NIK / No.Passport (for foreigners) - For the citizen, filled with NIK listed on the Family Card (KK) or KTP. As for foreigners filled with a valid passport number.
Gender - Gender PTK.
Place of birth - Place of birth PTK appropriate relevant official document.

Educational Qualification - Qualification PTK.

Formal Education Unit - Name of the education unit PTK. If not passed, according to current nomenclature. If passed, the appropriate nomenclature listed on the diploma.
