			<content:encoded><![CDATA[<p><a href="http://www.drpersky.com/wp-content/uploads/2012/05/la-kings-win.jpg"><img class="alignleft size-medium wp-image-5358" title="Dustin Penner, Slava Voynov, Jeff Carter, Mike Richards" src="http://www.drpersky.com/wp-content/uploads/2012/05/la-kings-win-300x229.jpg" alt="" width="300" height="229" /></a>Congratulations to the <strong>Los Angeles Kings</strong> for advancing to the <strong>Stanley Cup Finals</strong>!  It has come to our attention after enjoying ice hockey for many years that there is a propensity toward facial lacerations, broken noses, and loss of teeth. </p>
<p> The best treatment for a <strong>facial laceration</strong> is different for National Hockey League players as compared to the general population.  In ice hockey, the goal is to get the wound repaired as quickly as possible and to get the player back into the game immediately.  In general, lacerations should be cleaned, debrided, and sutured to maximize the best healing with minimization of future scarring.</p>
<p>The wound should be kept clean, and in most cases the sutures (stitches) are removed from facial injuries at five days after the repair.  Steri-strips are applied across the wound to minimize tension, decreasing the chance noticeable scar formation.  Once the stitches are out, either Vitamin E oil or Aloe Vera (preferably from a living plant leaf) should be applied to the wound two to three times a day for a few weeks (if there is any redness or irritation from this then it should be discontinued).  Within a week, <strong>Fraxel restore</strong> erbium laser treatment can begin.  There are usually about 3 to 4 quick and easy laser treatments separated by 2-3 weeks.  There is no downtime.  The laser treatments end when we can&#8217;t find the scar anymore.  This is effective even for some older scars, say after the better team wins the Stanley Cup, and the season is over.</p>
<p><a href="http://www.drpersky.com/wp-content/uploads/2012/05/alec-martinez.jpg"><img class="alignleft size-full wp-image-5363" title="alec martinez" src="http://www.drpersky.com/wp-content/uploads/2012/05/alec-martinez.jpg" alt="" width="153" height="94" /></a>King defenseman <strong>Alec Martinez</strong> appeared to suffer a nasal fracture in the last game against the <strong>Phoenix Coyotes</strong>, playing the remainder of the game with packing in his nostrils.  Kudos to Alec, it&#8217;s not easy to breath, let alone skate up and down the rink at full speed.  Hopefully it was a non-displaced fracture.   For those of you not vying for the Stanley Cup, after a nasal fracture we recommend that you be evaluated by a nasal surgeon to make sure that there is not a septal hematoma (internal bleeding within the nose in the septum which is really the only time that immediate surgical intervention is indicated).  If there is no swelling and the nasal bones are obviously displaced, the nasal bones can be reset immediately by manipulating them into place.  In the great majority of cases, there is significant swelling in which case the nose should be re-evaluated in about 5 days at which time it can be determined if the bones need to be placed back into their normal position.  In the meantime, we recommend elevation of the nose above the heart (sleep on 3 to 4 pillows), use baggies of frozen peas on the nose for the first two days, and unlike our hockey players, avoidance of exertion, lifting, or exercising.</p>

