In some cases , prolapsed disc may cause pain which runs down the buttocks. In the majority of cases, this pain will settle down with non-surgical treatments. Surgery is only recommended in people who are in a hurry to recover and do not want to be bothered with exercise and/or a weight loss program. Now the chiropractors have come up with their own treatments because they can’t not prescribe medications. Chiropractors offer spinal manipulations for treat almost every medical disorder under the sun. Cochrane reviews has looked at many of these therapies and found that spinal manipulations is no better than physical therapy, medication or exercise. Moreover, chiropractic spinal manipulations have to be undertaken for many weeks and each session can cost anywhere from $75-$200. Best advice- start your own exercise program, swim, walk, lose weight and stop smoking. This is a condition, which may affect more than one vertebra. Normally nerves in the spinal cord exit along the gaps in-between the vertebrae called foramen (openings). Sometimes this foramen can become narrow and impinge on the nerve. Aging and arthritis are two most common causes of foraminal stenosis. The symptoms of foraminal stenosis are similar to spinal stenosis. However, not all foraminal stenosis requires surgery. When the stenosis is ignored, some individuals can develop wasting of muscle and weakness- in such cases surgery is required.

Foraminotomy is often combined with laminectomy to provide the maximal benefit. If you have no symptoms or mild symptoms, surgery is best avoided. Does it have any benefits?

Low back pain is a very common and disabling disorder. There are hundreds of treatments for back pain but none is ideal. Over the past few years, many doctors and alternative health care workers have been offering spinal manipulation therapies or “hands-on treatments” for the spine. This basically consists of manipulation and mobilization of the back joints. The manipulations are carried in certain directions and the often the joints are “cracked.” These treatments are undertaken for many weeks and months and are quite expensive. By the end of 3-6 months, you will most likely have spent anywhere from $3,000-$8,000 for the treatments. So does the therapy work?

Analysis of data done by Cochrane reviews revealed that spinal manipulation therapy was no better than physical therapy or regular exercise. So what does this mean to the consumer?

If you have money to spend, then perhaps you may want to start with a physical therapy program. If you have no money, then you can obtain the same benefit by doing exercise at home or at the gym. The type of exercises to help increase muscle strength and mobility can be obtained from cyberspace for free. If you do not have internet, swimming and walking will do the trick.

The concept is simple. You sit or stand and a device gently pulls on either end of your body. This will supposedly relieve the pressure on the joint and you will be free of back pain. Each session can cost anywhere from $175-$300 and on average one needs 20-30 sessions. Low back pain is a major health problem and accounts for large number of people being absent from work, being disabled and depressed. So far, no one therapy seems to work for all people with back pain and now we have traction.

It does not work; it provides little or no pain relief and does not reverse your disability. The only people who benefit from traction therapy are the people who sell it- these health care providers are the ones who drive the fancy cars with your hard-earned money. There is not a single well-controlled randomized study that shows that traction is effective for low back pain. Of course, there are some bogus studies from the Orient or those published by the manufacturers, but the results are fraudulent.

Should I go for it?

The medical treatment of back pain sucks. In the last 2 decades, surgeons have gotten together with the pharmaceutical companies and developed devices that can be implanted into the spinal cord. The devices consist of an electrical generator that delivers pulses to a targeted area of the spinal cord. The devices are implanted using a surgical procedure and are powered by an implanted battery. How these devices work is not known but thousands of such devices are inserted each year. So do these implants work for pain relief?

From the literature, spinal cord devices for pain work in less than 3% of patients with only some types of back pain. The data show that these devices are absolutely crap for failed back surgery and any other type of complex back pain.

Unfortunately, the field of pain control is full of surgeons who have turned business people and have started to insert these devices in anyone who walks into their office. These pain devices are not reimbursed by any insurance company, are prohibitively expensive and most times do not work. So even if you have any common sense, one of the things you should do is avoid surgeons who want to scam you out of your hard earned money by inserting devices which basically suck.

Do you think carbamazepine may help?

Many people have chronic back pain for which there is no good therapy. While there are potent narcotics available, most doctors will not prescribe long-term narcotics because they are addictive, have serious side effects and there is always a hassle with the DEA. For many people with chronic back pain, it is a real struggle to lead a decent quality of life.

In the last 2 decades, many doctors have been prescribing a variety of other drugs for pain. One of these drugs is carbamezepine. This anti seizure drug is widely prescribed for a variety of chronic nerve pain syndromes but the question remains, “is it effective?”

Review of the literature indicates that carbamazepine does work for chronic nerve pain, at least in the short term. Most people report moderate pain relief. However, at least 1/3 of patient s also developed minor side effects ranging from skin rash to abdominal cramps. Therefore, if you have chronic nerve pain, perhaps you can ask your doctor for a trial of carbamazepine. The drug does take a few weeks to become effective and is not cheap.

<link rel='alternate' type='text/html' href='http://sbmedexback.blogspot.com/2011/01/i-have-had-severe-low-back-pain-from.html' title='I have had severe low back pain from a slipped disc for years. In the last 20 years, hundreds of traction devices have been developed for back pain. Most of these devices have been developed with the help of doctors who often own the company or have major shares. There are many types of traction therapies- some work while you stand, others work while you lay down on a table, some can be used at home and others work while you sleep. Cochrane review looked at the latest data on traction devices and found no evidence that these machines benefit anyone with back pain. Moreover these traction sessions go on for months and are costly. Some of the traction sessions cost $200 per hour. By the end of 16 weeks, you spend as much on these B.S. therapy as you would on brand new car- the only difference is that the doctor buys the car with your money and you continue to walk.

