Botox Safety Update By Mark R. Pulera M.D.
As you may know, the FDA (Food and Drug Administration) has recently released information regarding the safety of Botox. In order to understand this information, one must realize that Botox is and always has been a potent medication only available by prescription under a doctor’s supervision. Botox is prescribed at a wide range of doses for a wide variety of clinical conditions. Botox-related compounds have been medically studied for at least 100 years, and Botox has been approved for medical uses by the FDA for 18 years. Botox has been FDA approved for the cosmetic treatment of moderate to severe frown lines between the brows in adults for 8 years. Since 2002, over 13 million people have received Botox Cosmetic treatment. Reports of serious side effects are rare, but just like with any prescription medication, serious side effects including death are possible with treatment. Allergan, the manufacturer of Botox, maintains that no deaths have ever occurred with Botox treatment for cosmetic reasons. It should be noted that “Botox” and Botox Cosmetic” are the same medications marketed under different names by Allergan. The consumer group, Public Citizen issued a statement on January 24, 2008 petitioning the FDA to issue stronger warnings to patients and doctors about the use of Botox because of serious side effects including death. Public Citizen disputes Allergan’s assertion that no deaths have been caused by Botox Cosmetic. We do not have the details of the disputed case at this time. However, the FDA released a communication regarding an ongoing safety review of Botox on February 8, 2008 which can be found at www.fda.gov/medwatch/safety/2008/safety08.htm#botox. The FDA reviewed Botox treatments in variety of pediatric and adult patients and found a number of pediatric cases with serious adverse events including death. These adverse events were found most commonly in children with cerebral palsy who received relatively high does of Botox for limb spasticity. The doses were approximately 10 to 50 times hight than a usual dose of Botox used for cosmetic purposes. Among the adult cases of Botox treatment systemic effects after injection such as weakness or numbness were noted, however no deaths were reported. The doses of Botox used in these cases were approximately 2 to 15 times higher than Botox used for cosmetic purposes. The release of this communication from the FDA regarding the safety of Botox does not mean that a causal relationship has been established between Botox and the adverse events described. Nor does it mean that the FDA is advising healthcare professionals to discontinue prescribing these products. The FDA will provide further updates which it has reached a conclusion regarding this issue. In the meantime, we should all be aware of the potential systemic side effects of Botox treatment including swallowing difficulty, speech difficulty, breathing difficulty or weakness. At Malana Medical Spa, we have always been aware of the potent nature of medications such as Botox including the possibility of serious adverse reactions including death. We always have used our understanding of the risks of Botox to minimize potential untoward side effects in all of our treatments. We agree with notion that all healthcare professionals who administer Botox must have a thorough understanding of Botox in order to maximize patient safety. Anyone who desires Botox treatment should realize that although treatment is extremely safe in the hands of a knowledgeable healthcare professional, there are risks to any medical treatment. We would not offer Botox or any treatment that we did not feel to be completely safe and effective, and we will continue to treat ourselves and patients with Botox as long as the evidence continues to support its safety. SO I ASKED ABOUT IT AND THE PROCEDURE SEEMED SIMPLE ENOUGH. I DIDN'T REALLY LIKE THE NERVE BLOCK, BUT ITS BETTER THAN NOT HAVING IT AT ALL. I THINK EVERYBODY FINDS NUMBING TO BE A STRANGE FEELING, BUT OTHER WISE IT WAS FINE. MY LIPS WHERE SLIGHTLY SWOLLEN FOR ABOUT A WEEK . MY LIPS LOOK LIKE THEMSELVES BUT FULLER, MORE NATURAL WHICH IS WHAT I WAS LOOKING FOR. NO DUCK LIPS HERE!! I HAD SLIGHT BRUISING, BUT I'M ALSO VERY FAIR SKINNED, SO NO SURPRISE THERE. I have been getting Botox injections for the past two years and have not been completely satisfied with my results. My results have either looked unnatural or did not give me the look I was hoping to achieve. A friend recommended that I go to Malana Medical Spa and pay a visit to the Nurse, Stacy who performs "the best" injections she had ever had. After an in depth consultation, in which I was educated for the first time and given an honest answer about desired results, I was injected by Stacy. First of all, she was so gentle and the injections were painless. Secondly, she took her time and iced the areas before injecting me. Lastly, a full 10 days later the complete results are beautiful. It is not overdone and she proved to really know where and how much I needed for my particular needs! Thank you Stacy for the best BOTOX I have ever had.

