Diploma Candidates of the 2023 Cohort

Micah Sales is located in Athens, GA. He works as a clinical director in the outpatient clinic of FYZICAL Therapy and Balance Centers, seeing primarily orthopedic and neuro patient populations. Gary Dykes, a senior instructor for the McKenzie Institute USA, was strongly influential in Micah’s pursuit of the MDT Diploma Programme.

Ray Peralta is located in New York City, NY and serves in an outpatient orthopedic clinic attached to NYU Langone Health Hospital. Ray also owns an online business (Running Pain-Free) which is concerned with injury prevention and sports performance with runners, integrating MDT principles in this endeavor. Noticing the patient care outcomes of his colleagues, Ray decided to pursue the MDT Diploma Programme.

Daniel Paggen is located in Durham, NC and works within an outpatient clinic. He primarily sees the orthopedic patient population. Daniel began his MDT journey in 2011.

Perceptions of the MDT Diploma Programme and Take-Aways

Daniel discusses the issue of balancing family and home life, work, and time away from the clinic to undertake the program. Micah reflects upon the organization and time management required of the programme with his full-time position as a clinical director.

Clinical Residency and Patient Cases

Ray alludes to a patient case in which he oscillated with his differential diagnosis between a lumbar derangement or an SI joint. Micah pinpoints the thought process of the McKenzie Method, underlining the clarity that he gained of it for the sake of efficient patient care. Daniel recalls a patient who, without the prior experience and guidance of his clinical instructor, Daniel could have worsened.

Approaching the Final Exam

Ray continues educating himself on the recent literature and applying studies for the purposes of the oral portion of the exam. Ray also utilizes the MDT Diploma Manual and Robin McKenzie’s “Treat Your Own…” series for preparation. 

Micah and Daniel confer within study sessions, reviewing McKenzie’s older texts, the Diploma Clinical Concepts, and research articles (all from which they generate their personal notes).

The Literature and its Impact on Patient Education

Daniel refers to the concept of centralization as a prognostic indication for a patient’s chronic pain that had successfully centralized throughout her care. Educating this patient on centralization, Daniel also provided illustrations of peripheralization relative to centralization.

Referring to chronic pain patients, Micah regards Mark Werneke’s studies pertaining to centralization and directional preference. Micah also mentions Richard Rosedale's Extremity Pain of Spinal Source (EXPOSS) publications. Jason highlights the shared decision-making cultivated within the therapeutic alliance.

Psychosocial Aspect of the Biopsychosocial (BPS) Model

Regarding the contextual and cognitive-emotional drivers of pain and disability, Ray incorporates the Yellow Flag Risk Form (YFRF) into his patient intake, as well as the Modified Oswestry Low Back Pain Disability Questionnaire (MOLBPDQ) and STarT Back Tool.

Meet Clinical Instructor Chris Chase

Graduating in 1999 from St. Louis University, Chris earned his Master’s Degree in Physical Therapy. Chris became credentialed in the McKenzie Method in 2002; and he completed his diploma training in 2009. Chris soon joined the mentoring aspect of the MDT Diploma Programme, officially mentoring as an assistant diploma educator (2010) then appointed to Teaching Faculty with the McKenzie Institute USA (2012). Chris is now a Senior Instructor.

Structure of the Clinical Residency in Austin, TX

The clinical residency is a hospital-based clinic. It entails a total of 360 clinical hours, and this time is divided into sessions of manual therapy practice, tutorial themes and topics of the Diploma Manual, and, primarily, direct patient care (e.g., clinical reasoning, performing evaluations, performing follow-up visits, and discharging patients) under the supervision of clinical instructors Chris Chase or Kim Green. Roughly 60-70% of patient care is spinal; and the remainder is extremity.

“[We focus on] using the McKenzie Method at not only the credentialing level, but at the highest diploma level. You can’t [attain this] from just reading the books or attending the classes. You have to have somebody over your shoulder watching you make decisions [as the instructor] performs clinical mentoring/tutoring [with the diploma student] in a collaborative environment.” –Chris

Preparation for the Final Exam

Chris and Kim prepare diploma candidates for four verbal subparts of the final examination (lumbar spine, cervicothoracic spine, managing extremity patients, and research and other topics), creating most of their tutorials in the same fashion as the diploma exam.

Chris Expands Upon MDT Assessment, Classification, and Treatment

“If you have a strong sense of ‘I have my classification based on my initial history and baselines,’ then it’s okay to begin treatment; but know that you’re still always on that first visit critically analyzing ‘are the responses making sense?’ and ‘am I getting a response that I’m comfortable with [to continue progressive forces]?” –Chris

“A lot of people in the Mckenzie Institute [USA] think that we, [clinicians], don’t really want to use hands-on very much… I don’t actually use my hands to manage many patients, but I use them a lot in the evaluative process…for overpressure to learn. [Hands are] another tool to assess our patients when I don’t have a clear answer.” –Chris

“Ultimately, if you have a good knowledge base, then you can start to progress with management. If you have questions that need to be answered, choose the movement or testing strategy that’s going to answer the critical questions.”

