The Federal Government As Jabba the Hutt - Too Big and Too Slow to Get Anything Right

It occurred to be recently while watching television that the U.S. Federal government is really nothing more than the political incarnation of Jabba The Hutt from the Star Wars movies. As you may recall, Jabba was a formless, slow moving, overweight entity that ruled with an iron fist while devouring resources brought to him. He did not pay for anything, he just took them, and he was not there to help anyone but himself. Those he ruled over had no say in how their resources and wealth were used and had no chance in dislodging Jabba from his position of power. Kind of sounds like our political class currently sitting in DC.

Jabba came to mind today as I thought about some recent occurrences in which the Federal government has gotten so big that it is doubling back on itself and putting itself in some very weird situations and conflicts with the reality of the world around us:

- According to an article in the July 23, 2010 issue of The Week magazine, a Boston Federal judge has ruled parts of the Federal Defense of Marriage Act to be unconstitutional. This act forbids the Federal government from recognizing gay marriages and granting Federal benefits. The judge ruled that the Federal law forces Massachusetts to discriminate against it own citizens. Now for the weird part. the Obama administration is now forced to appeal the ruling, even though his administration and the Democrats in charge of Congress oppose the Defense of Marriage Act and want it repealed. The Federal government has gotten so big that it is expending legal resources for something that it does not want to exist in the first place.

- This weird situation is similar to the current illegal alien situation. A recent Associated press article reported that the latest statistics relative to illegal border crossers shows that the Federal government recently had the highest levels of prosecutions for illegal aliens and the highest deportation levels of illegals since they started keeping track of such statistics but at the same time this same Federal government was in court fighting the new Arizona state law that was trying to stem the flow of illegal aliens into that state.

- Getting back to gay marriages, according to a July 15, 2010 Associated Press article, the country of Argentina recently became the first Latin American country to legalize gay marriage. The article reported that Chile and several other South American countries are likely to try and follow suit. Do we think that our Jabba, parading as the American political class, has any chance of making that happen in this country when it finds itself in court defending against gay rights?

- According to an article in the August issue of Reason magazine, since 1996 fourteen states and the District of Columbia have legalized marijuana use for medical purposes and several other states are considering doing the same. This is in direct conflict with Federal law which has sometimes resulted in Federal raids of medical marijuana providers which are illegal under Federal law but legal under state law. This is also in conflict with Obama the campaigner who wanted to decriminalize the drug when he ran for President but now defends the outlawing of it at the Federal level. Again, government has gotten so big that we have entered the weird zone regarding medical marijuana where it is legal at the same time as being illegal.

- Speaking of drug conflicts, a recent Associated Press article reported that the Federal Veterans Affairs organization would allow its patients to use medical marijuana if those patients resided in the fourteen states where medical marijuana is legal. Thus, one arm of the Federal government (Veterans Affairs) is perfectly fine with medical marijuana use while other arms of the Federal government (FBI, DEA, Federal statue) wants to wipe it out.

- If you think the Federal government has a drug problem now, wait until the Oakland City Council votes on whether to allow industrial farming of marijuana to be established in city limits, industrial farms which would produce marijuana for medical use as well as for use in products ranging from baked goods to body oil. Winning applicants who would operate these farms would have to pay annual permit fees and eight percent of their sales to taxes as well as carry $2 million in liability insurance. Similar efforts are being pushed in other cities throughout the state along with a November ballot issue to legalize non-medical use of marijuana, according to the article. Now consider the contents of a short blurb in the July 23, 2010 issue of The Week magazine that reported on a Rand Study that concluded from their analysis that the legalization of marijuana would reduce the street price by up to 90%. Thus, the efforts in California may help reduce the street price of the drug which in turn would significantly reduce the power, wealth and influence of the Mexican drug cartels which would be a good thing. However, in the face of this good set of outcomes (more revenue for the local governments, less of a stigma of marijuana users, less law enforcement resources spent on busting marijuana users, the weakening of the Mexican drug cartels) do we think that the Jabba the Hutt beast in DC is nimble enough to understand what the benefits are or will it continue down its path of conflict at the Federal drug enforcement level?

- Consider a Washington Post article from May, 2007, entitled "Federal Loans Fuel Push For Coal Power Plants." The article discusses a leftover Depression era Federal program that provides low cost loans to build coal fired, high pollution power plants using taxpayer money. According to the article, "the [loan] support is a major force behind the rush to coal plants, which spew carbon dioxide that scientists blame for global warming." Thus, while the Obama administration is pushing a climate control bill in order to fight against global warming, the same government is funding power plants that do just the opposite. Makes no sense.

