Interesting situation in Test tube baby treatment

I got one couple, Husband 52 yrs and wife( A) 48 yrs,she had tubal block and treated but no pregnancy. The persong married another lady( B) ( accompaning them),this woman is of 32 yrs .

They come to me for testtube baby procedure, using the eggs of (B). But he B is suffering from Polycystic ovarian disease and hirsute. Chances of pregnancy is usually less in Poly cystic Ovarian disease( PCOD), but we get good number of eggs.

A is without periods but having good uterus,but not willing to undergo any sort of treatment as she was frustrated by earlier treatment

I convinced them that I will use both A and B's uterus for embryo transfer.

So retrieved eggs of B and made 4 blstocysts

We transferred two in each on 16.1.2011

Let us see what happens.......

I have examiined her 3 yrs back and told her to remove the large posterior wall fibroid before undergoing IVF, but as usual the patient goes for more short cuts and visits more doctors for any alternate treatment.
She was with endometriosis too.
But she had undergone IVF treatment at three centers without removing the fibroid and as usual it failed.
Psychologically troubled with her husband she approaches me again, with much larger fibroid and with more age...
Now myomectomy may cause trouble and may cost her uterus too. I feel bad

One couple came to me for fertility treatement from a remote area of northern MP.
The female has bilateral tubal block ,and I advice her to go for test tube baby or IVF treatment. She agreed and understood the pros and cons of the treatment.It was one month back.
Today she came with her husband and deposits the money.
Now it is my turn to make them understand ,how the treatment will be.
As I explained them , that they will require few frequent visits and may have to come to me daily for evaluation in the last leg of the treatment. They were not ready for frequent vists. Why?
I asked them why they are not willing to come
The answer was : Whenever we come outof village,pleople come to know that we are leaving. We hve to explain our relatives where we are going and we lie. We can not tell our people that we are going for test tube baby treatment ( as it is assumed that this child will be illegitimate). So it will not be possible for us to have frequent visits and we can only come once in a month. If you can do the treament in this way,then only we can take the treatment.
I refused as it is impossible.
They took their money and went back. They tell me ,that it is better to be childless than telling our people ,we are undergoing IVF treatment.
I really feel sorry,esp. for the lady ,who is taking all the brunt of being childless.

Twin pregnancy by Test tube baby (IVF) started well,but ........

A couple came to us for IVF-ICSI.

Woman is aged 36 yrs. IVF done in other centre ,failed.

we started our treatment,took her in short protocol.

6 Good quality oocytes retreieved. and 5 ICSIied.

We got three blastocyts on day5. two good quality. Looking at her age all the three blastocyts tranferred ,Embryo transfer was good

After 14 days ,pregnancy test done by beta hCG and it came 1200 mIU/ml.

After 10 days ,two intrauterine gestational sacs seen, and it followed and at 8 weeks both the embryos were live and growing well and treatment continued.

She was called at 11 weeks for NT scan.

Today 11.10.2011 ,I was shocked to see ,that one twin died ( heart stopped) and other one is alive but has a NT of 9.9 mm,rather it looks like cystic hygroma,with generalized skin edema...looks like Turner's or Down syndrome.

It is really a painful situation for all of us.

Now question is ,should she go for termination straight way or we should identify the defect in the alive baby.

We decided to do invasive testing of the alive baby and do FISH and/or karyotyping and then take the decision.

Even good looking blastocysts may tern into this, Is it a epigenetic problem of blastocyst transfer? or the Age matters...........?

Test tube pregnancy( IVF) in a lady who had no menstrual cycle

A woman aged 28 yrs, married life 6 yrs came to us . She did not have menses in her life( primary amenorrhea). On examiniation we found that she had an intact uterus and very small ovaries. On further examination we found that she was suffering from hypogonadotropic hypogonadism ( no hormonal drive from pituitary and hypothalamus).
We put her on gonadotropin and she responded, eight eggs retreived and ICSI done.
Blastocyt( two) transfered on 5th day of egg retrieval.
We did beta hCG today( pregnancy test) today and found positive. Now let us hope for the best....

