            <description>Abstract
Penile tumours are an important problem of male health affecting physical, mental and sexual health. Penile tumours can be subdivided into benign and malignant lesions. Their knowledge is important to prevent mutilating surgery in benign lesions. On the other hand, early recognition of malignancies is important for improved prognosis, and preservation of function. The most important tumour by epidemiology and prognosis is penile cancer. In contrast, malignant melanoma, sarcomas and lymphomas are rare. Clinical symptoms, histopathology and treatment options are discussed. Best possible treatment needs an interdisciplinary approach.             <description>We report a case of an 85‐year‐old man with a doughnut‐shaped basal cell carcinoma around the base of the penis invading the cavernosum urethra. In cases in which curative resection is planned, the penis is resected, cystostomy is performed, and postoperative self‐management is required. In line with the patient's wishes and the fact that he was an elderly person living alone without family support, we decided to perform penis‐conserving surgery. Relapse occurred once, for which additional resection was conducted. Two years after the resection, there was no evidence of tumor recurrence. The therapeutic plan for elderly patients should be modified after considering the patient's social background, wishes, activity and life expectancy. Primary urethral carcinoma (PUC) is a rare malignancy accounting for less than 1% of genitourinary cancers, with a predilection for men and African Americans. The sites and histology of urethral carcinoma vary by gender and anatomic location. Squamous cell carcinoma is most common among both genders but adenocarcinomas are noted in 15% to 35% of cases among women.
Obstructive or irritative symptoms and hematuria are common modes of presentation. Clinical evaluation includes cystourethroscopy with biopsy and examination under anesthesia. In each of these diseases, there are EGFR inhibitors that are approved for these conditions. EGFR is expressed in squamous cell penile cancer. The question: is it potentially useful in patients with squamous cell penile cancer? There are important parameters whether a target for treatment is important or not. Is the target over expressed, could it be inhibited, is it the predominant target, is it an important factor for growth for this cancer, and could it be given safely. Sixty‐six (22 pre‐cancerous and 44 cancerous lesions) tissue biopsies, received between 2004 and 2011 by the Anatomical Pathology Department at Dr. George Mukhari Hospital were selected for this study. Total DNA was extracted and genotyped using type specific real‐time quantitative polymerase chain reaction (qPCR) for 18 HPV types. Of 66 samples, only 51 were included in the analysis.     Abstract
    Data on the treatment of advanced penile carcinoma especially for chemotherapy are very limited. Due to the results of recent studies there has been a shift to taxan/cisplatinum based regimens. Initial case reports also showed a response to targeted therapy especially antibodies against epidermal growth factor receptor (EGFR). Nevertheless, a curative treatment by medicinal approaches alone does not appear to be possible for metastatic penile cancer and can be only cured by multimodal approaches, including neoadjuvant and adjuvant chemotherapy and salvage lymph node dissection. Palliative chemotherapy of advanced penile cancer has to be individually adapted and weighed against best supportive care for the final decision.

            <description>This study demonstrates the advantage of fusion technology in improving the cancer detection rate in a group of men in whom systematic 12-core TRUS guided biopsy often fails. We acknowledge that only patients with MR visible lesions underwent MR-US fusion biopsy, which could potentially cause bias. However, small lesions below the threshold of MR imaging most likely do not represent clinically significant prostate cancer (reference 29 in article). As mentioned, studies such as that of Matsuoka et al (reference 3 in comment), analysis from the fusion database reported by Kuru et al (reference 2 in comment) and others are essential to validate these results. Patients had preoperative prostate specific antigen greater than 20 ng/ml, Gleason 8 or greater, pT3b or a Mayo Clinic nomogram score of 10 or greater. Anterior PCa is less likely to be sampled by transrectal ultrasound guided biopsy, leading to an increased incidence of PCa in these regions after negative transrectal ultrasound guided biopsy (reference 9 in article). In the current series MP-MRI revealed an anterior cancer suspicious region in 29% of cases with no PCa on 12-core transrectal ultrasound guided biopsy, indicating the need for anterior biopsy to rule out PCa. This is oncologically sound in a period characterized by late presentation and high incidence of phimosis preventing early detection. Moreover, historical data labeled patients with penile cancer in lower socioeconomic classes with serious neglect of the first signs of cancer, necessitating an immediate solution, without too many choices. Changes in first presentation and patient characteristics, at least in Western Europe, led the way to less ablative surgery.
