<p>As discussed in our previous articles on Brachycephalic Obstructive Airway Syndrome, dog breeds that have “flat faces” such as the French Bulldog, Pug and English Bulldog, are classified as “brachycephalic”.<sup>1</sup> This means that they have a short, wide skull in comparison to other dog breeds.<sup>1</sup></p>

<p>Unfortunately, selective breeding for these extreme physical features has resulted in a plethora of health problems for these dogs, which we will expand upon below, specifically focusing on French Bulldog health issues.</p>
<h2>Most Common French Bulldog Health Issues</h2>

<h3>Breathing Issues (Respiratory Disease)</h3>
<p>Due to their abnormal, shortened skulls, brachycephalic dogs have an excess of soft tissue in their upper airway, which reduces their ability to breathe normally. They can also have an abnormally small or narrowed windpipe, termed a &#8220;hypoplastic trachea&#8221;, which further reduces their ability to get air down into their lungs.<sup>1</sup></p>
<p>These features are part of a condition called Brachycephalic Obstructive Airway Syndrome (BOAS), which means that they have abnormal airways which reduces their ability to breathe normally. In severe cases BOAS can even result in death due to complete obstruction of the airway or through secondary complications such as aspiration pneumonia.<sup>1</sup> Unfortunately, this is one of the most common french bulldog health issues.</p>
<p>Symptoms of BOAS include snoring, snorting, noisy breathing, exercise and heat intolerance.<sup>1</sup> These symptoms are so common among brachycephalic breeds such as French Bulldogs that they are incorrectly considered “normal for the breed” by many.<sup>2</sup></p>

