Si hay algo que pueda quedar claro del uso habitual de sistemas de Inteligencia Artificial es que suele ser más importante la pregunta que la respuesta. Por eso comparto una que me parece pertinente que respondamos cada cual, adelanto que llevo varios meses con ella en la cabeza.

La cuestión es generalizada en todos los países y culturas: pese a los avances tecnológicos cada vez es más difícil ejercer la medicina. Esto condiciona una tensión creciente en los que la ejercemos con picos de presión compartidos como pudo ser la pandemia, estacionales como las sobrecargas por enfermedades invernales o por falta de compañeros, o individuales por circunstancias personales.

Me atrevo a adelantar que la mayor dificultad es el desequilibrio creciente entre lo que los sistemas sanitarios ofrecen y las demandas incrementales de una sociedad cada vez más enferma. Entre medias la parte más elástica y vulnerable somos los profesionales de la salud que tratamos de estirarnos hasta llegar al límite de la rotura.

Alguno podría preguntar por qué la sociedad está hoy más enferma y la respuesta breve incluye tres razones. La primera por la mayor complejidad y aceleración del ritmo de vida que nos somete a una competitividad enfocada a la productividad basada en la autoexigencia. La segunda por una dilución de los vínculos familiares, vecinales y sociales tradicionales hacia una mayor individualización y soledad. Y la tercera por un cambio de las narrativas tradicionales religiosas, filosóficas e históricas hacia unas nuevas enfocadas al éxito material y personal, alimentadas de herramientas digitales que favorecen el ruido de fondo y merman la reflexión.

A esto hay que añadir la incertidumbre e incomodidad que produce la inestabilidad en la política internacional y nacional, las catástrofes naturales, la crisis económica y la devaluación de los empleos que dificultan el acceso a la vivienda y a una vida digna.

Resulta que la vida ordinaria produce cada vez más malestar y pareciera que nuestras capacidades para sostenerlo van mermando, seguramente por las razones aportadas. Como ha pasado siempre, seguimos sufriendo por las tensiones de la vida, por las desavenencias amorosas o por los conflictos en el trabajo. La diferencia es que antes lo arreglábamos en nuestro entorno y ahora acudimos al sistema sanitario. Ayer atendí en consulta a dos personas que sufrían rupturas de pareja, otra que se lleva mal con el jefe y otra que está de baja por desavenencias con la empresa… así todos los días. Lógicamente el sistema sanitario no tiene capacidad para absorber esa catarata de problemas que saturan desde Atención Primaria y Salud Mental hasta las urgencias y consultas de hospital.

Aumentan pues las listas de espera y la saturación de los profesionales con más citas diarias y más pacientes sin cita que atender. Pero hay un límite. La pandemia del Covid nos lo dejó muy claro. Existen líneas rojas que sin son cruzadas rompen a los profesionales.

Y si fuera poco tenemos un sistema sanitario que no se caracteriza por cuidar a sus trabajadores. Tengo muy presentes las enormes movilizaciones de hace tres años tras la pandemia para tratar de mejorar una Atención Primaria que se derrumbaba y las más recientes para conseguir que escuchen a los médicos para un Estatuto Marco que sigue consintiendo inequidades.

En la mesogestión estamos hartos de ver ejemplos de mala praxis. En la primaria madrileña llevamos muchos años, desde la instauración del área única, sin una cadena de liderazgo operativa. Antes teníamos a un gerente de área y a directores de enfermería y medicina accesibles, ya no. Están perdidos en despachos inaccesibles de los que nunca salen. Vemos de tarde en tarde a algún subalterno (directoras asistenciales o responsables de centros) que no tienen poder ejecutivo ni capacidad de respuesta para los problemas cotidianos. Todo deben “trasladarlo” a quien corresponda y así quedan sin solución los desgarrones que van socavando lo inmediato.

Y qué decir de la microgesión… un servidor intentó servir como director de centro de salud durante años hasta que tuvo que dimitir por sobrecarga (2400 pacientes de cupo sin horas de liberación para gestión, en un centro tensionado). Y al que acaban de expedientar por haber mandado “tres correos electrónicos a una multiplicidad de personas” para informar de una candidatura al Colegio de Médicos, no muy grata a la Consejería a tenor de los hechos. Un Colegio en el que por cierto una junta saliente (esta sí del gusto del Sermas) está protagonizando una situación deleznable de juego sucio y bloqueo tras una gestión que ha dejado en bancarrota a la institución y perdido el respeto a la asamblea de compromisarios que lleva un año sin convocar y a los colegiados a los que no reconoce su voto en las urnas.

La situación es muy oscura. Por eso vuelvo a la pregunta original mientras aprovecho los cinco días de suspensión de empleo y sueldo para tratar de dirimirla en primera persona. Algo me dice que nunca han hecho más falta médicos íntegros que aporten su ciencia y arte a los que lo precisan. Pero si los médicos enfermamos como colectivo, ¿quién aportará luz en esta noche larga? Espero que a los que se atrevan a formularla os resulte posible dar con una respuesta verosímil que deseo afirmativa. Si finalmente el juez decide que podemos entrar en el Colegio espero poder trabajar desde ahí para conseguirlo.

Is it still worth being a doctor today?

If one thing has become clear from the regular use of Artificial Intelligence, it is that the question is usually more important than the answer. That is why I am sharing one that I feel is pertinent for each of us to answer; I must admit, I have been turning it over in my mind for several months.

The issue is widespread across all countries and cultures: despite technological advances, it is becoming increasingly difficult to practise medicine. This creates growing tension for those of us in the profession, with shared peaks of pressure—such as the pandemic, seasonal surges due to winter illnesses or staff shortages—or individual struggles due to personal circumstances.

I would venture to say that the greatest difficulty is the growing imbalance between what healthcare systems offer and the increasing demands of a society that is becoming ever sicker. In the middle, the most elastic and vulnerable element is the healthcare professionals, who try to stretch themselves until they reach breaking point.

One might ask why society is sicker today, and the short answer includes three reasons. Firstly, the increased complexity and acceleration of the pace of life, which subjects us to a competitiveness focused on productivity based on self-perfectionism. Secondly, the dilution of traditional family, neighbourhood, and social ties in favour of greater individualisation and loneliness. And thirdly, a shift from traditional religious, philosophical, and historical narratives towards new ones focused on material and personal success, fuelled by digital tools that encourage background noise and diminish reflection.

To this, we must add the uncertainty and discomfort caused by international and national political instability, natural disasters, the economic crisis, and the devaluation of jobs, which make access to housing and a dignified life difficult.

It turns out that ordinary life is producing more and more malaise, and it seems our capacity to endure it is waning, likely for the reasons mentioned above. As has always been the case, we continue to suffer from the stresses of life, from heartbreaks or conflicts at work. The difference is that, previously, we resolved these within our own circles; now, we turn to the healthcare system. Yesterday, I saw two people in my surgery suffering from relationship breakdowns, another who gets on poorly with their boss, and another who is on sick leave due to disagreements with their company... it is like this every day. Logically, the healthcare system does not have the capacity to absorb this deluge of problems that saturates everything from Primary Care and Mental Health to A&E and hospital clinics.

Consequently, waiting lists grow, as does the saturation of professionals facing more daily appointments and more "walk-in" patients to see. But there is a limit. The Covid pandemic made this very clear to us. There are red lines that, if crossed, break the professionals.

And if that weren't enough, we have a healthcare system that is not known for looking after its workers. I remember vividly the huge mobilisations three years ago following the pandemic to try to improve a collapsing Primary Care system, and more recently, the efforts to make doctors' voices heard regarding a Framework Statute that continues to allow for inequities.

In "meso-management," we are sick of seeing examples of malpractice. In Madrid's primary care, we have gone many years—since the establishment of the "single area"—without a functional chain of leadership. We used to have accessible area managers and nursing and medical directors; no longer. They are lost in inaccessible offices they never leave. Every now and then, we see a subordinate (assistant directors or centre managers) who has no executive power or capacity to respond to day-to-day problems. Everything must be "passed on" to whoever it concerns, and so the tears that undermine our immediate work remain unresolved.

And what can be said of "micro-management"... I personally tried to serve as a health centre director for years until I had to resign due to overload (a list of 2,400 patients with no allocated management hours, in a high-pressure centre). And I have just been disciplined with a suspension for sending "three emails to multiple people" to inform them of a candidacy for the Medical Association (Colegio de Médicos)—one that was clearly not to the liking of the Regional Health Ministry, given the facts. An Association, by the way, where an outgoing board (this one very much to the liking of the health service) is playing out a despicable situation of foul play and obstruction following a management period that has left the institution bankrupt and lost the respect of the assembly of representatives, which has not been convened for a year, and the members, whose votes at the ballot box are not being recognised.

The situation is very dark. That is why I return to the original question, while I take advantage of these five days of suspension without pay to try to resolve it for myself. Something tells me that there has never been a greater need for doctors of integrity who bring their science and art to those who need it. But if we doctors fall ill as a collective, who will provide light in this long night?

I hope that those of you who dare to ask the question find it possible to reach a credible answer—one that I hope is a "yes." If the judge finally decides that we can enter the Association, I hope to be able to work from there to achieve it.

现如今，当医生还值得吗？

如果说日常使用人工智能系统能让我们明白一件事，那就是：提问往往比回答更重要。因此，我想分享一个我认为值得每个人去思考的问题；坦白说，这个问题已经在我脑里盘旋好几个月了。

这是一个在所有国家和文化中都普遍存在的问题：尽管技术在进步，行医却变得越来越困难。这导致我们从业者承受着日益增长的压力——既有像疫情期间那样的全民压力高峰，也有像冬季疾病爆发或人手短缺导致的季节性超负荷，还有因个人境遇而产生的个体压力。

我敢预言，最大的困境在于医疗系统所能提供的服务与社会日益增长的需求之间，存在着严重的失衡。在这个日益病态的社会中，我们医护人员成了中间最脆弱、弹性也最大的部分，不断地自我透支，直到达到崩溃的边缘。

有人可能会问，为什么现在的社会病得更重了？简而言之有三个原因。第一，生活节奏的复杂化和加速，使我们陷入了一种基于自我苛求、以生产力为导向的竞争中。第二，传统的家庭、邻里和社会纽带被削弱，取而代之的是个人主义和孤独。第三，传统的宗教、哲学和历史叙事转向了追求物质和个人成功的新叙事，而数字工具又滋生了嘈杂的环境，削弱了人们的思考能力。

除此之外，国际和国内政局动荡、自然灾害、经济危机以及职业贬值所带来的不确定性和不安感，也让人们更难获得住房和体面的生活。

事实证明，普通生活产生的不适感越来越多，而我们承受这些不适的能力似乎在减弱，这很可能就是上述原因导致的。正如历史上一直发生的那样，我们依然会因为生活的压力、感情的不顺或工作的冲突而痛苦。不同之处在于，以前我们会在自己的社交圈里解决这些问题，而现在我们奔向医疗系统。昨天我在诊室接诊了两名因失恋而痛苦的患者，另一名和老板关系不和，还有一名因为与公司产生分歧而请病假……每天都是如此。显然，医疗系统没有能力消化这一波又一波涌向基层医疗、精神卫生中心、急诊和专科门诊的问题。

于是，候补名单越来越长，医生的工作量也趋于饱和，每天要接诊更多的预约患者和临时加号。但凡事都有极限。新冠疫情让我们看得很清楚：存在着一些红线，一旦被逾越，医生就会崩溃。

更何况，我们的医疗系统并不以关爱员工著称。我清楚地记得三年前疫情后的大规模动员，那是为了挽救几近崩溃的基层医疗；还有最近为了让医生在《框架法令》（Estatuto Marco）中获得话语权的努力，但该法令至今仍在纵容不公。

在中层管理中，我们看够了糟糕治理的案例。在马德里的基层医疗体系中，自从实行“单一区域制”以来，我们多年来一直缺乏有效的领导链条。以前我们有区域经理，有可以随时沟通的护理和医疗总监，现在没有了。他们消失在那些从不迈出的、难以进入的办公室里。我们偶尔能见到一两个下属（助理总监或中心负责人），但他们既没有行政权力，也没有解决日常问题的能力。所有事情都必须“上报”给相关部门，于是那些破坏日常工作的裂痕始终无法得到解决。

至于基层管理……我曾尝试担任多年社区卫生中心主任，直到因负担过重而辞职（在压力巨大的中心负责2400名患者，且没有管理豁免工时）。最近，我竟因为发送了“三封抄送给多人的电子邮件”来告知医师公会（Colegio de Médicos）候选人名单而受到停职处分——从结果来看，这一候选名单显然并不合卫生部门的心意。说到公会，那届即将离任的理事会（倒是深得马德里健康服务中心欢心）正在上演一出卑劣的黑箱操作和阻挠大戏。他们的管理导致公会破产，失去了代表大会的尊重（已有一年未召开），且拒不承认会员们的投票结果。

局势非常暗淡。正因如此，在被停职停薪的这五天里，我回到了最初的问题，试图以个人身份去理清它。直觉告诉我，现在比以往任何时候都更需要正直的医生，将他们的科学和艺术奉献给需要的人。但如果我们医生作为一个群体病倒了，谁在这漫漫长夜中带来光明？

我希望那些敢于提出这个问题的人，能够找到一个令人信服的答案，并且我希望这个答案是肯定的。如果最终法官判定我们可以进入公会，我希望能在那里为此而努力。

Me crucé esta semana con el rey. En el puente de Segovia con la calle Mayor. Mientras esperaba a cruzar la calle Bailén cortaron el tráfico y pasó la comitiva de seguridad con el coche blindado de la banderita en el que viajaba su majestad a una importante reunión con embajadores en el palacio real. La asimetría de la situación era evidente pero un servidor ganaba en una cosa: grados de libertad. Disfrutaba de un día libre que me permitía ir y hacer lo que me viniera en gana sin ataduras de agenda ni problemas de estado.

Ese ejercicio de libertad no es fácil de implementar. Libertad no es ir a tomar cervezas como postulan algunas, es algo más. Es un proceso de elección de opciones y sus correspondientes acciones derivadas. Confundir libertad con libertad de consumo es un engaño más.

Lo cierto es que no es fácil abrirnos a más grados de libertad. Solemos protegernos por rutinas que nos ahorran el trabajo de pensar por cuanto la repetición nos exime de ponderar opciones. La ponderación desgasta. Lo habitual, como dice Jung, es que en lugar de fatigarnos pensando terminemos juzgando.

Un pequeño experimento que me gusta hacer al salir a la calle sin rumbo definido es observar qué camino termino realizando. Con frecuencia terminamos repitiendo los mismos. Pero en esas rutinas se esconde la posibilidad de hacer pequeños cambios, tal vez ahí resida nuestra verdadera y pequeña capacidad de libertad. O quizá al hablar o al escribir pase lo mismo, nuestro fuero interno nos suele llevar por caminos trillados que, a veces, se abren a alguna posibilidad distinta.

Llevo un año sin escribir. Un año sabático. Un disgusto hizo que perdiera el placer de compartir ideas, pensamientos y palabras. Afortunadamente he seguido caminando en silencio y voy recuperando la posibilidad de contar, de volver a hablar. He aprendido que es revolucionario guardar silencio en un mundo de gritos y ruido de fondo. Pero también que cuando uno calla otros ocupan su lugar, y en las plazas digitales lo hacen robots.