Sezgin</strong>, et. al.)  have investigated what women of different ages presenting for facial cosmetic surgery consultation focus upon.  They grouped patients into 4 groups, those in their 20&#8242;s, 30&#8242;s, 40&#8242;s, and over 50.  Each group was asked what facial feature bothered them the most, as well as which of their facial features they liked the most.  The list of facial features included <strong>skin, forehead, periorbital region (eyes), eyebrows,</strong> <strong>cheeks, jawline, lips, and nose</strong>.</p>
<p>The top features that women in their 20&#8242;s noticed when they looked at themselves in the mirror were their nose and skin.  The most liked feature was the lips (36%).  The most disliked feature and feature that they wished to change the most was the nose (42%).  Remember that this study was conducted in Turkey, home of many a large nose. </p>
<p>Women in their 30&#8242;s focused on their skin and periorbital area in the mirror.  Their favorite feature was their lips, and most disliked feature again their nose which also was the feature they most wished to change (32%).</p>
<p>Women in their 40&#8242;s stated that the periorbital region and their skin was most focused upon in the mirror.  Their favorite feature was  their nose (apparently by the time most women in Turkey have reached their 40&#8242;s they have either had a <strong>rhinoplasty</strong>, or have grown to love their nose.)  The periorbital region was their most disliked, 36% wished to change the area around their eyes.</p>
<p>Women 50 and above focused on their periorbital area and jawline.  Again their most liked feature was their nose.  The most disliked area was their jawline which along with their periorbital area was the feature that they most desired to change.</p>
<p><a href="http://www.drpersky.com/wp-content/uploads/2012/05/roy-lichtenstein-girl-in-mirror-c-1963.jpg"><img class="alignleft size-full wp-image-5340" title="roy-lichtenstein-girl-in-mirror-c-1963" src="http://www.drpersky.com/wp-content/uploads/2012/05/roy-lichtenstein-girl-in-mirror-c-1963.jpg" alt="" width="160" height="160" /></a>In summary, younger women&#8217;s self-beauty focal points were their nose, skin, and lips.  For older women it was their jawline and periorbital area.  Younger patients aim to achieve facial harmony (smaller, hump-free nose), while older patients seek to restore a more youthful appearance (eye and jawline rejuvenation).  </p>
<p>Another recent study by <strong>I. Springer</strong> showed that impaired well-being is associated with impaired facial perception.  Isn&#8217;t this what <strong>Billy Crystal&#8217;s</strong> character on <strong>Saturday Night Live</strong> was saying, &#8220;When you look Mah-vah-lous, you feel Mah-vah-lous!&#8221;?  While we are looking at studies, another showed that 20% of patients who undergo cosmetic surgery have a history of psychiatric disorders (on some days around the office it seems a lot higher than that!)  80% of those patients are clinically depressed.  Finally, there are multiple studies that have shown that a positive change in appearance can improve psychological well being, as well as symptoms of <strong>depression</strong> (<strong>Botox</strong> among treatments that can improve a patient&#8217;s attitude).</p>
<p>As Sezgin discusses in his article, &#8220;Throughout history, the relationship between a woman and her mirror has made its way  into children&#8217;s fairy tales and other literature.  The tug-of-war between the two can be continuous; if a woman constantly desires to be at peace with her reflection in the mirror and yet she is not, the desire to change and the process of changing begins.&#8221;  As we have said before, beauty comes from within, from the heart, from a free mind, and if that is not enough, then it may be time to consider an aesthetic consultation with a board certified professional.  Be well.</p>

			<content:encoded><![CDATA[<p><a href="http://www.drpersky.com/wp-content/uploads/2012/05/Kathie-Lee-Ulthera.jpg"><img class="alignleft size-full wp-image-5329" title="Kathie Lee Ulthera" src="http://www.drpersky.com/wp-content/uploads/2012/05/Kathie-Lee-Ulthera.jpg" alt="" width="120" height="90" /></a><strong>Ulthera</strong>, the revolutionary non-surgical skin lifting and tightening ultrasound device, was featured on <strong>The TODAY Show</strong> on May 3, 2012.  <strong>Dr. Pat Wexler</strong> and <strong>Dr. Francesca Fusco</strong> treated <strong>Kathie Lee Gifford&#8217;s</strong> right eye brow on camera.  Seeing is believing, and we have seen definite improvement in the vast majority of patients that we have treated with Ulthera.  Unfortunately, on the video clip of Kathie Lee&#8217;s treatment I was hard pressed to see any difference when the ladies were all raving about how they &#8220;&#8230;could already see the difference!&#8221; </p>
<p>After watching the clip again, it was apparent why I did not appreciate the difference that the doctors were claiming.  The area was treated with only a small superficial transducer that was set to a very low energy level and a very short length line of energy delivered.  Dr. Fusco treated Kathie Lee with only 10 pulses or lines.  The usual treatment involves at least 10 lines with a deep, higher energy transducer followed by 10 lines with the standard length more superficial transducer.  Ulthera treatment is uncomfortable, but tolerable.  Most likely the very weak settings were used on Kathie Lee to give the appearance that the treatment is not painful.</p>

<p>Ulthera is a great treatment that is safe and effective, but I am sorry that this live treatment performed on Kathie Lee does not represent the product as it should, especially the uncalled for enthusiastic reaction of the women claiming a visual &#8220;difference&#8221; when there was not one.  Hopefully, Kathie Lee received a full standard treatment off air and we will be able to see the difference in a few months.  Seeing is believing!</p>