<h3>GI Issues (Gastrointestinal Disease)</h3>
<p>There are several gastrointestinal disorders that have been recognized in brachycephalic dogs, such as gastro-esophageal reflux (also known as GORD or heartburn in people), esophagitis/gastritis and hiatal hernias (when a portion of the stomach inappropriately moves into the chest cavity).<sup>1</sup> These gastrointestinal disorders are linked to their respiratory disease.<sup>3</sup></p>
<p>Brachycephalic Obstructive Airway Syndrome results in chronic increased negative airway pressure, as these dogs must increase their breathing (inspiratory) effort to suck air past their obstructed upper airways.<sup>3</sup></p>
<p>You can think about this as creating a vacuum effect, which “sucks&#8217; &#8216; the contents of the stomach up into the esophagus, which is gastro-intestinal reflux. Chronic gastro-intestinal tract upset is uncomfortable and presents with symptoms of nausea (turning their nose up at food, licking lips), flatulence and reflux (bringing up white frothy foam).<sup>1,3</sup></p>
<h3>Musculoskeletal Issues</h3>
<p>French Bulldogs have an increased prevalence of fractures of the upper arm near the elbow (humeral condylar fractures).<sup>4</sup> They have also been found to be at an increased risk of dislocating their knee caps (patellar luxation).<sup>9</sup> Additionally, hip dysplasia is also observed at a high rate in brachycephalic breeds.<sup>6</sup></p>
<h3>Neurological Issues</h3>
<p>As an additional consequence of an abnormally shortened and widened skull, brachycephalic breeds are predisposed to neurological conditions such as excess cerebrospinal fluid on the brain (hydrocephalus) and a condition where the skull is too small for the size of the brain (Chiari-like malformation).<sup>6</sup></p>
<p>Another abnormal feature of these breeds includes vertebral abnormalities and malformations such as hemivertebrae, which can predispose to other spinal disease processes such as intervertebral disc disease (IVDD).</p>
<p>Intervertebral disc disease, also known as a “slipped disc” or “herniated disc” can result in permanent paralysis in severe cases. French Bulldogs with IVDD are particularly prone to recurrence, with up to 50% of Frenchies that had previously experienced a slipped disc going on to develop another slipped disc.<sup>8</sup> This is unfortunately another very common French Bulldog health issue that owners should be aware of.</p>
<h3>Reproductive Issues</h3>
<p>Brachycephalic breeds are predisposed to reproductive problems such as difficult or obstructed labor (dystocia). Brachycephalic puppies have large, wide heads, which when combined with the narrow pelvis of the brachycephalic bitch, results in an inability to give birth naturally.</p>
<p>One European study found that 94.8% of English Bulldogs are delivered via Caesarean section.<sup>10</sup> Another UK study found that French Bulldogs were 15.1 times more likely to have difficulty giving birth than other crossbreeds. <sup>11</sup></p>
<h3>Eye Issues (Ocular Disease)</h3>
<p>The abnormal skull shape of brachycephalic breeds has also predisposed them to eye (ocular) disease, which is termed “Brachycephalic Ocular Syndrome (BOS)”.<sup>12</sup> Their compressed skulls have resulted in shallow eye sockets and they also commonly have abnormal eyelid development.</p>
<p>Additionally, brachycephalic breeds have also been found to have less tear production than other breeds. These factors predispose brachycephalic dogs to corneal ulcers, which is damage to the cornea (the outer, clear layer of the eyeball). Corneal ulcers are painful and in severe cases may result in the patient needing to have their eye removed surgically.<sup>12</sup></p>
<h3>Skin and Ear Issues (Dermatological Disease)</h3>
<p>Dermatological (skin &amp; ear) disease is commonly seen in brachycephalic breeds. They are predisposed to environmental or food allergies) (atopic dermatitis) and skin fold dermatitis (e.g. facial fold or tail fold dermatitis). As well as skin disease, brachycephalic breeds are also over-represented for outer ear infections, which can progress to middle and inner ear infections.<sup>13</sup></p>
<h3>Dental Issues</h3>
<p>Finally, due to their abnormally shortened skulls, brachycephalic breeds are predisposed to malocclusions, which is when the teeth do not line up appropriately. This can cause pain as the teeth may then be pressing into the soft tissues of the mouth or other teeth. Additionally, crowing and rotation of teeth predisposes to periodontal disease.<sup>15,16</sup></p>
<h2>Welfare Concerns for French Bulldog Health Issues</h2>
<p>Given the number of diseases that are associated with brachycephalic breeds, there is no wonder why the explosion in popularity of French Bulldogs has been considered an animal welfare crisis. Dogs that suffer from Brachycephalic Obstructive Airway Syndrome (BOAS) that may be considered clinically normal have been shown to have chronically low blood oxygen levels (hypoxia).<sup>3</sup></p>
<p>BOAS is also associated with sleep apnea, with many photos of Frenchies sleeping while sitting up or with a toy in their mouth going viral as “cute” and “funny”, when in fact these dogs are trying to keep their airway open whilst they sleep. These dogs are often chronically tired (which may be mistaken as being “lazy”) as they wake several times through the evening and will often be seen falling asleep whilst sitting or standing.<sup>3</sup></p>

<p>Many veterinary professionals have reported that brachycephalic breeds, when undergoing anesthetic, will commonly tolerate their breathing tube much longer than other breeds. A breathing tube is a plastic tube that is placed down the airway while under anesthesia to keep it open and allow delivery of oxygen and anesthetic gasses.</p>
<p>When dogs begin to wake from the anesthesia, they dislike the feeling of the breathing tube and it is gently removed. However in brachycephalic dogs, they will commonly tolerate this tube until they are completely awake, as this is likely the best they have ever been able to breathe.</p>

<p>While BOAS surgery is an option for these dogs, BOAS is a progressive disease, meaning that it will generally worsen over time.<sup>1</sup> Surgery aims to slow the progression of secondary changes associated with the disease such as laryngeal collapse, but it is not a permanent or complete fix for these dogs.</p>
<p>In addition to their breathing issues, intervertebral disc disease (IVDD) has become a recurrent issue in French Bulldogs. IVDD can be particularly distressing for owners and the veterinary team, as in severe cases it can be fatal. It is particularly heart-wrenching to see French Bulldogs who have previously undergone spinal surgery and recovered, present to the veterinary hospital with another blown disc, which may occur less than 12 months after the first one.<sup>8</sup></p>

