A biopsy is a diagnostic test in which your doctor removes cells or tissue from your body for examination by a pathologist, a doctor with specialized training in diagnosing diseases using microscopes. Your doctor will probably order a biopsy to confirm a diagnosis of mesothelioma or other cancer.

If your doctor suspects mesothelioma, he or she will remove a sample of tissue from your chest or abdomen for testing at a pathology lab. This biopsy tissue will then be sent to your pathologist, who will issue a pathology report with a final diagnosis.

The biopsy may be done in different ways, depending on where the cancer is located. If the cancer appears to be a pleural mesothelioma, your doctor may order a core needle biopsy (CNB) or a fine needle aspiration (FNA) biopsy. He or she will use a CT machine or other imaging test such as an endoscopic ultrasound (EUS) to locate the abnormal tissue, and then insert a biopsy needle through the chest wall into the suspect lung tissue.

If your doctor is unable to get a large enough tissue sample through a needle biopsy, he or she may also make a surgical incision to remove tissue from the chest or abdomen. This is known as an incisional biopsy.

Hospice

Family members may reach the point where they need additional help to care for their loved one suffering from mesothelioma. Families may want to consider hospice care. Hospice provides care to patients who are in the terminal stages of an illness. In order to qualify for hospice care, your condition must be considered incurable and terminal and your doctor must certify that your life expectancy is six months or less.

Hospice teams include medical professionals, counselors, therapists, social workers, spiritual advisors, home health aides and trained volunteers. The goal of hospice is to care for those who are in the last stages of life. It endeavors to enhance the quality of life and make the last stages of life as comfortable as possible. It provides emotional, psychological, and spiritual care and support for you and your family.

Focus on Pain Management and Family Support

In hospice, the focus of care has changed. It is no longer concerned with aggressive measures to cure a disease. Instead, it focuses on relieving pain, anxiety, and other symptoms, so those who are dying may do so with dignity and in the comfort of their own homes with their loved ones caring for them. Often this is referred to as palliative care.

Hospice provides many benefits, including basic medical care with a focus on pain and symptom control, medical supplies and equipment, trained volunteer support, respite for your caregivers, counseling and guidance for you and your family, and counseling and support for your loved ones after you die.

Hospice care can take place in your own home or in a hospice center, sometimes in a nursing home, long-term care facility, or hospital. Most of the time, care will take place in your home and a family member or loved one will look after you. A member of your hospice team will visit you for an hour or so one or more times a week. The hospice team will work with your caregivers to help them give you the best possible care and to prepare them to handle situations that can arise in the home. Hospice programs offer services 24 hours a day, 7 days a week, and you or your caregivers may call the hospice team at any time. If necessary, a nurse can usually come to your home at any hour of the day or night. Hospice also provides respite care for your caregivers. Trained volunteers may be available to relieve your caregiver for a few hours each week. Some hospice programs provide respite for several days at a time. If you enter a hospice program, you can still remain under the care of your doctor, and your doctor will work with your hospice team to stay involved in your care. You may also be treated in a hospital and then return to hospice care. If you live longer than 6 months, you can remain under hospice care. If you get better, you can leave the hospice program.

Hospice care is covered by Medicare, Medicaid, and by most, but not all, private insurance programs. It is best to find out exactly what your insurance covers before you enter a hospice program. Many hospice programs will help you to investigate your coverage.

The Decision is a Personal One

The decision to enter hospice is a personal one. It is a decision that involves the patient, his or her family, and the patient's physician. It is often difficult for patients and loved ones to face the end of life, to acknowledge that it is approaching. Some people wish to pursue aggressive measures up to the very end, to do everything possible to live as long possible by any medical means possible. They may not be able to accept the concept of hospice care. Some patients and their loved ones simply do not know about hospice care or they are afraid that they will not be able to see their doctor anymore. They may also fear that they will not be able to go to a hospital or be able to leave the program if they get better.

Those who choose hospice care enter a comprehensive program, with dedicated professionals and volunteers who endeavor to help their patients to live out their last days in dignity and comfort, without anxiety and pain, and to help their loved ones to face the myriad issues that arise with terminal illness and death.

Latency

Latency refers to the length of time between the first exposure to a toxic or carcinogenic substance such as asbestos, and the onset of the disease caused by the toxin. Most asbestos-related diseases have a long latency period of at least 10 to 40 years.

For example, it typically takes 20 to 40 years to develop malignant mesothelioma after being exposed to asbestos. Likewise, it usually takes 20 to 30 years after the first exposure to develop pleural plaques, asbestosis, or asbestos-related lung cancer. In some cases, the latency period may be as short as 10 years after exposure. In other cases, it may take 50 years or more.