Estamos abocados a un mundo cada vez más ruidoso, cada vez más manipulado por algoritmos, cada vez menos libre. Perdemos grados de libertad cuando caemos en las redes de la prisa, la multitarea, la extenuación, la manipulación… cuando consiguen que nos callemos y que agachemos la cabeza.

Cometo errores, soy falible y también caigo al suelo. Pero tengo claro que merece la pena trabajar en provecho del bien común y compartir ideas en un tiempo que impone las suyas. Seguiremos intentándolo.

A royal encounter

I ran into the King this week. At the junction of the Segovia bridge and Calle Mayor. While I was waiting to cross Calle Bailén, they stopped the traffic and the security convoy went past, with the armoured car bearing the little flag in which His Majesty was travelling to an important meeting with ambassadors at the Royal Palace. The asymmetry of the situation was obvious, but I had the edge in one respect: degrees of freedom. I was enjoying a day off that allowed me to go wherever I pleased and do whatever I fancied, with no constraints of schedule and no affairs of state to worry about.

That exercise of freedom is not easy to put into practice. Freedom is not going for a few beers, as some would have it; it is something more. It is a process of choosing between options and the corresponding actions that follow from them. Confusing freedom with freedom as a consumer is just another kind of deceit.

The truth is that it is not easy to open ourselves up to more degrees of freedom. We usually protect ourselves through routines that save us the work of thinking, because repetition frees us from having to weigh up options. Weighing things up is draining. As Jung says, what usually happens is that instead of tiring ourselves out by thinking, we end up judging.

A small experiment I like to do when I go out without a set destination is to observe which route I end up taking. We very often repeat the same ones. Yet within those routines lies the possibility of making small changes; perhaps our true, modest capacity for freedom resides there. Or perhaps, when we speak or write, the same thing happens: our inner world tends to lead us down well‑trodden paths which, every so often, open up to some different possibility.

I have not written for a year. A sabbatical year. An upset made me lose the pleasure of sharing ideas, thoughts and words. Fortunately, I have carried on walking in silence and I am gradually recovering the ability to tell, to speak again. I have learnt that keeping silent in a world of shouting and background noise is a revolutionary act. But I have also learnt that when one falls silent, others take one’s place, and in the digital squares those others are robots.

We are heading towards a world that is ever noisier, ever more manipulated by algorithms, ever less free. We lose degrees of freedom when we get caught in the nets of haste, multitasking, exhaustion, manipulation… when they manage to make us keep quiet and bow our heads.

I make mistakes, I am fallible and I too fall flat on my face. But I am certain it is worthwhile to work for the common good and to share ideas in a time that imposes its own. We shall keep trying.

一次王室邂逅

这周我偶遇了国王。
在塞哥维亚桥与马约尔大街的交叉口。
当我正等着穿过巴列恩街时，交通被拦下，一支安保车队驶过，里面有挂着小国旗的防弹车，陛下正坐在车里，前往王宫参加一场与大使们的重要会议。
这种情境下的不对称显而易见，不过有一点是我占了上风：自由度。
我正享受着一个休息日，可以随心所欲地去任何地方、做任何事情，没有日程安排的束缚，也没有国事烦扰。

这种自由的实践并不容易。
自由并不是像有些人说的那样只是去喝几杯啤酒，而是更深一层的东西。
它是一个在不同选项之间作出选择，并承担随之而来的行动的过程。
把自由混同于消费自由，只不过是另一种欺骗。

事实上，提升我们的自由度并不容易。
我们往往用日常惯例把自己保护起来，这些惯例帮我们省去了思考的力气，因为重复会让我们免于反复权衡选择。
权衡是很消耗精力的。
正如荣格所说，我们通常不会让自己因思考而疲惫不堪，而是干脆转向评判。

每当我没有预设目的地就出门时，我都喜欢做一个小实验：观察自己最后走的是哪条路。
我们往往会重复走同样的路线。
然而，正是在这些惯例中，隐藏着做出微小改变的可能，也许我们的那一点点真正自由的能力就蕴藏于此。
或者，在说话和写作时也是如此，我们的内心世界往往引导我们走上那些早已踏平的小路，而这些小路有时会通向某种新的可能。

我已经一年没有写作了。
一个休假年。
一场不愉快让我失去了分享想法、思绪和文字的乐趣。
所幸的是，我一直在默默前行，如今慢慢重新找回了讲述、重新开口说话的可能。
我明白了，在一个充斥着呐喊和背景噪音的世界里保持沉默本身就是一种革命性的举动。
但我也明白，当一个人沉默时，别人就会占据他的位置，而在数字广场上，那些“别人”是机器人。

我们正走向一个愈发喧嚣、愈发被算法操控、愈发缺乏自由的世界。
当我们落入匆忙、多任务处理、精疲力竭和被操纵的罗网之中，当他们让我们噤声、让我们低头时，我们就失去了自由度。

我会犯错，我并不完美，我也会跌倒。
但我很清楚，为了公共利益而努力、在一个把自己的观念强加于人的时代分享想法，是值得的。
我们会继续努力下去。

La bofetada. The Slap in the Face. 记耳光 (Yī jì ěrguāng)

Hace un año me presenté como vocal de Atención Primaria en la candidatura de Tomás Merina al Colegio de Médicos de Madrid. Me costó tomar la decisión sabiendo que tendría un coste alto para mí, y efectivamente, ayer me comunican que seré suspendido de empleo y sueldo cinco días por haberme atrevido.

Pese a ganar con holgura las elecciones seguimos sin entrar en el Colegio debido a las maniobras del presidente saliente y sus abogados que no dejaron más opción que terminar en un recurso contencioso administrativo que aún no está resuelto.

La Consejería de Sanidad dejó claro que nuestra candidatura no era de su agrado negándose a recibir al candidato ganador y a las pocas semanas abriéndome un expediente por haber mandado tres correos electrónicos (uno informando de nuestra candidatura y otros dos recordando la fecha de votación y los lugares de voto). Lógicamente los correos del resto de candidaturas no fueron castigados, como tampoco lo son los que a diario recibimos de sindicatos, organizaciones o demás.

Reconozco que inicialmente me dolió como una bofetada. No entendía que mis superiores no me hubieran informado, apercibido o llamado al orden, decidiendo abrir directamente la vía legal. De ellos no he recibido correo, llamada o noticia hasta la notificación de ayer de la gerente de Atención Primaria. Me queda claro su nivel como directivos, como compañeros de profesión y como personas.

En este momento mi sensación es otra. Conozco bien la sanidad madrileña, sé lo que hay. Mi caso no tiene ninguna importancia, tan solo ilustra lo que hay. Mis pacientes tendrán que esperar una semana a ser citados y un servidor se irá esos días a tomar viento.

The Slap in the Face

A year ago, I stood as a Primary Care representative in Tomás Merina’s candidacy for the Madrid Medical Association. It was a difficult decision to make, knowing it would come at a high personal cost; and indeed, yesterday I was informed that I am to be suspended from work and pay for five days for having dared to do so.

Despite winning the elections by a clear margin, we have still been unable to take office due to the manoeuvres of the outgoing president and his lawyers. They left us with no choice but to file an administrative appeal, which remains unresolved.

The Regional Ministry of Health made it clear that our candidacy was not to their liking, refusing to receive the winning candidate and, within a few weeks, opening a disciplinary case against me for having sent three emails (one introducing our candidacy and two others reminding people of the voting date and polling stations). Naturally, the emails from the other candidates were not penalised.

I admit that, initially, it stung like a slap in the face. I could not understand why my superiors had not informed me, cautioned me, or called me to order, deciding instead to proceed directly via legal channels. I have received no email, phone call, or word from them until yesterday’s notification from the Primary Care Manager. Their calibre as managers, as doctors, and as people is now perfectly clear to me.

At this moment, my feelings have changed. I know the Madrid healthcare system well; I know how it works. My case is of no real importance; it merely illustrates the current state of affairs. My patients will have to wait a week for their appointments, and yours truly will be heading off for a few days to get away from it all.

一年前，我作为初级保健（Atención Primaria）代表，参加了托马斯·梅里纳（Tomás Merina）竞选马德里医学院（Colegio de Médicos de Madrid）领导层的候选名单。当时做出这个决定并不容易，因为我知道自己会付出高昂的代价。果然，昨天我收到通知，因我“胆敢”参选，我将被停职停薪五天。

卫生局（La Consejería de Sanidad）明确表示我们的候选名单并不合其心意，他们不仅拒绝接见胜选候选人，还在几周后对我展开了纪律调查，理由是我发送了三封电子邮件（一封是介绍我们的候选资格，另外两封是提醒投票日期和地点）。理所当然地，其他候选名单发送的邮件并没有受到任何惩罚。

我承认，起初这让我感到像挨了一记耳光一样疼。我不明白为什么我的上级没有事先通知我、警告我或约谈我，而是直接选择了法律手段。从他们那里，我没有收到过任何邮件、电话或消息，直到昨天收到初级保健经理的正式通知。他们作为管理人员、作为医生以及作为个人的水平，现在对我来说已经一目了然。

Reconozco que me ha impactado. El último videoclip de Rosalía es una obra de arte. Y una enorme demostración de poderío y de libertad. Para una cantante pop que pone su dinero para autoproducirse es un enorme riesgo meterse en un jardín tan complicado. La carrera de esta artista ha sido fulgurante dando pasos hacia un estilo propio cada vez más suyo. Pasamos del anterior trabajo, MOTOMAMI (neologismo con el que expresa su naturaleza dual con una faceta Moto, dura/fuerte/veloz, y otra faceta Mami, frágil/vulnerable/dulce) a BERGHAIN que es otra dualidad: montaña/arboleda (y no la discoteca alemana a la que aluden sus fans). En esta canción hay dos partes nitidamente separadas, daré unas pinceladas hemenéuticas atendiendo principalmente al plano musical, al simbólico y al místico. A sus 33 años (la edad de Cristo) no me cabe duda de que su proyecto LUX será impresionante.

1ª Parte. BERG, montaña

Rosalía sube a la montaña, abre la puerta con una llave unida a un rosario (sus creencias). Entra en la habitación y pasa entre una escultura de la Virgen a la derecha (nueva Eva que repara el pecado original) y una manzana al a izquierda en la mesa (mordida por la primera Eva...). Abre las cortinas y al entrar la luz aparece la filarmónica de Londres cuyos violines empiezan con arpegios frenéticos, tocados con la técnica 'sautillé', que consiste en una ejecución rápida del arco en saltos cortos para generar una sonoridad nerviosa y vivaz. Esta textura genera una sensación de urgencia y dinámica en la línea musical. Aparecen los pies calzados con rosarios, el relicario en forma de corazón (su esencia divina) y el terrón de azúcar (su fragilidad) mojado en el café. La siguiente escena que se superpone con la voz del coro (Su miedo es mi miedo Su rabia es mi rabia Su amor es mi amor Su sangre es mi sangre) y la de Rosalía cantando en alemán con un sorprendente registro de soprano lírica (La llama penetra en mi cerebro Como un oso de peluche de plomo Guardo muchas cosas en mi corazón Por eso mi corazón es tan pesado), crea tensión y riqueza sonora. Descalza y vestida de blanco intenta poner orden en su mundo interior planchando (alisa lo arrugado), lavando ropa (limpia arrodillada lo sucio) y haciendo la cama (ordena el desorden). No lo consigue.

Sale de nuevo al exterior acompañada con la orquesta (todos los componentes de su vida interior) a pedir ayuda a la consulta del cardiólogo y al joyero. Empieza a cantar en español (ahora es ella la que habla), bajando una octava, entregando mayor energía y un tono más cálido e íntimo que se intensifica poco a poco. El acompañamiento incluye ostinatos de arpegios y violines tocando con glissando (deslizamientos), que aportan dramatismo y misterio a la atmósfera musical: Yo sé muy bien lo que soy, Ternura pa'l café, Solo soy un terrón de azúcar, Sé que me funde el calor, Sé desaparecer, Cuando tú vienes, es cuando me voy. En el autobús, el timbal en redoble rápido (taquicárdico) está a su lado. El final del coro se suma la voz de Björk mientras el electrocardiograma se vuelve plano indicando parada cardiaca y el joyero indica que el relicario no se puede arreglar. Ha hecho todo lo que ha podido pero no ha conseguido salvarse. La música se detiene.

2ª Parte. Hein, arboleda.

Rosalía vuelve a casa, pero esta vez la puerta está abierta, la llave y el rosario ya no son necesarios para entrar. Todo está oscuro y hace frio, estamos en la noche oscura del alma a la que se accede tras una catástrofe vital o un gran dolor, cuando nuestro corazón ha dejado de latir. El papel pintado simula una arboleda nocturna, la música es gélida y la voz de Björk transmite inquietud: The only way to save us is through divine intervention, The only (the only) way (way) I will be saved (saved) is through divine intervention.

Rosalía está vestida de Blancanieves (pureza, fragilidad), las criaturas del bosque interior se acercan a ayudarla y un petirrojo (criatura alada angelical) transmite un mensaje de esperanza.

Empieza la pesadilla con la voz de Yves Tumor repitiendo: I'll fuck you till you love me (una frase que Myke Thyson dijo en un enorme ataque de ira). Rosalía duerme inquieta en su cama mientras sus sueños la atormentan, la voz que oye tal vez sea una distorsión a través de la tormenta que le dice que sufrirá hasta que no ame lo divino. En esa angustia se despierta, abre los ojos y se gira del lado oscuro de la cama hacia la luz, se rinde. Lo acaba de entender, eso basta para dejar entrar la Luz, vuelve a la vida. Comienza un ritmo lento de timbal que remeda la vuelta a latir del corazón. Aparecen de nuevo la joya rota y el terrón de azúcar para terminar con la imagen de la joya reparada y Rosalía transmutada en paloma blanca y negra que sale volando de la cama mientras se escucha love me, love me, love me.

Conclusión

Esta obra en dos actos transmite un mensaje, para salvarnos necesitamos lo divino. Se ayuda del pensamiento de Simone Weil sobre la gravedad y la gracia, compartiendo el mismo mensaje, la gravedad nos hunde, la gracia nos eleva. Nuestras fuerzas no consiguen sacarnos del lecho de postración por mucho que hayamos intentado lavar las manchas, estirar las arrugas y colocar todo en su sitio. Tampoco sirven la ciencia y los expertos. No podemos salvarnos nosotros mismos de nosotros mismos.

El lanzamiento del disco será en pocos días. Se sabe que usará 13 idiomas y que explorará la dimensión de la mística femenina como ella misma ha dicho. Qué quieren que les diga, me resulta algo fresco y novedoso en el panorama musical y cultural que nos rodea. Lo que he visto y oído hasta el momento lo he disfrutado mucho, espero que tenga el eco que merece.