- A recent Associate Press article reported how the Feds had busted 94 people for defrauding the Medicare program. This was a good development but why was our Jabba so slow in getting these arrests done? Medicare fraud has been going on since the day Medicare started several decades ago, why did it take such a painfully slow time to start arresting the cheats? One of those arrested had filed over 3,700 fraudulent claims under her name before she was arrested, how slow can you get?

We could go on and on. The U.S. government has gotten so big and so slow, just like Jabba the Hutt, that its many folds of skin hide waste, stupidity and the doubling back on itself, i.e. government laws and actions in conflict with itself or the desires of those running the government. We could go on and on about how slow, ineffective, and wasteful our Jabba is, about how our Jabba never solves a problem whether it is insecure borders, failing public schools, escalating health care costs, etc., how our Jabba wastes untold billions of dollars on earmarks, useless initiatives, and fraud-infested programs, or how our Jabba does not know how to regulate the economy, resulting in sky high national debt levels and a very creaky economic situation with low growth and high unemployment.

Jabba is very bad for all of us but he is difficult to dislodge. Through the earmark process, the gerrymandering of Congressional districts, do-nothing campaign finance laws, and other methods, Jabba has many defenses against defeat in an election, defenses that even a Jedi light saber could not easily pierce. Long term, it is critical we begin to impose term limits on politicians so that they never again get as fat, slow, wasteful, and ineffective as Jabba The Hutt. Short term, this November is critical since it begins the process of voting out the Jabba incumbents and finally getting in some sleek, effective, and courageous Jedi warriors who will make the difficult decisions to get the size of government under control and make that downsized government more effective and less weird and less conflicted.

Perkembangan dan Pertumbuhan Bayi Di Dalam Kandungan Dari Minggu ke Minggu | Trimester 1 hingga Trimester 3

Didalam kandungan, bayi terus tumbuh dan berkembang. seiring bertambahnya usia kandungan, organ tubuh bayi terus dibentuk. usia rawan kandungan adalah usia 3 bulan kebawah atau trimester pertama. karena pada usia ini merupakan tahap pembentukan organ tubuh bayi. oleh sebab itu, ibu perlu menjaga kandungannya sebaik mungkin agar tidak terjadi resiko bayi lahir cacat.
untuk mengetahui tahap tumbuh kembang bayi dari trimester pertama, trimester kedua hingga trimester ketiga, dapat dibaca artikel tahap-tahap pertumbuhan dan perkembbangan bayi berikut:

*Pekembangan dan pertumbuhan bayi pada Trimester Pertama (Usia 0-12 minggu/3 bulan)

Minggu Ke-1 (minggu pertama)

Pada minggu ini ibu mengalami menstruasi terakhir sebelum kehamilan. Perdarahan terjadi dan hormon-hormon di tubuh pun tengah mempersiapkan sel telur untuk dilepaskan.
Idealnya calon ibu berada dalam kondisi sehat optimal. Kebiasaan seperti merokok, minum beralkohol dan obat-obatan yang tidak perlu sudah seharusnya dihentikan pada masa ini. Suhu tubuh basal akan sedikit meningkat pada masa ovulasi dan berkisar antara 36,6 C dan berangsur - angsur akan meningkat. Uterus (dinding rahim) menebal dan mempersiapkan untuk tahap ovulasi.

Minggu Ke-3

Merupakan masa ovulasi (pelepasan telur), serta terjadi fertilisasi atau Pembuahan yang memerlukan waktu 4 hari, setelah telur dibuahi maka dinamakan zigot.pembuahan terjadi di tuba palopi.

Minggu Ke-4
zygote berpindah menuju rahim. Dengan berakhirnya minggu ini, ibu tidak mengalami menstruasi, dan menjadi pertanda pertama kemungkinan kehamilan. Pada beberapa wanita mendapatkan sedikit perdarahan dan disalahartikan sebagai menstruasi, sebenarnya perdarahan yang sedikit itu karena implantasi atau penanaman dari zygote ke dinding rahim ibu.

Minggu Ke-5
Pada masa ini, zygote berkembang menjadi embrio. Embrio berukuran sebesar buah apel. pada usia ini, bayi ibu sudah mempunyai detak jantung sendiri, plasenta dan tali pusat sudah berkerja sepenuhnya pada minggu ini. Vesikel-vesikel otak primer mulai terbentuk, sistim saraf mulai berkembang.

Minggu Ke-6
Embrio terlihat seperti berudu. Pada minggu ini bayi dapat mengenali kepala, ekor, tangan dan anggota badan yang masih seperti tunas. Pada minggu ini merupakan pembentukan awal dari hati, pancreas, paru-paru, kelenjar tiroid dan jantung.