This can make it hard to identify all of the sources of your past asbestos exposure after you are diagnosed with an asbestos-related disease. Your disease may have been caused by a product that you worked with during the 1960s , 1970s, or 1980s, and even as far back as the 1940s or 1950s. It can be difficult to remember all of the sources of exposure after so many years have passed. This is particularly true in cases of mesothelioma which may have been caused by very brief or low levels of exposure to asbestos, including household exposure or bystander exposure.

Mesothelium

The mesothelium is a thin, two-layer membrane that surrounds the entire surface of the lungs and heart as well as the organs in the abdomen. The mesothelial lining of the lungs is called the pleura, the lining of the heart is called the pericardium, and the lining of the abdomen is called the peritoneum.

The mesothelium protects your internal organs, and produces a lubricating fluid that allows your lungs and heart to glide easily against your chest wall. Without this protective lining, your lungs and heart would be unable to expand and contract freely. Thus, while few people have ever heard of the mesothelium, your body cannot function normally without it.

Exposure to asbestos can cause permanent damage to the mesothelium. Asbestos is a powerful carcinogen. Over time, asbestos fibers in the body cause the mesothelial cells to become malignant and form a cancerous tumor known as mesothelioma. Unfortunately, the widespread use of asbestos throughout the twentieth century has led to an epidemic of mesothelioma in the United States.

Navy Personnel are at Risk for Developing Mesothelioma

Veterans from all branches of the armed forces including the Army, Navy, Air Force and Marines are at risk for developing mesothelioma due to asbestos exposure sustained while on active duty serving their country. Mesothelioma Navy cases, however, are typically those recognized as the most common. Over time, these military veterans and particularly, Navy veterans and shipyard workers, have demonstrated a higher rate of developing mesothelioma than individuals with no military or navy history. These brave men and women, who so honorably served our country from World War II through the Vietnam War, were unknowingly exposed to high levels of airborne asbestos dust throughout their tours of duty. Until the late 1970 and early 1980, products containing asbestos were made and used extensively in military establishments and on Navy warships and submarines. Warships were constructed using many types of asbestos containing products including pipe-fittings, gaskets, cables, cement, joint compound and boiler insulation just to name a few. Military vehicles were equipped with asbestos containing gaskets, clutches and brakes. When constructing buildings on our nation army bases floor covering materials, cement mixtures and plumbing systems frequently contained asbestos based products. Exposed insulation and airborne dust from grinding and sanding or ongoing maintenance procedures all created dangerous opportunities for Navy veterans to be exposed to asbestos.

Asbestos is a naturally occurring mineral that possesses exceptional heat and fire resistant properties and is relatively inexpensive to produce. Once the dangers of asbestos exposure became incontestable in the early 80, however, the Navy and military in general curtailed the use of asbestos containing products. Because Navy warships and military establishments remain ”in service” for many years, however, they continue to harbor asbestos containing parts and materials that present exposure risks even today in any type of asbestos removal project or maintenance and repair effort. As a result, present day military personnel may still be at risk for asbestos exposure although many safety precautions have been put in place to minimize their risk.

Navy Ships and Shipyard Workers

Many Navy veterans who were responsible for building and maintaining warships are reported to be at particularly high risk for developing mesothelioma or other asbestos cancer diseases. This is because many different types of products containing asbestos were used on the ships and the ventilation systems were very poor. Consequently these Navy personnel were frequently exposed to highly concentrated levels of asbestos dust in fire and engine rooms, boiler rooms, navigation rooms, mess halls and sleeping quarters. In fact, because signs of malignant mesothelioma may not show up for 30 to 40 years after being exposed to asbestos, it is only recently that a rise in the number of mesothelioma cases among Navy veterans and shipyard workers is being reported.

Navy veterans who worked below deck on naval warships or submarines are those most likely to develop mesothelioma due to asbestos exposure. They worked (and sometimes lived) in extremely tight quarters with dangerously high levels of asbestos dust lingering in the air. Poor ventilation systems prevented adequate air exchange and resulted in highly concentrated levels of asbestos to remain in the air for Navy personnel to breathe in. Unfortunately, because Navy veterans and shipyard workers carried the asbestos dust and fibers home on their clothes it is not uncommon for loved ones, particularly those responsible for handling the laundry, to also find that they have been diagnosed with mesothelioma.

Where Was Asbestos Found Aboard Navy Ships?