<p>Another concern is the evidence that brachycephalic breeds live shorter lives than non-brachycephalic breeds. A recent UK study found that the average life expectancy for a French Bulldog is 4.53 years.<sup>17</sup> This is much lower than the commonly cited 10-12 years. Other studies comparing brachycephalic to non-brachycephalic breeds have shown with statistical analysis that flat faced dogs such as the <a href="https://frenchbulldog.org/english-bulldog-vs-french-bulldog/">English Bulldog </a>have much lower health, which is driven by their extreme body and facial features.<sup>18</sup></p>
<h2>Brachycephaly and French Bulldog Health: Owner Perceptions</h2>
<p>So with all these French Bulldog health issues, why do people keep buying Frenchies and other brachycephalic breeds? Several studies have demonstrated that owners of brachycephalic breeds do not perceive that their dogs are in ill health.</p>
<p>One such study that surveyed brachycephalic owners found that 20% had undergone at least one corrective surgery and 36.5% of owners reported that their dog had intolerance to heat. However, despite this awareness, 70.9% of owners considered their dog to be “in very good health” or “best health possible”.<sup>2</sup></p>
<p>There is clearly a disconnect between the health of flat-faced dogs and owner perception.</p>
<p>Unfortunately, symptoms of disease in these dogs such as snoring, snorting, heat and exercise intolerance are so common that it has become considered “normal for the breed”. It is important for us all to recognize that these symptoms are not normal and reduce the quality of life in these dogs.</p>
<h2>Ways to Improve French Bulldog Health Issues</h2>
<p>So what are the solutions? There are many opinions on how we should be addressing this animal welfare crisis and there are many different stakeholders involved, including veterinarians, animal welfare groups, breeders and owners.</p>

<p>In Norway, legislation was passed to ban breeding of the English Bulldog, with the court ruling that breeding of these dogs breached the county’s animal welfare laws. While banning brachycephalic breeds would seem to fix this animal welfare issue, there are concerns that this could ultimately lead to owners turning to underground, unregistered breeders. This legislation was reversed at the end of 2022, with the ban on breeding lifted.<sup>20</sup></p>

<p>Another approach that has been advocated in many other countries such as the UK, is working towards improving the current breeds by changing breed standards. The University of Cambridge and the Kennel Club have developed a Respiratory Function Grading Scheme, which is a non-invasive test to help determine whether the dog is affected by BOAS.<sup>19</sup></p>
<p>This tool is being used across the UK and internationally to help advise owners whether their dog is affected by BOAS and offer guidance to breeders to lower the risk of producing affected puppies (Kennel Club).<sup>19</sup> There are several registered breeders who are using this grading scheme as well as other screening tests in an effort to work towards a healthier breed.</p>
<h2>The Way Forward: French Bulldog Health Issues</h2>
<p>In response to the growing popularity of brachycephalic breeds and the concern for their welfare, the Brachycephalic Working Group (BWG) was formed in 2016. The BWG is a single body comprised of all stakeholders of dog welfare in the UK including vets, breeders, The Kennel Club, animal welfare groups and owners, that aims to improve the welfare of these dogs. The Brachycephalic Working Group encourages all potential dog owners to stop and think before buying a flat faced dog.<sup>21</sup></p>
<p>It is encouraging that there has been more concern and awareness for brachycephalic breeds in recent years. There are a tremendous number of campaigns, such as the “Love is Blind” campaign in Australia, that aim to increase public awareness of the issue. It is clear that education is key and all stakeholders (owners, vets, animal welfare groups, breeders) must work together to improve the welfare of these animals.</p>
<p>If you are a new French Bulldog owner or perhaps have had a Frenchie for a while but haven’t come across this information previously, we understand that reading about all these health and welfare concerns can be overwhelming and upsetting.</p>

