&lt;/div&gt;&lt;div style="color: blue; font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;"I am xxxx xxxx from the India at Maharashtra. I am working in the Business transcription company as Operation Associate. In the XXXX systems pvt. It is time we think about our education system.  This is just&amp;nbsp;&lt;/span&gt;&lt;/pre&gt;&lt;pre style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;an example of what is seen almost every where and every day.  &lt;/span&gt;&lt;span style="font-size: small;"&gt;The so called graduates&amp;nbsp;&lt;/span&gt;&lt;/pre&gt;&lt;pre style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;not able to even write a decent resume by themselves.&lt;/span&gt;&lt;/pre&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt; &lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;The education institutions are meant for generating a good and capable generation.&amp;nbsp; It starts right from the politicians (don't know if they really know the meaning and importance of Education).&amp;nbsp; The educational institutes have the prime interest in profitability.&amp;nbsp; Lastly, the teachers (with due respect to them).&lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;The politician.&amp;nbsp; He better be a politician and not interfere in the education as he is never educated enough to decide what students should be taught.&amp;nbsp;&amp;nbsp; He is normally educated in a field which is not wanted by the society (antisocial activities).&lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt; &lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;Educational Institutions.&amp;nbsp; They sure need to see the profitability to survive.&amp;nbsp; They are restricted by the politicians to recruit according to caste/creed and not by the brain or aptitude or knowledge base.&lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt; &lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;A teacher is a person who is the main source of how the next generation is going to be.&amp;nbsp; A teacher should be the highest paid person and the best and/or an authority in the field he teaches.&amp;nbsp; But the reality is the one who comes in the teaching line is the one who comes in this field as a last alternative.&lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt; &lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;The parents also need to think.&amp;nbsp; Their duty is not just to give birth and blame others for not making their child a capable human being.&amp;nbsp; The child is in the school for only 6-8 hours in a day.&amp;nbsp; So he is with his parents for a longer period.&amp;nbsp; The parent's prime need in life is money and not the future of their child.&amp;nbsp; They live in a world where others are to make their child a good human being because they gave birth to that child.&amp;nbsp; They teach the teachers how to teach their child.&amp;nbsp; Not to scold them, not punish them even if they make mistakes, promote them to next level even if the child has not learned enough.&amp;nbsp; All they want is a Degree embossed on their child's forehead.&lt;/span&gt;&lt;/div&gt;&lt;div style="font-family: Verdana,sans-serif;"&gt;&lt;span style="font-size: small;"&gt;&lt;br /&gt;

The bill was then sent to the President for his signature. Immediately after that vote was taken, the House passed changes to the Senate passed bill known the "Budget Reconciliation" bill (H.R.4872) by a vote of 220 to 211. The Senate will now consider this package of changes this week, which will shape the final outcome of the healthcare reform program.&lt;br /&gt;
This healthcare bill is a comprehensive overhaul bill that increases coverage to 94% of Americans and makes sweeping changes to our current healthcare system. There are new provisions in the bill for both individuals and employers. Although the bill is comprehensive, we address the changes that will affect small employers and their employees.&lt;br /&gt;
&lt;b&gt;Individual Requirements:&lt;/b&gt;&lt;br /&gt;
The bill requires all U.S. citizens to obtain the minimum essential coverage or pay a penalty of $95 in 2014, $325 in 2015, $695 in 2016 and $750 in 2017 and indexed thereafter. There will be a penalty cap of $2,250 per family.&lt;br /&gt;
Exemptions to the penalty will be granted for financial hardship, religious objections, and those with income below the tax filing threshold (in 2009 the threshold for taxpayers under age 65 was $9,350 for singles and $18,700 for couples).&lt;br /&gt;
Beginning in 2018, there will be a 40% excise tax on individual high-cost health plans costing more than $10,200 for individuals and $27,500 for families.&lt;br /&gt;
&lt;b&gt;Employer Requirements:&lt;/b&gt;&lt;br /&gt;
&lt;b&gt;Employers under 50 employees:&lt;/b&gt;&lt;br /&gt;
Exempt from required coverage and penalties.&lt;br /&gt;
&lt;b&gt;Employers more than 50 employees who do not offer coverage:&lt;/b&gt;&lt;br /&gt;
Employers with over 50 employees that do not offer healthcare coverage will be required to pay a fee of $2000 per total number of full-time employees if they have at least one full-time employee receiving a tax credit for premium assistance. There is a transition provision that exempts 30 employees from the total number of full-time employees for purposes of this calculation.&lt;br /&gt;