Brief Hermeneutic Analysis of Rosalía's BERGHAIN Music Video

I must admit it made an impact on me. Rosalía's latest music video is a work of art. A tremendous display of power and freedom. For a pop singer who self-funds her productions, it's a huge risk to venture into such a complex territory. This artist's career has been meteoric, moving steadily towards a style ever more her own. We go from her previous work, MOTOMAMI (a neologism expressing her dual nature with a "Moto" facet – tough/strong/fast – and a "Mami" facet – fragile/vulnerable/sweet), to BERGHAIN, which also presents a duality: mountain/grove (not the German nightclub alluded to by her fans). This song is clearly divided into two parts; I will offer some hermeneutical insights focusing mainly on the musical, symbolic and mystical planes. At 33 years old (the age of Christ), I have no doubt her LUX project will be impressive.

Part One: BERG, mountain

Rosalía arrives from the street into her home (inner world), unlocking the closed door with a key attached to a rosary (her beliefs). She enters the room passing between a sculpture of the Virgin (a new Eve who repairs original sin) and an apple on the table (bitten by the first Eve…). She opens the curtains and as the light enters, frenetic violin arpeggios begin, played with the ‘sautillé’ technique – a fast bow executed in short jumps to create a nervous and lively sound. This texture creates a sense of urgency and dynamism in the musical line.

We see feet shod with rosaries, a heart-shaped reliquary (her divine essence), and a sugar cube (her fragility) dipped in coffee. The subsequent scene overlaps with the choir's voice (“Your fear is my fear, Your anger my anger, Your love my love, Your blood my blood”) and Rosalía singing in German with a surprising lyrical soprano register (“The flame penetrates my brain like a lead teddy bear, I keep many things in my heart, that's why my heart is so heavy”), creating tension and sonic richness. Barefoot and dressed in white, she tries to restore order in her inner world by ironing (smoothing the wrinkled), washing clothes (cleaning the dirty), and making the bed (tidying disorder). She fails.

She leaves home again, accompanied by the orchestra (all elements of her inner life), going to ask for help at the cardiologist’s and the jeweller’s offices. She starts singing in Spanish, dropping an octave, delivering more energy and a warmer, more intimate tone that intensifies gradually. The accompaniment includes ostinato arpeggios and violins playing glissandi (sliding notes), adding drama and mystery to the musical atmosphere: “I know very well what I am, tenderness for the coffee, I am only a sugar cube, I know the heat melts me, I know how to disappear when you come, that’s when I leave.”

On the bus, the timpani’s rapid roll (tachycardic) is beside her. At the end of the chorus, Björk’s voice joins as the electrocardiogram flatlines signalling cardiac arrest, and the jeweller points out the reliquary cannot be fixed. The music stops.

Part Two: Hein, grove

Rosalía returns home, but this time the door is open, the key and rosary no longer needed to enter. All is dark and cold; we are in the dark night of the soul reached after a vital catastrophe or great pain. The wallpaper depicts a nocturnal grove, the music is icy, and Björk's voice conveys unease:
“The only way to save us is through divine intervention, The only (the only) way (way) I will be saved (saved) is through divine intervention.”
Rosalía is dressed as Snow White (purity, fragility), creatures of the inner forest approach to help her, and a robin (an angelic winged creature) brings a message of hope.

The nightmare begins with Yves Tumor’s voice repeating "I'll fuck you till you love me"(a phrase Myke Thyson uttered in a huge fit of rage). Rosalía restless in bed, tormented by her dreams. A slow timpani rhythm starts mimicking the heart's return to beating. The broken jewel and sugar cube reappear, concluding with the image of the repaired jewel and Rosalía transformed into a black and white dove flying from the bed as “love me, love me, love me” is heard.

Conclusion

This work in two acts conveys a message: to save ourselves, we need the divine. It draws on Simone Weil's thought about la gravedad y la gracia,(https://www.blogger.com/blog/post/edit/4060378544967004941/8289747196029543048#) sharing the same message, that gravity pulls us down, and grace lifts us up. Our strength cannot free us from our state of paralysis, no matter how much we try to wash away stains, straighten wrinkles and put everything in its place. Neither science nor experts can help. We cannot save ourselves from ourselves.

The album will be released in a few days. It is known she will use 13 languages and explore the dimension of feminine mysticism, as she herself has said. What can I say? It feels fresh and novel in the current musical and cultural landscape. What I have seen and heard so far I have enjoyed very much and hope it resonates as it deserves.

Recreación de Rosalía y Byung Chul Han unas horas antes de la explosión del Vesubio

Llevo unos meses reflexionando sobre la importancia de poner nombre a los valores para ayudarnos y ayudar a otros a orientarnos en la vida. Porque a las consultas nos llegan muchas personas perdidas y tal vez los que los recibimos también lo estemos. Este proceso empezó por invitación de mi querido amigo Francesc Borrell en el congreso de comunicación de este año, al lado de eminencias como el dr Broggi y doña Victoria Camps. Y continuó en el destilado de un par de artículos publicados este mes por la Fundación Letamendi y otro que saldrá en semanas en el Observatorio la Gestión Importa. Pero esto tiene poca importancia. Centremonos en lo que ha pasado en los últimos días.

Les compartiré dos vídeos, en el primero tienen el discurso de diez minutos de Byung Chul Han al recibir su premio Princesa de Asturias, en el segundo el último videoclip de Rosalía que es un alarde de creatividad y simbolismo. El filósofo viene a decirnos que como sociedad estamos completamente perdidos, "Estamos atrapados en un establo de la modernidad, donde la aceleración constante esclaviza nuestra existencia y erosionamos nuestra capacidad de estar presentes y ser libres."

La cantante (y su equipo), que hablan principalmente a la juventud, también nos dicen que estamos perdidos y necesitamos ayuda divina, "The only way I will be salved is through divine intervention".

Es lo que tienen las crisis, todo arde y salta por los aires. Y como saben los oportunistas, si no tienes claros tus valores, lo que para ti es más importante, te venderán los suyos. Por eso es inteligente preguntarnos ¿qué es lo más importante para mi? tanto a nivel personal como profesional. Sin tenerlo claro será imposible fijar un rumbo que nos haga salir del laberinto donde damos vueltas y vueltas como caballos en la noche.

Quizá haya que rescatar los aportes de la filosofía o la religión para encontrar el rumbo tal y como nos sugieren. Lo que seguro habrá que hacer es mirarnos por dentro y hacernos preguntas. Si queremos sanar nuestro corazón es necesario atrevernos a entrar en nuestra oscuridad y abrir cortinas.

Are we lost?

I have been reflecting for a few months on the importance of putting names to values to help ourselves and others find direction in life. Many lost people come to consultations, and perhaps those of us who receive them are also lost. This process began at the invitation of my dear friend Francesc Borrell at this year’s communication congress, alongside eminences like Dr Broggi and Mrs Victoria Camps. It continued in the distillation of a couple of articles published this month by the Letamendi Foundation and another that will appear in a few weeks at the Health Management Observatory. But this matters little. Let us focus on what has happened in recent days.

I will share two videos with you: the first is the ten-minute speech by Byung Chul Han on receiving his Princess of Asturias Award; the second is the three-minute latest videoclip by Rosalía, a display of creativity and symbolism. The philosopher tells us that as a society we are completely lost, “We are trapped in a stable of modernity, where constant acceleration enslaves our existence and erodes our capacity to be present and free.”

The singer (and her team), who mainly speak to the youth, also tells us that we are lost and in need of divine help: “The only way I will be salved is through divine intervention.”

That is what crises are like: everything burns and explodes. And as opportunists know, if you are not clear about your values, what matters most to you, they will sell you theirs. That is why it is wise to ask ourselves what is most important to me, both personally and professionally. Without clarity, it will be impossible to set a course that leads us out of the labyrinth where we go round and round like horses at night.

Perhaps we need to rescue the contributions of philosophy or religion to find the path forward as they suggest. What is certain is that we must look within ourselves and ask questions. If we want to heal our heart, we must dare to enter our darkness and draw back the curtains.

La sociedad está en shock que compartimos como ciudadanos. El impacto de televisar los conflictos de Gaza y Ucrania con coberturas mediáticas máximas ha conseguido meter la guerra en nuestros comedores y hacernos partícipes de ella o más bien convidados de piedra al no poder hacer nada más que opinar. Y nuestras opiniones valen poco como se ha visto a lo largo de estos meses por mucho que las tratemos de prodigar en redes sociales o conversaciones de salón. El activismo de sofá no mueve molino aunque muchos permanezcan en el engaño y se sumen a los ríos de hiel que nos rodean. Por mucho que queramos no podemos parar una guerra opinando. Hay que ir más allá. En primer lugar tomando conciencia de lo que está pasando. Darnos cuenta de en qué manera participamos con el agresor al compartir los intereses políticos y económicos de nuestro país o la Unión Europea que se alinean con el mismo. Dándonos cuenta de en qué manera participamos con la parte del bando agredido que mantiene la agresión y custodia los rescoldos que volverán a incendiarlo todo con el tiempo. Dándonos cuenta de que a fin de cuentas somos los mismos que están siendo agredidos por uno y otro bando, dado que las víctimas de todas las guerras son hermanas, y nosotros tenemos la nuestra bien reciente con los abuelos caídos en combate llenando los cementerios.

Hay sin embargo un efecto secundario bien conocido cuando se expone a alguien a una saturación mediática de un tema: la indiferencia. Por salud mental, desconectamos del horror como quien oye llover. Y esa desconexión nos deshumaniza, nos aleja de lo que verdaderamente somos. Pero ojo, también el que se pasa el día opinando a favor o en contra termina deshumanizado al no tener más remedio que terminar en una deriva que le lleva inevitablemente hacia la polarización y fanatización. La única forma de no deshumanizarse ante la catástrofe no es huyendo o agrediendo, es sosteniendo. Hace muchos siglos alguien se dio cuenta de esto allá en la India bajo el árbol del Bodhi ante el reto de sostener la mirada ante sus propios diablos. Esos que aparecen cuando tratamos de permanecer en nuestro centro el suficiente tiempo. Los mismos que se encontraron el Cristo en el desierto, Mahoma en la cueva y tantos santos en diferentes circunstancias. Por que nos sigue resultando más fácil tratar de escapar o unirnos a la turba enfurecida, que no es más que otra huida hacia delante. Permanecer es mucho más exigente. Esto ya lo sabía mi bisabuela Martín que pasaba sus días de senectud dedicando su tiempo a la oración y a hacer algún solitario con los naipes. Ella conocía bien a sus fantasmas y no se amedrentaba al sostenerles la mirada, fueran del tamaño que fueran. La bastaba tener fe, una fe pequeñita de que hay algo más grande que nosotros que lo sostiene todo.

En esta dinámica de contemplación que propongo surgirá pronto la pregunta ¿qué puedo hacer? Acompañada de un inevitable sentimiento de impotencia y frustración. A los que acompañarán otros más oscuros a lo largo del tiempo. Lo que es hacer podemos hacer poco. Toca ser y estar, verbos mucho más difíciles de conjugar pero más oportunos dado de lo que se trata es de no deshumanizarnos nosotros mismos. En esta línea compruebo que el dolor que siento ante las víctimas del conflicto es el que me mantiene despierto y abierto a la posibilidad de crear o encontrar caminos de salida, puentes de contacto, elementos sanadores… A lo mejor solo me sale un pequeño escrito, una sucinta reflexión, un breve poema, una conversación… Pero quién sabe, tal vez alguien consiga ver una solución o un nuevo camino que teniéndolo delante no veíamos.

Como médico me sorprendo de que ese dolor ante las víctimas es parecido al dolor que siento ante mis pacientes enfermos, ante los que se van muriendo, ante los que llegan a mi sumidos en diferentes catástrofes vitales. En un dolor que suena igual al que producen mis seres queridos cuando tienen problemas o el mio propio frente a los contratiempos de la vida. Compartir las fuentes de dolor nos hermana a todos los seres humanos y nos abre una oportunidad a humanizarnos si conseguimos entender que el dolor no un fin en sí mismo sino una oportunidad para buscar un sentido o un camino mejor.

No es fácil tomar partido por la humanización. Muchas voces nos exigirán que lo hagamos por uno de los bandos para seguir alimentando la batalla hasta que los otros claudiquen o se extingan. Nunca ha sido sencillo limitarse a sostener y no terminar huyendo o agrediendo. Pero la oportunidad la tenemos delante si somos capaces de entender lo que este y otros dilemas nos proponen.

Todos buscamos

El sentido a la vida

Que es ella misma

Si todo arde

Y nos rodean las llamas

También nosotros

The Challenge of Dehumanisation in the Face of War

Society is in a state of shock, which we, as citizens, all share. The impact of televising the conflicts in Gaza and Ukraine with maximum media coverage has managed to bring war into our living rooms and make us participants, or rather, passive observers, unable to do anything but offer our opinions. And our opinions are worth little, as has been seen over these months, no matter how much we try to express them on social media or in drawing-room conversations. Sofa activism doesn't turn a mill, even if many remain deluded and join the rivers of bitterness that surround us. As much as we may want to, we can't stop a war with our opinions. We have to go further. First, by becoming aware of what's happening. Realising how we participate with the aggressor by sharing our country's or the European Union's political and economic interests that align with them. Realising how we participate with the part of the attacked side that maintains the aggression and guards the embers that will set everything ablaze again in time. Realising that, at the end of the day, we are the same people who are being attacked by both sides, given that the victims of all wars are brothers and sisters, and we have our own very recent history with grandfathers who fell in combat filling the cemeteries.

There is, however, a well-known side effect when someone is exposed to a media saturation of a topic: indifference. For our mental health, we switch off from the horror as if hearing rain. And that disconnection dehumanises us; it distances us from what we truly are. But be careful, even the person who spends all day giving their opinion for or against ends up dehumanised, having no choice but to end up in a drift that inevitably leads to polarisation and fanaticism. The only way not to become dehumanised in the face of catastrophe is not by fleeing or by attacking, it is by enduring. Many centuries ago, someone realised this in India under the Bodhi tree when faced with the challenge of holding the gaze of their own devils. Those that appear when we try to remain centred for long enough. The same ones that Christ encountered in the desert, Muhammad in the cave and so many saints in different circumstances. Because it is still easier for us to try to escape or to join the enraged mob, which is nothing more than another flight forward. Remaining is much more demanding. My great-grandmother Martín, who spent her days in old age dedicating her time to prayer and playing solitaire with cards, already knew this well. She knew her ghosts well and wasn't afraid to hold their gaze, no matter what size they were. It was enough for her to have faith, a small faith that there is something bigger than us that sustains everything.

In this dynamic of contemplation that I propose, the question will soon arise: "What can I do?" Accompanied by an inevitable feeling of powerlessness and frustration. Which will be accompanied by others, darker ones, over time. As for what we can do, we can do little. It's time to be and to be present, verbs that are much harder to conjugate but more appropriate, since the point is not to dehumanise ourselves. Along these lines, I find that the pain I feel for the victims of the conflict is what keeps me awake and open to the possibility of creating or finding ways out, connecting bridges, healing elements... Maybe all I can produce is a small piece of writing, a succinct reflection, a short poem, a conversation... But who knows, maybe someone will manage to see a solution or a new path that we couldn't see even when it was right in front of us.

