<p><strong>Chiropractic for low back pain</strong> is suggested by the AMA and others. In a recent article published by the <i>Journal of the American Medical Association</i> (JAMA), it was recommended that patients looking for alternative treatment for low back pain first try chiropractic care before considering surgical procedures. The article states that many individuals can benefit from conservative chiropractic care, and that surgery is often only necessary when other more conservative treatments fail. Previous research has shown that chiropractic care for back pain is more cost effective than either surgery or drug therapy, and saves patients money. The first phase is often pain relief, and the second phase is to correct the problem. The goal is to help patients maintain a healthy, pain-free back. The information in the JAMA article reinforces the American Chiropractic Association’s (ACA) position, which states, “Conservative care options should serve as a first line of defense against pain.”</p>
<p>According to the Global Burden of Disease Study 2010, low back pain is the single leading cause of disability worldwide. Half of all working Americans admit to having back pain symptoms each year.</p>

<h2><b>The Causes of Back Pain</b></h2>
<p>The back is a structure composed of bones, joints, ligaments and muscles. <p>Samuel Moon, MD, MPH, associate director of education at the Duke Center for Integrative Medicine, weighs in on alternative treatments for back pain: <b></b></p>
<p>1) <strong><span style="text-decoration: underline;">Physical Therapy</span></strong>: Physical therapy for back pain can include the use of machines or traction, stretching exercises, or hands-on techniques.<b> </b>“The types of physical therapy shown to be most helpful for chronic back pain are hands-on soft tissue mobilization where the therapist moves the soft tissues of your back, as well as education about posture and home exercise,” says Dr. Moon. “Effective physical therapy should be goal oriented. It should always include learning what to do at home, and then doing it.”</p>
<p><b>2) <span style="text-decoration: underline;">Chiropractic Care</span>:</b> A chiropractic physician is a neuromusculoskeletal expert who is trained in the treatment and management of back pain. They use their hands to manipulate the joints and spine to help relieve the pain, and provide strategies for back-strengthening exercises and injury prevention. “Chiropractic is generally considered to be as good as physical therapy, and as good as patient education, in relieving back pain,” says Dr. Moon. “And, like physical therapists, chiropractors can be very good educators about back health.&#8221;<b></b></p>
<p><b>3)</b> <b><span style="text-decoration: underline;">Acupuncture</span></b><b>: </b>Acupuncturefor back pain involves inserting very thin needles into specific points on your body. This traditional Chinese therapy is thought to balance the flow of energy in your body, and is generally considered to be very safe. Dr. Moon commonly recommends acupuncture for back pain. “There is definitely some evidence that it&#8217;s effective,” he says.<b></b></p>
<p><b>4) <span style="text-decoration: underline;">Therapeutic Massage</span>: </b>Various massage techniques are used to relieve back pain. “Combined with other approaches, I think it can be useful,&#8221; says Dr. Moon.<b></b></p>
<p><b> </b><b>5) <span style="text-decoration: underline;">Mind-Body Therapies</span>: </b>Mind-body therapies for back pain include: Guided imagery, Hypnosis, Biofeedback, Relaxation therapy, and Talk therapy<b>. </b>Research has shown that these kinds of therapies can be effective in treating back pain. &#8220;For persistent back pain, I almost always recommend some type of mind-body therapy,&#8221; Dr. Moon says.</p>

<p>During this time of the year motor vehicle accidents are more frequent due to the winter weather conditions. Even from a minor fender bender you may still receive a whiplash injury. Whiplash is a term used to describe the forceful extension and then flexion of the neck. When the neck is initially forced into extension this causes a stretching of the flexor muscles including the SCM and scalenes in the front of the neck, and a jamming of the facet joints at the back of the spine. When the head forcefully moves into flexion it causes a stretching and tearing of the extensors muscles, including the cervical extensors, traps, and sub occipital muscles.</p>

