 4 times in the past, why is H. pylori positive again? Is it a case of resistance to antibiotics? ]]></description>

My 73 years old mother has started ATT (anti-tuberculous therapy) two and a half months back. She was hospitalised twice for high grade fever and severe weakness and was diagnosed with miliary tuberculosis. She is on rifampicin, Pz, levofloxacin and ethambutol. Steroids were also given. As soon as steroids were stopped, after a week the symptoms of fever 101 degree Fahrenheit returned along with listlessness and weakness. Repeat CT chest came absolutely clear. What causes fever and severe weakness? She has been started on second line ATT pending results of culture sensitivity for last 5 days. If second line ATT is taken for 5 days, then stopped for 10 days, and restarted, will there be any problem. ]]></description>

My 64 years old mother is suffering from high blood pressure and severe osteoporosis. Three years back, she was diagnosed with deep vein thrombosis (DVT) in her left leg and is on Acitrom since then. Recently, she underwent kidney function test as her left kidney is smaller in size (6.7 x 3.2 cm) and blood urea nitrogen was found to be 46 mg/dl and creatinine - 2.50 mg/dL. The doctor told us that the kidney is working 30% and after 6 months, dialysis would be required. She is suffering from hypertension for the last 10 years and was taking Telma 40.  For the past few days her blood pressure was continuously high so the doctor has replaced Telma 40 with S Numlo 5 and further increased it by 2.5 mg. She is not able to sleep at night for the last 3 days and has got swelling on the back of the leg along with diarrhoea. I have found that these may be it is the side effects of S Numlo 5 and also  read that Telma is not good for the kidneys. ]]></description>

I consulted  another doctor, who told me to stop Flotral. I stopped it and am not taking it for the last one month. I am a 37 years old female has been diagnosed with right ear drum perforation 20 years back, which has not healed till now. A few years back, I was diagnosed with left ear perforation also. Around five months back, I had a severe cold and went to a doctor who told me that there is no perforation in left ear but rather the ear cover is thrown back. But often when I have cold I experience temporary less hearing in right ear or some strange noises but after sometime it resolves on its own. I also suffer from vertigo. Yesterday I used an ear bud to clean wax from my right ear and since morning I am experiencing less hearing in right ear and echo sounds and strange noises again. I went to the doctor who said that there is inflammation in the ear and infection. He prescribed Droxyl (500mg) for 5 days. He has also given ear drop Betnesol-N. But I am quite apprehensive about taking Betnosol-N ear drops as it contains neomycin and it's clearly written on the medicine box also that not to be taken if there is perforation of tympanic membrane. I also read many times that when there is ear perforation no ear drops should be recommended as it enters the middle ear and inner ear and causes ototoxicity. Am I on right treatment? Is vertigo linked with my ear problem? Is tympanoplasty 100 percent successful or it can make hearing worse at times? ]]></description>

My 18 months old son is suffering from seizures after birth, but we recognise it 10 months back. The doctor suggested EEG and the result showed bilateral posterior hemispheric epileptogenic activity. Then we started these medicines – Biotin (20 mg) daily, Epilex syrup 1.5 ml thrice a day, Omnacortil (7.5 mg) daily in the morning and L-carnitine / cornitor 1 ml daily. MRI was also done which showed evidence of confluent hyper intensities involving bilateral peri-venticular white matter with mild atrophy. Subtle myelination of peri-venticular white matter and internal capsule is seen. There are capacious CSF spaces in bilateral Fronto-temporo-parietal regions. Rest of cerebral parenchyma is normal in signal intensity. Bilateral basal ganglia and thalami are normal in signal intensity. Ventricles are normal in shape, size outline and volume. Septum is in midline. Basal cisterns and sylvian fissures are normal. Sella and parasellar areas are normal. After a couple months, Omnacortil 7.5 mg was stopped and Epilex chorono (300 mg) was given and epilex syrup was stopped. After that he was alright. But four months back, seizures recurred at night with fever and remained for 20 minutes. How can recurrent seizures be managed? ]]></description>

