Many people take medications that cause their mouth to lack saliva( a side effect). The saliva is one of the protective mechanisms that the mouth has against gum disease and decay. If you have a dry mouth ( lacking in saliva) you are very likely to get oral disease. It is very important that you take precautions against the damage that will be done to your teeth and soft tissue. First ,you will need to consider getting your teeth cleaned and checked 3-4 times per year. Secondly, you need to have an Oral-B electric toothbrush(not a battery driven toothbrush). It has been my experience that the Oral_B rechargeable toothbrush is many times better than a hand held or battery driven toothbrush. Some of my patients require a flouride mouth rinse to halt the decalcification of the teeth( from being dried out) as well. Since the mouth is dry, you may need a mouth rinse to help lubricate and moisturize your mouth. The Biotene products are very helpful. They can be found at most pharmacies.

It is very helpful to avoid pop as it is acidic and will etch the teeth . This will cause decay very quickly and it may be wide spread as well. Acid reflux is another source of acid in the oral cavity and so you may need to see your physician about that if it is occurring. We have had a fair number of patients with dry mouth and acid reflux who have lost their teeth as a result. So this is a very serious problem that needs attention as soon as you recognize it.

This article is only a partial treatment of the problem, but hopefully it has been helpful. For more information you can call our office at 765-649-3388 or 317-578-1933 or send me question thru my web site at www.garrysmithdental.com.

Many people have a fear of going to the dentist because of bad, painful experiences. We have spent years refining our practice with patient comfort in mind. Many of our new technologies like Dental lasers, cad-cam equipment and air abrasion tools have gone a long way to making your dental appointments much more comfortable. Another way we make the patient more comfortable is staff who have been in the office a long time. We do not believe in turnover in our staff. We have 3 employees that have been in our office for 30+ years, 2 staff have 18+ years experience, and 5 staff have 5-7 years in our office. This helps patients feel a part of a family. These patients know our staff and therefore build a relationship and a feeling of confidence. We know their kids and in fact their kids are now bringing their kids. It is not uncommon for us to be seeing 3 generations of a family. We also have a friendly staff and the atmosphere is relaxed. We offer heated neck supports and warming blankets for those who feel the need. We try to make each patient feel special. All of this in a relaxed and friendly atmosphere. We like to see families and get to know them. You can learn more about us at www.garrysmithdental.com.

Many people today want their teeth whiter but not everyone can get “bleach white” color in their teeth. What the whitening materials do is take out the organic material that causes the bad color. The basic material used is peroxide based. So if you keep whitening it will remove all color from your teeth and leave them DARKER. The darker color comes from blood and the darker color of the tissue around the teeth. You cannot reverse the process.

So the answer to the question of “is whitening dangerous” is yes if you go to far. That is why recommend the process be monitored by a professional. The over -the-counter materials are not as strong as the in-office products. So the over-the-counter products are less effective but over time can do the same thing- cause permanently darker teeth. The only way to reverse it is by having crowns and veneers placed on your teeth. That will be an expensive mistake.

So my recommendation is to have your whitening process monitored and have a realistic expectation as to how white your teeth will become. Everyone cannot have bright white teeth. There 4 basic shade in the human teeth: white, yellow, brown, gray. You will have one of those genetic types in your tooth makeup. So whitening each of those types will dohe following:

white will get whiter

yellow will get less yellow, some whiter

brown will be less brown some whiter

gray will be a lighter gray and some whiter

It may be clear from this discussion that for the genetically yellow, brown and gray teeth can only be WHITE by doing crowns or veneers. Skin color is also a be factor in what looks white in your mouth. So you should contact your dentist to see how your whitening expectation n be attained before you start either in- office or over-the counter processes.

A true TMJ problem exists when there is clicking, grating, limited opening, and/or pain in one or both jaw joints. The most common causes are deep overbite, missing back teeth, incorrect bite, grinding your teeth and accidents involving a blow to the jaw. Most commonly a splint or grinding appliance can be used to “depressurize” and reposition the jaw joints and allow the time for internal joint healing to occur. It is unusual to have an adolescent with TMJ problems

The vast majority of TMJ patients are adults in the 20-50 age range. The major causes for TMJ dysfunction are accidents, crowded teeth, missing teeth and a bad bite. Regardless of the causative factors, a corrective splint or bite appliance (also called grinding appliance) is required. It will require a custom made hard plastic appliance not a flexible one as sold in pharmacies and sporting goods stores. Flexible ones will be “chewed” on and can cause the problem to get worse. The flexible appliances are only good for very short term relief until you can get to a dentist who treats TMJ problems. There are very few dentists trained in the treatment of true TMJ problems. We have been treating TMJ patients for 30 years, so we are well versed in all of the treatment modalities for TMJ problems.

If left untreated, the problem will get worse , it never gets better if it is a true TMJ problem. A grinding or bruxism problem is NOT a TMJ problem until the grinding problem has gone untreated for a time. So grinding and clenching problems , if treated, can be prevented from cause damage to the TMJ area.