Minggu Ke-7
Jantung sudah terbentuk lengkap. Saraf dan otot bekerja bersamaan unutk pertama kalinya. Bayi mempunyai reflek dan bergerak spontan (ibu belum dapat merasakannya). Akhir minggu ke-7 ini otak akan terbentuk lengkap.

Minggu Ke-8

Embrio sekarang berukuran panjang sekitar 25-30 mm. Lengan dan kaki sudah terbagi menjadi komponen paha, kaki,tangan, lengan, bahu. Organ reproduksinya mulai terbentuk begitu juga dengan kartilago dan tulang. Telinga luar sudah terbentuk sempurna, mata membentuk pigmen. Didalam otak, jaringan saraf berhubungan dengan lobi penciuman di otak.
Jantung sudah memompa dengan kuat dan irama teratur.

Minggu ke-9
Pergerakan pertama fetus dapat dideteksi dengan USG. Pada minggu ini perut dan rongga dada sudah terpisah dan otot mata dan bibir atas terbentuk.

Minggu ke-10
Tulang sudah menggantikan kartilago. Diafragma memisahkan jantung dan paru-paru dari perut. Otot leher terbentuk. Otak berkembang cepat dalam bulan terakhir ini sehingga proporsi kepala lebih besar daripada tubuh.

Minggu ke-11

Organ seks luar sudah terbentuk, juga folikel-folikel rambut dan gigi. Bayi sudah dapat menelan cairan amnion dan mengeluarkan kembali (kencing).

Minggu ke-12
Ukuran fetus anda sekarang sekitar 8 cm. Semua organ vital bayi sudah terbentuk. Dengan signal dari otak, otot akan merespon dan bayi sudah dapat menedang. Di akhir trimester pertama, organ-organ tubuh bayi sudah terbentuk. Rasa mual dan lelah pada anda sudah hilang. Perkembangan selanjutnay anda akan melihat pertumbuhan bayi (dan juga anda) bertumbuh lebih besar.

*Pekembangan dan pertumbuhan bayi pada Trimester Kedua (Usia 13-28 minggu)
Minggu ke 13 :
Panjang bayi sekitar 7,5 cm dengan trakea, paru-paru, perut, hati, pancreas, dan usu berkembang ke fungsi terakhir. Pita suara mulai terbentuk, dan tunas gigi muncul dengan 20 gigi bayi. Pada minggu ini jari tangan, telapak kaki mulai terlihat.

Pada minggu ini organ seks bayi sudah dapat dibedakan apakah laki-laki atau perempuan. Denyut jantung bayi berdetak kuat (detak jantung hampir 2x lebih cepat dari anda) dan anda dapat mendengarkannya di dokter nada. Bayi juga sudah siap memberi respon terhadap dunia di luar rahim ibu. Bayi mungkin akan bergerak bila anda mengusap perut anda, tapi anda mungkin masih belum dapat merasakannya.

Minggu ke 14:
Pada minggu ini organ seks bayi sudah dapat dibedakan apakah laki-laki atau perempuan. Denyut jantung bayi berdetak kuat (detak jantung hampir 2x lebih cepat dari anda) dan anda dapat mendengarkannya di dokter nada. Bayi juga sudah siap memberi respon terhadap dunia di luar rahim ibu. Minggu ke 16 :
Jika anda melakukan USG saat ini mungkin anda dapat mengetahui jenis kelamin bayi anda. Otot bayi sudah berkembang dan menjadi kuat. Gerakannya semakin aktif. Mulai menghisap ibu jarinya, menguap, merenggangkan tubuhnya, sudah mampu menelan, kencing dan cegukan. Pada minggu ini jika sinar terang diletakkan di perut anda bayi akan menggerakkan tangan dan matanya.

Minggu ke 17:
Masih banyak ruang dalam rahim anda sehingga bayi akan bergerak merasakan sekitarnya. Kulit bayi anda berkembang dan transparan. Terlihat merah sebab pembuluh darah masih terlihat jelas.

Minggu ke 18:
Sekarang bayi sudah dapat mendengarkan suara dari luar tubuh anda. Bayi akan bergerak atau melompat ketika mendengarkan suara keras. Otot bayi sudah dapat berkontraksi dan relaks, bayi sudah dapat menendang atau meninju. Bergerak sangat aktif, dalam minggu ini mungkin anda sudah dapat merasakan gerakan putarannya untuk pertama kali.

Minggu ke 19:
Sekarang panjang bayi anda sekitar 23 cm. Jika anda belum juga merasakan bayi anda, mungkin anda akan mulai merasakan gerakkannya pada minggu ini.