“Don’t make any assumptions; pick what you’re going to learn from.” –Chris

The Ideal Diploma Candidate

Chris lists candidates’ most common reasons for pursuing and undergoing the MDT Diploma Programme:

• To be appointed to Teaching Faculty

• To improve their research of the McKenzie Method

• To be a better clinician for their patients, and a better mentor to their colleagues

The candidates who typically do well in the program, Chris asserts, are those who declare:

“I want my patients to get better–as good as the people I’ve seen on these courses.”

Meet Todd Edelson and Diploma Candidate Tim Davis

Tim Davis currently works in a private practice outpatient clinic in Montana. His hopes for the clinical residency include improved communication with the patient and efficiency in clinical care.

Todd Edelson is a diplomate of the McKenzie Method of Mechanical Diagnosis and Therapy. Beginning in 2003, Todd has been directing the MDT certified clinic Montclair Physical Therapy in New Jersey.

Todd’s Diploma Clinical Residency

Montclair Physical Therapy is a primarily direct-pay private practice (services may also be covered by Medicare insurance). Patients’ motivation, therefore, typically differs from that which is observed in an insurance-based physical therapy clinic. 

Quotes:

“We train our patients to be independent rather than dependent, which is [an essential facet of the McKenzie Method].” –Todd

Clinicians also provide a 1:1 service to prevent double-booking, and every evaluation and follow-up is one hour and thirty minutes, respectively.

Todd continues:

“There’s plenty of time for the patient, and there’s plenty of time for us–the clinician and diploma candidate–to really go through a meaningful thought process. It’s not rushed.”

During any given residency, Todd has supervised one or two diploma candidates. In both cases, he has observed a general contentment of a single candidate or two candidates who share their clinical residency. In the latter case, Todd pinpoints the significance of personalities interacting and how well-meshed they are in the collaborative process.

“The benefit of having two people is that they can [collaborate]. They help each other: if I ask a question and one doesn’t know, then the other comes in like an older sibling to help out.”

How to Be a Life-Long Learner: Todd’s Advice for the Diploma Candidate

Todd refers to Robin McKenzie’s quote (“My patients taught me everything I know”), highlighting the importance of the patient in the clinician’s continued learning and education.

“Listen to the person [who’s] sitting in front of you, because [he/she] knows more about [his/her] problem than anybody [else].” –Todd

“[The patient] is going to have plenty of time to tell [the clinician] his/her story, and we need to hear it.”

An Enlightened Moment in a Patient Case

Tim recalls a moment in the evaluation process in which he conducted a ~20-25 min. history intake before noticing the patient’s right lateral shift upon standing. Reflecting on this, Tim presumed that he would have been aware of the shift if he’d observed the patient walk into the clinic prior to the evaluation, consequently saving time during the evaluation and efficiently correcting the shift.

In episode 368 of the Mechanical Care Forum, we’re beginning season 11, highlighting actual patients who have a remarkable story and noteworthy results through a thorough movement and symptom assessment and treatment process and when possible we have their therapist join us too. We’re allowing the experiences and perspectives of these patients to reinforce the potential that exists with a proper mechanical exam and through a sound biopsychosocial approach for common musculoskeletal conditions. Our first patient is Makesh and he’s accompanied by his physical therapist Dr. Rhad Brown. Makesh struggled with persisting leg pain, sciatica as it's commonly called, which failed to resolve with conventional medical care. You’ll hear him describe a previous episode of leg pain involving his other leg and how that led to surgery, what he had been told about this new condition and how he had little hope that he might be able to avoid a second surgery. He’ll share how he discovered he could get control over his pain and, with some education from Dr. Brown and his own stick-to-itiveness, he is fully recovered and staying that way more than 2 years later. For Makesh, he tried the McKenzie Method of Mechanical Diagnosis and Therapy and it worked like magic.

In episode 344 of season 9, we’re shifting our conversation once again this season to dizziness and balance with Ms. Bridget Sholljegerdes. She shares how she found herself working with this patient population, the primary mechanisms she understands these symptoms to be coming from, some patterns of what effectively resolves these symptoms, and more. Bridget Scholljegerdes is a licensed physical therapist and clinical director at Fyzical Therapy and Balance Center in Las Cruces, New Mexico, where she has been employed for eleven years; all the while, working alongside owner and fellow physical therapist and MDT diplomat, Denise Campbell. After graduating in 1999 with a bachelor’s degree in health science from the University of Missouri-Columbia, she has had the opportunity to work in several states including: Wyoming, North Dakota, and New Mexico.  Bridget was introduced to mechanical diagnosis and therapy (MDT) in 2010 and became certified in MDT in December of 2014. In addition to MDT, she also specializes in complications with balance and gait, vestibular and vertigo disorders, diaphragmatic breathing, and sacroiliac joints, to name a few. 

