There are a number of options for the treatment of fine lines and wrinkles, each with varied expense, recovery time and results. The following are the most common conventional treatments used today:

Nonprescription and Prescription Topicals: The simplest method of skin treatment is the topical application of wrinkle creams. Widely available everywhere from pharmacies to health food stores, these creams are beneficial for the skin, helping to moisturize and nourish it. Laser Resurfacing

In dermatology, lasers adds another dimension for the treatment of skin diseases. Lasers (short for Light Amplification by Stimulated Emission of Radiation) work by producing an intense beam of light that travels in one direction. This beam can gently ablate skin tissue to improve wrinkles, scars or blemishes. In addition, lasers can also be used to destroy visible blood vessels or to cut skin tissue. Using a laser hand piece, undesired skin cells and wrinkles disappear and are replaced by fresh skin cells. An ointment such as Vaseline or Aquaphor is then applied. Strict wound care is intended to prevent scab formation and facilitate healthy wound healing. Typically, the areas heal in 10-21 days.

Once the areas have healed, makeup may be worn to camouflage the pink to red color that is generally seen after laser skin resurfacing. Most dermatologist recommend oil-free make-ups as well as green-based foundation since they neutralize the red color. The redness in the laser-treated sites generally fades in two to three months but may take as long as six months to completely disappear.

People with blonde or red hair may have a prolonged period of recovery. Conversely, patients with darker skin tones have a greater risk of healing with darker pigmentation (referred to as post-inflammatory hyperpigmentation). This may be minimized by use of a bleaching agent before laser skin resurfacing as well as continued use of this agent after healing.

Laser resurfacing is a relatively safe procedure. However, as with any other medical procedure, one should be aware of complications that may arise. Milia, or small white bumps, may appear in the laser-treated areas during healing. These lesions may be removed by gentle cleansing with a washcloth and do not scar. As mentioned above, increased pigmentation, and rarely decreased skin pigmentation, may result in the laser-treated areas. Lastly, reappearance of a cold sore may occur, especially after laser skin resurfacing around the mouth. This can be prevented by pre treatment with antiviral medication a day prior to the procedure.

The final result of laser resurfacing can take up to six months to fully appear. These results are long-lasting, but not permanent. Some people will require re-treatments over the years. Protection from the sun and the institution of a good skin-care regime can help in maintaining a rejuvenated look.

A scar is a natural part of the healing process. Skin scars occur when the deep, thick layer of skin (the dermis) is damaged. The worse the damage is, the worse the scar will be. Most skin scars are flat, pale and leave a trace of the original injury that caused them. The redness that often follows an injury to the skin is not a scar, and is generally not permanent. The time it takes for it to go away may, however, range from a few days to, in some serious and rare cases, a few years. Various treatments can speed up the process in serious cases. Scars form differently based on the location of the injury on the body and the age of the person who was injured. To mend the damage, the body has to lay down new collagen fibers (a naturally occurring protein that is produced by the body by fibroblasts). In wounded tissue these fibers are over expressed with collagen, but not so in non wounded tissue. Stable forms of topical vitamin C have been shown to improve collagen formation^[2]. Recent research has also implicated the gene product osteopontin in scarring and University of Bristol have developed a gel that inhibits the process^[3]^[4].

An injury does not become a scar until the wound has completely healed. And because the wounded body tissue over expresses collagen on the fibers and cannot re-build, the new tissue will have a different texture and quality than the surrounding normal tissue. This collagen producing process results in a fortuna scar.

Transforming Growth Factors (TGF) play a critical role in scar development and current research is investigating the manipulation of these TGFs for drug development to prevent scarring from the emergency adult wound healing process. As well, a recent American study implicated the protein Ribosomal s6 kinase (RSK) in the formation of scar tissue and found that the introduction of a chemical to counteract RSK could halt the formation of Cirrhosis. This treatment also has the potential to reduce or even prevent altogether other types of scarring.^[5]

[edit]Abnormal scars

Two types of scars are the result of the body overproducing collagen, which causes the scar to be raised above the surrounding skin. Hypertrophic scars take the form of a red raised lump on the skin, but do not grow beyond the boundaries of the original wound, and they often improve in appearance after a few years. Keloid scars are a more serious form of scarring, because they can carry on growing indefinitely into a large, tumorous (although benign) growth. Keloid scars can occur on anyone, but they are most common in dark-skinned people.^[6] Keloid scars can be caused by surgery, an accident, by acne or, sometimes, from body piercings. In some people, keloid scars form spontaneously.