<p>The <em>Newsweek</em> Pet of the Week this week is Gigi, a French bulldog from Willoughby, Ohio.</p>
<p>The 6-year-old dog went to live in a Cleveland suburb with owner Jackie Good when she was just a year old.</p>
<p>&#8220;Her previous owner worked quite long hours, so they wanted her to have a better lifestyle,&#8221; Good told <em>Newsweek</em>.</p>
<p>Gigi is playful and loving is now a firm part of the family and loves toys. &#8220;She is always ready to play fetch, tug rope, or to run around being a total goofball,&#8221; said Good.</p>
<p>In a video sent to <em>Newsweek</em>, Gigi showed off just how great she is at playing fetch with her tennis-ball shooter.</p>

<p>The Federation of European Companion Animal Veterinary Associations (FECAVA) recently announced the recipient of the <a href="https://www.fecava.org/didier-noel-carlotti-award/dr-jane-ladlow-honored-with-the-fecava-didier-noel-carlotti-award-2023/" target="_blank" rel="noopener">Didier-Noël Carlotti Award for 2023</a>. The annual award recognizes remarkable individuals who have made notable contributions to the field of veterinary medicine and is named in honor of Didier-Noeëll Carlotti, a veterinary surgeon who initiated FECAVA and served as its first President for four years.</p>
<p>Dr. Jane Ladlow, MA VetMB CertVR CertSAS DipECVS MRCVS, was honored with the 2023 award after being nominated by the British Small Animal Veterinary Association (BSAVA) for her trailblazing work on brachycephalic dog health and dedicated efforts to increase awareness. Dr. Ladlow is renown for her expertise in veterinary surgery and is a University Senior Lecturer in Small Animal Surgery at the University of Cambridge. Significant advancements have been made in veterinary science due to her research, which focused on brachycephalic obstructive airway syndrome (BOAS).</p>
<p>As French Bulldog owners well know, BOAS is a <a href="https://frenchbulldog.org/french-bulldog-breathing-issues/" target="_blank" rel="noopener">breathing issue</a> that impacts many members of the breed. The health of brachycephalic dogs has long been subject to <a href="https://mrcvs.co.uk/en/news/17154/Can-brachycephalic-dogs-be-healthy" target="_blank" rel="noopener">debate among veterinarians</a>, with Dr. Ladlow advocating the promotion of healthy examples of brachycephalic breeds and encouraging dog owners to seek veterinary advice.</p>
<p>Dr Ladlow’s research involved the objective assessment of airway function and the development of effective <a href="https://frenchbulldog.org/boas-surgery" target="_blank" rel="noopener">surgical strategies</a> to improve the health and wellbeing of brachycephalic canine companions. This research led to one of her most notable contributions to the field: Respiratory function scoring for brachycephalic dogs.</p>
<p>Dr. Ladlow began her research in 2019, with her work resulting in the establishment of a national scheme in the UK that provides independent, veterinary-led respiratory function scoring. Brachycephalic dog owners will be pleased to hear the data obtained through the scheme offers valuable insights into the scope and severity of respiratory issues in brachycephalic breeds.</p>
<p>The data further empowers veterinarians and breeders to undertake proactive measures to improve the overall health of these popular breeds.</p>
<p>The dedication and research efforts of Dr. Ladlow have had a valuable impact, both in the UK and globally, due to her continued efforts to promote brachycephalic dog health through her work. <h2>What is BOAS?</h2>