ART or the assisted reproductive technology is a term used for the procedure carried out to the couples who are failed to reproduce on their own ,through a natural mating. It comprises of artificial insemination, intra-uterine insemination and ICSI-IVF.
To provide any treatment to any infertile couple, their should be a logic for that particular treatment. Indiscriminate treatments leads to a great stress, both psychologically and financially.
For that , their should be a streamlined approach to find out the reason or etiology of the infertility. In the past , couple would undergo an exhaustive and lengthy infertility evaluations that could last for months. The battery of diagnostic tests included multiple semen analyses, several blood tests, a hysterosalpingogram(HSG), BBT charts, endometrial biopsies, postcoital test, various immune tests on cervical mucus and/or sperm and a laparoscopy. Some of these procedures were not only painful and invasive but results were inconclusive so they had to be repeated frequently, thus prolonging the time of the workup and its associated discomforts and risks. Most of these tests are now considered unreliable and in retrospect waste of time. Today , the infertility evaluation is streamlined and essentially encompasses only three basic tests : a cycle day 3 FSH/E2 or AMH, hysterosalpingogram, and semen analysis along with a thorough transvaginal sonography.
On the treatment part : the couple should be offered with an overview of all treatment options from the most conservative to the most aggressive. The level s of the treatment depend on the condition and the cause of infertility. The level one is with oral medication, with or without IUI, level two is with injectable medications ( gonadotropins) with or without IUI and the third is ICSI-IVF.
There should be direct interaction with the couple with the physician, as fertility treatment is very stressful. We should be honest with the patients with their chances of success. Doing IVF-ICSI in big batches should be avoided in small set-up as physician can not do justice with the patients due to various reasons. IVF treatment is team effort and success depends on their quality too. The patient drop out is common due to psychological stress, hence the whole team should handle them very sensitively

A lady( Ta...) from Saudi Arabia came to me with her husband with a marital life 4 yrs. They came tome with a history that doctors told there,that she is not producing eggs and will not be able to become mother. They visited 4-5 doctors in Saudi Arab ,and she was treated indescriminately with clompihene citrate and gonadotropins and after sonography(?) she was told that the eggs are not growing and it is not possible to treat her here. They were totally panicked
They contacted me through website and within two days they came to me. They were totally lost their hopes.
Fortunately ,I did an ultrasound ,and found that there are multiple eggs in her ovary and all are stimulated. ( she is a case of Polycystic Ovarian Disease).
I convinced them for oocyte retreival and ICSI-IVF. They agreed . After giving a maturation injection of hCG ,we did oocyte retrieval. 26 eggs retreived ,with 4 in MII stage ( stage where ICSI is possible).
My wife ( embryologist) Dr. Rinku did ICSI in 4 eggs , and later 3 fertilized.
Two embryos went upto blastocyst stage ,one with good quality.
Embryo transfer done with ease.
She went into ovarian hyperstimulation syndrome and treated .
On 14 th day Pregnancy test came positive.
We asked her to stay in my city, we followed her
Today I did an ultrasound and found that she is carrying a healthy pregnancy of 16 weeks.
Conclusion : Totally lost hopes,but courage and instant decisions made the things possible

twin pregnancy by Egg Donation after 4 failures in premature ovarian failure

A 32 yrs . female Reeta ,lost her periods at the age of 28 yrs. She came to us 2 yrs back with loss of menses and childlessness. We investigated and found to be premature ovarian faliure ,chromosomal analysis did not reveal any thing. We took her in egg sharing program ,and after giving high amount of estogen ,her endometrium did not go beyond 6-6.5 mm. We tried four attempts and failed. After that I put her on " anti tubercular treatement"( I should have started this treatment earlier) emperically( as DNA PCR did not show M.tb in her curettings). This continued for 6 months. After finishing this course we tried another egg donation attempt. Fortunately ,this time endometrium goes to 8 mm. Two blastocysts transferred on 23/5/2011 and pregnancy test done on 5th june 2011.
Today I did a transvaginal sonography: There are two well defined intrauterine sac with fetal heart activity.
Few lessons : We should not give-up. I should have started anti-tubercular treatment earlier.