&lt;b&gt;Employers with over 50 employees who offer healthcare coverage:&lt;/b&gt;&lt;br /&gt;
Employers with over 50 employees who offer healthcare coverage must pay at least 60% of their employees’ premiums or employee premiums payments cannot exceed 9.5% or more of their income to avoid fees. Fees may be imposed at the rate of $3,000 per employee who receives a tax credit for premium assistance.&lt;br /&gt;
How does an employee qualify for a premium assistance?&lt;br /&gt;
• An individual is eligible to receive a tax credit for premium assistance if their employer pays less than 60% of the premium cost or if the employee share of the premium exceeds 9.5% of their income.&lt;br /&gt;
&lt;b&gt;Employers more than 200 employees:&lt;/b&gt;&lt;br /&gt;
Employers with 200 or more employees will be required to automatically enroll new full-time employees in coverage.&lt;br /&gt;
&lt;b&gt;Insurance Market Reforms:&lt;/b&gt;&lt;br /&gt;
Beginning in 2014, the bill puts in place reforms to the current small group insurance market and changes the way insurers determine eligibility. In addition, there will be a “market shift” to pool the self-employed, non-profits and small businesses into the same group under new marketplaces called exchanges.&lt;br /&gt;
These market reforms include:&lt;br /&gt;
&lt;ul&gt;&lt;li&gt;Individuals and small businesses will be guaranteed health coverage and will have the ability to renew coverage annually&lt;/li&gt;
&lt;li&gt;Prohibits insurers from basing eligibility on: pre-existing conditions, health status, medical condition, claims experience, receipt of health care, medical history, genetic information, disability, and evidence of insurability&lt;/li&gt;
&lt;li&gt;Prohibits lifetime or annual limits on benefits&lt;/li&gt;
&lt;li&gt;Prohibits deductibles greater than $2,000 for individuals and $4,000 for families&lt;/li&gt;
&lt;li&gt;Requires insurers to expend no less than 85% on medical coverage&lt;/li&gt;
&lt;li&gt;Coverage for non-dependent children up to age 26&lt;/li&gt;
&lt;/ul&gt;&lt;b&gt;What’s in it for Small Business?&lt;/b&gt;&lt;br /&gt;
&lt;b&gt;New Marketplaces to Shop for Healthcare Coverage:&lt;/b&gt;&lt;br /&gt;
By 2014, all 50 states will be required to establish state-based exchanges. These state-based exchanges, or marketplaces will be offered to employees of small businesses, the self-employed and individuals.&lt;br /&gt;
These pools will be run by states. States may form regional exchanges or allow more than one exchange to operate in a state.&lt;br /&gt;
These state-based exchanges will also include private insurance plans, multi-state health plans, and Consumer Operated and Oriented Plans (CO-OPs). There will be no public option, however, each state exchange will be required to offer at least two multi-state health plans negotiated by Office of Personnel Management (OPM). CO-OPs are non-profit, member driven and owned health insurance issuers that will offer plans through the exchanges.&lt;br /&gt;
&lt;b&gt;Small Business Eligibility for the Exchanges:&lt;/b&gt;&lt;br /&gt;
Individuals, the self-employed and small businesses up to 100 full-time employees will be eligible to obtain coverage through state exchanges. However, before 2016, a state may limit pool to businesses up to 50 employees. Beginning in 2017, the state exchanges will expand to small businesses more than 100 employees.&lt;br /&gt;
It is important to note that small employers eligible to obtain coverage in an exchange who grow beyond the employee limit will continue to be treated as a small employer and eligible to participate in their qualified health plan until the employer decides not to enroll in that plan.&lt;br /&gt;
&lt;b&gt;Benefits Under the Exchanges:&lt;/b&gt;&lt;br /&gt;
The exchanges will include private insurers that offer four benefit categories of qualified health plans. The four levels of coverage for benefits will be: bronze, silver, gold and platinum.&lt;br /&gt;
• Benefit levels include:&lt;br /&gt;
Bronze plan covers 60% of the benefit costs of the plan&lt;br /&gt;
Silver plan covers 70% of the benefit costs of the plan&lt;br /&gt;
Gold plan covers 80% of the benefit costs of the plan&lt;br /&gt;
Platinum plan covers 90% of the benefit costs of the plan&lt;br /&gt;
• All insurance plans in the exchanges must include these services: ambulatory patient services, emergency services, hospitalization, maternity and newborn care, mental health and substance use disorder services, including behavioral health treatment, prescription drugs, rehabilitative and habilitative services and devices, laboratory services, preventive and wellness services and chronic disease management, and pediatric services, including oral and vision care.&lt;br /&gt;
&lt;b&gt;Increased Coverage Options:&lt;/b&gt;&lt;br /&gt;
&lt;br /&gt;
The bill will establish simple cafeteria plans to allow small employers and the self-employed to offer tax-free benefits.&lt;br /&gt;
Employers who offer insurance but not through the exchanges will be required to offer “free choice vouchers” to employees to purchase coverage through the exchanges.&lt;br /&gt;