<p>Brachycephalic Obstructive Airway Syndrome refers to the respiratory distress that is experienced by dogs with brachycephaly. The term “brachycephaly” or “brachycephalic” is used to describe the shortened facial structure that has resulted from generations of breeding by humans to create dogs with squished facial characteristics. Brachycephalic breeds include the French Bulldog, Pug, English Bulldog, Boston Terrier, Shih Tzu, Cavalier King Charles, and Shih Tzu.<sup>2</sup> Symptoms of brachycephalic obstructive airway syndrome include:</p>
<ul>
<li>Heat stress</li>
<li>Exercise intolerance</li>
<li>Snoring &amp; snorting noises</li>
<li>Sleep apnoea</li>
<li>Regurgitation &amp; Vomiting</li>
<li>Dyspnoea (difficulty breathing)</li>
<li>Cyanosis (blue gums/tongue indicating a lack of oxygen in the bloodstream) &amp; syncopal events (fainting)<sup>2</sup></li>
</ul>
<p>The symptoms of brachycephalic obstructive airway syndrome can be subtle and unfortunately considered “normal” for the breed. For example, the snorting noises that French Bulldogs commonly make when excited or when exercises are associated with their upper airway (nose, mouth, and entrance to the windpipe) obstruction caused by excessive soft tissue.<sup>2</sup></p>
<p>Generations of breeding for a flattened face has resulted in a shortened skull, however the same amount of soft tissue remains. This causes obstruction of the upper airway with an enlarged tongue, tonsils, aberrant nasal turbinates, everted laryngeal saccules, and an elongated soft palate in these dogs. These obstructive tissues create resistance to airflow, generating increased negative pressure which causes secondary changes such as laryngeal collapse. This syndrome reduces these dog’s quality of life and can be life-threatening in severe cases.<sup>1,2</sup></p>
<h2>What is BOAS Surgery?</h2>
<p>So how do veterinarians treat BOAS with surgery? There are many contributing components associated with BOAS as illustrated in the diagram below, however not all of these can be corrected surgically. As it is not possible to change the shape of the skull of these dogs, surgery aims to remove some of the enlarged soft tissue in the upper airway. There are several components to BOAS surgery, including alarplasty, staphylectomy, tonsillectomy and sacculectomy.<sup>2,3</sup></p>
<h3>Alarplasty</h3>
<p>Alarplasty aims to address the stenotic nares (constricted nostrils) typically seen in brachycephalic dogs. There are two main procedures, the wedge alarplasty and the ala-vestibuloplasty. The wedge alarplasty technique involves taking a wedge of tissue from the outside wing of the nostril to widen the opening. The wedge is sewn closed by the surgeon with very fine, dissolvable suture material.<sup>3</sup> The ala-vestibuloplasty is a newer technique that is being adopted by some BOAS surgeons which involves taking a larger section of tissue deeper into the nostril as well as a wedge of tissue on the nostril wing. This technique does not use any suture material and has been recommended to further improve airflow.<sup>2</sup></p>

<h3>Staphylectomy</h3>
<p>Staphylectomy refers to the removal of a portion of the elongated soft palate. If you run your tongue over the top part of your mouth you will feel the hard bony palate but as you reach the back of the mouth, you will feel it soften – this is the soft palate. In brachycephalic dogs, this commonly extends all the way to the entrance of the airway, blocking airflow and causing snorting and snoring. During a staphylectomy, the surgeon will thin and shorten the soft palate. This procedure may be performed using a special laser or with surgical instruments such as specialized scissors. If using the surgical instruments, your dog will have some dissolvable stitches placed in the shortened palate.</p>

<h3>Tonsillectomy</h3>
<p>The tonsils may become everted in brachycephalic dogs, which is thought to be secondary to negative airway pressure. Inflammation of the tonsils was also found in 20% of dogs with BOAS in one study, most commonly affecting the French Bulldog.<sup>2</sup> The tonsils may be removed with cautery (a special blood vessel sealing device) or surgical instruments.<sup>7</sup></p>

<h3>Sacculectomy</h3>
<p>The laryngeal saccules sit at the entrance of the airway. If they are found to be everted and obstructing airflow your surgeon will delicately remove them.</p>