Although they can be a cosmetic problem, keloid scars are only inert masses of collagen and therefore completely harmless and non-cancerous. However, they can be itchy or painful in some individuals. They tend to be most common on theshoulders and chest.

Alternately, a scar can take the form of a sunken recess in the skin, which has a pitted appearance. These are caused when underlying structures supporting the skin, such as fat or muscle, are lost. This type of scarring is commonly associated with acne, but can be caused by chickenpox, surgery or an accident.

Scars can also take the form of stretched skin. These are called striae and are caused when the skin is stretched rapidly (for instance during pregnancy, significant weight gain or adolescent growth spurts), or when skin is put under tension during the healing process, (usually near joints). This type of scar usually improves in appearance after a few years.

[edit]Treatments for skin scars

Currently no scar can be completely removed,^[7] although healing in embryos and some animals even after extended injuries can occur without scarring.^[8] Therefore all treatments will leave a trace, but a number of approaches have been tried with silicon gel sheeting and steroid injections having the most widely accepted role in general scar treatment.^[9]

In historical sequence, corticosteroid therapy by injection into the scars was introduced in the 1960s, from the early 1970s pressure garment therapy was introduced for widespread burn scars, and silicone gel sheets from the 1980s.^[10]

[edit]Needling

Needling is an inexpensive process where the scarred area is continuously needled to promote collagen formation. Once needled the area is allowed to fully heal, and needled again if required depending on the intensity of the scar. Scarring needles and needling rollers are available for home use; however, needling should not be done on parts of the face or areas where major nerves are located without professional medical supervision. Needling at home must also be done in line with hygienic and sterilization requirements.

[edit]Pressure garments

Pressure garments should be used only under supervision by a medical professional. They are most often used for burn scars that cover a large area, this treatment is only effective on recent scars.

Pressure garments are usually custom-made from elastic materials, and fit tightly around the scarring. They work best when they are worn 24 hours a day for six to twelve months.

It is believed that they work by applying constant pressure to surface blood vessels and eventually causing scars to flatten and become softer.

[edit]Steroids

A long term course of steroid injections under medical supervision, into the scar may help flatten and soften the appearance of keloid or hypertrophic scars.

The steroid is injected into the scar itself; since very little is absorbed into the blood stream, side effects of this treatment are minor. However, it does cause thinning of the scar tissue so it does carry risks when injected into scars caused by operations into ruptured tendons. This treatment is repeated at 4-6 week intervals.

Topical steroids are ineffective.^[11]

[edit]Silicone sheeting and gel

Silicone scar treatments improve scar appearance and are often used to prevent and treat hypertrophic scarring. The exact mechanism of action is unknown though some studies suggest a manipulation of local ionic charges or a decrease in production of “pro-inflammatory” substances like TGFβ2.^[12]

Dimethicone silicone gel appears to be as effective as silicone sheeting in improving scar appearance.^[13]

[edit]Dermabrasion

Dermabrasion involves the removal of the surface of the skin with specialist equipment and usually involves a general anaesthetic. It is useful with raised scars, but is less effective when the scar is sunken below the surrounding skin.

Collagen injections can be used to raise sunken scars to the level of surrounding skin. Its effects are however temporary, and it needs to be regularly repeated. There is also a risk in some people of an allergic reaction.

[edit]Laser surgery & resurfacing

The use of lasers on scars is a new form of treatment. Several cosmetic lasers have been FDA approved for the treatment of acne scars by using laser resurfacing techniques. Vascular lasers have been proven to greatly reduce the redness of most scars 6–10 weeks after the initial treatment. It has been theorized that removing layers of skin with a carbon dioxide or Erbium:YAG laser may help flatten scars.^{[citation needed]}

Surgical excision of hypertrophic or keloid scars is often associated to other methods such as pressotherapy or silicone gel sheeting. Lone excision of keloid scars however shows a high recurrence rate close to 45%. A clinical study is currently ongoing to assess the benefits of a treatment combining surgery and laser-assisted healing in hypertrophic or keloid scars.