As a doctor, I'm surprised that this pain for the victims is similar to the pain I feel for my sick patients, for those who are dying, for those who come to me plunged into different life catastrophes. It's a pain that sounds just like the one caused by my loved ones when they have problems or my own when faced with life's setbacks. Sharing the sources of pain unites all human beings and gives us an opportunity to humanise ourselves if we manage to understand that pain isn't an end in itself but an opportunity to seek meaning or a better path.

It's not easy to take a stand for humanisation. Many voices will demand that we do so for one of the sides to continue feeding the battle until the others surrender or are extinguished. It has never been easy to simply endure and not end up fleeing or attacking. But we have the opportunity in front of us if we are capable of understanding what this and other dilemmas are proposing to us.

Inteligencia artificial en Atencion Primaria, ¿empezar por el tejado? Artificial intelligence in Primary Care – Should we start with the roof? 基层医疗中的人工智能——我们是不是应该从屋顶开始？

La reciente noticia de la Consejería de Salud de Madrid de invertir en un sistema de transcripción de la conversación entre profesionales y pacientes da pie para una reflexión. No entraré en juzgar el posible peso de esta herramienta que ya existe en el mercado y que he podido probar por gentileza de Frederic Llordans, CEO de Llamalítica. Tan solo diré que sin estar integrada en la historia clínica electrónica me servía de muy poco. Cuando lo esté probablemente aumente el texto libre y mejore quizá algún parte de interconsulta o algún informe, pero poco más. En AP no hacemos los informes detallados de alta de las consultas de hospital, de la urgencia o de la privada y la utilidad será mucho menor. Tendremos historias clínicas mejor redactadas y con más información, eso puede ser bueno o no según distintas variables (orden/desorden, diligencia del clínico, duplicación de información, aumento del ruido de fondo, pérdida de información relevante…) Como suele pasar lo sabremos con el tiempo según lo vayamos usando dado que nadie se va a molestar en validarlo ni estudiarlo previamente.

Tal vez sea más interesante empezar a hablar del suelo en lugar del tejado. Habría que poner el foco en las tareas que en este momento hacen los médicos y no deberían hacer antes de empezar a jugar con herramientas que digitalicen acciones que quizá sean de competencia administrativa o de otras categorías profesionales. Digitalizar lo inapropiado solo consigue mantener la ineficacia con una capa de pintura digital.

Lo siguiente sería mejorar los sistemas de historia clínica electrónica que en nuestro país son legión dado que cada Comunidad Autónoma tiene las suyas con distinto grado de interoperatividad dentro de su territorio y sin ninguna fuera de él. Estos sistemas siguen sin capturar la información que se introduce como texto libre para transformarla en datos que el programa pueda entender y normalizar (DGPs). De este modo si un profesional escribe una exploración o una tensión como texto libre, habrá de repetirlo en las casillas correspondientes para que el programa lo entienda y se rellenen los distintos protocolos diagnósticos y terapéuticos esenciales para el control de calidad y de gestión. Esto sí que consume horas y horas del valioso tiempo de los profesionales. Aquí tienen el filón para introducir la IA, pero dudo lo consigan dado que al ser incontables los sistemas se requeriría otra legión de informáticos y saldría muy caro. Si quisiéramos ser eficientes e inteligentes habría que unificar de una vez los sistemas para invertir en mejorar y mantener uno. Eso no lo veremos.

Seguiremos pues con una base de historia clínica electrónica primitiva a la que se irán poniendo parches (con fondos europeos). Y los clínicos seguiremos sufriendo con sistemas que muchas veces son una pesada cruz que obliga a mantener la mirada en la pantalla y las manos haciendo innumerables clics en lugar de en escuchar, atender y dedicar tiempo de calidad a los pacientes.

Y los pacientes seguirán teniendo que soportar médicos que apenas les miran aunque en breve podrán disponer de registros informáticos más completos que usarán sus palabras y les proveerán (quizá) de informes más completos.

Artificial intelligence in Primary Care – Should we start with the roof?

The recent announcement by the Madrid Health Authority to invest in a system that transcribes conversations between clinicians and patients invites some reflection. I won’t pass judgement on the potential impact of this tool, which is already available and which I’ve had the opportunity to test thanks to Frederic Llordans, CEO of Llamalítica. Suffice to say that, without integration into the electronic health record, it was of little use to me. Once it is integrated, it may well increase the amount of free‑text and perhaps improve elements such as interconsultation notes or certain reports, but little else. In primary care we do not prepare the detailed discharge reports from hospital visits, A&E or private consultations, so its utility will be much more limited. We will have better‑written records with more information, which may be positive or otherwise depending on various factors (order/disorder, clinician diligence, information duplication, increase in background noise, loss of relevant data…). As often happens, we’ll only know with time and practical use, since no one will take the trouble to validate or study it beforehand.

Perhaps it would be more sensible to begin at the foundation rather than the roof. We ought to focus on the tasks that doctors currently perform but shouldn’t have to, before deploying tools that digitise actions more appropriate to administrative staff or other professional categories. Digitising the inappropriate merely preserves inefficiency beneath a veneer of digital paint.

The next step should be to improve the electronic health record systems, which in our country are legion, with each Autonomous Community having its own system, varying degrees of interoperability within its territory, and none between regions. These systems still fail to capture free‑text entries and convert them into data that the software can understand and normalise (like DGPs). Thus, if a professional enters an examination finding or a blood pressure reading as free text, they must re‑enter it into the appropriate fields for the system to recognise it and populate essential diagnostic and therapeutic protocols needed for quality control and management. This indeed consumes hours and hours of precious professional time. Here lies the real opportunity for AI – but I doubt it will be seized, since the sheer number of disparate systems would require yet another army of IT professionals and prove extremely costly. If we truly wanted to be efficient and smart, we would unify these systems once and for all and invest in improving and maintaining one. But that will not happen

So we will continue with a primitive electronic health record base, patched over time (with European funding). And clinicians will continue to suffer under systems that often feel like a burdensome cross, forcing our eyes on screens and our hands to perform countless clicks instead of listening, caring and spending quality time with patients.

And patients will continue to endure doctors who barely look at them, even though soon they will have access to more comprehensive electronic records built from their own words, possibly generating more detailed reports.

下一步应该是改进电子病历系统。在我国，这类系统多如牛毛，各个自治区都有自己的系统，区域内部可互操作，却无法实现跨区互通。这些系统至今仍无法将自由文本录入的信息转换为软件可以理解和标准化的结构化数据（比如 DGP）。因此，如果专业人员以自由文本方式记录体格检查或血压数值，就必须再将其输入对应的字段，才能让系统识别并填充用于质量控制与管理的各种诊疗协议。的确，这种重复性录入消耗了医护人员大量宝贵时间。这里才是真正应用人工智能的巨大潜力所在——但我对此抱有怀疑，因为系统数量众多，需要巨额人力成本和资金。要实现高效与智慧，理应统一系统，共同投资一个可持续维护和改善的单一平台。但这是不太可能实现的愿景。于是，我们将继续使用一个原始的电子病历系统，并不断打补丁（可能靠欧洲资金）。临床医师将继续在繁重的系统中挣扎——这些系统常常像十字架一样沉重，让我们眼睛盯着屏幕，手指不停地点击，而不是倾听、照护和将时间倾注给患者。

而患者仍将忍受几乎不被医生关注的境况，尽管不久之后，他们将能获取由自己言语生成的更完整的电子记录，这些记录可能会生成更详实的报告。

Comunicar ciencia no es fácil. Los que nos dedicamos a la salud lo sabemos. Por eso me animé a participar en el libro Comunicando ciencia con ciencia para el capítulo La aventura de comunicar salud en redes sociales junto a Gema Revuelta y Heber Longas que lo ilustró con una excelente infografía que podrán ver más abajo.

El libro es el tercero de una serie de la fundación Lilly que ha conseguido crear un estilo de edición muy agradable a la lectura con gran cantidad de gráficos, imágenes y, en esta ocasión, infografías. Abunda en recursos bibliográficos con múltiples enlaces a páginas web y códigos QR para facilitar su acceso.

En el grupo de autores encontrarán expertos en comunicación trabajando en pareja con profesionales de perfil científico o, en mi caso, sanitario. Ha sido un privilegio colaborar y aportar desde mi perspectiva de médico rural con una pequeña faceta divulgadora en medios digitales.

A Book to Communicate Science Better

Communicating science isn't easy. Those of us in healthcare know this well. That's why I was keen to participate in the book Comunicando ciencia con ciencia (Communicating Science with Science) for the chapter titled 'The Adventure of Communicating Health on Social Media,' alongside Gema Revuelta and Heber Longas, who illustrated it with an excellent infographic.

This book is the third in a series from the Lilly Foundation, which has successfully created a very reader-friendly editing style with numerous charts, images, and, in this instance, infographics. It's rich in bibliographic resources, with multiple links to web resources and QR codes for easy access.

Among the authors, you'll find communication experts working in pairs with professionals from scientific backgrounds or, in my case, healthcare. It was a privilege to collaborate and contribute from my perspective as a rural doctor with a small public outreach facet in digital media.

Basta con leer un único periódico, escuchar un único programa de radio o ver un único telediario. Cualquier observador ser da cuenta de inmediato de que vivimos rodeados de un enorme ruido de fondo. Es un mal globalizado pero en nuestra piel de toro lo hemos refinado. Los chicos de la prensa y las herramientas digitales favorecen el griterío y la bronca hasta unas cotas cada vez más elevadas. Hay más visibilidad para los bocachanclas que para los sabios. Por eso Byung Chul Han me encanta por ir contra corriente: además de ser sabio es silencioso.

El salto cultural e idiomático fue demasiado fuerte y no podía leer a una suficiente velocidad para la misión que quería conseguir. Terminó haciendo filosofía porque había que pensar más y leer y escribir menos. Fue un acierto. La segunda cosa que le tengo que agradecerle tras su alteridad es la limpieza de ese pensamiento lento que se destila en lo que escribe. De hecho la forma en que escribimos es fiel reflejo de nuestra manera de pensar. Por eso sus libros son breves y concisos. Evitan la prodigalidad del filósofo tipo que suele gustar (y abusar) del devaneo, el circunloquio y las innumerables citas.

Otro ejemplo que me viene a la cabeza de pensador lento es su compatriota June Juh, un joven matemático que ha conseguido ganar una medalla Fields (el premio Nobel de las matemáticas) por sus ideas en geometría combinatoria. Un caso de mal estudiante en su infancia, y de friki que no sabía freírse un huevo en su juventud, que terminó consiguiendo una privilegiada visión numérica pensando despacito. Son biografías distintas pero coincidentes en esa forma de pensar que llega más lejos apagando pantallas y pensando sin prisas.

Byung escribe sobre la vida corriente y nos ha regalado conceptos como "sociedad del cansancio", "psicopolítica" autoexplotacion, infocracia, hipertransparencia... en un formato de libros breves que, si bien no es apto para todos los públicos, se abre a audiencias más amplias que las de sus homólogos con más fama como Žižek, Habermas, Taylor o Nussbaum.

Me alegra que le hayan concedido el premio Princesa de Asturias, una buena excusa para que salga de su casa berlinesa, y de sus dos pianos, y se dé un paseo por Oviedo. Yo le daría veinte premios más, reconozco un conflicto de intereses por todo lo que ha enriquecido mi propio pensamiento. Esa deuda siento que es generalizada en nuestro medio. A los que aportan claridad, brevedad, pedagogía y consciencia habría que ponerles en un pedestal y de paso quitar a los influencers, famosos y políticos mediocres que los ocupan. Si me permiten una pista, sean generosos con ustedes y atrévanse a leer cualquiera de sus pequeños libros.

Why Do We Spaniards Owe a Debt to Byung-Chul Han?

You only need to read a single newspaper, listen to a single radio programme, or watch a single news bulletin. Any observer quickly realises that we live surrounded by an overwhelming background noise. It's a global ailment, but here in Spain we've refined it to an art form. The press and digital tools feed the shouting and bickering to ever more unbearable levels. Loudmouths get more visibility than the wise. That’s why I’m so fond of Byung-Chul Han — he goes against the grain. He’s not only wise but silent.

We know he was born in Korea in 1959 and, after starting work in industry, he eventually emigrated to Germany intending to become a literature professor. He didn’t succeed. The cultural and linguistic leap was too great; he couldn’t read fast enough to keep up with the demands of that goal. He ended up studying philosophy instead — a field that, he realised, required more thinking and less reading and writing. It was a fortunate turn. The second reason I am grateful to him, after his otherness, is the clarity of that slow thought which permeates everything he writes. In fact, the way we write is a faithful reflection of how we think. That’s why his books are brief and concise. He avoids the usual extravagance of the typical philosopher, who tends to delight in — and overuse — rambling, circumlocution, and endless references.

Another example that comes to mind of a slow thinker is his compatriot June Juh, a young mathematician who won a Fields Medal (the Nobel Prize of mathematics) for his work in combinatorial geometry. A poor student in his early years, and an awkward youth who didn’t even know how to fry an egg, he eventually developed a privileged mathematical vision — by thinking slowly. These are different biographies, yet they share a way of thinking that reaches further by switching off screens and allowing ideas to mature without haste.

Byung writes about everyday life and has gifted us with concepts like the burnout society, psychopolitics, self-exploitation, infocracy, and hypertransparency — in short books which, while not always accessible to the general public, are more approachable than those of more famous contemporaries like Žižek, Habermas, Taylor or Nussbaum.

I was pleased to hear that he’s been awarded the Princess of Asturias Prize — a good excuse for him to leave his Berlin home, and his two pianos, and take a stroll through Oviedo. Personally, I’d give him twenty more awards — I admit a conflict of interest given how much his work has enriched my own thinking. I suspect this debt is shared by many in our cultural landscape. Those who bring clarity, brevity, pedagogy, and awareness should be put on a pedestal — and perhaps we could knock down the mediocre influencers, celebrities, and politicians who currently occupy them.

If I may offer a suggestion: be generous with yourself and dare to read any one of his little books.

