Sex has always been an intricate issue. Says Dr. Rishma Dhillon Pai, Consulting gynecologist at Lilavati and Jaslok hospital,“ In the initial stages because the frequency of sex is very high there can be cases of tearing which might require stitching, infection or dryness. But more than physical it is a psychological thing. From a young age sex is portrayed as evil and scary. Most women have the notion that it is “painful procedure”. And once that idea is engraved in the mind, it spills over to the physical aspect. And that is when it becomes harmful and a task. It leads to a psychological block, which results in bad, painful sex which in turn leads to various emotional complications. This condition is called Vaginismus. The key is to remember that it is not the frequency that could cause problems but the technique. It is okay if you don’t have sex every night. It should not become a task or an everyday chore. It is a pleasurable act and should be done with a relaxed mind. Ultimately the idea is to have fun and also be safe”.

36 % of women suffer from Vaginismus which is a spasm of the vaginal muscles, caused mainly by fear of being hurt. This spasm is often so painful that intercourse is impossible - sometimes for years.

She says..“I come from a very conservative family and talking about sex was a strict no. I got married when I was 22 to very nice man. I was a virgin and literally clueless about sex and everything related to it. On the first night of the marriage, we consummated our relationship. It was very painful experience and uncomfortable for me but I didn’t complain. Our sex life was pretty ok but even after six months of marriage it was a painful act for me. I thought it was normal and didn’t talk to anybody about it or bothered to go to a gynaecologist. Some times when the sex was rough I even used to bleed. Without knowing and subconsciously that experience shut me off emotionally. I got phobic of sex. I refused to let my husband even touch me. My fear combined with the lack of sex drive from my side affected our marriage. We grew distant. It was quite traumatic for me because I couldn’t talk to anybody about it. I became quite antisocial and aloof which was bad because I am a complete extrovert by nature. I didn’t socialise much, didn’t step out of my home if not necessary, shunned out friends. When things started getting really bad and I couldn’t take it anymore I convinced myself and visited the doctor. The tests revealed that I had developed an infection and had a tearing, which got aggravated every time we did it. I needed stitches. It was a relief and a shock at the same time to know that because it meant that there wasn’t anything wrong with it and me and it was treatable. There was PT-141, by Palatin Technologies of New Jersey, a drug inhaled via nasal spray and found, in preliminary tests, to produce friskiness in both men and women, leading to buzz in 2005 that people would soon be able to snort themselves into the mood. "The first real, honest-to-God, horny-making, body-shaking, equal-opportunity aphrodisiac," panted New York magazine that year.

Nuh-uh. The FDA stopped the clinical testing, citing concerns about a side effect high blood pressure.

There was Intrinsa, a patch made by Procter & Gamble that transmitted testosterone into the bloodstream through the skin. (Testosterone, associated with sex drive, is produced naturally in women, though in far lower quantities than in men.) Also dinged by the FDA.

With men, all a medication needs to produce is arousal, a.k.a. an erection. A guy will conjure lust on his own. A woman, on the other hand, can have the physical signs of arousal and remain uninterested in sex. That's why Viagra doesn't work for women, even though it produces roughly the same physical effect — rushing blood to the nether regions — on them as on men.

Arousal for women does not always lead to desire: Even Pfizer had a hard time grasping that concept. The company tested 3,000 women over the course of eight years before finally abandoning hope, in 2004, that Viagra itself could be the female Viagra.

"What we know is that very little of what's going on with women and sex is below the waist," says Anita Clayton, a professor at the University of Virginia's Center for Psychiatric Clinical Research and co-author of Satisfaction: Women, Sex and the Quest for Intimacy. "Almost all of it is above the neck."

Which gets us to another complexity: If lust is "above the neck" for women, how do you measure it? But that's hard to define, and it doesn't necessarily translate as "orgasm."

So pink Viagra must clear a higher hurdle than Viagra ever did — it has to spark desire that also leads to satisfying events. To Clayton, it seems as if the government has set a standard that is unreasonably high.

"It's as though the government is worried women will be turned into nymphomaniacs," she says. "Look, the FDA blocked the morning-after pill for ages. Why? Either they're worried we'll be bad girls, or they don't understand women's sexuality."