<p><a href="http://www.drpersky.com/wp-content/uploads/2012/04/da-vincis-proportions1.jpg"><img class="alignleft size-thumbnail wp-image-5305" title="da vincis proportions" src="http://www.drpersky.com/wp-content/uploads/2012/04/da-vincis-proportions1-150x150.jpg" alt="" width="150" height="150" /></a>I have given two lectures in the past three days on &#8220;<strong>Beauty Secrets</strong>&#8220;.  Part of my presentation discusses <strong>Leonardo da Vinci&#8217;s </strong>devine proportions of beauty.  While watching  Ms. Blunt in the film,  it was apparent that her facial proportions are consistent with the vertical fifths and horizontal thirds that da Vinci describes.  Her forehead appeared to have just the right amount of neuromodulator (<strong>Dysport</strong>, <strong>Botox</strong>, or <strong>Xeomin</strong>) as it appeared smooth yet had some movement (not available during da Vinci&#8217;s times).</p>
<p>Producer <strong>Judd Apatow</strong>, there are some &#8220;toe&#8221; puns in the movie, captured the intricacies of today&#8217;s man-woman and parent-child relationships.  The story line took this baby boomer back to <strong>Dustin Hoffman&#8217;s</strong> relationship in &#8220;<strong>The Graduate</strong>&#8221; with the on again, off again nature of the main characters relationship (as well as the partial Northern California location of the shoot).  The film was brilliantly cast, though as a <strong>rhinoplasty </strong><a href="http://www.drpersky.com/wp-content/uploads/2012/04/Brian-Posehn.jpg"><strong><img class="alignright size-full wp-image-5302" title="Brian Posehn" src="http://www.drpersky.com/wp-content/uploads/2012/04/Brian-Posehn.jpg" alt="" width="99" height="99" /></strong></a><a href="http://www.drpersky.com/wp-content/uploads/2012/04/Tracee-Chimo.jpg"><strong><img class="alignleft size-full wp-image-5300" title="Tracee Chimo" src="http://www.drpersky.com/wp-content/uploads/2012/04/Tracee-Chimo.jpg" alt="" width="214" height="314" /></strong></a><strong>surgeon</strong> I thought that the delicatessen employee&#8217;s noses were a bit too Semitic (<strong>Tracee Chimo</strong> and <strong>Brian Posehn</strong> right and left  respectively).  <strong>Alison Brie </strong>was well cast as Ms. Blunt&#8217;s sister.  Ms. Brie is very beautiful herself, and also has da Vinci like facial proportions. </p>

<p>Cellulaze is a new technology used to treat <strong>cellulite</strong>.  It seems to be promising, but as with all new devices, I prefer to be conservative in their use.  If you are young with minimal cellulite, it won&#8217;t hurt to wait a year or so to determine the safety and effectiveness of Cellulaze in cllinical use.  The last thing that you want is to have an adverse event when your concerns were not so bad to begin with.  As with all treatments, be sure to choose your treating physician carefully.  Good luck and be well.</p>

Dr. Persky just returned from a multi-speciality aesthetic  conference (<strong>Facial Aesthetics State of the Art 2012</strong>)  that was not only for core multi-speciality aesthetic physicians (plastic surgery, facial plastic surgery, dermatology, and opthalmo-plastic surgery), but also included &#8220;aesthetic extenders&#8217; and non-aesthetic core physicians.   An internist without any experience in plastic surgery approached me during a break  asking me what I thought about the surgical tool in the exhibitors hall that she was told &#8221; With this you can do a blepharoplasty (eye lid surgery)&#8221; , and whether she should purchase the surgical tool, take it back home to her office, and perform blepharoplasty.  I  was  shocked and succinctly dissuaded her from attempting any surgery without a proper training residency.    At a recent Scuptra injection conference in Beverly Hills  with premiere injector <strong>Danny Vllegar, MD</strong> (Vienna),  I was not surprised by some of the improper injecting techniques that were being used by nurses, physician&#8217;s assistants, and other &#8220;extenders&#8221;  from high profile practices when they discussed their techniques.   Patients should also beware that there is an increase of  Botox and Laser &#8220;centers&#8221; in Urgent Cares, and strip mall stores.   Needless to say, too ofter patients &#8220;get what they pay for.&#8221;  An advantage of being treated by a facial plastic surgeon with artistic vision is that you may not only need that touch of Botox, but by adding a little something here or there, your aesthetic result is greatly enhanced.  Please choose your  facial aesthetic provider carefully, it is much easier for us to take care of you from the beginning, and much more difficult to correct problems.</div>

