&lt;li&gt;&lt;span style="font-size: small;"&gt;&lt;b&gt;Close and open Outlook again. &lt;/b&gt;&lt;/span&gt;&lt;/li&gt;
&lt;li&gt;&lt;span style="font-size: small;"&gt;&lt;b&gt;Go to Tools\Options\Other\Advanced Options\Custom Forms\Manage Forms again and click Install Change the Files of Type to fdm. &lt;/b&gt;&lt;/span&gt;&lt;/li&gt;
&lt;li&gt;&lt;span style="font-size: small;"&gt;&lt;b&gt;Navigate to where you saved the form file in step 3 and select the filename.fdm file. &lt;/b&gt;&lt;/span&gt;&lt;/li&gt;
