Just wanted to add this note about vaginal mesh alternative surgery for pelvic organ prolapse.

Bladder suspension refers to surgery that helps place a
sagging bladder back into its normal position. Is this type of surgery
right for you? Here are answers to some of the most common questions,
from who may need this procedure to possible complications.

Why Is Bladder Suspension Surgery Done?

Bladder suspension (or bladder neck suspension)
surgery is a procedure used to treat urine leakage that occurs when a
person sneezes, laughs, or coughs -- a condition called stress
incontinence. Childbirth, as well as hormonal changes that come with
menopause, can cause a woman to lose muscle tone along the pelvic floor.
That can lead to stress incontinence and, if you decide to do it,
bladder suspension surgery.

“Thank You God. We can finally get the word out to women.”
Those are the words of Christine Scott, 53, a former runner who had a vaginal mesh implant to treat stress urinary incontinence in 2008. After that her health went downhill as the synthetic mesh eroded into her colon.
KGET-TV 17 in Bakersfield reported late Monday night (here) that the Bakersfield woman won a $5.5 million lawsuit she filed against mesh maker C.R. Bard. In the product liability action, Scott said she had the Avaulta Plus Biosynthetic Support System to correct some urine leakage, but it left her in chronic pain, incontinent, and unable to have intercourse.
“I don’t know if I have one or 100 surgeries ahead,” she said to the tv station.
The jury returned the verdict late Friday in Kern County Superior
Court before Judge William Palmer. Scott told jurors she could only
urinate with a catheter for five months. She then had to undergo eight
additional removal surgeries. The Avaulta remains in her body as many surgeons feel the permanent implants cannot be safely removed.
The jury awarded Scot $5 million and her husband $500,000 for the loss of their relationship.
The lawsuit is a landmark victory and the first of its kind in the
nation against a major mesh manufacturer. There are another 650 mesh
cases pending against C.R. Bard and thousands pending against five other
mesh manufacturers consolidated in multidistrict litigation.
Tested on Rabbits and Sheep
Attorney Elaine Houghton said, “ They tested this on they tested 16
rats 12 rabbits 4 sheep and by their own research the next living
product this went into was a women.”
Houghton and her law partner, Gene Lorenz represented Scott.
KGET reports Bard stopped selling the
Avaulta medical device on July 1, 2012 in the United States because the
FDA wanted more clinical trials done.

The Bakersfield woman said the hardest part was having to keep quiet
watching women continue to be hurt. In 2010 an estimated 200,000 women
had synthetic mesh implanted to treat stress urinary incontinence. The
actual complication rate is unknown since the U.S. does not monitor
medical devices after they are implanted.
Synthetic meshes used to treat incontinence are petroleum-based
polypropylene. They are considered a Class II medical device and
manufacturers are not required to conduct clinical trials for safety and
efficacy before they are marketed.
When I got that verdict it was, Thank you God. Now we can do something,” said Scott.
FDA- Complications “Not Rare”
Scott filed a lawsuit against Bard and her doctor in January, 2009.
In October 2008, the U.S. Food and Drug Administration filed the first
of two Safety Notifications warning the public that there was a degree
of “rare” but “serious” complication association with
mesh placement in some women. One year ago, the federal agency revised
that notification and said that complications were “not rare.”
Many doctors have assumed that the FDA warnings primarily focus on synthetic transvaginal mesh for pelvic organ prolapse, not incontinence. This judgment flies in the face of that assumption.
Scott says she will start a support group. Patient Right to Know.org for people who have questions about surgical mesh.
C.R. Bard of Murray Hill, New Jersey plans to appeal. The Avaulta Plus is no longer sold in the U.S. but is sold globally.

Although this product is no longer sold, other manufacturers are still in the marketplace.

Dr. Michael Hulse ob gyn Hickory Flat Ga. transvaginal mesh complications

may 20th my fiancee' had the vaginal mesh implanted. may20th evening we were in the emergency room of Cherokee hospital due to a 'mistake' on medications. following week was a complete hell ...bleeding, pain and misery. Surgery was scheduled for 'correction of the mesh ' due to it being too 'tight'. surgery was completed , and she ONLY had to pay for the hospital , not another 'surgeon' fee. Today, she still is in pain along her leg and hip, uses a prescribed cream for what ? and the story continues. Contact with the doc has only resulted in comments..that it will get better. we are in the month of january now, and I cannot see any positive results...anywhere. The bladder problem continues but now we have additional pain along her hip and leg and back. DO NOT do the VAGINAL MESH! Any reputable doctor will not recommend a procedure that results in so many failures, as evidenced by the FDA warnings. She was influenced by Dr. Hulse to continue with the procedure even though my research suggested other wise. feel free to contact me at my email for further 'complications' too personal to publish
Matter of fact, if the doctor you are using even suggests the vaginal mesh, GO TO ANOTHER DOCTOR.
Feb. 5 2012...update
well....she went back to see Dr. Michael Hulse for her stomach pain. He prescribed 4mg tablets of pregnisone...another mistake. Do not use this if you have high blood pressure..is one of the first warnings ...which he ignored...or made another 'mistake' like prescribing the neurotin thinking it was Advil....obviously, we discontinued the medicin after her blood pressure skyrocketed...avoid this guy or any doctor who recommends the mesh....period.
Per the Food and Drug Administration:

=== === === FDA NEWS RELEASE === === ===
For Release at 12 p.m. on July 13, 2011
From: Karen Riley
FDA: Surgical placement of mesh to repair pelvic organ prolapse poses
risks.
Agency says other options may expose
women to less risk than transvaginal procedure.

