The music of containment: Addressing the participants in mother-infant psychoanalytic treatment

The author discusses the psychoanalyst’s address in mother-infant treatments. He brings out the infant as an important though often neglected addressee. A clinical example is used in which a telephone call during the session before triggered fretting in a 3-month-old girl and distress in her mother. He describes how non-verbal levels of his interventions reached the girl and contained her, and suggests his containment worked along similar lines as the communicative musicality between mother and baby. He thus links Bion’s psychoanalytic concept of containment with Trevarthen’s concept of communicative musicality. He also brings out the mother’s need for containment and discusses when it is essential to focus on either participant in the therapy room. This choice is guided both by explicit deliberations and by unconscious factors comprising his countertransference. Our wish to grasp the countertransference is countered by our unwillingness of being reminded of our own infantile helplessness. The baby’s ability to receive her mother’s caretaking is compromised when the mother’s conscious and unconscious messages diverge. This was the case when she tried to soothe her daughter but was preoccupied with anger at the therapist to an extent she was not fully aware of.

Coparenting and pregnancy: an ultrasound study

Transition to parenthood involves a transformation of mental representations for mothers and fathers. During pregnancy the coparenting and triadic relational subsystem emerge. Ultrasound during pregnancy could promote paternal involvement, reassures parents and allows them to share fantasies about the baby. This study aims to explore parental and coparental identity during pregnancy and the role of baby’s image through the 4D ultrasound. Results evidence some differences between the observed couples and some indicators of affiliative, parental and coparental processes.

Pyshoanalytic psychotherapy with infants and their families

When a family requires consultation for an infant, family members often bring a very powerful load of affects and raw, confused and undifferentiated emotional elements into the consulting room. It is apparently difficult, if not even impossible, to carry out separation interventions. Respecting the fusion set-up of these families appeared to be important, as it allowed them to be resilient to traumas and unbearably painful internal or external experience. Family psychoanalitical psychotherapy proves to be a valuable therapeutical tool: thanks to the presence of several members of the family, the initial cluster of raw emotional experience can be split into multiple configurations and versions. It involves working in a territory where attention is paid both to intrapsychic dimension and interpersonal relations. It is therefore possible to start family psychotherapy treatments very early: early infancy psychotherapists have the opportunity of intervening when mental circuits can still be very easily modulated, before environmental difficulties intervene, enter the body and are deeply structured. Moreover, in family psychoanalitical psychotherapy it is possible to treat older children suffering from severe diseases, where concrete mental functioning prevails. This paper describes some movements from the destructive acting-out and somatization dimension to the symbolic dimension of emotion-centered reflective thinking.

The use of observation in parent-infant psychotherapy when parents suffer from psychiatric disorders

This paper explores the demanding work with parents with mental illnesses and their baby. The family was referred to a C.A.M.H.S. by the health visitor, concerned about issues of bonding. A limited piece of work was contracted with the family, who was already receiving much help from several agencies. The therapist’s task was to share the observations and understanding of the 4-month-old girl’s mind and her enfolding relationship with her parents in the here-and-now of sessions. The parental disturbances inevitably were played out and were addressed in an alternating dialogue with both parents and infant. The splits, projections and countertransference aspects of working with these parents are high- lightened.

Paternal identity and perinatal depression

The article descrives the importance of having had a paternal and maternal meaningful experience when one becomes a parent. How to address the difficulties fathers experience after the babies birth. May we talk of paternal postnatal depression since it is the new family dynamic that sets off such depression? What is the paternal postnatal care like? We try to answer these questions on the base of discussions with fathers in a small group once they become fathers.