There is tremendous fear of pain during IVF procedure. People have various notions regarding IVF procedure.
I kept a large board in my reception denoting the facts of ICSI-IVF and,It is visible very clearly to couple they come to me for IVF-ICSI. Along with the facts of success and costs other points:It is :
1. You do not have any pain during IVF procedure,esp. during egg retrieval,as it is always under general anesthesia and it takes hardly 4-5 minutes. The embryo transfer is not done under any anesthesia ( mostly in cases we do). It is our strong belief that ,while placing ( not injecting) the embryos inside the uterus, the procedure should be painless. If there is any cramp in the uterus and the lady complains about it,we stop the procedure and do it later on. Because any pain means uterus is not quiescent and it will throw the embryo out.
2. You do not require any hospital admission.
3. You don't require any surgery. ( Egg retrieval is from vagina under sonography guide,with a needle)
4. YOU DO NOT REQUIRE ANY BED REST ,CAN GO HOME AFTER ONE HOUR OF EMBRYO TRANSFER AND INTERCOURSE IS ALLOWED AFTER THAT.
Hope these informations will help to those couples who are planning to undergo this procedure. EXCEPT SOME PAIN DURING THE INJECTIONS,this procedure is very patient friendly.This writing is in response to one of my patient from London ,who has lots of fear regarding this procedure.

Today I did a Cesarean section for IVF triplet pregnancy. It was a premature delivery as she had premature repture of membrane today in morning. The pregnancy was of 31 weeks and all the three babies are alive. But I donot know what will happen to them as they are very premature.
IVF in private practice is very tricky business. Here the couple spend the money from their own pocket( Not sponsored by any Govt funding or Indurance company). We want that the couple ( and us too) should succeed in getting pregnancy,for that we put 2-3 embryos . When all of them gets implanted,then there is problem of plenty. Multifetal pregnancy is one of the biggest trouble maker in my work now. We are trying to get the courage to do a single embryo transfer,but some times patient refuses for single embryo transfer,they want twin. The logic is " we will get two at one price"
As we are doing blastocyst transfer, we are facing the spontaneous twinning in some pregnancies.
By the way ,my patient,who was carrying IVF twins and aborted one ,is doing well with one live fetus.

I feel really frustated ,when I see the educated lot ,(even after having a succcessful IVF treatment in my clinic ),reluctant to say ,that they concieved by this procedure. When you apply your knowledge, skill and toil to achieve a very high scientific feat, and the couple does not acknowledge it( I am not generalizing),it is really agonising.
Society still not see the test tube baby pregnancy with respect,and feel that,the baby created by In-vitro fertilization is an alien.
I feel the "Test tube Baby " word is disturbing,but we can not help it. For the sake of glamour it has been created ,and now even the educated segment of the society feels that the baby is coming from " Mars".
May be,Because of these compulsions , the couple try to hide their identity in my clinic, and even if they are pregnant ,will say that this pregnancy is a natural one and they are here for a regular antenatal check-up.
I feel sad on their psychology, and try to placate them ,Amen

IVF twin pregnancy in trouble

Today we were in great dilemma. A IVF -ICSI pregnancy done 3 and half months back and it was successdul twin pregnancy in first attempt.
Every thing was fine till 3 days back ,but suddenly the contractions of uterus started and there is release of water. I did ultrasound today ,and found that both the babies are alive.One is lying in vagina and other is inside the uterine cavity and membranes and liqour intact. the cervical canal is closed.
As second baby is alive and healthy. we took out the baby present in vagina and left the placenta and the other baby inside,with a hope that it may continue,as there is no contraction and bleeding at present.
We explained our fear to the relatives regarding : Bleeding, Expulsion, Coagulation defects,infections etc.
Let us hope for the best ,and this may go as spontaneous fetal reduction........