为什么西班牙人欠卞之焕（Byung-Chul Han）一份情？

你只需读一份报纸、听一个广播节目，或看一档新闻节目。任何观察者都会立即意识到，我们生活在巨大的背景噪音中。这是一个全球性的问题，但在我们的“斗牛士之国”，我们却将它精炼得更为极致。媒体和数字工具助长了喧嚣和争吵，愈演愈烈。爱嚷嚷的人比智者更容易获得关注。这也是为什么我特别喜欢卞之焕——他逆流而行，不但智慧，还保持沉默。

我们知道他1959年出生于韩国，最初在工业界工作，后来移居德国，希望成为一名文学教授。但他未能如愿。文化和语言的鸿沟太大，他的阅读速度跟不上他想要达成的目标。于是他转向了哲学——因为在这个领域，需要的是更多思考、更少阅读与写作。这个选择十分正确。我感激他的第二点，是他那种“缓慢思维”的清澈逻辑，在他的文字中渗透着沉稳和条理。其实，一个人的写作风格正是其思维方式的真实写照。因此，他的书简短而精炼，避免了许多哲学家常有的冗长、绕弯子以及滥用引用的毛病。

我还想到另一位“慢思考者”的例子：他的同胞、年轻数学家June Juh。他因在组合几何方面的创见获得了菲尔兹奖（数学界的诺贝尔奖）。小时候他是个差学生，青年时甚至不会煎鸡蛋，最后却通过“慢思考”获得了卓越的数学洞见。他们的经历虽然不同，但都表明，关闭屏幕、放慢思考的节奏，能让人走得更远。

卞之焕的写作聚焦于日常生活，他提出了许多富有启发性的概念，如“疲惫社会”“心理政治”“自我剥削”“信息统治”“过度透明”等。他的书通常篇幅不长，虽然不一定适合所有大众，但比起更为知名的哲学家如齐泽克、哈贝马斯、泰勒或努斯鲍姆，更加平易近人。

我很高兴他获得了阿斯图里亚斯王妃奖，这也许能成为他离开柏林家中、离开他那两架钢琴、走一趟奥维耶多的理由。我愿意再给他二十个奖项——我承认我有“利益冲突”，因为他的思想极大地丰富了我自己的思维。在我看来，这种“亏欠感”在我们的文化环境中是普遍存在的。那些带来清晰、简洁、教育性与觉知的人，应该被捧上神坛，而那些蹭流量的网红、平庸的名人和政客，则应被请下来。

Soy de los que cuando se comentan películas o series en un grupo se queda en fuera de juego. Lo mismo me pasa con el fútbol y otras muchas cuestiones. En estas dos frases pinto una autocaricatura que no se aleja mucho de la realidad. Y pese a todo he conseguido llegar a médico consultor, lo que significa muchos años de ejercicio, sin enloquecer ni desaparecer. Probablemente las humanidades tengan mucho que ver en esto. La capacidad de quedarme mirando la luna o escribiendo esta nota en lugar de tirarme al sofá a ver una tele que en mi caso no tiene apenas uso.

Por mi consulta suelen rotar unos cuantos estudiantes de medicina y residentes al año. A todos les recuerdo el valor de la llave de las humanidades, una llave que la mayoría tiene olvidada en un cajón bajo kilos de apuntes y material de las academias MIR. Una llave que abre muchas puertas, algunas con el poder de sanar nuestras heridas o proporcionarnos sentido o belleza, incluso en la tormenta.

Según la Wikipedia: Las humanidades de la salud son un campo de estudio interdisciplinario que se basa en aspectos de las artes y las humanidades en su enfoque de la atención médica, la salud y el bienestar.^[1] Implica la aplicación de las artes creativas o bellas artes (incluidas las artes visuales, la música, las artes escénicas) y las humanidades (incluidos los estudios literarios, los idiomas, el derecho, la historia, la filosofía, la religión, etc.) a cuestiones de salud humana y bienestar.^[2]

En mi juventud me sirvió de alimento la revista JANO que desgraciadamente se dejó de publicar. En ella descubrí lecturas, autores, músicas y propuestas de diversa índole. Mucho después recalé en la revista FOLIA HUMANÍSTICA y el blog Humanidades Médicas de la fundación Letamendi Forns. La tenaz labor de mi amigo Francesc Borrell a lo largo de los años es uno de esos ejercicios de generosidad que habría que poner en un pedestal. Sus comentarios de libros e ideas regados con la salsa de su erudición y criterio son valiosísimos.

Hoy me levanté con la pregunta de si habría otra forma mejor de referirse al concepto de humanidades médicas y tras reflexionar elegí el término Artes y Humanidades para profesionales de la salud. Me salió algo largo pero es claro e inclusivo dados los diferentes perfiles profesionales que existen y el bien que estas realidades producen en todos ellos.

En tiempos como estos donde las ciencias de la salud están abducidas por la tecnología, en una espiral de aceleración como nunca habíamos visto en la historia, es cada vez más necesario recuperar un necesario contrapeso para que las cabezas de los que nos dedicamos a la salud no estallen.

Pero si quieren usar la llave de las Humanidades recuerden que es muy fácil, basta con mirar la luna o lo que haya al otro lado de la ventana. Basta con escribir un verso. Basta con emocionarse con alguna canción, o mejor aún bailarla o cantarla o tocarla con algún instrumento.

I am one of those people who, when films or TV series are being discussed in a group, is left completely out of the loop. The same happens with football and many other topics. In these two sentences, I sketch a self-caricature that’s not too far from reality. And yet, despite all that, I’ve managed to become a consultant physician — which implies many years in the profession — without going mad or vanishing. The humanities probably have a lot to do with it. The ability to stare at the moon or to write this note instead of collapsing on the sofa to watch a television that I hardly use.

Every year, a number of medical students and residents rotate through my clinic. I always remind them of the value of the key offered by the humanities — a key that most have left forgotten in a drawer, buried under piles of lecture notes and materials from MIR exam prep academies. It’s a key that opens many doors, some of them capable of healing our wounds or offering us meaning or beauty, even in the midst of a storm.

According to Wikipedia: Health humanities is an interdisciplinary field of study that draws on aspects of the arts and humanities in its approach to healthcare, health, and wellbeing. It involves the application of the creative or fine arts (including visual arts, music, performing arts) and the humanities (including literary studies, languages, law, history, philosophy, religion, etc.) to issues of human health and wellbeing.

In my youth, I was nourished by the magazine JANO, which sadly ceased publication. Through it, I discovered readings, authors, music, and proposals of many kinds. Much later, I came across the journal FOLIA HUMANÍSTICA and the Medical Humanities blog of the Letamendi Forns Foundation. The tireless work of my friend Francesc Borrell over the years is one of those acts of generosity that deserves a pedestal. His book reviews and reflections, enriched by his erudition and sound judgement, are truly invaluable.

This morning I woke up wondering whether there might be a better way to refer to the concept of medical humanities. After some thought, I settled on the term Arts and Humanities for Health Professionals. It’s a bit long, but clear and inclusive, considering the range of professional profiles out there — and the good these approaches do for all of them.

In times like these, where the health sciences have been abducted by technology, caught in a whirlwind of acceleration unlike anything we’ve seen in history, it is more necessary than ever to recover a vital counterweight — so that the minds of those of us working in healthcare don’t explode.

我是那种在群聊中讨论电影或电视剧时会插不上话的人。足球也是如此，还有很多其他话题。以上两句话可以说是我自画像，并不夸张。尽管如此，我还是成为了一名资深的医生——这意味着多年从医，却没有疯掉或消失。人文学科可能在这其中发挥了重要作用。它让我有能力静静地看着月亮，或是写下这段文字，而不是倒在沙发上看电视（我家电视几乎不用）。

每年都会有一些医学生和住院医生在我的诊室轮转。我总会提醒他们：不要忘记那把属于人文学科的钥匙。这把钥匙常被许多人遗忘在抽屉底下，压在厚厚的讲义和考试资料之下。但它能打开很多扇门，其中一些能疗愈我们的伤口，为我们带来意义和美，哪怕是在风暴中。

根据维基百科：健康人文学科是一个跨学科研究领域，它结合了艺术与人文学科的内容，用以探讨医疗保健、健康与福祉等问题。它涉及对创造性艺术或美术（包括视觉艺术、音乐、表演艺术）及人文学科（包括文学、语言、法律、历史、哲学、宗教等）的应用，以解决人类健康与福祉的问题。

年轻时，我经常阅读《JANO》杂志，那是我的精神食粮，可惜后来停刊了。通过这本杂志，我认识了许多书籍、作家、音乐和各种思想。很多年后，我又发现了《FOLIA HUMANÍSTICA》期刊和Letamendi Forns基金会的医学人文博客。我朋友Francesc Borrell多年来所做的坚持不懈的工作，是一种应该被铭记的慷慨行为。他的书评和思想评论，结合他丰富的学识和独到的见解，极具价值。

今天早上，我思考是否有更好的方式来称呼“医学人文学科”这个概念。最后我选择了“面向健康专业人员的艺术与人文学科”这一说法。虽然有点长，但它明确而包容，能涵盖各种专业背景，并且这些内容确实对所有从业者都大有益处。

如今，健康科学似乎被技术“绑架”，被卷入了史无前例的加速旋涡中，因此我们更需要一个有力的平衡点——以免我们这些致力于健康事业的人“头脑爆炸”。

Obituario del doctor Fernando Casado

Desde tercero de carrera hemos navegado juntos. Muchos años de travesía en la que ha habido de todo. Tuve la suerte de conocer a sus padres que, de alguna manera, codicionaron que terminara viviendo en la sierra madrileña como ellos y otros muchos actores. Tuve la suerte de compartir con él muchísima vida personal y profesional y de caminar cerca en las subidas y bajadas que fueron aconteciendo a lo largo de los años.

Como médico de familia tuvo un comienzo muy duro con años de guardias y refuerzos en primaria para luego pasar por consultas muy sobrecargadas que terminaron minando su salud. Recuerdo especialmente la del centro de salud de Alameda, un sótano sin ventanas, que su gran sensibilidad no pudo resistir. Con los años pudo ejercer la medicina de familia en Mallorca, lugar que conocía desde niño y en la que había pasado temporadas en su juventud. Fue crítico con los gestores del sistema sanitario por padecer de primera mano la deshumanización de el sistema con sus médicos.

Su muerte me afecta especialmente al haber sido un medio hermano para mi. Compartir apellido nos hizo bromear sobre esto desde siempre, incluso los amigos con guasa nos llamaban los dos Casados cuando empezamos a movernos en entornos digitales en los que fue un pionero con una visión clara de tender puentes entre profesionales y pacientes. Hizo muchas cosas, pero lo más importante para mi es cómo las hizo. La presencia y humanidad que irradiaba y su carisma cercano y sensible que acogía a todos.

Para mi ha sido inesperado, nuestra última conversación fue hace unas semanas y no me verbalizó ningún problema. Comparto aquí mi dolor y su recuerdo.

Celebrando el día de la Atención Primaria expedientado. Celebrating Primary Care Day under Disciplinary Action. 庆祝初级医疗日却面临纪律处分

Durante el año 2024 se fraguó una candidatura de junta directiva para las elecciones del Colegio de Médicos de Madrid. Me ofrecieron la oportunidad de ser vocal de Atención Primaria y me costó tomar la decisión dado que al no ser nuevo en la profesión conozco bien tanto esa institución como el Servicio Madrileño de Salud.

Pese a que el Sermas nos prohibió poner cartelería, hacer reuniones en los centros de salud y hacer cualquier tipo de difusión ganamos las elecciones con amplia mayoría. Hace un mes me abrieron expediente disciplinario por mandar dos correos electrónicos dando información sobre el día y las mesas de votación y otro por informar de mi candidatura. Presunta falta grave.

Tengo el dudoso privilegio de ser el primer miembro de una junta directiva electa de la historia del Colegio de Médicos de Madrid a quien la administración sanciona por algo parecido, lo que deja claro la posición de esta favoreciendo a una candidatura a la que premia y halaga y atacando a otra que no es quizá de su gusto. Por supuesto no hay sanciones a las otras dos candidaturas en liza que sin duda usaron también el email para lo mismo como se ha hecho toda la vida.

En el día de hoy no puedo negar cierta tristeza. Como bien explica el filósofo Byung Chul Han sufrimos una gran crisis de la narrativa. Estamos perdiendo la capacidad de dialogar y cada vez más asuntos terminan dirimiéndose en el juzgado. La transición en el Colegio de Médicos se habría podido arreglar tomando un café y no en un contencioso administrativo, las dudas de la Dirección Asistencial Norte de Atención Primaria de Madrid con mis correos con una llamada y no en los juzgados y el bloqueo de la reforma estatuto marco escuchando a los médicos y no con estos protestando en las calles.

Desgraciadamente frente a la estupidez los propios dioses luchan en vano. La Atención Primaria que conocíamos desaparece como un charco tras salir el sol y los responsables actuales y pasados miran hacia otro lado sabiendo que no tendrán que dar la cara cuando ya no exista, pese a que muchos seamos testigos de su culpa y su vergüenza. Los números son cada vez más duros y las propuestas son cada vez más ignoradas. Y la ciudadanía sigue siendo incapaz de cuestionar a los políticos que vota, limitar sus desmanes u obligarles a cumplir sus obligaciones de gestión de lo público.

Si me lee algún médico de la sanidad pública de Madrid sepan que esos gestores que les piden continuamente que participen en comisiones, grupos de trabajo, docencia y demás funciones probablemente no se lo agradezcan nunca pero que les tendrán bajo su lupa y les castigarán si sospechan que sus opiniones no coinciden con las suyas. Sean prudentes en lo que dicen y escriben.

Siento no poder ser optimista, tampoco evitar plantearme abandonar el barco.

Celebrating Primary Care Day under Disciplinary Action

In 2024, a candidacy for the board of directors was formed for the elections of the Madrid College of Physicians. I was offered the opportunity to be the representative for Primary Care, and it was a tough decision to make, as, being no stranger to the profession, I am well acquainted with both that institution and the Madrid Health Service (Sermas).

Despite Sermas banning us from putting up posters, holding meetings in health centres, or engaging in any kind of campaign promotion, we won the elections with a substantial majority. A month ago, I was subjected to disciplinary proceedings for sending two emails—one providing information about the election day and polling stations, and another announcing my candidacy. This was deemed a presumed serious offence.

I have the dubious privilege of being the first elected board member in the history of the Madrid College of Physicians to be sanctioned by the administration for something like this, which clearly reveals their bias in favouring one candidacy—praising and rewarding it—while attacking another that may not be to their liking. Naturally, there are no sanctions against the other two competing candidacies, which undoubtedly also used email for the same purpose, as has always been done.

Today, I cannot deny a certain sadness. As the philosopher Byung-Chul Han aptly explains, we are suffering a profound crisis of narrative. We are losing the ability to engage in dialogue, and increasingly, matters end up being settled in court. The transition at the College of Physicians could have been resolved over a coffee, not through administrative litigation; the concerns of the North Primary Care Assistance Directorate in Madrid about my emails could have been addressed with a phone call, not in the courts; and the blockage of the statutory framework reform could have been tackled by listening to doctors, not with them protesting in the streets.

Regrettably, as the saying goes, even the gods struggle in vain against stupidity. The Primary Care we once knew is vanishing like a puddle after the sun comes out, and current and former leaders look the other way, knowing they won’t have to face the consequences when it’s gone, even though many of us bear witness to their guilt and shame. The numbers grow ever harsher, and the proposals are increasingly ignored. And the public remains unable to question the politicians they vote for, curb their excesses, or hold them accountable for their obligations in managing public services.

If any public health doctor in Madrid is reading this, know that those managers who constantly ask you to participate in committees, working groups, teaching, and other duties will likely never thank you. Instead, they’ll keep you under scrutiny and punish you if they suspect your opinions differ from theirs. Be cautious about what you say and write.

I’m sorry I can’t be optimistic, nor can I avoid contemplating abandoning ship.