Retired world famous facial plastic surgeon <strong>Frank Kamer, MD</strong> has also said that though our surgical goal is to create symmetry,  slight asymmetries in rhinoplasty are okay, even lending to a more natural appearing nose. <strong>Rhinoplasty</strong> remains the most difficult of all facial plastic surgery aesthetic surgeries (other than the miraculous full facial transplants that we are currently witnessing).  The key to a great nose is that it &#8220;gets out of the way&#8221; of the patient&#8217;s eyes and other facial features.  While in the past, an obvious nose job was a sign of status, today natural and unoperated upon appearing noses are preferred .  We are using more conservative techniques such as using <strong>sutures</strong> to position and re-shape nasal cartilages instead of removing excessive cartilage.  <strong>Diced cartilage grafts</strong> wrapped in temporalis fascia allows the rhinoplasty surgeon to augment the nasal profile in a most natural manner.</div>
<div><strong>Sculptra</strong> and <strong>Ulthera</strong> are excellent non-invasive, non-surgical treatments to restore facial volume, shape, and tightness.  In combination they are even more effective.</div>

<p><a href="http://www.drpersky.com/wp-content/uploads/2012/04/persky-lecture4.jpg"><img class="alignleft size-medium wp-image-5228" title="persky lecture" src="http://www.drpersky.com/wp-content/uploads/2012/04/persky-lecture4-224x300.jpg" alt="" width="224" height="300" /></a>I was honored and humbled to be included among many of the world&#8217;s experts in facial plastic surgery this past week at the 20&#8242;th Annual <strong>State of the Art in Facial Aesthetics 2012</strong> multi-speciality course.  <strong>Phil Langston, MD</strong> (Memphis, TN) chaired an excellent meeting that covered all of facial plastics.  I presented, <strong>&#8220;Effective Clinical Use of Micro-Focused Ultrasound (Ulthera/Ultherapy) to Lift and Tighten the Skin&#8221;, &#8220;Effective Clinical Use of Cryolipolysis (CoolSculpting)&#8221;, &#8220;The Treatment of Stretch Marks with Non-Ablative Fractionated Laser</strong>&#8220;, and participated in panel discussions on skin care, injectables, and non-ablative lasers.</p>
<p>Highlights of the meeting included <strong>rhinoplasty, facelift, blepharoplasty, injectable, and non-invasive aesthetic procedures</strong>.  <strong>Ronald Gruber, MD (Oakland, CA)</strong> and <strong>Richard Zeph, MD (Carmel, IN)</strong>  wowed attendees with their artistic rhinoplasty results.  Course director Phil Langston, MD, and world renowned <strong>Gaylon McCollough, MD</strong> shared their invaluable rhinoplasty experience and wisdom.</p>
<p><strong>Sam Lam, MD</strong> led a day long hair transplant seminar in addition to sharing pearls from his successful practice throughout the week.  <strong>Catherine Maley</strong> led lectures in improving aesthetic patient experiences.</p>
<p>So what&#8217;s new in facial aesthetics?  Surgery remains the gold standard for lifting and tightening the face.  Non-invasive technologies remain in demand.  Fillers such as <strong>Juvederm, Restylane, Radiesse, and Sculptra</strong>plus neuromodulators (<strong>Botox and Dysport</strong>) are as popular as ever.  <strong>Ulthera</strong> is the currently best non-invasive method to lift and tighten skin.  <strong>CoolSculpting</strong>is easy, safe, and effective in treating unwanted fat bulges that have been resistent to diet and exercise.  Dr. Gruber shared new and innovative techniques that will better my rhinoplasty results.</p>
<p>All in all, it was great to be back in Buckhead!  For those of you not familiar with Atlanta, Buckhead is a suburb where I spent my medical school days while at <strong>Emory University Medical School</strong>&#8230;many years ago.  Springtime in Atlanta is a most wonderful time bringing back many fond memories while attending the conference with old colleagues and friends, meeting new ones, and sharing our facial aesthetics knowledge to promote better aesthetic outcomes for our patients.  Be well y&#8217;all!</p>