As a young child (2 yrs- 7 yrs old) a soft hand tooth brush is best. For them the vibration of the electric brush is scary.I never recommend a battery driven toothbrush, they do not clean the teeth and gums well enough . As they get older I would recommend the Oral-B Vitality electric tooth brush. It is half speed (as compared to adult electric brushes) and cleans 20 times better than the hand held brush. An adolescent or adult would need the Oral-B Triumph electric brush. It comes with a floss- aide head and is estimated to be 40 xs better than a hand held brush. Proper technique is to hold it at the tooth-gum junction and move it to the next tooth then the next and so on. Do not scrub horizontally with any brush as it may cause erosions at the gum line. Oral-B has over 90% of the electric tooth brush market and spends a lot of money on research and development. The good thing is their product is top of the line.Although they do not replace flossing or dental check ups, you will see a big difference in your oral condition and lack of dental work needed. Currently you can get these at SAM’S Club or COSCO at a price of 2/ $100. The regular cost is $138 each, so this is a great buy!

All dentists are trained to do crowns which cover all 5 surfaces of the tooth ( the whole tooth). Onlays are different in that they only cover part of the tooth ( three or four surfaces). To design and prepare an onlay requires a good deal more designing skill than doing a full coverage crown. When done properly, either one will bring the tooth back to 100% of its original strength.

The reason an onlay is better is that good tooth structure is saved. The original tooth structure is superior to any man made material and so if we can save any tooth structure, the patient is far better off. Crowns and onlays can be made out of gold, all porcelain, porcelain fused to gold, or plastics. We use Cerec Technology ( CAD-CAM instrument) to fabricate porcelain bonded restorations in one appointment. Bonding is superior to cementation and, since metal does not bond well to tooth structure,we choose porcelain or plastics over metal anytime. Crowns or onlays can be done to replace large mercury fillings, resin fillings, to repair fractured teeth or to replace existing crowns or onlays.

Unless you have this CAD-CAM technology, these procedures are much harder to do and require multiple appointments. That means more shots and temporaries in between appointments. Patients have told us they love having one appointment instead of multiple appointments because there is only one shot , no sensitivity and less lost hours at work.

Traditional orthodontics has always been taught as a “wait and see” process. By that, I mean to wait until all permanent teeth have erupted and then sort out the crowded mess that has occurred. This often requires four permanent teeth to be removed. Often a “headgear” is used to move the upper front teeth back to close the spaces that remain. This can lead to TMJ problems later since the lower jaw is limited in movement by moving the upper teeth backward.

A better approach is to take advantage of the child’s growing potential early on (6-7 yrs of age) by placing expansion retainers, grow the necessary bone and when permanent tooth is erupted, straighten the teeth with braces (metal, clear, or invisible) without extracting any teeth.

We take a panoramic x-ray of your child at age 6-8 to see if there will be crowding by looking at the developing teeth in the bone on the film. If there is crowding visible, we can make models of the childs mouth and measure how much bone is required. We have used this treatment method for over 29 years with great success. We rarely extracted permanent teeth to treat our cases.

One of the problems with extracting teeth and pulling the front teeth back with head gear is that the child may develope TMJ problems later in life. This happens because the upper front teeth ,when they are brought back, keep the lower jaw from going foreward into its resting zone. This creates strain in the the Jaw joint (TMJ).Very few children have grown up experiencing tmj symptoms using this method of early intervention.. In fact, that is why I got involved with learning this treatment method because I treat tmj problems and found I needed to move the teeth differently from the traditional orthodontic way. If your child is in the 6-8 year old age group you may want to seek out an early intervention orthodontic dentist, like us.

Over 90% of the dentist in the USA today must send your crowns or onlays to a dental laboratory for your crown/onlay to be completed. That is 2 appointments separated by 2-3 weeks. This means you have to be careful with a temporary in between the appointments. We have had Cerec technology in our office for over 5 years now. This new technology allows for your crown/onlay in one appointment. It takes a total of 2-3 hours and you walk out with your finished product ,since you had no temporary, you will not have sensitivity in between appointments.and no lost time for the patients second appointment. Less that 10% of all dentists have Cerec technology available. Not only does the Cerec save time for the patient, the crown is perfect every time. We have noticed less breakage and fewer bite problems since we have used the Cerec. The crown/onlays are all porcelain instead of the old porcelain fused to metal crowns that could have that dark line you have seen. Also, we bond these crowns ( it is “like” superglue but safe for the oral cavity). The old way is to use cement but it will dissolve in time and allow for leakage and new decay formation. Bonding is superior since it is not soluble in saliva and therefore will never leak like cemented restorations tend to do in time. For more information on this technology contact our office at msmith8544@aol.com,gsmithdds@comcast.net,or thru our web site at www.garrysmithdental.com

If you’re a partial denture wearer, ask yourself a question: “Would I rather not have to remove my teeth ever again?” Or, if you’re a full denture wearer, “Would I like to have my dentures firmly attached to my jaw, without slipping or sore spots?” Most people answer “Yes.” Dental implants can help you attain these goals, which is why more and more people have become eager to learn about them.

If you are missing a tooth or having to have one removed, an implant is the best choice. There are several reasons. First the implant is stronger than the original tooth by 50%, so chewing strength is added not removed. Secondly, the implant will not decay or fracture as a cemented bridge would. So as a cost comparison, a bridge for 1 missing tooth would be approximately $3300 and 1 implant ,post and crown would be $4400. For the extra benefits ( more strength, won’t decay or fracture) the cost is reasonable.

Implants are made out of titanium . Once the implant is placed and the bone heals around it, a post is placed into the implant and a crown is made to fit over the post. There are three components to what you, the patient, think of as an implant tooth.

Implants are 98% successful. The main reason for failure is gum disease. We do not recommend implants to patients who cannot properly care for their mouth.