IVF-ICSI in batches should be avoided

1. Should we do IVF-ICSI in batches ?
It has been observed that there is a growing trend In opening new IVF centers in many cities. Every gynecologist who has a good practice have a dream of starting a IVF or test tube baby center in his premises to make the institution complete. We must welcome this attitude, but do they ever understand the pros and cons of this project ? My personal feeling is that they do understand the problems of the project and take multiple precautions to make it a economically and “ pshycologically” viable . This type of projects are started at those places where there is no in-house embryologists, and in some places they do not have in-house Reproductive endocrinogists ( usually a gynecologist dealing exclusively with endocrinology) and an Embryologist. Here the practicing gynecologist collects infertile couples and makes a batch. This exercise is must because ,when there is no in-house facility and the team is called from out side for 2-3 days , they have to look after the economics. The number of couples should be as maximum as possible, and, the stay of the coming team should have to be as minimum as possible. This is a fact that , the coming Gynecologist and/or Embryologist will charge their fee ,per couple and ,the stay and food had to be born by the IVF center. It is important for the visiting doctors that ,this project have to be economically sustainable for them also., because they are coming here leaving there own practice behind( ..?)
But my simple queries are:
1. How many couples you can take in one batch ?( I have heard about taking 10-15 couples in a day by some centers)
2. Does all the female partners can be down regulated in same way ,or do all respond in a same way?
3. Does all the female partners require same protocols ?
4. How do you time the hCG injection for the female 36 hours prior to oocyte retrieval.
5. Can a single embryologist handle so many eggs together, even he/she takes them for conventional IVF . and if they take them for ICSI, how he/she can handle them for maturation, teasing and denudation and identification for MII stage.
6. Do they have more than one embryologist so that they can handle more patients.
7. Do they have more work stations and micromanipulators for multiple patients.
8. If more than 4-5 patients are taken ,do they have multiple incubators. It is important ,because more the opening of the doors, more will be fall in temperature and disturbance in pH.
9. Who will maintain the pH of the incubators for this particular batch of culture media, prior to the day of oocyte retrievals, Do these lab people come early to check the quality control of the laboratory equipments, at least two –three days prior to actual work.
10. Who will decide that particular batch of the culture media requires what concentration of CO2 to get the appropriate pH inside the incubator?
11. Can they go for blastocyst transfer, because it requires 5/6 days of embryo culture, and may require a prolong stay of the visiting team ( costs too much for center….)

I am writing all these stuff, because I got a call from one of my colleague, maintaining her IVF center in the same way and now feel frustrated because of poor outcome. She was continuously told by the visiting faculties that the work is going well. But at the end of the day , the results are so poor ,that ,for her it is difficult to answer her own patents, who are banking on her. The vendors put their most expensive machines ( with the opinions of visiting faculties..?),still there is poor results. Then faculties asked the IVF center owner for Laser assisted hatching and same vendor puts the Laser..(?) at owners cost. As the results are still poor, she gets infuriated and frustrated. As she had very good name in regular and advance gynec work, this work and its result ( which is not her domain) ,makes a dent in her reputation.

Here I am not an authority to give sermons and preaching, but it is my humble advice to all those gynecologists, aspiring for IVF center (because they can afford it)., think again and again to start it. If results are not consistently good , then your regular practice might go well , but these instruments will rot in a corner of your hospital after some years. It is better to be the captain of the ship and along with that you should know how to row it too. If this is possible then go for purchasing the ship for your own. Amin

We did 15 IVF -ICSI cases in first 1/2 of April 2011 ,out of this 11 conceived and confirmed by fetal heart. All are blstocyst tranfers. Three twin pregnancy.