庆祝初级医疗日却面临纪律处分

2024年，马德里医师协会的选举中成立了一个董事会候选团队。我被邀请担任初级医疗的代表，这个决定并不容易，因为我对这个行业并不陌生，深知该机构和马德里健康服务系统（Sermas）的运作。

尽管Sermas禁止我们在健康中心张贴海报、举行会议或进行任何形式的宣传，我们仍以压倒性多数赢得了选举。一个月前，我因为发送了两封电子邮件而被提起纪律处分程序——一封提供了选举日和投票站的信息，另一封宣布了我的候选资格。这被视为涉嫌严重违纪。

我成为了马德里医师协会历史上首位因类似事件被行政部门处罚的当选董事会成员，这无疑暴露了他们的偏见：他们支持并赞扬一个候选团队，而攻击另一个可能不合他们心意的团队。当然，另外两个竞争的候选团队显然也使用了电子邮件做同样的事情——这是历来的惯例——却没有任何处罚。

今天，我无法否认内心的某种悲伤。正如哲学家韩炳哲（Byung-Chul Han）所阐述，我们正面临叙事的深刻危机。我们正在失去对话的能力，越来越多的事情最终在法庭上解决。医师协会的过渡本可以通过一杯咖啡解决，而不是通过行政诉讼；马德里北部初级医疗协助部门对我邮件的疑问本可以通过一通电话解决，而不是诉诸法庭；法定框架改革的阻碍本可以通过倾听医生的意见解决，而不是让他们走上街头抗议。

遗憾的是，正如俗语所说，面对愚蠢，连神明也无能为力。我们所熟知的初级医疗正在像太阳出来后的水坑一样消失，现任和前任负责人却视而不见，他们知道当它彻底消失时，他们无需面对后果，尽管我们许多人见证了他们的罪责与羞耻。数据越来越严峻，建议却越来越被忽视。公众依然无法质疑他们所投票选的政治家，遏制他们的过度行为，或迫使他们履行管理公共服务的职责。

如果有马德里公共卫生系统的医生读到这篇文章，请注意，那些不断要求你们参与委员会、工作小组、教学等职责的管理者，可能永远不会感激你们。相反，他们会密切监视你们，如果他们怀疑你们的意见与他们不一致，就会惩罚你们。请谨慎对待你们的言行。

Estos son mis valores, pero si no le gustan tengo otros…

Este año el congreso del grupo Comunicación y Salud de Semfyc ha sido especial. De alguna forma siempre lo son, es uno de los superpoderes de este congreso de formato alternativo y sin publicidad industrial, auspiciado por uno de los grupos más creativos y abiertos a lo humano del mundo de la medicina. Llevo años acudiendo y participando, habitualmente con un taller original que junto a la Dra Isabel Núñez preparamos con mimo y ofrecemos en exclusiva sin que haya réplicas posteriores. Esta forma de trabajar espolea a su vez nuestra creatividad y nos abre a temas y posibilidades nuevas cada año.

En esta ocasión no preparamos taller. No pudimos. Francesc Borrell me abrió la posibilidad de participar en una mesa sobre valores junto a, nada más y nada menos, Marc Antoni Broggi, presidente de la fundación Letamendi, y Victoria Camps, probablemente la filósofa española contemporánea más importante.

Ante la exigencia de esta invitación no tuve más remedio que replegar velas y centrarme en exclusiva al comprender al instante la importancia del asunto. Meter una mesa de valores en un congreso médico es parecido a colocar la traca final de las Fallas en el Senado. Porque dentro de la complejidad que afrontamos a diario en los centros de salud casi todos sabemos de la crisis institucional, profesional y social que padecemos pero pocos imaginamos el calado de la crisis de valores que fluye por debajo y pocos ven. Superpónganlo si quieren a a cualquier ámbito social. ¿Y esto por qué? Apuntó tres razones de la muchas que hay:

1. Ceguera axiológica. Nos cuesta ver y nombrar los valores. Ya nos resulta difícil con las emociones. Los valores son todavía más etéreos.

2. Burbujas éticas y morales. La sociedad tiende a colocarnos dentro de burbujas que compartimos con personas parecidas. En el mundo virtual somos etiquetados según gustos, creencias, valores y actitudes. Posteriormente nos agrupan para recibir información y mensajes que refuercen nuestra condición y nos opongan a grupos de rivales.

3. Crisis narrativa. La mezcla de verdad y mentira, los mensajes reforzadores de identidad que recibimos, la polarización del discurso que impide el diálogo y la confrontación tranquila, la aceleración social que dificulta la reflexión, la pérdida de palabras y la simplificación del discurso y el pensamiento… hacen que el mundo de los valores sea cada vez más ajeno.

Por todo esto no queda más remedio que tomarnos el tema en serio y repasar nuestro valores personales y la jerarquía de los mismos. Para ello no hay mejor pregunta que autoformularse que: ¿qué es lo más importante para mi? Y tratar de desvelar aquello que valoro más y en consecuencia tiene capacidad de orientarme en la vida. Si no somos capaces de responder otros lo harán por nosotros. Así ha sido siempre. Tras responderla pueden desdoblar la pregunta y aplicarla también al ámbito estrictamente profesional. Verán que interesante.

La fundación Letamendi que impulsó la iniciativa es una rara avis del mundo de la salud por sus propuestas de gran calidad que animan las humanidades y favorecen que los que nos dedicamos a la salud recordemos que en nuestra labor hay más aspectos de interés además de la ciencia. Tanto su revista como el blog de propuestas están en abierto, accesibles de forma gratuita por quien desee beber de sus inmensas fuentes. Llevo años recomendándola y lo seguiré haciendo.

Tras las dos horas de la mesa, a la que acudieron unas 90 personas, siguió una jornada trepidante con múltiples talleres y una presentación nocturna del congreso con doblete del propio Francesc Borrell que nos regaló una conferencia delicatesen sobre arte con colofón musical de copla catalana de sorpresa. Al día siguiente otra buena traca de talleres que culminó con la tradicional cena de clausura. No comentaré mucho más del congreso que es la segunda propuesta de Semfyc a nivel nacional, con más de 400 inscritos. Un esfuerzo con la misión de transmitir una llama de luz y de sentido de profesional a profesional. Bien por Josep María Bosch y el equipo organizador.

Communication and Values in Healthcare

This year, the congress of the Communication and Health group of Semfyc has been special. In some way, they always are—it’s one of the superpowers of this alternative-format congress, free from industrial advertising, and supported by one of the most creative and human-focused groups in the world of medicine. I’ve been attending and participating for years, usually with an original workshop that Dr Isabel Núñez and I carefully prepare and offer exclusively, without any subsequent repeats. This way of working, in turn, spurs our creativity and opens us up to new topics and possibilities each year.

This time, we didn’t prepare a workshop. We couldn’t. Francesc Borrell offered me the chance to take part in a panel discussion on values alongside none other than Marc Antoni Broggi, president of the Letamendi Foundation, and Victoria Camps, arguably the most significant contemporary Spanish philosopher. Faced with the weight of this invitation, I had no choice but to batten down the hatches and focus entirely on it, immediately grasping the importance of the matter. Introducing a discussion on values into a medical congress is akin to setting off the grand finale of the Fallas fireworks in the Senate. Because amid the daily complexity we face in health centres, almost all of us are aware of the institutional, professional, and social crises we endure, yet few imagine the depth of the crisis of values flowing beneath, unnoticed by most. Overlay this onto any social sphere if you like. And why is this? I pointed out three of the many reasons:

1. *Axiological blindness.* We struggle to see and name values. Emotions are already hard enough; values are even more ethereal.

2. *Ethical and axiological bubbles.* Society tends to place us in bubbles alongside like-minded people. In the virtual world, we’re labelled according to tastes, beliefs, values, and attitudes, then grouped to receive information and messages that reinforce our condition and pit us against rival groups.

3. *Narrative crisis.* The blend of truth and lies, the identity-reinforcing messages we receive, the polarisation of discourse that prevents dialogue and calm confrontation, the social acceleration that hinders reflection, the loss of words, and the simplification of discourse and thought—all make the world of values increasingly alien.

For all these reasons, we have no choice but to take the topic seriously and review our personal values and their hierarchy. There’s no better question to ask ourselves than: *What matters most to me?* And then try to uncover what I value most, which, in turn, has the power to guide me in life. If we can’t answer this, others will do it for us. That’s how it’s always been. After answering, you can extend the question and apply it to the strictly professional sphere too. You’ll see how interesting it gets.

The Letamendi Foundation, which spearheaded the initiative, is a rare gem in the healthcare world due to its high-quality proposals that enliven the humanities and remind those of us in healthcare that our work involves more than just science. Both its journal and its blog of ideas are freely accessible, open to anyone who wishes to drink from their vast wells of wisdom. I’ve been recommending it for years and will continue to do so.

After the two-hour panel, attended by nearly 90 people, the day continued with an exhilarating lineup of workshops and a night-time congress presentation featuring a double act from Francesc Borrell himself, who treated us to a delicate lecture on art, capped off with a surprise musical finale of Catalan *copla*. I won’t say much more about the congress, which is Semfyc’s second national event, with over 400 attendees. It’s an effort with a mission to pass a flame of light and meaning from professional to professional. Well done to Josep María Bosch and the organising team.

Compartiendo valores en el Equipo de Atención Primaria. Sharing Values in the Primary Care Team. 初级医疗团队共享价值观

La presentación “Compartiendo valores en el Equipo de Atención Primaria (EAP)” analiza la influencia de los valores en la práctica profesional y la convivencia en equipos de salud. Forma parte de la mesa de humanismo médico del XXXIV Congreso Comunicación y Salud de Semfyc y aborda la tensión entre el apostolado (defensa idealista de valores) y la complicidad (adaptación pragmática al entorno), así como entre sacrificio y supervivencia en la labor médica.

Se cuestiona qué significa ser un buen médico y cómo los sesgos cognitivos (efecto Dunning-Kruger, sesgo de autoservicio y de confirmación), la presión social, la falta de autocrítica y la desconexión entre valores y acciones pueden distorsionar la percepción del propio desempeño. La cultura del éxito y la dificultad de recibir críticas honestas refuerzan una autoimagen idealizada, limitando el aprendizaje y la mejora profesional.

La discusión sobre valores en el equipo sanitario es compleja debido a su subjetividad, el temor al conflicto y la posible hipocresía percibida. Se proponen estrategias como el enfoque dialógico, la autocrítica y el humor para facilitar estos debates sin caer en discursos moralistas. Se resalta la importancia de valores como equidad, transparencia, humanización y trabajo en equipo para una atención primaria efectiva.

También se analiza el impacto de la bioética y el principialismo en la toma de decisiones éticas, resaltando la importancia de la visión bioética en el trato con personas enfermas, sus familias y su comunidad. Finalmente, se plantea si pacientes y comunidad deben compartir nuestros valores, destacando la necesidad de sensibilidad cultural, participación y adaptación a su realidad.

El cierre reflexiona sobre la relación entre valores y sacrificio, explorando dilemas éticos y situaciones reales donde la coherencia con los principios profesionales exige renuncias y compromisos personales.

Sharing Values in the Primary Care Team

The presentation “Sharing Values in the Primary Care Team” analyses the influence of values on professional practice and teamwork in healthcare settings. It is part of the Medical Humanism panel at the Communication and Health Congress organised by Semfyc and explores the tension between apostolate (the idealistic defence of values) and complicity (the pragmatic adaptation to circumstances), as well as between sacrifice and survival in medical practice.

It questions what it means to be a good doctor and how cognitive biases (such as the Dunning-Kruger effect, self-serving bias, and confirmation bias), social pressure, lack of self-criticism, and the disconnection between values and actions can distort the perception of one’s own performance. The culture of success and the difficulty in receiving honest feedback reinforce an idealised self-image, limiting learning and professional growth.

Discussing values within healthcare teams is complex due to subjectivity, fear of conflict, and potential perceived hypocrisy. Strategies such as a dialogical approach, self-criticism, and humour are proposed to facilitate these discussions without resorting to moralistic discourse. The importance of values such as equity, transparency, humanisation, and teamwork is highlighted as fundamental to effective primary care.

The impact of bioethics and principlism on ethical decision-making is also examined, emphasising the significance of a bioethical perspective in interactions with patients, their families, and the wider community. Finally, the question arises of whether patients and communities should share the same values as healthcare professionals, underlining the need for cultural sensitivity, participation, and adaptation to their realities.

The presentation concludes with a reflection on the relationship between values and sacrifice, exploring ethical dilemmas and real-life situations where consistency with professional principles demands personal sacrifices and commitments.

¿Hemos aprendido algo tras la pandemia? Have we learned anything after the pandemic? 疫情过后我们学到了什么吗？

Los Seminarios de Innovación en Atención Primaria (SIAP) son uno de esos casos de éxito del mundo de la salud que no aparecerá en las cabeceras de los principales medios de comunicación. Una jornada científica independiente, libre, multidisciplinar, abierta y de amplias miras. Un evento sin patrocinadores industriales. Un foro que trata de formular preguntas y de cuestionar el pensamiento único que también amenaza a la medicina y las ciencias biosanitarias.

Este año se celebraban los 20 años de seminarios con su edición en Torremocha del Jarama con un monográfico sobre la pandemia Covid. El mismo Josep Laporte reconocía que ha sido uno de los escasos foros de reflexión tras la pandemia a nivel nacional. Seguimos sin una evaluación técnica ni una evaluación oficial. Seguimos sin hacer una valoración profesional y sin poder afirmar con claridad dónde estuvieron los errores o se pudo hacer mejor. Tampoco nadie ha pedido perdón ni ha asumido ninguna responsabilidad. Ni los políticos que tomaron las medidas y gestionaron el dinero, ni los epidemiólogos que las propusieron, ni los profesionales que las implementamos, ni la sociedad que las acató.

Comparto la pregunta: ¿Qué se pudo hacer mejor?

Y ofrezco diez puntos iniciales.

1. Dotar a los profesionales de medios de protección. Estuvimos meses cubiertos con bolsas de basura y sin mascarillas.

2. Haber proporcionado evidencia científica y datos fiables para respaldar las medidas tomadas. Estuvimos casi toda la pandemia a ciegas.

3. No usar fármacos y tratamientos de forma creativa (cloroquina, azitromicina, corticoides...).

4. Calibrar los confinamientos. Permitir en todo caso la deambulación en solitario o en entornos naturales.

5. Proteger especialmente los derechos de los niños y las niñas.

6. Proteger a los ancianos, especialmente a los institucionalizados.

7. Velar para que la Atención Primaria sea la base del sistema sanitario impidiendo que colapse por razones burocráticas (partes de baja), falta de personal y mala gestión. Colapsó en cada ola

8. Si hay que sobredimensionar la atención telefónica habría que priorizar la atención a los más frágiles y vulnerable y no dedicarla fundamentalmente a burocracia (bajas y demás).

9. Hacer una evaluación general sistemática y reglada para cuantificar aciertos y errores y hacer propuestas de mejora para próximas pandemias.

10. Mejorar la inversión y la organización de la Atención Primaria y el resto del sistema a tenor de los resultados de la evaluación general.

