New treatment for macular degeneration

November 18, 2011 -- The U.S. Food and Drug Administration approved Eylea (aflibercept) to be used in treatment of patients with wet (neovascular) age-related macular degeneration (AMD), which is one of major causes of vision loss and blindness in ages 60 and older.

AMD gradually destroys a person’s sharp, central vision. It affects the macula, the part of the eye that allows people to see fine detail needed to do daily tasks such as reading and driving.

There are two forms of AMD, a wet form and a dry form. The wet form of AMD includes the growth of abnormal blood vessels. The blood vessels can leak fluid into the central part of the retina, also known as the macula. When fluid leaks into the macula, the macula thickens and vision loss occurs. An early symptom of wet AMD occurs when straight lines appear to be wavy.

“Eylea is an important new treatment option for adults with wet AMD,” said Edward Cox, M.D., M.P.H, director of the Office of Antimicrobial Products in FDA’s Center for Drug Evaluation and Research. “It is a potentially blinding disease and the availability of new treatment options is important.”

The safety and effectiveness of Eylea was evaluated in two clinical trials involving 2,412 adult patients. People in the study received either Eylea or Lucentis (ranibizumab injection). The primary endpoint in each study was a patient’s clearness of vision (visual acuity) after one year of treatment.

Eylea is injected into the eye either every four weeks or every eight weeks by an ophthalmologist. The studies showed that Eylea was as effective as Lucentis in maintaining or improving visual acuity.

The most commonly reported side effects in patients receiving Eylea included eye pain, blood at the injection site (conjunctival hemorrhage), the appearance of floating spots in a person’s vision (vitreous floaters), clouding of the eye lens (cataract), and an increase in eye pressure.

Eylea should not be used in those who have an active eye infection or active ocular inflammation. Eylea has not been studied in pregnant women, so the treatment should be used only in pregnant women if the potential benefits of the treatment outweigh any potential risks. Age related macular degeneration does not occur in children and Eylea has not been studied in children.

Other FDA-approved treatment options for wet AMD include: Visudyne (verteporfin for injection) approved in 2000, Macugen (pegaptanib sodium injection) approved in 2004, and Lucentis (ranibizumab injection) approved in 2006.

1. Informed consent.

2. Inform the patient about fluorescein angiography. Obtain verbal or written informed consent.

3. Dilate patient pupil.

4. Prepare fluorescein solution, vein needle, and syringe.

5. Prepare fundus camera:
a-clean front lens.
b-load color and black-and-white film into two camera backs.
c-focus eyepiece crosshairs.

6. Take identification photograph, which includes the patient name, the date, and other data (number of fluorescein angiogiogram, patient vision of right and left eye, referring physician and so forth)

7. Position the patient for alignment,focus,and comfort.

8. Align and focus camera.

9. Complete color photography.

10. Switch to camera with black-and-white film for fluorescein photography.

11. Take red free photographs.

12. Insert vein needle.

13. Simultaneously start timer from zero and inject fluorescein dye.

14.Take preinjection color photographs,shoot at exact start of injection as timer is turned on and shoot second shoot at exact finish of injection(length of injection is automatically recorded)

15. Start fluorescein photograph 8 seconds after start of injection in young patients and after 12 seconds in older patients.

16. Follow fluorescein angiography plan.

17. When photography is done, reassure the patient that all went well and remind him or her that the urine will be discolored for aday or so. Have patient wait an additional 20 minutes for observation for possible reactions to fluorescein.

is a property of substance to alter the wave length of the reflected light on exciting. Angiography means recording of the angios or the blood vessels. It is basically recording and visualization of the blood vessels of the retina using fluorescein dye.

General overview on Procedure :

In FFA, the chemical or dye is injected in the vein of the hand and this dye circulates all over the body and in every blood vessel. Since retina is the only place in the body where blood vessels can be seen, it is used to visualize the retinal vasculature, that is blood vessels of the retina.
A special machine called the fundus camera, which has got special lens filters, is used to take photographs of the retina. We can also attach digital camera system or a printer.
When blue light falls on retina which has Fluorescein dye the reflected light is of green colour & not blue. This property of dye is used to study the normal or abnormal anatomy of retina. This dye and the reflected light is greenish in color. The fundus camera records this. This dye would go wherever the blood vessels are present in retina and we would get the picture of that.

If blood vasculature of the retina is normal, then we would get the normal pattern of the retina. If there were blockage then the dye would not go beyond that and would stop at that particular point. If there is leakage, the dye would also leak out along with the blood in the retina and we would be able to pick up that. If there are certain other defects in the retina then depending on the various features seen, whether we get leakages or staining or blocked fluorescence or window defects whether the dye seen in early phase, late phase or in very late phase or delayed phase and is initially less and increasing etc, we can get very good idea of patients disease.

1• series of colour photographs are taken of the back of your eye.

2•A small needle is inserted into a vein in your arm or hand.

3• The lights will be turned off and the dye injected.

4• Once the dye reaches the back of the eye, (this only takes a few seconds), the photographer will begin to take lots of pictures.
5• The lights from the camera flash are quite bright but it is very important to keep your eye open, a nurse will help you if necessary. You will then have a short rest.

6• Another series of photographs will then be taken a few minutes later. The needle will then be removed and you will be asked to wait in the waiting area for about 15 minutes.
7• You will usually be given another appointment to discuss the results and plan your treatment with the eye doctor, although this is sometimes possible on the same day as the test.