The U.S.
Food and Drug Administration today issued an updated safety communication
warning health care providers and patients that surgical
placement of mesh through the vagina to repair pelvic organ prolapse may expose
patients to greater risk than other surgical options.
The safety communication also says that with the exposure to greater
risk comes no evidence of greater clinical benefit such as improved
quality of life.
Pelvic Organ Prolapse (POP) occurs when the internal structures that support
the pelvic organs such as the bladder, uterus and bowel, become so weak or
stretched that the organs drop from their normal position and bulge or prolapse
into the vagina. While not a life-threatening condition, women withPOP often experience pelvic discomfort, disruption of their sexual, urinary, and
defecatory functions, and an overall reduction in their quality of life.
Surgery to repair POP can be performed through the abdomen or transvaginally,
through the vagina, using stitches, or with the addition of surgical mesh to
reinforce the repair and correct the anatomy.

“There are clear risks associated with the transvaginal placement of mesh to
treat POP,” said William Maisel, M.D., M.P.H., deputy director and chief
scientist at the FDA’s Center for Devices and Radiological Health. “The FDA is
asking surgeons to carefully consider all other treatment options and to make
sure that their patients are fully informed of potential complications from
surgical mesh. Mesh is a permanent implant — complete removal may not be
possible and may not result in complete resolution of complications.”

In 2010, there were at least 100,000 POP repairs that used surgical mesh.
About 75,000 of these were transvaginal procedures.

The FDA issued a safety communication in 2008 due to increasing concerns
about adverse events associated with the transvaginal placement of mesh. Since
then, the number of adverse events has continued to climb. From 2008 to 2010,
the FDA received 1,503 adverse event reports associated with mesh used for POP
repair, five times as many as the agency received from 2005 to 2007. The reports don’t always differentiate between transvaginal and
abdominal procedures.
The most frequently reported complications from surgical mesh used to repair
POP include mesh becoming exposed or protruding out of the vaginal tissue
(erosion), pain, infection, bleeding, pain during sexual intercourse, organ
perforation from surgical tools used in the mesh placement procedure, and
urinary problems. Some reports cited the need for additional surgeries or
hospitalization to treat complications or to remove the mesh.

The FDA also conducted a review of scientific literature published between
1996 and 2010 comparing mesh surgeries to non-mesh surgeries. The agency review
suggests that many patients who undergo transvaginal POP repair with mesh are
exposed to additional risks, compared to patients who undergo POP repair with
stitches alone. While mesh often corrected anatomy, there was no evidence that
mesh provided any greater clinical benefit than non-mesh surgeries.

FDA recommends that health care providers:

Recognize that in most cases, POP can be treated successfully without
mesh.

Know that surgical mesh is a permanent implant that can make any future
surgical repairs more challenging and can put the patient at risk for additional
complications and surgeries.

Consider that mesh placed abdominally for POP repair may result in lower
rates of mesh complications compared to transvaginal POP surgery with mesh.

Be sure that patients are aware of the risks and benefits of transvaginal POP
repair with mesh, and inform patients if mesh is being used.

The FDA recommends that patients:

Ask the surgeon before surgery about all POP treatment options, including
those that do not involve mesh: understand why the surgeon may be recommending
treatment of POP with mesh.

Continue with routine check-ups and follow-up care after surgery. Notify the
surgeon if complications develop (persistent vaginal bleeding or discharge,
pelvic or groin pain during sex).

Those who have had POP surgery but don’t know if the surgeon used mesh should
find out during their next scheduled visit with their health care
provider.

The FDA also announced that an outside panel of experts in obstetrics and
gynecology will meet on Sept. 8-9, 2011, to discuss the safety and effectiveness
of surgical mesh used to treat POP and stress urinary incontinence (SUI), a
leakage of urine during physical activity. The panel will discuss the risk of
transvaginal POP repair, clinical studies that may be necessary to address risks
and benefits of this type of surgery, and FDA’s interim recommendations for
health care professionals and patients.

“Input from the clinicians, manufacturers and other experts will help the FDA
better understand the safety and effectiveness of surgical mesh for POP and SUI
repair, including any changes that would improve our oversight,” Maisel
said.

Today’s safety communication is limited to the transvaginal placement of mesh
to repair POP. It does not address the safety and effectiveness of mesh used to
treat SUI or mesh implanted abdominally.

Associated with Transvaginal Placement of Surgical Mesh for Pelvic Organ