<h2>Signs of whiplash injury</h2>
<p>It is not uncommon to feel little to no discomfort immediately following an accident as the body will release natural pain killers in the form of endorphins as well as the heightened adrenaline in the body. Once this wears off and the inflammation begins to set in is when the pain and restricted motion begins. In many cases, even minor accidents can cause symptoms of whiplash which can show up as late as 2-6 weeks post injury. The reason this can happen is because even in a minor accident there is still a stretching and tearing of tissue, so the body’s response is to lay calcium or scar tissue down in the area. After weeks of scar tissue deposition, this may lead to further symptoms, such as headaches, jaw pain, trigger points, and restricted range of motion.</p>
<h2>Treatment for whiplash</h2>
<p>It is important after any type of whiplash injury to seek out your doctor to assess the level of damage, and develop a treatment plan which will not only relieve your temporary symptoms but also prevent further damage down the road. At Mountain View Pain Center, we see many injuries that are due to improperly treated whiplash injuries.  What our doctors most commonly see in this instance, are injuries to the Cervical intervertebral discs. After an accident the neck becomes very stiff as a self-protective mechanism to prevent further damage. The problem with this is if motion is not properly restored, the Cervical spine no longer functions with optimal range of motion. This will speed up the degenerative process of the spine, and begin to wear down the disc spaces at a rapid rate. A disc injury in the neck can lead to very intense pain in the neck, shoulder, and arm. In many cases with a Cervical disc injury a patient may only feel symptoms in the arm, such as numbness, radiating pain, weakness, or tingling.  In the event a disc injury does occur it can still be treated with very positive results; however the natural treatment process of the disc is 90 days which can lead to a lengthy treatment plan.</p>

<p>In the world of Chiropractic, Physical Therapy, and Sports Medicine, there is much debate and discussion revolving around the topic of “core strength.”  The reason this topic has been so highly discussed is usually in regards to the fact that research has shown us that individuals possessing greater core strength and stability often have reduced incidences of things like low back pain, sports injuries, and postural dysfunction.  With this article I would like to discuss exactly what the core is and the role it plays in helping us stay injury and low back pain free.</p>
<h2>Core Muscles</h2>

<p>The core is made up of a lot of different muscles and tissues all working in synergy to provide a solid foundation for the body to operate upon.  One thing that people often mistake about the core is the actual muscle groups it incorporates.  It is not just the abdomen that needs to be strong in order to have a strong core; in fact it is much more than that.  The core does include the abdominal musculature like the rectus abdominis, and the obliques, but the back or posterior part of your body needs to be equally if not greater in strength and stability.   Having strong back muscles, along the spine as well as having deep “stabilizer” muscles will help your body better react to everyday forces that are applied to it.  Along with having strong back muscles, having strong and flexible leg muscles is also extremely important.  The hamstrings need to be flexible and firing properly, along with the big muscles like the gluteus maximus or your “butt muscles.”  Yes, there should be muscle there, not just a seat cushion!  If muscles like the gluteus maximus and hamstrings are strong and doing their job, they will help stabilize the pelvis, which in turn will help decrease some of the forces and load that is placed on the lumbar spine or “low back.”</p>

<p>This week’s discussion deals with<strong> facet syndrome</strong>. Facet syndrome is one of the most common causes of pain originating from the spine. In order to understand facet syndrome we must first start by defining what facet syndrome is by looking at the anatomy of a typical spinal vertebrae.</p>

<p>The facets are on the posterior section of the vertebral body and are made up of two articulating surfaces the inferior articular process and the superior articular process. The facet joint is formed when the superior portion of a lower vertebral body meets the inferior portion of the vertebral body above it.  The facet joint is further held together a capsule of ligaments forming a joint commonly known as the zygapophyseal joint (z-joint). When these joints are jammed together from either injury, inflammation, or degeneration we get the facet syndrome. Causes of this jamming could lead from trauma, excessive weight, decreased disc height, and an increased lumbar curvature. Degenerative changes and bad posture caused by muscle imbalance is the leading reason why people develop facet syndrome.</p>
<h2> Signs and Symptoms of Facet Syndrome</h2>

<p>Common symptoms of facet syndrome is neck pain and low back pain that hurts more during rest, extension, or is relieved with motion. The neck pain will be aggravated by extension, rotation and repetitive movements. Low back pain is aggravated by extension, rotation, prolonged sitting and repetitive movements. Most patients describe facet pain as sharp and in a well-defined area, normally feels superficial and one sided.</p>
<p>People also report that they have diffuse morning stiffness. As we can see the signs and symptoms of facet syndrome is every similar to those of a herniated disc. This is why it’s so important to get the proper diagnosis so you can receive the proper treatment for you problem.</p>