If you have any common sense, avoid traction nonsense and save your hard earned money. In any case, the decision to undergo surgery is complex and needs to be thoroughly discussed with the surgeon. You need to get a surgeon who is more interested in your back than the money. Plus, the surgeon must be competent. Thirdly, check out the surgeon’s experience and record. Ask to talk to his/her former patients. And most important, get to know the procedure and its possible complications. Surgery for back pain has a very poor track record of success. In the entire USA, you are not likely to meet more than a handful of people who will claim that their back surgery was satisfactory. Not only are some back surgeons terrible, they also do not really care about their outcomes.

In the majority of cases this may take anywhere from 6-16 weeks. Of course, you will need some pain medications for the first few weeks. However, since many patients have no idea how serious their back disorder is, they may not always be aware of all the possible options. Patients are always at a major disadvantage when it comes to dealing with surgeons. Physicians and surgeons in general will recommend therapies which they know are expensive- irrespective whether they work or not. The best advice is to read up on your back pain, talk to other people and not agree to any treatment without a second opinion. It is the rare patient who ever improves with back surgery

Plain x-rays are often done by doctors but these imaging studies are a waste of money and often delay proper treatment in some cases. However, even with a diagnosis of herniated disc the treatment is somewhat difficult. There is no one treatment that works in everyone. Even the same treatment may not work in the future. For most patients with back pain, the treatment has to be individualized. Unfortunately, most health care professionals simply write a pain prescription, send the individual for physical therapy exercise clinic (most likely owned by the physician or his colleagues), and then recommend surgery.

I have been told I have a herniated disc. Basically, all of us have a jelly like substance in the vertebral joints. This substance playa a vital role in protecting the nerves and also cushions the joints from compressing against each other. Sometimes, during trauma or lifting weights this jelly-like substance leaks and impinges on the nerves which are leaving the spinal cord. When the nerves are pinched or compressed, the patient feels pain. When a herniated disc develops, it can limit how an individual functions on a daily basis- the chief reason is because of continuous pain. The pain is a dull ache which often radiates to the back of the legs. The majority of herniated discs occur in the lower back.

<link rel='alternate' type='text/html' href='http://sbmedexback.blogspot.com/2011/01/i-have-been-told-i-have-herniated-disc.html' title='I have been told I have a herniated disc. There are some data to suggest that heat therapy is helpful when the pain is acute and the duration of pain is less than 3 months. However, many doctors continue to recommend these treatments for people who have had back pain for many years.

Heat treatments comes in many forms –from hot water bottle, heating pads, electrical appliance pads and infrared lamps. The cold treatments come as cold ice pack, gels, cold towels and ice massages. However, the question is, “do warm or cold compress help relieve back pain which is long standing?”

A recent review of major studies on low back pain where both heat and cold treatments were used revealed the following. There was little benefit from both ice and heat treatment if the back pain was of more than 3 months in duration. However, if the back pain was of short duration, heat treatments only provided minimal relief that was of short duration. In some cases, addition of exercise did help reduce the pain and helped improve function. As far as cold treatment were concerned, there is very little data to support their use. Most people did have any benefit from cold treatments

So what should the consumer do?

If you have new onset of back pain, try some heat and start an exercise program. As far as ice packs are concerned, save your money for the pain pills.

Is there a permanent cure for back pain?

Unfortunately No. Back pain affects millions of people and accounts for high morbidity. For most people, the only way to relieve back pain is with the use of pain killers. All these either provide temporary relief or are worthless remedies. In most cases, use of these back pain remedies will only worsen your pain and make you go broke.

As far as surgery is concerned, keep away from back surgeons unless it is an absolute emergency. It is the very rare patient who ever benefits from back surgery. For every one happy individual, there are thousands more who are incapacitated and in worse shape then before. Moreover, back surgery is often associated with serious complications.

You will live a lot longer (and get to keep your money) if you keep away from back surgeons and the nonsensical traction devices.

Does traction therapy help back pain? Part 2

That traction is at best considered sham or a scam- which means it did diddly squat. Traction was no more effective than taking a sugar pill (placebo). A variety of traction devices were analyzed and the conclusion was the same- traction devices are nothing more than a scam. Irrespective of whether the individual underwent continuous traction or intermittent traction, no one benefitted from this type of therapy In fact many individuals developed more complications from traction and some even required emergent surgery.

The authors concluded that traction alone as a treatment for low back pain is as good as garbage.

So if you have back pain, save your hard earned money on these sham devices and keep away from scamming health care professionals who are more interested in your money than your back.
Part 1

Low-back pain (LBP) affects millions of individuals in North America and is a major health problem. The medical costs of low back pain are enormous resulting in absenteeism, disability and poor quality life style. No one satisfactory treatment for low back pain exists and almost every week a new therapy is developed. In the last 2 decades, traction has been advocated as a panacea for back pain. There are several type of devices ranging from mechanical and manual traction.

Today, many physicians have fancy traction devices in their clinics which they hype up as a cure for sciatica and other types of back pain. It is recommended that one enroll in traction treatment sessions 1-2 times a week for 12-16 weeks and voila, you will be cured. One should not forget that traction treatments do not come cheap. All the data on traction treatment and its benefits come directly from the mouth of people who make these machines or physicians who promote these devices.

So do they work?

Researchers looked at randomized controlled studies to determine if traction really worked. More than 2200 patients who received 1045 sessions of traction were evaluated from several clinical trials.

And what did the researchers find?

<link rel='alternate' type='text/html' href='http://sbmedexback.blogspot.com/2010/02/does-traction-therapy-help-back-pain.html' title='Does traction therapy help back pain?