&lt;b&gt;Assistance to Small Businesses to Reduce Coverage Costs: Tax Credits&lt;/b&gt;&lt;br /&gt;
The bill contains small business tax credits to help offset the cost of providing healthcare coverage to employees.&lt;br /&gt;
Starting in 2010, a three-year small business tax credit will be available to businesses up to 25 employees and average wages below $50,000. The full tax credit, 35% of the employer’s contribution toward the employee’s health insurance premium, would be available to small businesses with 10 or fewer employees with average wages below $25,000. Employers must contribute at least 50% of the total premium cost to be eligible for a tax credit.&lt;br /&gt;
In 2014, a tax credit will be available to businesses up to 25 employees and average wages below $50,000 who purchase health coverage for their employees through state exchanges. The maximum allowable tax credit will increase to 50% of the employer’s contribution, if the employer contributes at least 50% of the total premium cost.&lt;br /&gt;
The employer would cover the premium costs for their employees during the year and receive the tax credit at the end of the year when filing income taxes.&lt;br /&gt;
These small business tax credits will be applicable to tax exempt small businesses, 501(c)(3)s (i.e. charitable organizations), but the maximum credit will be limited to 25% of the employer’s contribution toward the employee’s health insurance premium in 2010-2013, and will increase to 35% in 2014.&lt;div class="blogger-post-footer"&gt;&lt;img width='1' height='1' src='https://blogger.googleusercontent.com/tracker/8169802247479498850-2631260112507189390?l=hemant-atpune.blogspot.com' alt='' /&gt;&lt;/div&gt;

Spell out the number or recast the sentence. &lt;/span&gt;&lt;/li&gt;&lt;li&gt; &lt;span style="font-family:MS Sans Serif,Arial,Helvetica;"&gt;Dictated as: 10 milligrams of Reglan was administered stat.  Transcribe as: Reglan 10 mg was administered stat. &lt;/span&gt;&lt;/li&gt;&lt;li&gt; &lt;span style="font-family:MS Sans Serif,Arial,Helvetica;"&gt;Exception - It is acceptable to begin a sentence with a date. Use semicolons to separate items which contain commas: &lt;br /&gt;Example: The patient was seen June 1;  February 4;  August 3, 2000;  January 2, 2001; and February 5, 2001. But with the recent bomb blast in the German Bakery in Koregaon Park, it certainly raises the question if it really still remains a Pensioner's paradise.&lt;br /&gt;&lt;br /&gt;Though the incident was cowardly act by the terrorist it was good to find that the victims got medical help very quickly with the nearby citizens coming to the rescue.  Thanks and hats off to their help and effort and the presence of mind shown to provide immediate help to the victims and injured.  The media though, made a show out of it with questions and remarks like idiots. To solve this problem, it took them one decade and $12 million.&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: bold;"&gt;And what did the Russians do...??&lt;/span&gt;&lt;br /&gt;&lt;span style="font-weight: bold;"&gt;They used a pencil.&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;CASE 2&lt;br /&gt;&lt;br /&gt;One of the most memorable case studies on Japanese management was the case of the empty soapbox, which happened in one of Japan's biggest cosmetics companies. The company received a complaint that a consumer had bought a soapbox that was empty. Immediately the authorities isolated the problem to the assembly line, which transported all the packaged boxes of soap to the delivery department. For some reason, one soapbox went through the assembly line empty. Management asked its engineers to solve the problem. Post-haste, the engineers worked hard to devise an X-ray machine with high-resolution monitors manned by two people to watch all the soapboxes that passed through the line to make sure they were not empty. No doubt, they worked hard and they worked fast but they spent a whooping amount to do so.&lt;br /&gt;&lt;br /&gt;But when a rank-and-file employee in a small company was posed with the same problem, he did not get into complications of X-rays, etc., but instead came out with another solution. He bought a strong industrial electric fan and pointed it at the assembly line. He switched the fan on, and as each soapbox passed the fan, it simply blew the empty boxes out of the line.&lt;br /&gt;&lt;br /&gt;&lt;span style="font-weight: bold;"&gt;Moral: Always look for simple solutions. Devise the simplest possible solution that solves the problems. Always focus on solutions &amp;amp; not on problems. While in the ER, she was examined, X-rated and sent home.&lt;br /&gt;17. The skin was moist and dry.&lt;br /&gt;18. Occasional, constant, infrequent headaches.&lt;br /&gt;19. Patient was alert and unresponsive.&lt;br /&gt;20. Rectal exam revealed a normal size thyroid.&lt;br /&gt;21. She stated that she had been constipated for most of her life, until she got a divorce.&lt;br /&gt;22. I saw your patient today, who is still under our car for physical therapy.&lt;br /&gt;23. Both breasts are equal and reactive to light and accommodation.&lt;br /&gt;24. Exam of genitalia reveals that he is circus sized.&lt;br /&gt;25.