Minggu ke 20:
Otot bayi anda semakin kuat setiap minggunnya. Jika sebelumnya mungkin anda masih belum dapat merasakan gerakannya, maka sekarang ini anda pasti akan merasakannya. Tapi mungkin anda salah mengartikan gerakannya pertama kali sebagai angin, karena terasa seperti letupan gelembung-gelembung di dalam.
Bayi anda bergerak sekitar 200 kali sehari, tapi anda hanya akan merasakan sedikit dari semua gerakan ini.

Minggu ke 21 :
Panjang bayi sekitar 28 cm. Anda akan merasakan lebih banyak gerakan bayi anda sekarang karena bayi makin aktif.

Minggu ke 22:

Bayi anda sadar akan lingkungannya dan ia akan merasa lebih tenang ketika mendengarkan suara dan sentuhan anda di perut. Tubuh bayi mulai memproduksi sel darah putih. Ini penting untuk bayi dalam melawan penyakit dan infeksi.

Minggu ke 23:
Sekarang ukuran kepalannya sudah sesuai dengan tubuhnya. Saat ini bayi terlihat sama seperti yang akan lahir nanti, tapi masih lebih kecil dan kurus. Anda dapat mendengarkan detak jantung bayi anda dengan stestoskop. (letakkan stestoskop langsung di perut anda). Pada saat ini pertumbuhan otoknya sangat cepat.

Minggu ke 24:
Pendengaran bayi sudah terbentuk sempurna. Bayi akan bergerak dengan suara musik dari luar. Bayi mulai membentuk pola kapan saat ia tidur dan kapan saat bangun, Anda akan merasakan suatu saat bayi bergerak terus, dan saat yang lain tidak.

Minggu ke 25:
Bayi mulai berlatih bernafas dengan menghirup dan menghembuskan cairan amnion, yang mana kadang membuat bayi seperti cegukan, dan anda akan dapat merasakannya juga.

Minggu ke 26:
Bayi sudah mempunyai lemak dibawah kulit, yang akan membantu mengontrol suhu tubuhnya pada saat lahir. Wajah dan tubuhnya secara umum akan terlihat saat lahir. Panjang sekitar 28-32 cm dengan berat sekitar 680 gram.

Minggu ke 27:

Minggu ke 30:
Bayi anda mengisi hampir seluruh ruang di rahim anda. Ketika bayi menendang atau mendorong, anda dapat melihat kaki atau tangannya bergerak di bawah kulit perut anda. Otak berkembang sangat cepat.

Minggu ke 31:
Bayi makin bertumbuh besar, sehingga ruangan rahim menjadi lebih sedikit, ini membuat bayi kekurangan ruang untuk bergerak. Bayi kemungkinan dalam posisi melengkungkan badan dengan dengkul dilipat, dagu di dadanya dan tangan dan kaki saling bersilang.

Minggu ke 32:
Bayi berada dalam posisi kepala dibawah sampai lahir nanti. Bayi akan tetap menedang, gerakan rata-rata sehari meningkat 375 per hari. Tapi anda tidak akan merasakan semuannya ini, dan 10 gerakan sehari sudah normal.

Minggu ke 33:
Anda akan semakin merasakan gerak bayi karena bayi mengisi hampir seluruh ruang rahim. Gerakan akan menjadi aktif suatu waktu yang bisa membuat anda merasa tak nyaman, terutama ketika kakinya di bawah tulang rusuk anda. Pada minggu ini seluruh rambut bayi telah tumbuh.

Minggu ke 34:
Pada minggu-minggu ini terjadi pertumbuhan yang pesat terhadap otak. Semua sistem tubuh sudah terbentuk dengan sempurna, meskipun paru-paru masih tetap belum matang. Bayi memberi respon terhadap suara yang familiar.

Minggu ke 35:
Bayi terus menambah cadangan lemak bawah kulitnya. Kepala bayi sudah mulai memasuki panggul.

Minggu ke 36:
Mulai dari minggu ini bayi sudah mempunyai ukuran dan kematangan yang siap untuk lahir. Jika bayi lahir pada minggu ini bayi lair premature tetapi bayi akan baik-baik saja. Pada bulan terakhir kehamilan ini bayi akan mendapat antibody dari ibunya, seperti campak. Lemak akan terus bertambah dibawah kulit bayi setiap hari.

Minggu ke 37:
Bayi akan terus berlatih untuk menggerakkan paru-parunya, karena bayi akan bernafas setelah dilahirkan. Jika posisi kepala bayi dibawah maka kemungkinan kepala sudah memasuki panggul ibu pada minggu ini.

