The Navigator will be based out of three counties (Knox. Lincoln and Waldo) and will guide patients through the care continuum, establishing contact with newly diagnosed and recurrent cancer patients and their family members/caregivers. The Navigator will work closely with the health care systems within these counties, as well as outside healthcare facilities that the patient is referred to for care. The Navigator will actively identify and address barriers that might keep the patient from receiving timely and appropriate care for their cancer diagnosis and will connect patients with resources, health care and support services as needed. The Navigator will also participate in quality improvement initiatives and other activities evaluating the effectiveness of the regional navigation role. This is a grant funded position and the expiration date is April 2012. Although the Navigator will be a Maine Medical Center Cancer Institute employee the position is based in the region of the three counties, Knox, Lincoln and Waldo.<br> <u>Minimum Knowledge, Skills, and Abilities Required:<br> </u>1.