[edit]Radiotherapy

Low-dose, superficial radiotherapy, is used to prevent re-occurrence of severe keloid and hypertrophic scarring. It is usually effective, but only used in extreme cases due to the risk of long-term side effects.

[edit]Natural remedies

Research shows the use of vitamin E and onion extract as a treatment for scars is ineffective. Vitamin E causes contact dermatitis in up to 33% of users and in some cases it may worsen scar appearance.^[11]^[14] Vitamin C normalizes collagen production and encourages the production of an organized, healthy collagen framework ^[15] which improves scar appearance. Vitamin C and some of its esters also fade the dark pigment associated with some scars.^[2]

Laser Treatments
There are 2 major categories of laser used in acne scar therapy. They are the resurfacing (aka ablative) lasers and the non-ablative lasers.
Ablative lasers literally remove the outer layers of the skin, burning away scar tissue and stimulating the dermal collagen to tighten, reducing the amount of scar visibility. The ultrapulsed carbon dioxide laser and erbium YAG laser are most frequently used in laser resurfacing.
Because the skin is injured and unprotected tissue exposed, great effort must be put into wound care and infection prevention. The skin may remain reddened for several months or a year afterwards.

Triplet sisters scarred in fire have new hope

Innovative laser treatment may help their childhood burn scars disappear

They were three little 17-month-old blondes — identical triplets cute as matching buttons, all nestling together in the same bed — when their house went up in flames. And for the next 20 years, as they grew into beautiful young women, they thought that they would have to live forever with the thick and disfiguring scars that covered their arms, backs, chests and faces.

Despite the dramatic advances in treatment for burn victims in recent years, there wasn’t much that could be done for the type of old scar tissue that Trae, Jordan and Chandra Berns were living with, NBC News’ chief medical editor Dr. Nancy Snyderman told TODAY’s Matt Lauer Thursday in New York.

The Berns triplets, now 22 years old, sat on the couch between Snyderman and Lauer, eager to share how a new laser treatment has dramatically improved their appearance and given them hope that their scars can all but be erased.

Dramatic difference
“When you take scars that are 18 to 20 years old, it’s healed. They’re scarred, and these guys know how stiff and inflexible that tissue is,” Snyderman said.

Now, three months after undergoing treatment with lasers that are normally used to treat severe acne scars, the triplets showed off faces and arms that are dramatically smoother and more uniform in color than the ridged and blotched skin they had lived with for almost their entire lives.

“With my scars, the coloring is definitely all uniform. The scars are so much smoother,” Jordan said, showing off a forearm that looked smooth and only slightly discolored. “It was a lot thicker, and now it’s just really smooth,” she added of her skin.

“My scars are a lot smoother and the texture is a lot better,” Trae agreed.

Trae was the triplet who learned on the Internet of a pioneering treatment that Florida dermatologist Dr. Jill Waibel had adapted for treating burn victims, called an ultrapulse fractional ablative laser. She talked her sisters into joining her for a two-day drive from their Texas home to Waibel’s West Palm Beach clinic. When Waibel met the three young women, she and Lumenis, the manufacturer of the laser, agreed to treat them free of charge.

Double laser
The treatment consists of two separate lasers. The first shoots tiny beams that penetrate less than a millimeter into the scar tissue, heating it to the boiling point and vaporizing it. The second laser flattens and evens out the texture and coloring of the remaining scar tissue. Treating all three women took an entire eight-hour day, and more treatments will follow.

The full benefit of the treatment will take six months to show, but after three months, Trae, Jordan and Chandra were ecstatic with the results, which were readily apparent when comparing their skin now to before pictures.

No matter how much people talk about how it’s what’s inside that counts, living with scars is not easy, Snyderman said.

“The social stigma of walking around with scars on your body is unbelievably difficult for most people, especially when you see three beautiful girls,” she told Lauer.