		<title>Allergan (Juvederm/Botox)—Merz (Radiesse/Belotero/Xeomin) Wars</title>

			<content:encoded><![CDATA[<p>Aesthetic physicians received a fax from <strong>Merz Pharmaceuticals</strong> late Friday afternoon informing them that they will not be allowed to promote Merz facial fillers <strong>Radiesse</strong> and <strong>Belotero</strong> and its neuromodulator <strong>Xeomin</strong> to their practices until further notice.  Radiesse will still be available, but Xeomin will not, and the release of Belotero, a highly anticipated new facial filler, may be delayed until court details of a lawsuit brought by <strong>Allergan</strong> (maker of <strong>Botox/Juvederm</strong>) against Merz are worked out.</p>
<p>According to <strong>Michael J. Sacopolus, JD,</strong> legal analyst for <strong>Plastic Surgery Products</strong> &#8220;Earlier this week a Federal Court Judge in Santa Ana, California entered into an injunction against Merz. The injunction stopped Merz from marketing Xeomin to the facial aesthetic community. This injunction came at the request of Allergan whose Botox product would directly compete with Xeomin. The judge sided with Allergan in finding that former Allergan employees had blatantly misappropriated Allergan’s proprietary information when they switched employment to Merz. The judge next indicated the full extent of the injunctive order as the scope, terms and duration would be released Friday March 9<sup>th</sup>.&#8221;</p>
<p>Merz recruited and hired ex-Allergan employees and used their Allergan data, sales strategies, targets, physician lists, and much more in promoting Xeomin and Radiesse.  Xeomin will likely not be available for the remainder of 2012.  For those of our patients wanting to get a dose of Xeomin before who knows when, <strong>Persky Sunder Plastic Surgery</strong> has two vials left.  <strong>Dysport</strong> and Botox are still available as usual.</p>
<p>Now if Allergan could just do something about that darn <strong>Medicis </strong>company and their neuromodulator Dysport, they would have their &#8220;Botox Monopoly&#8221; back!  Good luck and be well!</p>

<p><em>Since early childhood we knew that when Pinocchio told a lie his nose would grow larger.  In the recent case of an Egyptian botched nose job incident, a gentleman&#8217;s nose was made smaller and he lied.</em></p>
<p><em><a href="http://www.drpersky.com/wp-content/uploads/2012/03/anwar-bilkimy-2.jpg"><img class="alignleft size-full wp-image-5197" title="anwar bilkimy 2" src="http://www.drpersky.com/wp-content/uploads/2012/03/anwar-bilkimy-2.jpg" alt="" width="124" height="90" /></a>They are calling it the curious case of the nose job in Egypt.  Not since Cleopatra has aesthetics been so in the Cairo news.  Anwar Bilkimy, an ultraconservative<a href="http://www.drpersky.com/wp-content/uploads/2012/03/Anwar-Bilkimy.jpg"><img class="alignright size-full wp-image-5198" title="Anwar Bilkimy" src="http://www.drpersky.com/wp-content/uploads/2012/03/Anwar-Bilkimy.jpg" alt="" width="126" height="84" /></a> Islamist member of the newly elected Egyptian parliamnent attempted to &#8220;mask&#8221; his nasal plastic surgery by convincing fellow lawmakers that his bandaged and bruised face resulted from an attack by thugs.  Ironically, Bilkimy is a member of the parliament&#8217;s subcommittee on health!   He initially drew the sympathy of his colleagues when they visited him at the hospital, that was until one of his physicians, Dr. Mahmoud Nassef (any relation to &#8220;Housewives of Beverly Hills&#8221; Paul?) stunned by the lie that crossed the bounds of religion and vanity, told investigators the truth.  Bilkimy immediately submitted his resigation from parliament in public embarrassment. </em></p>
<p><em>Though cosmetic surgery in Beverly Hills and Encino is not condemned as tampering with God&#8217;s creation as it is by many ultraconservative Islamists, we too are asked by many of our patients to be party to a &#8220;fib&#8221; about their surgery.  &#8220;Please don&#8217;t show my before and after photos to anyone, I want them to think I was born this way&#8221;, &#8220;We are having her rhinoplasty done this summer between middle school and her new high school, we don&#8217;t want anyone to know&#8221;, &#8220;I&#8217;m doing this while my husband is out of town&#8221;, and &#8220;I&#8217;ll just say that I bumped into a cabinet&#8221; are often heard around our office.  We respect our patients privacy and wishes, but don&#8217;t be angry with us when we bump into you in public and &#8220;don&#8217;t remember where we know you from.&#8221;  </em></p>