No podemos cerrar una convulsión política, sanitaria y social sin analizar éxitos y fracasos, sin reparar daños. La pandemia covid19 convulsionó a la sociedad y llevó a la toma de decisiones de dudosa ciencia y ética y a imposiciones de dudosa legalidad. Han sido escasos los ámbitos públicos en que se hayan analizado los aciertos y errores, más allá de los extremos laudatorios o culpabilizadores. Los Seminarios de Innovación en Atención Primaria (SIAP) establecieron uno virtual, siapcovid19 (“Del Estado de Alarma al Estado de Solidaridad”) https://covid19siap.wordpress. desde marzo de 2020, con más de 300 profesionales cuya página-bitácora queda como testimonio de que se podrían haber hecho las cosas de manera científica y humana y de que las críticas y sugerencias no son “a posteriori”. Hubo alternativas que se ignoraron, de ciencia, ética, solidaridad y ternura.
Cinco años después hemos tenido un SIAP virtual y presencial (en Torremocha de Jarama, Madrid), en febrero y marzo de 2025, para analizar tanto el daño de la pandemia como los aciertos y el daño de las reacciones políticas a lo largo de la pandemia (https://seminariossiap.es/siapcovid2025/) En esta sesión virtual se resume dicho #siapTorremocha25.

Have we learned anything after the pandemic?

The Primary Care Innovation Seminars (SIAP) are one of those success stories in the health world that won't make the headlines of major news outlets. An independent, free, multidisciplinary, open, and far-sighted scientific conference. An event without industrial sponsors. A forum that seeks to ask questions and challenge the monolithic thinking that also threatens medicine and biomedical sciences.

This year marked the 20th anniversary of the seminars with its edition in Torremocha del Jarama, featuring a monograph on the Covid pandemic. Josep Laporte himself acknowledged that it has been one of the few forums for reflection after the pandemic at the national level. We still lack a technical evaluation or an official assessment. We still haven't conducted a professional appraisal and cannot clearly state where the mistakes were or where things could have been done better. No one has apologised or taken any responsibility either. Not the politicians who made the decisions and managed the money, nor the epidemiologists who proposed them, nor the professionals who implemented them, nor the society that complied.

I share the question: What could have been done better?

And I offer ten initial points.

Provide professionals with protective equipment. We spent months covered in bin bags and without masks.
Provide scientific evidence and reliable data to support the measures taken. We were in the dark for almost the entire pandemic.
Avoid using drugs and treatments creatively (chloroquine, azithromycin, corticosteroids...).
Calibrate lockdowns. Allow solitary walks or walks in natural environments in all cases.
Protect the rights of children especially.
Protect the elderly, especially those in institutions.
Ensure that Primary Care is the foundation of the healthcare system, preventing it from collapsing due to bureaucratic reasons (sick leave forms), staff shortages, and mismanagement. It collapsed in every wave.
If telephone care must be over-dimensioned, prioritise care for the most fragile and vulnerable, and do not dedicate it primarily to bureaucracy (sick leave and others).
Conduct a systematic and regulated general evaluation to quantify successes and failures and make proposals for improvement for future pandemics.
Improve investment and organisation in Primary Care and the rest of the system based on the results of the general evaluation.

我提出十个初步要点。

为专业人士提供防护设备。我们有好几个月都穿着垃圾袋，没有口罩。
提供科学证据和可靠数据来支持所采取的措施。在整个疫情期间，我们几乎都是盲目的。
避免创造性地使用药物和治疗方法（氯喹、阿奇霉素、皮质类固醇等）。
调整封锁措施。在任何情况下都允许单独或在自然环境中行走。
特别保护儿童的权利。
保护老年人，特别是那些住在养老院的老年人。
确保基层医疗是卫生系统的基础，防止其因官僚原因（病假单）、人员短缺和管理不善而崩溃。每一波疫情都导致了崩溃。
如果必须过度扩大电话护理，应优先为最脆弱的人群提供护理，而不是主要用于处理官僚事务（病假等）。
进行系统的、规范的全面评估，量化成功和失败，并为未来的流行病提出改进建议。
根据全面评估的结果，改善对基层医疗和卫生系统其他部门的投资和组织。

Hace tres años fuimos testigos del colapso sanitario que las diferentes oleadas pandémicas produjeron sin que ninguna medida política, técnica ni epidemiológíca lo evitara. Para muchos médicos fue una epifanía. La cruda realidad nos enseñó que pese a hacer todo los esfuerzos posibles la sanidad tenía un límite, vaya si lo tenía. En Atención Primaria algunos ya lo habíamos visto con anterioridad pero tuvo que ser la pandemia global (y la postpandemia) la puerta necesaria para entrar a un nuevo nivel de comprensión.

En Madrid los pediatras y médicos de familia tuvimos que salir a la calle junto con manifestaciones ciudadanas multitudinarias pidiendo que se reforzara el sistema. Tras varios meses la protesta termino cediendo tras algún parche de la administración, mejoraban algo las condiciones económicas y por primera vez se ponía un límite al número de consultas diarias para evitar aplastar a los profesionales. Esto ha tenido un cumplimiento desigual, lo que ha mantenido la tensión sobre el colectivo y ha hecho evidentes listas de espera no vistas antes en los centros de salud.

A lo anterior se suma la modificación del Estatuto Marco que es la ley que condiciona las relaciones de los profesionales de la salud con las distintas administraciones. Una ley que muchos médicos consideran injusta al permitir condiciones de trabajo peores que las del resto de los trabajadores. Fundamentalmente para los que hacen guardias con un exceso de horas que no se reconocen como extras ni computan para la jubilación. Está surgiendo una protesta nacional que los sindicatos canalizan en las negociaciones con el Ministerio pero faltan muchas voces por oír.

Nos enfrentamos a grandes retos que amenazan los sistemas sanitarios públicos europeos. Por el lado poblacional vemos un aumento del envejecimiento y la cronicidad, a nivel social exigencia de inmediatez y mayor dificultad para tolerar el malestar asociado a la vida, a nivel político mayor dificultad para nivelar presupuestos crecientes con fármacos y productos sanitarios cada vez más caros y a nivel profesional un incremento de la presión asistencial y de exigencia de resultados sin suficiente protección ni contraprestación. Todo esto en una coyuntura geopolítica que nos quiere vender la narrativa de que es necesario invertir en defensa un dinero que tendrá que salir de otros servicios públicos…

Ningún actor ha conseguido dar una respuesta a los retos que tenemos encima de la mesa a modo de nudo gordiano que no hay quien desate. Seguramente será necesario abrir nuevas líneas de reflexión y de diálogo, algo que en estos tiempos en los que todos van a lo suyo cuesta bastante. Por eso defiendo la intuición de que es preciso rescatar la voz de los Colegios profesionales para aunar la de todas las cabezas pensantes que sea posible. Desgraciadamente en Madrid esto no es posible. Pese a que ha habido elecciones recientes, la institución se haya sumida en un bloqueo administrativo dado que la junta saliente ha cerrado de malas formas el camino a la entrante con un recurso contencioso administrativo, de motivación muy cuestionable, que deja a los 50.000 médicos de Madrid sin el amparo y la representación de su Colegio.

No es fácil aventurar qué va a pasar, pero lo que sí podemos decir es que será fundamental priorizar una nueva narrativa que se aleje tanto del optimismo fácil de los que tratan de vender una sanidad “de las mejores del mundo” como de los agoreros que solo ven hundimiento y llamas. Creo que la sociedad precisa médicos que la atiendan, orienten y curen trabajando con unas condiciones dignas que permitan que lo den todo sin dejarse su propia salud por el camino. Por eso seguiré defendiendo esta postura desde mi humilde consulta rural y quizá, si me dejan, desde el propio Colegio.

Medical Journey in the Desert

Two years ago, we witnessed the healthcare collapse triggered by the successive waves of the pandemic, with no political, technical, or epidemiological measures able to prevent it. For many doctors, it was an epiphany. The harsh reality taught us that despite all possible efforts, healthcare had a limit—and what a limit it was. In Primary Care, some of us had seen this before, but it took the global pandemic (and the post-pandemic period) to serve as the necessary gateway to a new level of understanding.

In Madrid, paediatricians and family doctors had to take to the streets alongside massive citizen demonstrations, demanding reinforcement of the system. After several months, the protests subsided following some administrative patches: economic conditions improved slightly, and for the first time, a cap was placed on the number of daily consultations to prevent overwhelming professionals. Compliance with this has been uneven, keeping tension alive within the workforce and making unprecedented waiting lists in health centres all too apparent.

Added to this is the amendment of the Framework Statute, the law governing the relationship between healthcare professionals and various administrations. Many doctors view this law as unfair, as it permits working conditions worse than those of other workers—particularly for those on call, with excessive hours that are neither recognised as overtime nor counted towards retirement. A national protest is emerging, channelled by unions in negotiations with the Ministry, though many voices remain unheard.

We face immense challenges threatening Europe’s public healthcare systems. On the population side, we see increased ageing and chronic illness; socially, there’s a demand for immediacy and less tolerance for the discomfort inherent in life; politically, it’s harder to balance rising budgets with increasingly expensive drugs and medical supplies; and professionally, there’s growing pressure and expectations for results without sufficient protection or compensation. All this unfolds in a geopolitical context pushing the narrative that defence spending is essential, using funds that will inevitably come from other public services.

No stakeholder has managed to address the challenges on the table—a Gordian knot no one can untie. New lines of reflection and dialogue are likely needed, something hard to achieve in times when everyone is focused on their own interests. That’s why I believe we must reclaim the voice of professional associations to unite as many thinking minds as possible. Sadly, in Madrid, this isn’t feasible. Despite recent elections, the institution is mired in administrative deadlock: the outgoing board has bitterly blocked the incoming one with a highly questionable legal challenge, leaving Madrid’s 50,000 doctors without the support and representation of their College.

It’s hard to predict what will happen, but what we can say is that crafting a new narrative will be crucial—one that avoids both the facile optimism of those peddling a “world-class” healthcare system and the doomsayers who see only collapse and flames. I believe society needs doctors to care for, guide, and heal it, working under decent conditions that allow them to give their all without sacrificing their own health. That’s why I’ll keep advocating for this stance from my humble rural practice—and perhaps, if permitted, from the College itself.

我们面临着威胁欧洲公共医疗系统的巨大挑战。从人口角度看，老龄化和慢性病增加；在社会层面，人们要求即时性，对生活中固有的不适越来越难以容忍；在政治层面，平衡不断增长的预算与日益昂贵的药物和医疗产品变得更加困难；在职业层面，医疗压力和对成果的要求不断增加，却没有足够的保护或回报。所有这些都发生在一种地缘政治背景下，这种背景试图让我们相信必须投资于国防，而这笔钱将不可避免地从其他公共服务中抽取。

没有任何一方成功应对我们面前的这些挑战——这是一个无人能解的戈尔迪之结。很可能需要开辟新的反思和对话路径，而在如今人人只顾自身利益的时代，这相当困难。因此，我坚信必须重新启用专业协会的声音，团结尽可能多的有识之士。遗憾的是，在马德里这是不可能的。尽管最近举行了选举，但该机构却陷入行政僵局：即将离任的理事会以极具争议性的行政诉讼方式恶劣地阻断了新理事会的道路，使马德里50,000名医生失去了他们学院的支持和代表。

很难预测未来会怎样，但我们可以说，至关重要的是要优先制定一种新的叙事，既避免那些试图兜售“世界一流”医疗系统的简单乐观主义，也远离那些只看到崩溃和火焰的悲观主义者。我相信，社会需要医生来照顾、引导和治疗他们，而医生需要在体面的条件下工作，这样他们才能全力以赴，而不至于牺牲自己的健康。因此，我将继续从我简朴的乡村诊所捍卫这一立场——如果允许的话，或许还能从学院内部这样做。

En el mundo de las instituciones médicas, donde la ética y la profesionalidad deberían ser los pilares fundamentales, la Junta Directiva saliente del Ilustre Colegio Oficial de Médicos de Madrid (ICOMEM) parece haber optado por un enfoque más... digamos, "creativo". En un ejercicio de ironía, uno podría preguntarse si han decidido que la comedia de errores es el nuevo género de gestión institucional. La situación actual de bloqueo es tan ridícula que parece sacada de una telenovela mal escrita, con la diferencia de que aquí los actores son médicos, supuestamente de los más racionales de entre nosotros.

Comencemos con la elección del nuevo presidente, Tomás Merina, un evento que en cualquier organización seria sería una celebración de democracia y liderazgo. Sin embargo, en el ICOMEM, parece más bien un juego de sillas musicales donde, al parecer, el ganador no solo tiene que ser el más votado, sino también el que mejor salte sobre las zancadillas y los obstáculos burocráticos. Merina fue elegido presidente con el beneplácito de la comisión electoral y el apoyo del 47% de los sufragios, pero ¡oh sorpresa!, la comisión de recursos decidió anular su elección un mes después, alegando que no cumplía con los requisitos de estar en el ejercicio activo de la medicina. Es inevitable preguntarse, ¿acaso los votos de los colegiados son menos importantes que la letra pequeña de unos estatutos que parecen haber sido interpretados de manera torticera?

La ironía de esta situación es tan espesa que se podría cortar con bisturí. Merina, quien ha sido médico desde 1979, con 46 años de experiencia en la sanidad privada, la gestión y la universidad, se ve cuestionado por su "falta de actividad". Uno podría pensar que, en una institución que debería valorar la experiencia y la trayectoria profesional, este tipo de detalles serían irrelevantes. Pero no, para la Junta saliente, parece que la antigüedad o el propio ejercicio de la medicina es un juego de números y no de méritos. La comisión de recursos, en un acto de cinismo magistral, decidió que Merina, quien ha estado colegiado en Madrid durante décadas, no cumplía los requisitos. ¿De verdad? Uno casi espera que la próxima reunión del colegio incluya un juego de Monopoly para decidir quién es el próximo presidente, dado el nivel de seriedad con el que se toman estas decisiones.

Es triste reconocer que la Junta Directiva saliente, que ha sido rechazada por la colegiación, sigue en el poder como una "Junta Okupa", y como dice el propio Merina “no se van ni con agua caliente”. ¿Cómo es posible que una organización que representa a profesionales de la salud opere con menos seriedad que una chirigota gaditana?

La resultante de esta situación es el gran malestar creado entre los 50.000 médicos madrileños que asisten al espectáculo con una mezcla de incredulidad y estupor que les lleva a pensar otra vez si el Colegio servirá para algo, hartos de una historia que ha sido generosa en ejemplos poco edificantes. Porque ¿dónde está la dignidad profesional cuando una junta saliente se aferra al poder en una institución que ha quedado bloqueada?

En conclusión, los siete miembros que quedan de la anterior Junta Directiva parecen haber adoptado una estrategia de gestión que combina lo absurdo con lo patético. La cuestionable anulación de la elección de Merina, la permanencia de una Junta saliente no deseada, y el estado de estupor de los colegiados frente a esta crisis nos muestra una institución que en lugar de ser un faro de profesionalismo, se ha convertido en un circo donde el mal perder es el payaso principal. Uno solo puede esperar que esta comedia de errores llegue pronto a su fin, y que los médicos de Madrid recuperen no solo su Colegio, sino también su dignidad profesional, antes de que los largos plazos legales del recurso contencioso administrativo terminen hundiendo la institución.

What is Happening at the Madrid Medical Association?