<p>Did you know that one of the most common causes of numbness, weakness, and pain down your arm could be related to a herniated cervical disc?</p>
<p>Other signs and symptoms of a Cervical disc injury in patients are pain/numbness starting in the lower neck which can radiate to the chest, trap, shoulder, and even down through the arm into the hands and fingers.</p>
<h2>Definition of a Herniated Cervical Disc</h2>
<p>Most commonly defined as a rupture of the annulus fibrosus (commonly known as the rings of the disc that hold the nucleus made up of a jelly like substance in the center). When this rupture occurs the nucleus will herniate/bulge pinching the cervical nerves causing the pain or numbness to occur. This herniation is most commonly one sided and often occurs at C5 or C6.</p>
<h3>Causes of Herniated Cervical Discs</h3>
<p>There are several reasons why discs herniate. One of the more common causes of a disc herniation/bulge is progressive degeneration. As we continue to age there is increased pressure, and decreased function of the spine which causes a higher risk for injury. Bad posture, trauma, and whiplash injuries are just some of the mechanism of injury that can cause a herniated disc. These factors show how important it is to take care of your spine.  Even a minor sports injury can cause decreased function of the neck, which as we mentioned early puts the disc at risk and increase likelihood of further injury .</p>

<p>One of the most common causes of low back pain is dysfunction of the <strong>Sacroilliac joint</strong> (also knows as the SI joint). The sacroilliac joint is one of the largest joints in the body, and is an articulation of the sacrum and the iliac bone (a portion of the pelvis). This bone can become restricted in several ways such as acute trauma, repetitive motion, pregnancy, or poor posture.</p>
<p>When this joint becomes restricted it causes inflammation in the joint, which can cause a very sharp pain directly over the area. Due to the muscle attachments in the area, inflammation in the joint may also cause low back pain, hip pain, or even radiating pain in the leg.  Any condition or injury to the leg that causes an altered stride can also affect the joint. For example an ankle injury can cause someone to alter the weight they put on that leg. This can cause more pressure on the opposite SI joint, rotation of the pelvis, or a functionally short leg. If the joint has suffered chronic restriction and inflammation this area can degenerate rapidly causing chronic low back pain that can cause an altered gate pattern or limp.</p>

<p>Are you having trouble moving around because of a hip problem?  Are you having difficulty walking up or down stairs?  Is it painful to turn your body on the bed?  Hip pain is a common complaint that can arise from a myriad of conditions ranging in levels of severity and complexity.  The hip is also an area where pain can be felt that is generated from distant sources (referred pain).  Hip pain can be very painful and cause a lot of your daily activities to be very challenging.  However, the good news with hip pain is that often times these problems are easily treatable.</p>
<h2>Common Causes of Hip Pain</h2>

<p>There are many reasons that can be causing your hip pain; one of the most common is referred pain coming from the back.  In older individuals osteoarthritis of the hip joint is a problem that affects 25% of women and 15% of men over the ages of 60.  Osteoarthritis is the most common disease affecting the hip joint.  Other factors that can predispose you to osteoarthritic changes are prior incidents of trauma such as fracture or dislocation, family history of this disease, obesity, or other diseases affecting the joint.  Muscles and ligaments of the hip joint can become sprained or strained and cause sudden intense pain.  The joint itself can become fixated or restricted, which causes a decrease of range of motion and function, which can affect your daily activities and movement patterns.</p>
<h2>Treatment of Hip Pain</h2>