Asbestos could be found in nearly any area of a naval ship, as it served myriad uses within these vessels. However, most often, asbestos was used in an insulation capacity. Some of the areas asbestos was found include boiler/steam valves, piping, and related fixtures. Those who worked closest to asbestos within vessels were often those who conducted repairs of this type of fixtures. Oftentimes, the repairs of these materials necessitated disturbing older products by chipping and grinding- which would release asbestos fibers into the surrounding air. Because of this, it is not unusual for those who have worked extensively aboard naval ships or in naval shipyards to develop asbestos-related health complications like pleural mesothelioma, asbestosis, and other respiratory conditions as a result of exposure to asbestos.

Those who have knowingly worked with asbestos product, and particularly those who worked with products in the capacity discussed above, should be aware of the warning signs of mesothelioma disease and other asbestos related health complications. Mesothelioma symptoms closely mimic those associated with other respiratory conditions (including the common cold or chronic cough) but those who have worked with asbestos should share their asbestos exposure history and mesothelioma risk factors with their doctor or oncologist to ensure that they identify possible asbestos-related health complications.

Veterans can develop mesothelioma as

a result of asbestos exposure during their military service.

For decades, the men and women from all four branches (Army, Navy, Air Force and Marines) of the United States military have defended our country, both in times of war and in times of peace. Veterans put their lives on the line each and every day, both on the battlefront and at the many U.S. military bases located around the world. Of all the individuals in the United States that have been diagnosed with mesothelioma cancer, the veteran population has been affected the most.

The simple reason for this increased risk is exposure to asbestos. Asbestos, the only known cause of mesothelioma, was used by the U.S. military in literally hundreds of applications. Because exposure was unavoidable for some, mesothelioma remains one of the most serious diseases affecting veterans today, particularly those who served between World War II and the Vietnam Conflict.

Asbestos was used in all branches of the military for many years, especially during the years of World War II and the Korean Conflict. At that time, shipbuilding was at its peak. In fact, about 4.3 million Americans worked in shipyards during World War II and because of the daily tasks they were required to perform, many were at risk for developing asbestos-related diseases.

Navy veterans are at the greatest risk to develop mesothelioma as asbestos was widely used in Naval ships and shipyards.

Navy veterans were exposed to high levels of asbestos present in many areas of navy ships including boiler rooms, engine rooms, galleys and sleeping quarters. While the soldiers who lived and worked aboard the ships - including gunmen, boilermen, and firemen - were susceptible to inhaling asbestos, those who built and repaired the ships were even more prone to developing diseases associated with the toxic mineral. They were tradesmen such as pipefitters, plumbers, mechanics, shipfitters, electricians, welders and boilermakers to name a few.

There have even been instances where military base secretaries, and others who did not work directly on ships, developed mesothelioma cancer through second-hand asbestos exposure. Second hand exposure occurs when asbestos dust is inhaled from the clothes and hair of others who worked with asbestos on the base or from an abundance of asbestos circulating through the air. Loved ones of shipyard workers have also been known to develop the disease due to the same type of secondary exposure.

Because mesothelioma can remain dormant for several decades, many veterans who served during the 1950s to the 1970s are just being diagnosed with the disease. These brave men and women were unaware that they would face a terminal illness in their later years when they had hoped to be enjoying retirement and extra time with their family. Mesothelioma is a particularly difficult disease to battle, and though there have been great advances in the area of mesothelioma treatment in recent years, the overall prognosis for the disease is not a favorable one.

There has also been some controversy surrounding the U.S. government and their responsibility to those who have developed mesothelioma and other asbestos diseases as a result of their service to the country. While the Veterans Administration offers some guidance to afflicted personnel, it is clear that more must be done to support veterans with mesothelioma, including compensation for medical expenses, loss of income, and suffering.

Dr. Taub’s Guide to Navigating Mesothelioma Clinical Trials

Step 1. Making a treatment decision.

Patients must first decide whether they will participate in conventional therapies or, for the reasons of lack of effectiveness or otherwise, to explore participation in a clinical trial. Conventional chemotherapy for malignant pleural mesothelioma is limited to a combination therapy of Alimta and Cisplatin, a cocktail which is currently the only FDA-approved method for systemic chemotherapy treatment of the disease.

Those patients who decide to explore clinical trials often do so because primary treatment protocols are not effective or not markedly effective in treating their tumor. Volunteering for a clinical trial is an important decision. If the decision is indeed made to participate in a clinical trial they must next decide what phase of the trial they will participate in.

Step 2. Know the Phases of Clinical Trials

Phase one clinical trials focus on the maximum toleration of toxicity in patients who participate in them. Once a patient develops toxic symptoms, the dosage is scaled back depending on the patient. Phase one clinical trials for mesothelioma are rare given the fragility of patient health.