Minggu ke 38:
Reflek bayi sudah terkoordinasi, bayi sudah dapat mengedipkan mata, menggerakkan kepala, memegang, dan merespon suara, sentuhan dan cahaya. Bayi sudah dapat membedakan antara terang dan gelap.

Minggu ke 39-40:
Bayi sudah siap untuk lahir. Sekarang ini bayi sudah terlihat cukup montok. Dengan lemak yang terbentuk dibawah kulitnya, akan mampu mengatur suhu tubuhnya saat lahir. Kulit bayi halus dan lembut. Berat bayi sekitar 2.7 kg – 3.2 kg dengan panjang sekitar 50 cm.

IDEALIS Versus PRAGMATIS

Sering terpikirkan dalam benak saya ketika saya masih menginjak bangku kuliah di sistem informasi di Institut Teknology 10 November Surabaya (ITS Surabaya). Saya termasuk orang yang egois dan apatis dan selalu berusaha melawan sistem yang telah ada. Dalam benak saya selalu terpikirkan apakah tujuan saya kedepan ini akankah saya akan menjadi mahasiswa yang idealis ataukah menjadi mahasiswa pragmatis.

Saya sering mendengarkan orang mengatakan hal-hal negatif mengenai orang yang punya IDEALISME tertentu. Entah itu dari sindiran hingga secara terang-terangan telah banyak ditunjukkan kepada orang-orang yang mempunyai kesetiaan tertentu terhadap ide yang yang mereka yakini benar. Orang-orang indonesia, terutama sekali masyarakat perkotaan, menganggap bahwa idealisme adalah suatu konsep yang harus ditinggalkan jauh dalam menjalankan hidup agar mendapatkan hidup yang baik. Benarkah itu?

Sebelum menilai antara mahaiswa atau seseorang atau bahkan kelompok yang idealis atau pragmatis maka alangkah baiknya saya memberikan sedikit ulasan antara idealis dengan pragmatis.

Idealisme adalah suatu keyakinan atas suatu hal yang dianggap benar oleh individu yang bersangkutan dengan bersumber dari sebuah pengalaman, pendidikan, kultur budaya, maupun kebiasaan. Idealisme akan tumbuh secara perlahan dalam jiwa seseorang dan termanifestasikan dalam sebuah bentuk perilaku,kemudian sikap, ide, maupun cara berpikir dari orang atau sekelompok orang tersebut.

Pengaruh idealisme tidak hanya terbatas pada tingkat individu, tapi juga hingga ke tingkat negara. Nilai-nilai idealisme mempengaruhi individu contohnya adalah keyakinan mengenai pola hidup, nilai-nilai kebenaran, gaya mengasuh anak, karir dan sebagainya. Sedangkan idealisme pada tingkat negara adalah seperti ideologi Pancasila, Komunisme, Liberalisme dan masih banyak lagi.

Pentingnya idealisme pada pembentukan karakter mahasiswa adalah akan menciptakan mahasiswa yang berpola pikir kritis. Bukan berarti idealisme akan terus bersikeras mengikuti pemahamannya akan suatu masalah. Idealis berarti memiliki pemahaman yang berkarakter. Hal seperti itu akan menjadi lebih baik daripada hanya sekedar menjadi manusia yang tidak berkembang.

Idealisme memang memiliki kekurangan atau dampak negatif. Kekurangan itu adalah jika seseorang idealis bertemu dengan orang idealis lainnya. Maka akan saling mempertahankan pemahaman masing-masing. Dampak inilah yang menyebabkan terjadinya pertentangan akan pemahaman satu sama lain. Untuk itu mahasiswa berkarakter juga memerlukan sikap toleran agar tidak terjadi pertentangan dari pemahaman yang lain. berusaha mengambil dampak positif dari pemahaman idealis tersebut. Tidak salah menjadi idealis, akan tetapi menjadi terlalu idealislah yang menyebabkan masalah.

Sedangkan pragmatisme berarti pemahaman yang bersifat langsung bagaimana penerapannya sesuai dengan keadaan. Pragmatisme dapat diartikan juga sebagai motivasi. Yang berarti orang yang berpemahaman seperti ini akan selalu mempunyai motivasi untuk mendapatkan apa yang ia inginkan. Ia akan melakukan semua yang menurutnya benar untuk mencapai keinginannya.