Special precautions to take before having an angiogram:

Patients with diabetes, kidney diseases, high blood pressure or receiving anticoagulants have to take special precautions as following and an angiography should only be done only when it is necessary.

*	If you have kidney problems an angiography should only be done if clinically indicated as the dye is removed from the blood by the kidneys and can cause further impairment of kidney function.
*	If you are diabetic patient: this dye test gives false high readings in the urine and on blood sugar tests. You should not adjust your insulin or any of your diabetic treatments based on these results during the first 2 days following the Fluorescein angiogram.
*	If you suffer from high blood pressure (hypertension) then this should be well controlled with tablets before angiography is done as there is a higher risk of bleeding from the puncture site.

Safety of Fluorescein :

Topical, oral, and intravenous use of fluorescein can cause adverse reactions including nausea, vomiting, hives, acute hypotension, anaphylaxis and related anaphylactoid reaction, cardiac arrest, and sudden death. Intravenous use has the most reported adverse reactions, including sudden death, but this may reflect greater use rather than greater risk. Both oral and topical uses have been reported to cause anaphylaxis, including one case of anaphylaxis with cardiac arrest (resuscitated) following topical use in an eye drop. Reported rates of adverse reactions vary from 1% to 6% The higher rates may reflect study populations that include a higher percentage of persons with prior adverse reactions. The risk of an adverse reaction is 25 times higher if the person has had a prior adverse reaction. The risk can be reduced with prior (prophylactic) use of antihistamines and prompt emergency management of any ensuing anaphylaxis. A simple prick test may help to identify persons at greatest risk of adverse reaction.

Management of simple hazards:

Most people do not experience any side effects apart from blurring of vision and some dazzle from the camera’s flash.

The nausea usually is transient and subsides quickly. Hives can range from a minor annoyance to severe.
A single dose of antihistamine may give complete relief.

The dye can tint your skin a slight yellow colour and your urine can
turn bright yellow, both of which will fade within 24 hours.
You should avoid exposure to direct sunlight during this time.
If the dye leaks outside the vein it may cause temporary pain.
This can be relieved by cold compression.
Drinking plenty of water during this time will help to flush the dye out of your system

More complex side effects:

Also, retinal imaging is important for diagnosing retina diseases.

Fundus camera:
Is a device for photographing the retina.
It is based on the indirect ophthalmoscopy principle, where the Observer’s eye is replaced with a camera.

So, A fundus camera or (retinal camera)
Is a specialized low power Microscope with an attached Camera designed to Photograph the interior surface of the Eye, including the Retina, Optic disc, Macula, and Posterior pole i.e. the Fundus (eye).

Optical principles:
The optical design of fundus cameras is based on the principle of monocular indirect ophthalmoscopy.
A fundus camera provides an upright, magnified view of the fundus. A typical camera views 30 to 50 degrees of retinal area, with a magnification of 2.5x, and allows some modification of this relationship through zoom or auxiliary lenses from 15 degrees which provides 5x magnification to 140 degrees with a wide angle lens which minifies the image by half. The optics of a fundus camera are similar to those of an indirect ophthalmoscope in that the observation and illumination systems follow dissimilar paths.

The observation light is focused via a series of lenses through a doughnut shaped aperture, which then passes through a central aperture to form an annulus, before passing through the camera objective lens and through the cornea onto the retina.The light reflected from the retina passes through the un-illuminated hole in the doughnut formed by the illumination system. As the light paths of the two systems are independent, there are minimal reflections of the light source captured in the formed image. The image forming rays continue towards the low powered telescopic eyepiece.
When the button is pressed to take a picture, a mirror interrupts the path of the illumination system allow the light from the flash bulb to pass into the eye. Simultaneously, a mirror falls in front of the observation telescope, which redirects the light onto the capturing medium, whether it is film or a digital Charge-coupled device.

Because of the eye’s tendency to Accommodation (eye) while looking though
a telescope, it is imperative that the exiting Vergence is parallel in order for an in focus image to be formed on the capturing medium.

Pre-examination setting:

1. The examiner
Before attempting photography, must become very familiar with the camera through a training session and by learning the terminology of the camera operation Manual .

2. Examination room

The room lights are dimmed so that only the lights of the camera and computer monitors remain on.

The majority of the exam is performed in the dark.

3. The camera

The camera is mounted on a motorized table for easy Height adjustment, and both the examiner and the SP have an adjustable stool to sit on

Before each SP is photographed, the objective lens should be checked and cleaned if necessary.

High humidity or temperatures must be avoided. Dusty conditions mean that the camera will need frequent cleaning.

It its extremely important that camera lens be inspected and cleaned, if dirty, before each photograph. The retinal camera should remain covered when not in use and the lens should be capped after each SP photograph.

A checklist itemizing procedures for preparing the camera at the start of each day is posted on the wall near the fundus camera.

Technicians responsible for the fundus equipment should carefully follow the daily procedures.

4. The patient

Photography begins with a complete explanation of the procedure to the SP by the examiner.

It is important to reassure the SP that no retinal damage is caused by this procedure.

The camera flash is bright and the SP should know when to expect a flash. Since pictures will include the macula (area of central vision), it is normal for the SP to experience a blue or red tint to vision immediately following the flash.

This totally disappears within two to five minutes. No dilation drops are used for this examination, In non mydriatic procedure and the eyes will not be touched.