Phase two clinical trials are considered the preferred choice for mesothelioma patients. These trials test the safety and effectiveness of a drug against a given cancer. The majority of mesothelioma patients participating in clinical trials will begin with a phase two trial. If a given drug or drug combination demonstrates efficacy or patient response in 20% or more of patients, it will often become a phase three trial.

Phase three clinical trials are considered the best in the field of general oncology but are rare among mesothelioma patients. These trials test the effectiveness of a given drug against a current accepted treatment protocol. In some cases, multimodal therapies, including surgery and chemotherapy, will be tested against the effectiveness of surgery alone. These studies require many patients and present the most conclusive evidence-based treatment protocols.

3. Seek a Second Opinion!

Nearly all patients should seek the opinion of not only their primary physician, but also those working in tandem with their primary oncologist on multimodal therapies. Patients who are undergoing surgery should consult with radiation oncologists, medical oncologists, and pathologists to explore how multimodal therapies may benefit them. As Dr. Taub frequently says, “If you’re oncologist has no time to speak with you- go elsewhere immediately.”

Overview

Dr. Harvey Pass is a renowned thoracic surgeon that has dedicated his career to bringing increased awareness of the dangers of asbestos exposure and its causal relationship to pleural mesothelioma to both the medical community and the public at large. One of his most important research findings occurred while at the Karmanos Cancer Center from 1996-2005. While there, Dr. Harvey Pass and his colleagues completed a key study through which they identified a protein called osteopontin that presents at elevated levels in patients diagnosed with pleural mesothelioma. This important medical discovery now allows doctors to use a simple blood test to screen individuals with a history of asbestos exposure to help diagnose pleural mesotheliomas in earlier stages (stage 1 and stage 2) where they can be treated more successfully.

The majority of Dr. Sugarbaker's clinical and laboratory research has centered on the development of what has been termed a "tri-modal" therapeutic approach to the treatment of diffuse malignant pleural mesothelioma. This three-pronged method uses a combination of radical surgery, chemotherapy and radiation therapy to treat patients diagnosed with the disease. Specifically, Sugarbaker’s highly-regarded treatment for mesothelioma patients includes:

Extrapleural pneumonectomy - This is a serious surgical procedure that includes the removal of the diseased lung and the areas surrounding it. Sugarbaker is one of the most skilled in the country at this surgery but patients must be in overall good health to be a candidate for the procedure. Certain other parameters exist as well, including the fact that the disease must be limited to just one pleural surface. Cardiac and kidney function must be optimal as well.

Pleurectomy - In some cases, Sugarbaker’s tri-modal approach involves the use of pleurectomy instead of the more serious extrapleural pneumonectomy. A pleurectomy involves the removal of both layers of the lining of the lungs, known as the pleura. Again, the patient must be in otherwise good health to be eligible for this procedure.

Chemotherapy - Adjuvant chemotherapy is employed after surgery in order to combat any remaining cancer. As such, Sugarbaker and his team have been experimenting with intraoperative, intracavitary hyperthermic chemotherapy in order to better target cancer cells.

Radiation - Adjuvant radiation of the external beam variety is also used to kill any cancer cells remaining after surgery.

Dr. Sugarbaker's research and experience, as well as his application of this tri-modal procedure, has led to significant improvements in survival rates among malignant pleural mesothelioma patients.

In addition, Sugarbaker’s research focusing on the better tailoring of cancer drugs to individual patients was profiled in the Wall Street Journal in 2008. As the leader of a small study at Brigham and Women’s concerning genetic mutations in cancer cells, Sugarbaker’s team discovered that every patient’s tumor boasted a different group of mutated genes and that no gene was mutated in more than one patient, explaining why chemo works for some and not for others. The research team argues that the research of these mutated genes in each cancer patient would better assist doctors in choosing the correct drug(s) for treatment and, hence, better the prognosis for many patients.

Additional Work

In addition to his surgical work and laboratory research, Dr. Sugarbaker also travels worldwide to deliver lectures on the topic of mesothelioma treatment, including his approach in particular. He has been the keynote speaker at many mesothelioma-related functions including not only symposiums for medical professionals but also conferences for mesothelioma patients and their families.

In an effort to address the growing number of mesothelioma cases worldwide, Dr. Sugarbaker founded the International Mesothelioma Program at the Brigham, focused not only on treatment and research of the disease but also on the support of patients with malignant pleural mesothelioma as well as their families. The program - made up of a multi-disciplinary team of doctors from some of the Boston area’s best institutions - strives to develop new treatment strategies that will extend life expectancies for mesothelioma victims and brings together a wide array of researchers who can work in tandem to translate their findings into better patient care for those with the disease. The program also employs social workers, palliative care professionals, and spiritual counselors to offer emotional support to those afflicted with the disease in hopes of improving quality of life for victims of mesothelioma. In addition, by building a database of those with the disease, the International Pleural Mesothelioma Program hopes to establish satellite centers in selected hospitals where the incidence of the disease is high, creating a far-reaching network of professionals who can collaborate with each other to establish consistent protocols for treating this aggressive from of cancer.