My lips have always been a bother to me. They were always plain. I called them
vanilla lips, never very exotic or intriguing. So, of course, there was something that had to be done about that. I have always been an avid fan of all beauty magazines. One day I came across a glossy advertisement that made me envious and propelled me to seek the lips of this goddess. So with the magazine in hand and the excitement of finally achieving my beautiful lips I showed the ad to Dr. Pulera. Showing Dr. Pulera exactly what I wanted I put the immensely comfortable white Malana robe and sat in the treatment room as excited as ever. Listening to the exotic sounds of the Hawaiian music in the background and the sweet scents emanating from the candles made me so relaxed. Then the doctor came in and explained that I would be injected with anesthesia throughout my mouth. Expecting the shooting pain to come, I closed my eyes and opened my mouth. To my surprise I felt nothing but a tiny poke when the needle went in. Shortly after my whole mouth became numb. Then Dr. Pulera began his artistry in sculpting Juvederm injections both to my upper and lower lips. I felt nothing. Then the mirror was handed to me so that I could analyze my lips. They were the exact lips that I wanted in the ad. I had the goddess lips!!!!!! So after about an hour or so the anesthetic wore off and I could not stop looking at them. Any chance I got I would run to the mirror. I glossed them up, and off I went that same night to a have dinner with friends. When I walked in I was met by stares and open mouths. They loved them and I loved them. After a week I began getting complements on how natural and full they looked. I love my new lips they complement my face and make me look so much better .... They are my goddess lips!

I am so excited about my results, I just have to tell everyone!
I recently bought laser hair removal packages at Malana Medical Spa. I am doing it all, legs bikini, underarms and that embarassing hair on my lip and chin. I tried another place in Los Angeles for Laser hair removal and their laser did not give me the results that I am getting at Malana. I have only done two treatments out of six so far and I have not shaved in three weeks! The treatments are easy to tolerate and I really like the Nurse too. She makes the experience really comfortable and the rest of the Staff is so nice and helpful.

Laser skin resurfacing is the latest technology to improve the appearance of scars, fine lines, wrinkles and poor skin texture. Let's give a quick overview of how this technology works.

Pixel laser skin resurfacing uses microthermal treatment zones. Every laser pulse creates tiny ablative zones that remove the epidermis and dermis. This triggers the body's own healing process and stimulates the growth of new healthy tissue surrounding the ablative zones. By treating smaller areas and leaving surrounding tissue intact, the skin heals quickly with dramatic results. An added benefit of Pixel treatment is also increasing collagen production, which will continue for several months after treatment.

Now that we have a better idea of how the Pixel laser works, what is it used for? The Pixel is great for wrinkles around the eyes and mouth. Dr. Pulera graduated from Mount Sinai School of Medicine 1993 and completed his Residency at UCLA School of Medicine in 1997. He is a Board-Certified Neurologist and has been in private practice in the Los Angeles area for over 8 years. Dr. Pulera’s interest in cosmetic medicine has developed from multiple scientific disciplines including mechanisms of cellular death, laser science and dermatology. Dr. Pulera has completed extensive, physician-supervised training in cosmetic laser treatment as well as Botox injections and dermal fillers.

Question: Does Malana Medical Spa provide any surgical cosmetic treatment?

Answer: No, all of our treatments at Malana Medical Spa are non-surgical. Our treatments include laser treatments, photofacials, fractional ablative skin resurfacing, Botox, dermal fillers, Cosmelan masks and European facials.

Question: Will my treatment cause any down time?

Answer: Most of our treatments such as IPL laser, Botox and Dermal fillers cause no undesirable visible changes in the skin. You can resume normal activities immediately after these procedures and no one will notice anything except your enhanced beauty! Treatments such as the Pixel laser can cause redness or peeling of the skin for several days after the procedure, but can offer the most dramatic results for conditions such as fine lines and wrinkles and poor skin tone and texture.

Question: Will my treatment hurt?

Answer: Some of our cosmetic procedures can cause minor discomfort. However, Malana Medical Spa utilizes several methods of pain control including topical anesthetic, skin cooling and nerve blocks. Our anesthetic treatments can completely eliminate any discomfort you may experience from our procedures.

Question: Are the treatments safe?

Answer: At Malana Medical Spa, your safety is our top priority. All of our staff has had comprehensive, physician-supervised training. We use only the most technologically-advanced equipment and latest techniques. We only perform treatments that we feel are completely safe and effective.