Menurut Abraham Maslow (1943;1970) manusia melakukan tindakan untuk memenuhi kebutuhannya. Dalam tgeori hierarki of need menyatakan bahwa pada dasarnya semua manusia memiliki kebutuhan pokok. (serbasejarah,2011)

Pemahaman seperti ini juga mempunyai kekurangan dan kelebihan dalam penerapanny. Jika seorang mahasiswa memiliki karakter seorang pragmatisme berarti ia akan selalu melakukan apapun demi mencapai keinginannya. Positifnya ia akan selalu berusaha dalam pencapaiannya. Negatifnya ia akan melakukan apapun sehingga tidak mengindahkan batasan-batasan pada pencapaiannya. Pemahaman seperti ini harus dibarengui dengan pemahaman moral dan etika.

Dari kedua pemahaman diatas,jika mahasiswa penerus bangsa ini ditanamkan karakter yang berbeda secara bersamaan, dengan pertimbangan sisi positif dari kedua pemahaman tersebut, maka akan tercipta penerus bangsa yang berkarakter membangun bangsa. Idealisme yang mempunyai pemahaman akan cita-cita dan perubah. Pragmatisme yang mempunyai pemahaman selalu memotivasi bersungguh-sungguh dan tekun. Jika kedua pemahaman ini ditanamkan sehingga menghasilkan penerus bangsa yang berkarakter membangun. Tidak perlu diragukan lagi, ,mahasiswa bangsa ini tidak perlu lagi bersitegang masalah pendapat dan pemahaman mereka sehingga menyebabkan terjadinya kerusuhan. Sehingga dapat mengurangi tingkat kerusuhan dan tumpah darah yang konyol. Karakter yang selalu membangun ini, akan menumbuhkan peikiran-pemikiran yang inovatif dan kesungguhan dalam pelaksanaannya. Demi bangsa, bersama-sama, bergandeng tangan, untuk Indonesia LEBIH MAJU!!!!!!

SISTEM INFORMASI RUMAH SAKIT

A. SIRS
Sistem Informasi Rumah Sakit (SIRS) dimulai sejak tahun 1952 dan di revisi pertama kali
pada tanggal 01 Januari 1973. Sistem ini berlaku untuk semua Rumah Sakit baik yang
dikelola pemerintah seperti Departemen Kesehatan RI (Depkes RI), Pemerintah Propinsi,
Pemerintah Kabupaten/Kota, TNI, Polri dan Departemen lainnya termasuk Badan Usaha
Milik Negara serta Rumah Sakit yang dikelola sektor Swasta, seperti Yayasan Sosial,
Organisasi Keagamaan, Badan Usaha dan lain sebagainya.
SISTEM INFORMASI RUMAH SAKIT

A. SIRS
Sistem Informasi Rumah Sakit (SIRS) dimulai sejak tahun 1952 dan di revisi pertama kali
pada tanggal 01 Januari 1973. Sistem ini berlaku untuk semua Rumah Sakit baik yang
dikelola pemerintah seperti Departemen Kesehatan RI (Depkes RI), Pemerintah Propinsi,
Pemerintah Kabupaten/Kota, TNI, Polri dan Departemen lainnya termasuk Badan Usaha
Milik Negara serta Rumah Sakit yang dikelola sektor Swasta, seperti Yayasan Sosial,
Organisasi Keagamaan, Badan Usaha dan lain sebagainya.
Sakit Umum, Rumah Sakit Jiwa, Rumah Sakit Ketergantungan Obat, Rumah Sakit Tuberkulosa Paru,
Rumah Sakit Kusta, Rumah Sakit Mata, Rumah Sakit Orthopedi & Prothese, Rumah Sakit Bersalin dan
Rumah Sakit Khusus Lainnya.

Sistem ini telah dikaji ulang kembali agar dapat menunjang pemanfaatan data yang optimal serta
mengikuti perkembangan Ilmu Pengetahuan dan Teknologi dan juga agar dapat mengantisipasi
peningkatan kebutuhan data pada era globalisasi. Sistem ini diharapkan mampu menunjang
indikator atau parameter yang handal untuk mendorong keberhasilan Pembangunan Kesehatan di
Indonesia, khususnya bagi Rumah Sakit di Indonesia dalam mengukur mutu penyelenggaraan
pelayanan kesehatan sesuai standar yang berlaku, sehingga pada gilirannya dapat diupayakan aplikasi yang relevan sesuai dengan hasil penyajian atau keluaran dari Sistem Informasi Rumah Sakit.