In addition to his work with mesothelioma patients, Dr. Sugarbaker treats those with congenital chest wall deformities, performs volume reduction emphysema surgery, and also focuses on the treatment of individuals with esophageal cancer and a variety of other motility disorders.

Dr. Sugarbaker is also involved in the Tissue and Blood Repository at the Brigham and Women's Hospital, which was founded in 1991, and is Chair of Surgery of the hospital's Cancer and Leukemia Group. These roles permit Dr. Sugarbaker to develop new clinical trial opportunities, as well as non-invasive surgical methods and other beneficial malignant mesothelioma treatment options.

In addition, Dr. Sugarbaker is a member of The Cardiothoracic Surgery Network and is Vice President of the Graham Education and Research Foundation committee. Dr. Sugarbaker's research and clinical experience has also earned him recognition at both national and international levels. Dr. David Rice

Alternative Remedies Treament Option

Some patients may also seek out alternative remedies to complement or supplement the conventional cancer treatments prescribed by their oncologist and medical team. These include acupuncture, massage, yoga, biofeedback, and herbal remedies. Alternative and complementary therapies can promote relaxation and help ease the pain and nausea experienced by many mesothelioma patients. They may also help to alleviate some of the unpleasant side effects of conventional cancer treatments such as radiation, chemotherapy, and surgery.

There is no evidence to suggest that any of these alternative remedies have the power to cure cancer, but some patients find them very effective as a way to cope with the symptoms of mesothelioma and improve their quality of life. However, these alternative remedies should not replace your medication or other conventional treatments, and you should not attempt any alternative remedies without carefully discussing them with your medical team.

Acupuncture

Acupuncture is an ancient medical practice that originated in China more than 2,000 years ago. The best-known type of acupuncture involves putting thin, metallic needles into your skin at specific body points to improve health and well-being. However, there are many different acupuncture techniques. Some use sound waves, others use tiny electrical charges, and they may or may not use actual needles.

Research has shown that acupuncture may be somewhat effective at reducing nausea and vomiting after surgery and chemotherapy. It can also relieve pain. Scientists do not fully understand how acupuncture works, but some researchers believe that it may assist in the release of endorphins in the body. It may also affect the way the body regulates blood pressure and flow.

Biofeedback

Biofeedback is a relaxation technique that attempts to treat pain and insomnia by changing your heart rate, skin temperature, perspiration, or muscle tension. Essentially biofeedback attempts to make the patient aware of bodily processes that are normally thought to be involuntary, so that the patient can gain some conscious control over these processes and diminish their level of pain.

The biofeedback therapist will hook you up to a monitor that is connected to your skin by electrodes. The monitor measures changes in your bodily functions, and will produce a tone or image to let you know when you achieve the desired results in modifying the targeted function. This process is repeated until you have "trained" yourself to use conscious thought to change certain body functions such as your heart rate, temperature and muscle tension, just to name a few.

Biofeedback was evaluated and found to be of possible benefit for treating chronic pain and insomnia by an independent panel convened by the National Institutes of Health (NIH). There is no scientific evidence to support claims that biofeedback can cure cancer, or even slow its progression, but it may help to improve quality of life for some people.

Massage

Some health professionals recommend massage as a way for cancer patients to reduce stress, anxiety, and pain. There is some empirical evidence that massage can measurably decrease stress, anxiety, depression, pain, and fatigue among cancer patients. Most people do feel better after a massage, which at a minimum can help patients cope with the emotional stress of mesothelioma and the unpleasant side effects of most conventional treatments.

Herbal Remedies

There are a number of herbs, vitamins, and botanical supplements that are promoted for use by cancer patients. This includes a number of herbal remedies used in Chinese medicine, as well as other over-the-counter herbal and vitamin supplements sold in health food stores and drug stores. Some of these supplements may be helpful, and in some cases your doctor may even prescribe specific vitamin supplements to counteract some of the harmful side effects of chemotherapy or radiation.

However, it is very important to consult your doctor before trying any of these remedies. You should be particularly careful about using any herbal remedies or dietary supplements. While these may be "natural", they are not necessarily safe. They may decrease the effectiveness of certain anti-cancer drugs, and they may actually be harmful when taken in large doses or in combination with other conventional drugs.

Mind-Body Approaches
