MedCo Services has always taken that extra step to help patients find a way to meet their financial obligations to our healthcare clients. We understand that our clients have a community to care for. A good financial return and positive community relationships are paramount to maintain patient quality of care. This due diligence approach to each account will increasingly become a greater asset over the next few years. We have observed that under the ACA model not only are our client hospitals under increased financial pressure, but so are the patients they care for. We hear that across the nation insurance premiums are increasing, as well as the deductible and out of pocket share on employer based insurance. For example, the policies for the State of Georgia’s 650,000 employees now has a deductible of $2,500 individually and $5,000 for family coverage. This is in-network health care. Out-of-network healthcare has a $10,000 deductible. MedCo would like to share some statistical information from the CDC, and a focus study reported by Kaiser Family Foundation. The CDC studies show 1 in 3 Americans, or 104.6 million people, are affected by healthcare debt. 1 in 5, or 62.7 million people, have to pay this […]

The early out process is an integral part of your hospital’s revenue cycle. In an ideal world, patients would all make their co-pays and deductibles at or before the time of their procedure; as we know, this almost never happens. With the changes in healthcare reform we continue to see deductibles increase leaving more responibility left to the patient and ultimatly more responsibility on the Early Out company to collect while keeping patient satisfaction scores up. Some may wonder why so many upfront dedicatubales are going unpaid… it’s not always the patient’s fault; they may not know ahead of time what their co-pay or deductible is going to be and the hospital may not have the tools to help them identify what the amount actully is. Add to that the fact that the billing system in many hospitals is woefully inefficient, and you have a recipe for lost revenue and high early out balances. Here are some key factors in how the early out process is serving hospitals today: The early out process maintains patient interaction. This is critical. The more points of contact the hospital has with a patient post-procedure, the more likely the hospital is to collect. Left by […]

Most facilities can never get patients registered more than 1-2 days out because the work load is so massive and cumbersome. For a 400-600 bed facility there are likely 200-400 patient contacts that need to be made each day just to try and keep pace with the ever-increasing needs of Pre-Registration. Like most departments, staff and management are being asked to do more with less. This has created a huge burden for most providers, as this part of the revenue cycle is becoming crucial to ensuring insurance claims and patient balances are paid the first time they are submitted, otherwise a backlog of denials start stacking up. If we can be honest about the issues facing Pre-Registration staff, I think we will identify the most pressing issues within this department. Staffing Problems Pre-Registration staff are generally at the lower end of the pay scale, comparable to an entry level employee at most fast food chains or department stores. Yet these employees have monumental responsibilities pertaining to the success of the revenue cycle. Many Pre-Registration staff have to access multiple systems while interacting with a patient, interpret complicated and confusing insurance benefits, and sometimes handling complaints from the patient’s previous […]

The Office of E-Health Standards and Services (OESS) from the Centers for Medicare and Medicaid Services (CMS), Baltimore, has delayed the enforcement of HIPAA Version 5010 through June 30 for covered entities that are required to comply with the updated transactions standards, according to a new release issued by the American Physical Therapy Association (APTA), Alexandria, Va. While the new standards went into effect January 1, OESS announced in November 2011 that it would delay enforcement until the end of March 2012. This was an effort to allow covered entities to complete outstanding implementation activities, including software installation, testing, and training. The enforcement discretion period has now been extended an additional 3 months, although APTA reports that covered entities have been making steady progress toward this final goal. Through May of this year, OESS plans to step up its existing outreach to include more technical assistance for covered entities. It is also reportedly participating with industry groups, the Medicare fees-for-service program (FFS), Medicaid in order to help expand technical opportunities and eliminate any remaining barriers. APTA recently launched a new Web site that can serve as a resource to help physical therapists transition to HIPAA Version 5010. The site reportedly […]

Despite various efforts to reduce emergency department (ED) crowding–posting wait times, diverting patients–visits to the ED jumped 10 percent in one year to a new high of more than 136 million visits in 2009, according to preliminary data from the Centers for Disease Control and Prevention. Ninety-two percent of ED patients were considered urgent and more than 85 percent were insured, according to an American College of Emergency Physicians (ACEP) press release. In addition to a growing demand for healthcare services, emergency physicians credit the upsurge in visits to enhancements that enable EDs to treat patients faster, reports the San Francisco Chronicle. They also maintain that deterring patients from going to the ED for care won’t cut back on healthcare costs. “We’re efficient. We take care of acute patients and that’s what we do well,” Dr. Paul Kivela, managing partner of Napa Valley Emergency Medical Group and ACEP board member, told the Chronicle. Meanwhile, a new ACEP study examined why patients choose to go to the ED for minor complaints over their primary care provider (PCP). Many of the patients perceived their complaints to be urgent (33.4 percent) or were referred to the ED by their PCPs (33.4 percent). Others […]