“Emotionally, just knowing there’s a treatment that can change the look of our burns and hopefully heal them completely, that in itself is just an amazing feeling,” Jordan affirmed.

Emotional as well as physical
The scars from the fire are more than skin-deep. The current issue of Glamour magazine tells the full story of how the Berns triplets were just 17 months old when they were trapped in a fire in their Texas home. The blaze broke out around 6 p.m., and the girls were already sleeping in the crib they shared.

According to the article, their father, who told investigators he had been sleeping on the couch when the fire broke out, rescued the girls. Firefighters pulled the girl’s mother, Patti, out of the flames, but she died two days later from the effects of smoke inhalation.

The little girls spent a long time in the hospital, unaware that their father, who battled drug addictions his entire adult life, had been charged with setting the blaze that disfigured them and killed their mother. He was acquitted in trial when his defense convinced the jury that any one of three other people seen near the house at the time of the blaze could have set it.

All of that was kept from the girls as they grew up living alternately with grandparents, their father and an aunt. Only when their father died of a drug overdose when they were 16 did they begin to learn the full story of what had happened to them.

Still, the triplets told Lauer that despite the extensive and constant physical therapy and special compressing sleeves they had to wear, their childhoods were remarkably normal.

“When we were growing up, we didn’t notice that we had burns. We were regular children. We went out. The people that we grew up with, all the children, they pretty much knew what happened, and everybody pretty much accepted it,” Chandra said.

Hope for others
It was only when they got to high school and met new people that they were made acutely aware of their disfigurement by other children who pointed at them and called them vicious names.

They’re students at the University of Texas at San Antonio now, and Trae and Jordan plan to enter a master’s program in occupational therapy in the fall. Chandra will join them at UTSA and hopes to one day start a foundation for burn patients.

That’s why they’re going public with their story — to give hope to other burn victims.

“Just knowing there’s hope out there for other burn survivors to help their appearance, that’s just really good to know,” said Trae. What you are like inside is what’s important, she said, but that doesn’t mean what’s outside doesn’t count.

“It doesn’t matter what you look like,” Trae told Lauer. “But every person, if they have a flaw, they want to do something to try to fix that.”

ActiveFX v Fraxel

First, let me give you a little background on ActiveFX.
1. The procedure uses the 10,600nm wavelength which is considered by dermatologists and plastic surgeons to be the "gold standard" for removing wrinkles, dyschromia, etc

2. In the past, systems using this wavelength provided excellent results -- but the down-time was prolonged (as much as 6-8 weeks before make-up can be applied!)

3. Consumers wanted noticeable effects with minimal downtime.

We did months of work to identify the correct wavelength, settings, etc.
We found that the ideal laser would need to ablate the top layers of the skin and also simultaneously provide heat to the lower layers.
The ablation causes immediate contraction of wrinkles and loose skin (up to 30%) and also removes any pigmentation problems.
The heat causes new collagen promotion -- which serves to plump up from below AND to re-build the skin's scaffolding to prevent future wrinkles.
However, if you go TOO deep, you can get dermal scarring or dimples from fat atrophy -- both of which are long-lasting (if not permanent) side effects.

4. Guess what... we already had something close! But, we needed to completely change the settings, the spot size, and other things.

We reduced the spot size so that we don't have as much collateral heat. This is important to reduce the redness and swelling so that we have a fast recovery
The system uses a Computerized Pattern Generator (CPG) to place up to 600 of these tiny dots in a second! However, we wanted to make sure that none of these dots were placed side-by-side in a sequential manner. If we do, having several small dots placed nearly-simultaneously is almost as bad as one big dot. So, we made the computer program to put the dots in a non-sequential manner (called CoolScan) so that each individual tiny dot cools off before the one next to it is placed. Again, this is to reduce erythema, edema, etc
We found that the dots should be spread out so that they never touch each other. This means that we leave little "bridges" of healthy, untouched skin between the tiny dots. This "fractional" technique (a fraction of the surface is treated instead of 100%) also really speeds up the re-epithelialization and healing process.
We took several more months to study the energy settings. In the end, we came up with settings that are JUST enough to vaporize/ablate (shrinkage and remove damaged skin) AND enough to provide some heat in the dermis (new collagen) without giving us a long downtime or dermal side effects.