In the realm of medical institutions, where ethics and professionalism should be the fundamental pillars, the Outgoing Board of Directors of the Illustrious Official College of Physicians of Madrid (ICOMEM) seems to have opted for a more... let’s say, "creative" approach. In an exercise of irony, one might wonder if they have decided that a comedy of errors is the new model for institutional management. The current deadlock is so ridiculous that it seems taken from a poorly written soap opera, with the difference that here the actors are doctors—supposedly among the most rational of us.

Let’s begin with the election of the new president, Tomás Merina, an event that in any serious organisation would be a celebration of democracy and leadership. However, at ICOMEM, it appears more like a game of musical chairs where the winner not only has to be the most voted but also the one who best navigates bureaucratic obstacles and hurdles. Merina was elected president with the endorsement of the electoral commission and the support of 47% of the votes. But—oh, surprise!—the appeals commission annulled his election a month later, arguing that he did not meet the requirement of actively practising medicine. One cannot help but wonder: are the votes of the members less important than the fine print of statutes seemingly interpreted in a twisted manner?

The irony of this situation is so thick it could be cut with a scalpel. Merina, a doctor since 1979 with 46 years of experience in private healthcare, management, and academia, finds his "lack of activity" questioned. One would think that in an institution that should value experience and professional trajectory, such details would be irrelevant. But no, for the outgoing Board, it seems that seniority or even the practice of medicine is a game of numbers rather than merit. The appeals commission, in a masterful act of cynicism, decided that Merina, who has been registered in Madrid for decades, did not meet the requirements. Really? One almost expects the next college meeting to include a game of Monopoly to decide the next president, given the level of seriousness with which these decisions are taken.

It is disheartening to acknowledge that the Outgoing Board of Directors, rejected by the membership, remains in power like a "Squatter Board," and as Merina himself says, "they won’t leave even with boiling water." How is it possible that an organisation representing healthcare professionals operates with less seriousness than a carnival troupe from Cádiz?

The result of this situation is the great discontent among the 50,000 Madrid doctors who watch the spectacle with a mix of disbelief and astonishment, leading them to once again question whether the College serves any purpose, fed up with a history rich in less than edifying examples. Because where is professional dignity when an outgoing board clings to power in an institution that has been left paralysed?

In conclusion, the seven remaining members of the previous Board of Directors seem to have adopted a management strategy that combines the absurd with the pathetic. The questionable annulment of Merina’s election, the persistence of an unwanted outgoing Board, and the state of shock among members in the face of this crisis show us an institution that, instead of being a beacon of professionalism, has become a circus where sour grapes are the main clown. One can only hope that this comedy of errors comes to an end soon and that Madrid’s doctors reclaim not only their College but also their professional dignity—before the long legal deadlines for the administrative appeal end up sinking the institution.

El estado del bienestar, en su más pura esencia, nos ha regalado comodidades que antes ni soñábamos, pero también un lado oscuro: el consumismo nos ha vuelto vagos y cómodos. Vamos sobrados de sofá, pantallas, series, móviles y demás, creando un entorno donde la actividad física y mental se reducen a un mínimo histórico.

Hablamos de "obesidad mental", un fenómeno que no es nada más que la sustitución de neuronas por grasa. Es la erosión de nuestra capacidad cognitivo-emocional, nuestra creatividad y nuestra motivación, reemplazadas por una adicción silenciosa a la inactividad y al entretenimiento pasivo.

Nos hemos convertido en consumidores de tiempo, no en gestores de vida. Estamos llenando nuestras horas con contenidos vacíos que no nutren ni el cuerpo ni el alma, y esto es tan perjudicial como una dieta alta en azúcar y grasa.

Es fundamental que nos pongamos las pilas. La solución no está en la última serie de Netflix o en el algoritmo de TikTok, sino en volver a lo básico: cuidar la dieta, tanto la de nuestro cuerpo como la de nuestro cerebro. Necesitamos recuperar el placer de la actividad física, el estímulo del pensamiento crítico y la satisfacción de crear o hacerlo uno mismo antes que comprar o consumir. Y si lo hacemos acompañados o en un entorno natural mucho mejor.

En un mundo donde todo se nos ofrece en bandeja, es nuestro deber, por salud y por conciencia, no caer en la trampa de la comodidad extrema. La verdadera salud, mental y física, no se compra, se cultiva.

Mental Obesity

The welfare state, in its purest form, has gifted us comforts we could once only dream of, but it has also brought a dark side: consumerism has made us lazy and complacent. We have an abundance of sofas, screens, series, mobile phones, and more, creating an environment where physical and mental activity has reached a historic low.

We speak of “mental obesity,” a phenomenon that is nothing more than the replacement of neurons with fat. It is the erosion of our cognitive abilities, our creativity, and our motivation, replaced by a silent addiction to inactivity and passive entertainment.

We have become consumers of time, not managers of life. We are filling our hours with empty content that nourishes neither body nor soul, and this is as harmful as a diet high in sugar and fat.

It is crucial that we take action. The solution does not lie in the latest Netflix series or the TikTok algorithm, but in returning to the basics: taking care of our diet, both for our body and our mind. We need to rediscover the joy of physical activity, the stimulation of critical thinking, and the satisfaction of creating or doing things ourselves rather than just buying or consuming.

In a world where everything is handed to us on a silver platter, it is our duty, for the sake of our health and awareness, not to fall into the trap of extreme comfort. True health, both mental and physical, cannot be bought—it must be cultivated.

Hablando esta semana con Juan José Rodríguez Sendín, me decía que hay gente malvada y estúpida en todas partes, entre los médicos también. Y sufriendo en primera persona el escándalo que atraviesa el Colegio de Médicos de Madrid no puedo evitar recordar ese comentario, no por emitir un juicio personal sino para constatar cuánto nos gusta a los españoles montar chiringuitos o cortijos. Nos encantan. Pero suele ser un error porque al final todo se sabe, máxime en estos tiempos donde todo se vuelve transparente. Cuando en unos meses se haga pública la próxima auditoria de cuentas veremos de qué hablo, aunque ya los periodistas lo están empezando a mostrar.

Me animé a formar parte de una candidatura al Colegio de Médicos por que creo que hay que rescatar su verdadero papel. Es una institución que pese a no tener poder ejecutivo sí lo tiene a la hora de representar, defender, apoyar y promocionar a sus colegiados, a la profesión médica de Madrid. Y para ello hay que priorizar valores como la transparencia, la honradez, la valentía y la buena gestión antes de promocionar las ideas, creencias o intereses personales de su junta directiva, reformar cafeterías o montar escándalos mayúsculos. Sin considerar la dilapidación de recursos gastando 4 millones de euros en reformar una cafetería (presupuestada en 1 millón), mal gestionar el inmueble propiedad del Colegio en la calle Esparteros o tener la mesa de Atención Primaria sin convocar desde hace tiempo y muchas comisiones bloqueadas por los problemas de una junta que está en pleitos legales entre ellos con la aseguradora de por medio.

En primer lugar constato que no tenemos ninguna experiencia en catástrofes, nunca han ocurrido, no hay plan de contingencia. Hubiera sido un desastre similar al que hemos visto.

En segundo compruebo que los recursos con los que cuenta un municipio de 9.000 habitantes no son muchos: ayuntamiento, policía local, puesto de la guardia civil, centro de salud, ambulancia municipal (ahora parada por motivos técnicos). Pero tenemos un hospital y bomberos a 23 minutos (Collado Villalba), otro hospital a 35 en coche (Madrid)… Si ocurriera algo probablemente tendríamos ayuda rápida. 1. Lavarte las manos te protege.

2. Haz un uso prudente de la mascarilla (si eres vulnerable para protegerte o cuando tengas síntomas para proteger a otros).

3. Si estornudas usa pañuelos de papel o el codo.

4. Ventilar y evitar hacinamiento es muy útil.

5. La vacuna puede ayudar, ya puedes citarte.

6. Si necesitas baja pide cita telefónica.

7. Recuerda que la gripe se cura con reposo, antitérmicos y autocuidados.

8. En el pico epidémico haz un uso prudente de las urgencias.

9. Si tienes dudas tu enfermera te puede ayudar.

10. Hagas lo que hagas la gripe durará una semana.

El Ministerio de Sanidad está preparando una encuesta para saber que falta en Atención Primaria. Creo que lo saben, el Secretario de Estado de Sanidad es un buen médico de familia y conoce la finca. De todas formas llevamos décadas diciéndolo. Hay estudios, libros blancos, artículos, blogs, jornadas, grupos de trabajo, más grupos de trabajo, infinitas reuniones, infinitos programas, que lo han estudiado a fondo.

Y aquí seguimos desnudos en mitad de la plaza viviendo en un tonel como el filósofo griego. Y seguimos llorando, porque al menos eso sí lo sabemos hacer bien. Sin lágrimas habríamos perdido hace tiempo la poca salud mental que nos pueda quedar.

Pero las lágrimas no mueven molino. Los cambios que precisamos son de profundidad y afectarían al diseño de toda la organización sanitaria. No hay político ni gestor que se atreva. Ni lo va a haber. Es una cirugía demasiado arriesgada y muy sangrante (en votos), no se van a atrever. Por otro lado los tiburones del mercado siguen dando vueltas ante una presa cada vez más hundida relamiéndose ante el inminente festín. Porque será el mercado el que al final acometa la reforma fagocitando los trozos del barco sanitario uno detrás de otro. Eso ya lo saben en los despachos, ya está pasando.

Durante un tiempo creí que el reto estaba en conseguir contarlo de forma comprensible a la sociedad. Pero me equivoqué. Hace dos años hicimos una huelga muy prolongada en Madrid que consiguió manifestaciones millonarias. La gente era consciente de que su Atención Primaria estaba siendo desmantelada y salió a la calle a protestar para pocos meses después dar mayoría absoluta a quien lleva décadas gestionándola. Y lo peor es que si no hubiera sido así seguiríamos probablemente igual.

¿Cómo conseguir más sonrisas y menos lágrimas en Atención Primaria? pregunto al aire mientras deshojo la margarita. Y la respuesta viene sola con la primera luz del día: asumir lo que hay y hacer lo que se pueda. No me pidan una hoja de ruta más allá de la mera supervivencia, no la tengo. Pero si veo claro que hay que seguir dando masaje cardiaco allá donde podamos y poniendo las manos sobre los puntos más sangrantes para evitar el shock. Para esto necesitaremos revitalizar asociaciones y organizaciones profesionales, tirar de todo lo que pueda ayudar

En las siguientes semanas me verán dando la batalla para entrar en el Colegio de Médicos de Madrid como vocal de Atención Primaria en la candidatura del doctor Tomás Merina que lidera un equipo muy potente de médicos independientes con una misión clara: conseguir que el Colegio esté del lado de los médicos. En los últimos años muchos hemos sentido que no ha sido así. Vamos a necesitar ayuda para ello, tu ayuda, para que el día de las elecciones hagas el esfuerzo de votar. Nos gustaría demostrar que para que algo cambie es fundamental la suma de muchos pequeños actos. Gracias de antemano.

More smiles and fewer tears (in Primary Care)

The Ministry of Health is preparing a survey to find out what is lacking in Primary Care. I think they already know; the Secretary of State for Health is a good family doctor and knows the field. Anyway, we’ve been saying this for decades. There are studies, white papers, articles, blogs, conferences, working groups, more working groups, endless meetings, endless programmes, that have studied this thoroughly.

And here we are, still standing naked in the middle of the square, living in a barrel like the Greek philosopher. And we continue to cry, because at least we know how to do that well. Without tears, we would have long lost what little mental health we might have left.

But tears do not turn the mill. The changes we need are profound and would affect the design of the entire healthcare system. No politician or manager dares to take them on, and there will not be any who do. It’s a surgery that is too risky and too damaging (in terms of votes); they won’t dare. On the other hand, the market sharks keep circling, eyeing a sinking prey and licking their lips at the imminent feast. Because it will be the market that eventually undertakes the reform, swallowing the pieces of the healthcare ship one by one. The people in the offices already know this, it’s already happening.

For a while, I believed the challenge was to explain it clearly to society. But I was wrong. Two years ago, we held a very long strike in Madrid, which resulted in massive demonstrations. People were aware that their Primary Care was being dismantled and took to the streets to protest, only to give an absolute majority to those who have been managing it for decades a few months later. And the worst part is that, even if this had not happened, we would probably be in the same situation.

How can we get more smiles and fewer tears in Primary Care? I ask myself as I pluck the petals of a daisy. And the answer comes with the first light of day: accept what is and do what you can. Don’t ask me for a roadmap beyond mere survival, because I don’t have one. But I can clearly see that we must keep administering CPR wherever we can, and place our hands over the most serious wounds to avoid shock. For this, we will need to revitalise professional associations and organisations, drawing on everything that can help.

In the coming weeks, you will see me fighting to join the Madrid Medical Association as the Primary Care representative in the candidacy of Dr Tomás Merina, who leads a strong team of independent doctors with a clear mission: to ensure the Association is on the side of the doctors. Lo más difícil para un ser humano es dejar que los demás le coman, permitir que se alimenten de ti. Esta condición requiere una metamorfosis para pasar de depredador a humano pleno, porque hay plenitud en la entrega, en la entrega total. Manuel Vilas lleva muchos años regalándonos narrativas cada vez mejores, la última propuesta es su novela “El mejor libro del mundo” con 592 páginas donde se deja la piel a tiras. Crea una sucesión de escenas donde nos va alimentando con sus cuitas, viajes, encuentros y obsesiones. En la portada aparece un señor vestido de oscuro colgado de un árbol, una mezcla entre Pessoa y el arcano del loco del Tarot. Yo hubiera puesto un cristo con dos pistolas para avisar que el texto no va a dejar a nadie indiferente y que va a haber mucha sangre, sangre de autor. Vilas se despelleja hasta el tuétano y al hacerlo se convierte en un espejo que descubre fantasmas, neurosis y heridas reflejando con precisión las de los que se asoman. Con la línea argumental de cumplir 60 años nos ofrece un recorrido por paisajes interiores poblados por autores, música y literatura abundante. Es bello ver como todas esas narraciones y canciones le van transfigurando al encarnarse en él. Al escribir las trasvasa gota a gota al lector como en una donación de sangre y aprovecha para disparar al mismo tiempo contra toda celebridad a su alcance sin dejar títere con cabeza. Dispara al rey de España, a la infanta, al presidente, al Papa de Roma y a Jesucristo… dispara a personajes públicos y anónimos, a su propia familia y por supuesto a él mismo. Para dar ejemplo se acribilla y como buen maño lo hace a conciencia. De esos agujeros manará un fuego que impregnará todo lo que toca, todo lo que escribe. Hay páginas que casi queman y obligan a soltar el libro para tomar aliento. En otras vemos los estragos que su extrema sensibilidad provoca en su alma. En algunos capítulos provocará sonrisas, en otros lágrimas, pero si me permiten destacaría dos cosas, la existencia de un incierto punto de fuga que ofrece esperanza pese a las llamas y la posibilidad de amar que se nos permite a los vivos por el simple hecho de ser humanos. Vilas nos dice con esta novela que comamos y bebamos todos de él, de su carne, de sus letras. Y al hacerlo nos ofrece su vida en bandeja con una honestidad fuera serie (que llega más lejos que las propuestas de Emmanuel Carrère y otros autores). Merece la pena que en compensación compremos el libro y lo leamos.