B. Formulir Standar
Untuk berbagai data yang dikumpulkan melalui Sistem Informasi Rumah Sakit, digunakan formulir
standar sebagai berikut :
RL1 - Data Kegiatan Rumah Sakit
RL2a - Data Keadaan Morbiditas Pasien Rawat Inap
RL2b - Data Keadaan Morbiditas Pasien Rawat Jalan
RL2a1 - Data Keadaan Penyakit Khusus Pasien Rawat Inap Rumah Sakit
RL2b1 - Data Keadaan Penyakit Khusus Pasien Rawat Jalan Rumah Sakit
RL2c - Data Status Imunisasi
RL3 - Data Dasar Rumah sakit
RL4 - Data Ketenagaan Rumah Sakit
RL5 - Data Peralatan Medik Rumah Sakit dan Data Kegiatan Kesehatan Lingkungan.
RL6 - Data Infeksi Nosokomial.
1. Formulir RL1 merupakan formulir rekapitulasi laporan yang mencakup berbagai kegiatan
Rumah Sakit seperti rawat inap, pengunjung rumah sakit, kunjungan rawat jalan, kegiatan
kebidanan dan perinatologi, kegiatan pembedahan (menurut golongan dan spesialisasi),
kesehatan jiwa, pelayanan rawat darurat, kunjungan rumah, kegiatan radiologi,
(radiodiagnostik, radiotherapi, kedokteran nuklir, imaging pencitraan), kegiatan pelayanan
khusus, pemeriksaan laboratorium (patologi klinik, patoligi anatomi, toksikologi), kegiatan
farmasi rumah sakit (pengadaan obat, penulisan dan pelayanan resep), pelayanan
rehabilitasi medik, kegiatan keluarga berencana, kegiatan penyuluhan kesehatan, kegiatan
kesehatan gigi dan mulut, pemantauan dokter & tenaga asing lainnya, transfusi darah,
latihan/kursus/penataran, pembedahan mata, penanganan penyalahgunaan NAPZA,
kegiatan bayi tabung, cara pembayaran dan kegiatan rujukan.

2. Formulir RL2a memuat data kompilasi penyakit/morbiditas pasien rawat inap yang
dikelompokkan menurut Daftar Tabulasi Dasar KIP/10. Untuk masing-masing kelompok
penyakit dilaporkan mengenai jumlah Pasien Keluar menurut golongan umur dan menurut
jenis kelamin, serta jumlah pasien mati untuk masing-masing kelompok penyakit.

3. Formulir RL2b memuat data kompilasi penyakit/morbiditas pasien rawat jalan yang
dikelompokan menurut Daftar Tabulasi Dasar KIP/10. Untuk masing-masing kelompok
penyakit dilaporkan mengenai jumlah kasus baru menurut golongan umur dan menurut jenis
kelamin dari kasus baru tersebut dan jumlah kunjungan.

4. Formulir RL2a1 memuat data keadaan morbiditas survailans terpadu pasien rawat inap
rumah sakit.

5. Formulir RL2b1 memuat data keadaan morbiditas survailans terpadu pasien rawat jalan
rumah sakit.
6. Formulir RL2c memuat data status immunisasi sebagai lampiran dari formulir RL2a
7. Formulir RL3 memuat data identitas Rumah Sakit, Nama Rumah Sakit, Alamat Rumah
Sakit, Kelas Rumah Sakit, Surat Izin, Penyelenggara, Direktur Rumah Sakit, Fasilitas Tempat
Tidur, Fasilitas Rawat Jalan.

8. Formulir RL4 memuat data jumlah tenaga yang bekerja di Rumah Sakit menurut kualifikasi
pendidikan dan status kepegawaian.

9. Formulir RL5 memuat data jumlah dan jenis peralatan medik, jumlah, umur, kondisi, ijin
operasional, sertifikat kalibrasi serta data kesehatan lingkungan rumah sakit.

10. Formulir RL6 memuat data infeksi nosokomial di rumah sakit.
Frekuensi dan Periode Laporan
Frekuensi dan periode pelaporan disesuaikan dengan jenis data yang dikumpulkan yaitu :

a. Data Kegiatan Rumah Sakit - Formulir RL1
Formulir RL1 dibuat setiap triwulan oleh masing-masing Rumah Sakit berdasarkan
pencatatan harian yang dikompilasi setiap bulan. Data yang dilaporkan mencakup keadaan
mulai tanggal 1 bulan pertama sampai dengan tanggal 30/31 bulan ketiga pada setiap
triwulan yang bersangkutan.