5. All of these things added together give us consistent, reproducible results! We say "ActiveFX is for people with ACTIVE lifestyle who want noticeable EFFECTS -- dramatic improvement in Tone, Texture and Tightening -- all in a single treatment!" It is really incredible to watch a procedure -- you can see the skin "shrink-wrap" before your eyes!

OK, so now (finally) to your question about Fraxel.

1. Both ActiveFX and Fraxel use "fractional" coverage. Fraxel covers about 20% and ActiveFX covers about 80%. The studies we did showed that we got VERY consistent results with 80% (less coverage had too little results on some patients and more caused too much down-time). While 20% will reduce down-time, it also required multiple procedures or you have diminished results.

2. If one has an "aggressive" treatment with Fraxel (to try to eliminate the need to go back and forth to the doctor for multiple procedures), the Fraxel brochures indicate that down-time is 14-20 days! Let's ignore the "subtle" results that you would get and focus on down-time. With ActiveFX, you get VERY noticeable results with downtime much less than a week. In fact, most people can put make-up on in 3-4 days and ALL pinkness is gone in less than a week. Now let's consider the PAIN associated with an aggressive Fraxel treatment: with ActiveFX, most people have little/no pain and can drive themselves home!

3. Fraxel uses NON-ablative technology. Without ablation, there is no immediate contracture. And, it takes a long time for dyschromia to diminish -- immediate with ActiveFX. (Do you remember the old CoolTouch system? It used a 1320nm wavelength to try to reduce wrinkles -- this is a very similar wavelength to Fraxel. That system has basically died now -- no noticeable results despite multiple treatments!)

4. According to their Physician Guide, Fraxel's heat penetrates to 900microns. The eyelid's epidermis + dermis is only about 300microns thick. Therefore, can you safely use that system there? That is why their documentation and photos never show treatment within the orbital rim. However, the ActiveFX procedure is impressive on the eyelids, especially when you consider that this is a single treatment with minimal downtime

Ablative Fractional CO₂ Laser Resurfacing for Acne Scars

This approach offers more benefits than nonablative laser treatment and fewer adverse effects than traditional ablative resurfacing.

Traditional ablative laser resurfacing can significantly improve acne scars, but with considerable downtime and a real risk for hypopigmentation and other adverse sequelae. Nonablative modalities achieve minimal-to-mild improvement of acne scars with minimal downtime and without serious complications. In this prospective study, 15 patients with moderate-to-severe acne scars underwent two to three full-face treatments at 1- to 2-month intervals with an ablative fractionated CO₂ laser. Patients were administered topical anesthesia with nerve blocks and oral oxycodone/acetaminophen and diazepam for pain control. Treatment energies ranged from 20 to 100 mJ per pulse, with total densities of 200 to 1200 microthermal zones per cm². Clinicians used higher energies for deeper scars and lower energies in areas without scars and on thin skin. Investigators and patients graded improvement on a quartile scale (0=no improvement; 4=75% improvement). In 10 patients, the depth of a preselected scar was measured before and 3 months after the last treatment using the Primos 3-D optical imaging system.

Both investigators and patients gave mean improvement scores in the 25% to 50% range for scar atrophy, texture, and overall improvement. The degree of improvement increased with each treatment and continued to increase between the 1-month and 3-month follow-up visits after the last treatment. Objective topographic assessment demonstrated a mean reduction in scar depth of 67%. The most severe acne scars that were treated with the highest energies achieved the best improvement (51%–75% at 3 months after last treatment). Mild-to-moderate erythema immediately after treatment decreased to trace levels by 1 month, and mild-to-moderate edema resolved to trace levels by 1 week. Transient petechiae developed in half the subjects. Oozing and crusting occurred in 80% of patients and resolved in all but two subjects within 7 days of treatment; in the remaining two, the oozing persisted for up to 1 month after the third treatment. Transient, trace hyperpigmentation developed in one subject, but none had scarring or hypopigmentation.
