<p><em>New customers</em>: Since Galloup StoreFront is currently only available to customers with Galloup accounts, you&#8217;ll need to fill out a <a href="http://www.galloup.com/creditapps.php">credit ap</a> and send it our way before <a href="https://storefront.galloup.com/storefrontCommerce/requestAccount.do">requesting a StoreFront login</a>.</p>

<p><strong>How it works</strong>: Fill out our <a href="../../downloads/backflow_kit_shipping_form.pdf">printable shipping form</a>. Then you can either UPS your test kit to a Smith Location (<a href="http://www.galloup.com/warren.php">Warren</a> or <a href="http://www.galloup.com/grandrapidssmith.php">Grand Rapids</a>), give it to your salesman, or call your <a href="http://www.galloup.com/locations.php">local branch</a> to arrange for pick it up.</p>