b. Data Keadaan Morbiditas - Formulir RL2a, RL2b
Data morbiditas pasien rawat inap dan rawat jalan, dibuat setiap triwulan oleh masing-
masing Rumah Sakit berdasarkan pencatatan harian yang dikompilasi setiap bulan. Data
yang dilaporkan mencakup keadaan mulai tanggal 1 bulan pertama sampai dengan tanggal
30/31 bulan ketiga pada setiap triwulan yang bersangkutan. c. Data Keadaan Penyakit
Khusus - Formulir RL2a1, RL2b1 dan RL2
c.Data keadaan penyakit khusus rawat inap dan rawat jalan dibuat setiap bulan serta data
status imunisasi yang dilaporkan keadaan mulai tanggal 1 sampai dengan tanggal 30/31
setiap bulan yang bersangkutan. Laporan ini sebagai pelaksanaan laporan surveilans terpadu
di rumah sakit.

d. Data Dasar Rumah sakit - Formulir RL3
Formulir RL 3 diisi satu kali dalam setahun. Data yang dilaporkan pada RL3 merupakan data
dasar sesuai dengan keadaan pada tanggal 31 Desember setiap tahunnya.

e. Data Keadaan Ketenagaan - Formulir RL4
Formulir RL 4 dibuat dua kali setahun. Data yang dilaporkan sesuai dengan keadaan pada
tanggal 30 Juni dan tanggal 31 Desember.

f. Data Peralatan Medik Rumah Sakit dan Data Kegiatan Kesehatan Lingkungan Rumah Sakit -
Formulir RL5
Formulir RL5 dibuat satu kali setahun, sesuai dengan keadaan pada tanggal 31 Desember.

g. Data Infeksi Nosokomial Rumah Sakit - Formulir RL6
Formulir RL6 dibuat satu kali sebulan, sesuai dengan keadaan pada tanggal 31 tiap bulannya.
D. Jadual Pengiriman
Agar data yang dikumpulkan dapat diolah sesuai dengan jadwal yang ditentukan, maka
pengiriman formulir standar yang telah diisi dari Rumah Sakit dilakukan paling lambat 15
hari sesudah jangka waktu data yang dilaporkan.

E. Saluran dan Mekanisme Pengiriman
a. Formulir standar (kecuali RL2.1, RL2.2, RL2.3, dan RL4a serta RL2a1, RL2b1 dan RL2c)
dibuat dan dikirimkan ke Depkes RI cq. Ditjen Pelayanan Medik dengan alamat :

b. Selain itu laporan tersebut juga dikirimkan kepada semua instansi kesehatan di daerah
dan untuk arsip Rumah Sakit yang bersangkutan sebagai berikut :
1) Kepala Dinas Kesehatan Propinsi
2) Kepala Dinas Kesehatan Kabupaten/Kota
3) Bagi Rumah Sakit yang tidak diselenggarakan oleh Depkes/Pemda satu eksemplar laporan
dikirimkan kepada pemilik/penyelenggara Rumah Sakit yang bersangkutan.
4) Arsip Rumah Sakit

c. Khusus formulir individual pasien rawat inap dan ketenagaan (RL2.1, RL2.2, RL2.3)
dikirimkan ke Dinas Kesehatan Kabupaten/Kota setempat.

d. Khusus untuk RL4a hanya untuk Rumah Sakit Umum dan Khusus dengan status
kepemilikan Depkes RI.
F. Prosedur

Setiap Rumah Sakit yang tercatat di Depkes RI wajib mengikuti Sistem Informasi Rumah Sakit
yang telah ditetapkan, sesuai dengan prosedur sebagai berikut :

a. Setiap Rumah Sakit harus mempunyai surat izin penyelenggaraan Rumah Sakit
(tetap/sementara) dari Dinas Kesehatan atau Depkes RI atau surat penetapan sebagai
Rumah Sakit dari instansi yang berwenang.

b. Surat izin penetapan Rumah Sakit dikirimkan ke Direktorat Jenderal Pelayanan Medik
untuk mendapatkan nomor kode Rumah Sakit sebagai identitas dari Rumah Sakit yang
bersangkutan.
Setiap Rumah Sakit yang telah mempunyai nomor kode berarti telah tercatat di Depkes RI.

c. Nomor kode Rumah Sakit ditetapkan oleh Direktorat Jenderal Pelayanan Medik Depkes RI,
dan disampaikan ke Rumah Sakit yang bersangkutan, dengan tembusan dikirim ke Kantor
Dinas Kesehatan Propinsi atau Dinas Kesehatan Kabupaten/Kota.
d. Untuk mendapatkan gambaran tentang fasilitas Rumah Sakit tersebut maka setiap tahun
Rumah Sakit diminta untuk mengisi formulir RL3, dan mengirimkan ke Depkes dan instansi
yang telah ditetapkan dalam saluran pengiriman data.
e. Pengiriman formulir standar sesuai dengan periode dan jadual yang telah ditetapkan,
sebagaimana tergambar dalam Resume Sistem Informasi Rumah Sakit.