<h3>Additional Procedures</h3>
<p>In severely affected individuals, additional surgery to remove additional tissue within the nostrils to further increase airflow can be considered. The LATE procedure, which stands for laser-assisted turbinectomy, involves using a special medical laser to remove abnormal, crowded tissue within the nasal cavity.<sup>8</sup> Regrowth of tissue is common post-surgery however it has been demonstrated to be an effective treatment for dogs with intranasal abnormalities and poor response to the common surgical treatments previously discussed.<sup>8</sup></p>
<h2>What happens on the day of surgery?</h2>
<p>One of the most common questions from owners is how long does BOAS surgery take? The exact answer to this question will depend on multiple factors, such as how many of the above procedures are being performed and whether any additional procedures such as chest x-rays or bronchoscopy are also being performed. BOAS surgery is a day procedure, which means that your dog is under anaesthesia for no more than 1-2 hours. Your veterinarian may prescribe medication for your dog to take prior to surgery and will advise of fasting procedures.</p>
<p>Generally, you will be advised to fast your dog overnight prior to surgery, which means they can have dinner but no breakfast on the day of surgery. Depending on your dog’s symptoms, your veterinarian may advise you to feed a small handful of food early in the morning prior to surgery. Once you get to the clinic, your dog will be admitted to hospital and all the pre-surgery checks will be performed.</p>
<p>Once recovered from surgery, your pet will be monitored very closely for any signs of difficulty breathing or regurgitating. Depending on several factors (surgeon assessment of your pet, recovery from surgery, type of clinic – 24hr vs GP), your dog may be discharged home the same day of surgery or may be advised to stay overnight for ongoing monitoring.</p>
<h2>What to expect after surgery</h2>
<p>Your pet will be discharged home with instructions from your surgeon. They will usually be required to wear an Elizabethan collar to prevent rubbing of the stitches on the nose. Your pet should be rested as advised by your surgeon, with no vigorous play, running or jumping. Your pet may also have a small amount of nasal discharge post-surgery. If you are at all concerned about the discharge, make sure to contact your veterinarian for advice. Finally, soft food is recommended post airway surgery.</p>

<h2>Complications – is BOAS surgery dangerous?</h2>
<p>As with any surgical procedure, there are potential complications. Brachycephalic obstructive airway syndrome and gastro-oesophageal reflux carry risk for anaesthetic complications, however anaesthesia on these dogs is successfully performed daily in veterinary hospitals across the world.<sup>6</sup></p>
<p>The main complications of airway surgery reported in one study included dyspnoea (difficulty breathing), regurgitation, aspiration pneumonia (inhalation of gastric contents) and respiratory or cardiac arrest. Difficulty breathing post-surgery can vary from mild, only requiring oxygen supplementation to more severe, requiring repeat anaesthetic to place a breathing tube down the throat.<sup>5</sup></p>
<p>If there is persistent swelling of the airway, a temporary tracheostomy tube may be required. This is a procedure where an incision into the windpipe is made, and a tube placed to bypass the swollen airway.<sup>5</sup> This allows your pet to breathe even when their upper airway is swollen. This tube usually stays in place for a couple of days until the swelling resolves and is removed once no longer required. While BOAS surgery does come with risks, the improved quality of life your pet needs to be strongly considered and your surgeon will provide their recommendations for your dog.</p>

<h2>BOAS surgery cost</h2>
<p>Although most owners would like to know how much brachycephalic airway syndrome surgery will cost, this will vary between veterinary practices.<sup>1</sup> There will likely be a cost difference between specialist referral practices and primary care or general practices (GP’s). The difference in this cost is due to level of hospitalization provided (e.g. 24hr intensive care vs standard veterinary practice), qualifications of the surgeon and whether any other procedures are being performed (e.g. chest x-rays, bronchoscopy). Your regular veterinarian should be able to recommend BOAS surgeons in your area.</p>
<p>Given that nearly all brachycephalic dogs would benefit from surgery, it is strongly recommended that any prospective or current owners ensure that they take out pet insurance. It is important to read the product disclosure statement (PDS) closely when comparing pet insurance policies so that you are aware of any exclusions.</p>
<p>Unfortunately brachycephaly is associated with several other health conditions such as eye, dental, spinal, skin, and orthopedic disease.</p>
<p>Prioritizing pet insurance as part of your pet budget will ensure that if your French Bulldog develops any of these conditions, you are financially prepared to provide them with the best health care.