A number of drug companies say they can clear the FDA's bar. In January, clinical trials began across the country for LibiGel, a testosterone gel made by BioSante. Women rub a dab of LibiGel into their upper arms once a day. (It builds up testosterone levels over the course of months.) A German company, Boehringer Ingelheim, discovered that a drug it developed for depression didn't lift anyone's mood but boosted desire in women. It hopes to win FDA approval in 2009.

The sooner the better, say many physicians in the field of sex medicine. They describe FSD as a silent epidemic. Studies have found that most women who are distressed by their sex lives are too embarrassed to discuss it with their doctors. Those who do speak up are often told they need a shrink.

"When I started this field in the '70s, 90 percent of the treatment was psychological," says Irwin Goldstein, director of sexual medicine at San Diego's Alvarado Hospital. "It was just assumed that every couple needed therapy."

The success of Viagra changed that. It redefined impotence as "erectile dysfunction" and revealed that a lot of men didn't need to plumb their psyches at all; they needed greater vasodilation of the corpus cavernosum. In other words, they needed a pill.

It contradicts all the cultural beliefs we have about the way men are and/or are supposed to be, but the dirty little secret is... American men are flagging in their desire for sex.

"Men are so ashamed of speaking up about low sexual desire," observes Michele Weiner-Davis, a marriage therapist from the Chicago area. It violates their own sense of masculinity. But "low desire in men is America's best-kept secret," she says, and estimates that it affects "at least 20 to 25%" of adult males.

For women, the figure is thought to be much higher, somewhere between 40 and 50%. A woman ducking out of sex, the headache thing, "is as American as apple pie," says Weiner-Davis. It's a staple of every comedian's routine.

But it strikes terror into the heart of a guy to even think he might not be interested, because his sense of self is usually tied up in his virility. So no one has real information on just how many men are affected.

Nevertheless, there appears to be a great and growing gap between the reality of the current state of male desire and the cultural mythology surrounding it. Men are more and more having it less and less. Weiner-Davis is seeing it among the couples who turn up at her door for help.

And their low sex drive often has little to do with hormones or biology and a lot to do with the women in their lives. Men today, often enough, are angry at their wives.

The first inkling that something unusual was going on in the bedroom occurred some years ago, Weiner-Davis reports. "I was working with a couple that wasn't making much progress in their relationship. The husband, a high-powered attorney, said in an offhand way, 'I guess we don't really touch that much.' My immediate thought was that the wife wasn't interested. But he said, 'No, actually it's me who isn't interested.'"

When she asked him what that was about, he said, "You know, my wife is so critical of me. And she hurts my feelings. She finds fault in everything that I do. I just don't want to be anywhere near her."

What's happening, Weiner-Davis says, is the couples are working hard in the office. And women are also working hard at home. And they are getting on their husband's cases. "In theory, she says, "women are equipped with the language to ask for change. But they don't; instead, they bitch."

They don't express appreciation for what their husbands see as their own contribution of hard work to the family. And it's emasculating.

Instead of saying "I'd really like to spend more time with you," or "I really enjoy your company and the last time we went to a movie together I really had a good time," the husbands more often hear: "You never want to do anything."

And that can shut off sexual desire as quickly as a terrorist attack.

Whether lack of desire originates with husband or wife, the end result is the same. There is a lack of physical contact, which is experienced by the other partner as the ultimate rejection.

"When one partner is yearning for more physical closeness and touch, and the other spouse is too preoccupied, too stressed or too angry, it's a big deal," Weiner-Davis insists. The sex-starved marriage is really all about feeling wanted.

In the presence of a mismatch of desire, all intimacy drops out on all levels in addition to the sexual. Couples stop having meaningful conversations. They wind up at risk of infidelity and divorce.

So, not surprisingly, Weiner-Davis has some counsel for couples in relationships marked by mismatched desire. It basically comes down to what she calls the Nike approach: JUST DO IT!!! This is her advice for the low-desire spouse, and admittedly it's provocative.

She points out that the quickest way to change feelings is to take action, that most people have to make things happen. We know this, and take action, in other areas of our life, like exercise. But somehow we make sexuality a forbidden zone, outside the laws of mortals.

For most people, desire doesn't just happen by itself. The way to get people moving is to take action. As the Italians say, the appetite comes while eating.