 <title><![CDATA[Am I taking the right treatment for tuberculosis? ]]></title>

I am a 32 years old man diagnosed with abdominal tuberculosis (TB) based on analysis of  fluid aspirated  from my abdomen. Ten months back I started Omnacortil 60mg which was tapered off over 6 weeks.  I also took Rimactazid Plus, Pyrazinmide for first 2 months, Ethambutol and multi-vitamin syrup. Though there was a marked improvement in my condition, I was still having low grade fever (around 100 degrees) every evening, even after 2 months of treatment. I consulted a gastroenterologist who advised ultrasonography  (USG) and CT scan , which showed localised fluid collection in pelvis. It also revealed fatty infiltration of liver, small residual fluid collection in right iliac fossa and small fluid collection in right hypochondrium. Also, PCR for M. tuberculosis did not detect Mycobacterium tuberculosis DNA and the TB culture report showed ‘no growth of acid fast bacilli'. Medicines Neksium, Benadon and Motinorm were added. Ascitic fluid examination after 4 months of treatment showed ADA level of 35 U/L, Protein 5.9 g/dL, Albumin 3.2 g/dL, Lymphocytes 98%. Latest chest X-ray shows lobular opacity in right paratracheal region. I recently underwent laparoscopy and the findings were: thickened peritoneum, dense omental adhesions present to the abdominal wall, no ascites,and non-visualisation of liver due to adhesions. Biopsy showed granulomatous peritonitis of probable tuberculous aetiology. The improvement in my condition has been very slow and even after r 5 months of regular treatment I still have low grade fever every evening (99.2-99.5 degrees F) along with weakness. I have gained some weight over last 2 months and am just 3-4 kg less than my normal weight. The doctor says that I am still having TB and would get well in due course of time and asked me to continue with the same treatment. Am I taking right treatment? What do the USG / CT /X-ray and Laparoscopy findings indicate? Why does PCR and TB culture show negative reports as far as TB is concerned? Do I need to go for some other tests to get a final confirmation? ]]></description>

 <title><![CDATA[Am I suffering from tuberculosis? ]]></title>

I am a 35 years old female weighing 94 kg. I am suffering from thyroid from the last five years. My doctor suggested thyroxin (25 mg) and then increased the dosage to 100 mg. For the past four years, I am planning for my second child. My first child is seven years old. My doctor told me that it is difficult to conceive as I am over weight and told me that I am suffering from endometriosis. I was given thyroxin (25 mg) and metformin (500 mg). My doctor also suggested me to undergo tuberculosis test. Tests were done four months back, which showed TB-PCR-1 was negative, ELISA for tuberculosis (IgM) - 0.69 and ELISA for tuberculosis (IgG) – 170. After that doctor gave me AKT-4, Penzole (4 gm), Thyroxin (25 mg), Glycomet (500 mg). These tests were done two years back, follicle stimulating hormone - 6.35, luteinising hormone (LH) - 6.83, estradiol – 94 and testosterone - 0.61. Thyroid profile showed T3 - 2.24, T4 - 1.17 and TSH - 3.016.  Serum bilirubin test result showed total as 0.87mg/dl, direct as 0.34mg/dl and indirect 0.53mg/dl. I want to lose 10 kg weight. Please guide. ]]></description>

 <title><![CDATA[What does my mother's mastectomy indicate? ]]></title>

My 50 years old mother is a breast cancer patient. A lump was identified in her breast about nine months back. Then first she went through 8 cycles of chemotherapy (no biopsy before chemo, only mammogram and FNAC was done) and clinician termed it as T4dN1M0. About a week back, she underwent mastectomy and clinician sent mastectomy specimen for biopsy. The report diagnosed breast cancer - received a mastectomy specimen measuring 18 x 16 x 6 cm with axillary tail measuring 14 x 5 x 2 cm and the overlying skin measuring 15 x 8 cm. On serial slicing, a tumour is seen in the upper out quadrant measuring 3 x 3 cm extending 2 cm away from the DRP and far away from the nipple and areola. The skin is unremarkable. Adjacent breast, on serial slicing, doesn't reveal any other tumour. 10 lymphnodes identified. Section studied from breast shows areas of necrosed tumour cells with focal multinucleated giant cell reaction is seen, however there are no viable tumour cells identified. There are areas of dense sclerosis with chronic inflammatory infiltrate with multinucleate giant cells and focal area of lymphoid aggregate with areas of fat necrosis identified. Deep resection plane is free of tumour. Nipple areola and overlying skin are unremarkable. What does it mean? ]]></description>