Bridget also owns Elevate Gymnastics and Athletics in Las Cruces, New Mexico where she is the head coach and executive director. She has been a gymnastics coach, level 10 judge, and professional member with USA gymnastics for the past twenty years. She is also a member of the National Association of Women’s Gymnastics Judges (NAWGJ) state board. 

In her spare time, she enjoys spending time with her husband and daughter, exercising, and participating in triathlons, as well as traveling domestic and abroad.

Dr. Krauss is an Associate Professor in the School of Health Sciences at Oakland University in Rochester, Michigan. He completed his Baccalaureate of Science degree in Physical Therapy in 1990 at Oakland University. Dr. Krauss received his Orthopedic Manipulative Therapy Certification through the International Seminar of Orthopedic Manipulative Therapy in 1994. He completed his orthopedic residency training and earned his Graduate Certificate in Orthopedic Manual Physical Therapy and Master of Science degree in Physical Therapy through Oakland University in 1995. Dr. Krauss is a Board Certified Orthopedic Specialist through the American Physical Therapy Association and is a Fellow of the American Academy of Orthopedic Manual Physical Therapists. Dr. Krauss completed his PhD in Physical Therapy at Nova Southeastern University in 2005. Dr. Krauss has presented at the state, national, and international levels, authored and coauthored peer-reviewed papers, and authored and coauthored book chapters, Dr. Krauss primary research focuses on the efficacy of manual therapy and orthopedic intervention in the management of spinal and extremity disorders across the lifespan.

Certification of Functional Manual Medicine

Basic Concepts

The osteopathic profession subscribes to the following tenets:

• The body is capable of self-regulation, self-healing, and health maintenance.

• Illness is often caused by mechanical impediments to normal flow of body fluids and nerve activity.

• The body is a unit, and the person represents a combination of body, mind and spirit.

• The body is capable of self-regulation, self-healing, and health maintenance.

• Structure and function are reciprocally interrelated.

• The human body is a dynamic unit of function.

Somatic Dysfunction may be diagnosed by the presence of some, but not necessarily all, of the following:

• Tissue Texture Abnormalities -- Surrounding tissues may feel hypertonic, rigid, fibrosed, atrophied, or edematous.

• Asymmetry of Bony Landmarks and/or Function -- Bones, muscles, or joints may lack symmetry when compared to related structures.

• Restriction of Motion -- A joint with a somatic dysfunction will have abnormal characteristics of motion. During the diagnostic process you should be constantly asking yourself the following four questions:

  1. Is the total range of motion symmetric?

  2. Is the total range of motion reduced from normal?

  3. What is the quality of the motion?

  4. What is the characteristic feel at the end point of motion?

• Tenderness to Touch -- Pain may result from palpation of tissues associated with somatic dysfunction.

In episode 316 of season 8, we’re joined by Dr. John Childs, the founder and CEO of Evidence in Motion (EIM). He shares a bit if his professional journey, some of the background of EIM, it’s primary tenants, his thoughts on continuing education, dogma, gurus, and more. This week on MCF!

Dr. Childs is a founder and CEO of Evidence In Motion and partner in Confluent Health, which includes EIM; a network of 70+ physical therapy clinics in Indiana, Kentucky, North Carolina, Oklahoma, and Texas; and Fit for Work, which provides employers consultative services to decrease injuries and workers’ compensation costs.

A graduate from the U.S. Air Force Academy with a BS in biology (1994), he completed his MPT from US Army-Baylor University (1996), MBA from the University of Arizona (2000), Master of Science in Musculoskeletal Physical Therapy from the University of Pittsburgh (2002), PhD in Rehabilitation Science from the University of Pittsburgh (2003), and DPT from the Evidence in Motion Institute of Health Professions (2010).

Dr. Childs is a board-certified Orthopaedic Clinical Specialist from the American Board of Physical Therapy and a Fellow in the American Academy of Orthopaedic and Manual Physical Therapists. A professor in the Doctor of Physical Therapy Program at South College and Associate Professor in the US Army-Baylor Doctoral Program in Physical Therapy, Dr. Childs served a 20-year career in the Air Force, having collaborated on more than $10 million in grant funding and published more than 150 papers in leading scientific journals. He currently serves as an Associate Editor for both the Physical Therapy and Journal of Orthopaedic and Sports Physical Therapy journals. Dr. Childs was recently recognized as a “Health Care Hero” in San Antonio and is the youngest ever Fellow of the American Physical Therapy Association, the profession’s highest honor recognizing individuals for professional excellence and making a profound impact in advancing the profession.