<p>With the NFL continuing into the playoffs, many of us are sitting by watching the injury reports to see which of our favorite players may or may not be participating in this week&#8217;s games. Even what many consider a minor injury such as a spasm, sprain, or strain can keep a player from performing to the best of their abilities, or even being able to participate at all.  Sprains and strains are two of the most common types of soft tissue injuries.</p>
<p>To provide a better understanding of how these injuries can affect the body, let’s have a quick look at the body&#8217;s musculoskeletal system. The skeletal system is made up by the bones in your body, which form your skeleton.  The skeletal system provides for your body&#8217;s shape and also protects your vital organs such as the heart, lungs, and brain. The muscles that move your body are attached to the bone with tendons. The contraction of the muscle provides the ability for you to move your bones at the joints. The joints are held together with the connections of the muscles and ligaments. The ligaments are made up of a fibrous tissue which attaches a bone to another bone.</p>
<p>Spasms, sprains, and strains have very similar symptoms such as pain, swelling, and decreased range of motion, and they all involve soft tissue, but they are each a very different type of injury.</p>
<p>Spasm – A spasm is an involuntary contraction of muscle. This can occur suddenly, due to trauma, and can eventually lead to a chronic spasm, which will shorten the muscle over time. A muscle which has shortened overtime has decreased range of motion, and an increased risk for re-injury.</p>
<p>Sprain – A sprain happens when a ligament is stretched beyond its normal length, which causes a stretching, weakening, or possible tearing of the ligament.  Sprains can occur at any joint in the body, as they all have a ligamentous component, and  the most common joint sprained is the ankle.</p>

<p align="center">Strain – A sprain is an overstretched ligament, where a strain is an overstretched muscle or tendon. This is most commonly referred to as a ‘pulled’ muscle or tendon.</p>

<h2>Causes and Symptoms of Muscle Spasm&#8217;s Sprians and Strains</h2>
<p>The symptoms for these three injuries are relatively similar, but have some distinctions:</p>
<p>Spasm- A muscle spasm can occur from an acute injury or prolonged contraction due to chronic muscle imbalance. What we see mostly in sports is a short painful muscle contraction due to overuse.  The symptoms of the muscle spasm may feel different depending on which muscle in the body is affected. For example, a spasm in the cervical musculature may be interpreted as a headache, and a spasm in the pectorals or chest muscle can be interpreted as a tightness or deep chest pain.  The most common symptoms of a skeletal muscle spasm are pain, tightness, and decreased range of motion, and many muscle spasms can be palpated (felt) through light touch. The musculature affected by the spasm will often feel like a lump or ball in the muscle, will often be tender to the touch, and may produce radiating pains, similar to what you experience with a trigger point.</p>
<p>Strain- A strain is generally caused by an acute injury to the muscle due to overuse or a trauma. The strains can vary from minor, which will not keep you out of much physical activity, to a tear, which can keep you out for usually around 6 weeks. Most common symptoms of a muscle strain are pain and swelling. You could also experience bruising to the area of injury, pain while resting, or pain while moving the injured muscle.</p>
<p>Sprain- A sprain is almost always caused from an acute injury. The most common sprain is a lateral ankle sprain, also referred to as rolling the ankle. Sprains can range from a minor stretch or inflammation of the ligament to a full blown tear. A complete tear through the ligament will cause major instability and will require surgery. The ankle sprains we most commonly see in football come from lineman rolling over on another player’s foot, or from a running back being tackled. The most common sprain in a quarterback or receiver is an AC joint sprain. This can happen while throwing, or falling to the ground &#8211; jarring the shoulder from a tackle.  Depending on the severity and location of the sprain, the treatment can last from two weeks to more than six. The most common symptoms of a sprain are localized pain, decreased range of motion, inability to bear weight on injured joint, and instability of joint. If a sprain is not properly cared for, you run the risk of chronic sprains to that joint, along with future pain and lost motion due to the buildup of scar tissue.</p>
<h2>Treatments of Muscle Spasm&#8217;s, Sprains and Strains</h2>
<p>The treatments for all these injuries in the acute phase are very similar.  The key at the beginning of treatment is to reduce inflammation, which will increase range of motion and decrease pain. If you injure yourself in a sporting event or simply just at home working around the house, your first step should be to get ice on the area right away. The ice will help control pain and decrease swelling. The next step should be to see your doctor as soon as possible and get a proper diagnosis, which will determine your treatment plan. Once the appropriate diagnosis has been made, your doctor should give you recommendations as to when you can appropriately return to activity and how long treatment should last.</p>
<p>In addition to ice, another therapy we use at Mountain View Pain Center to reduce inflammation is to use an electric current called interferential. This electric current will push inflammation out of the joint and reduce pain. In addition to the electric currents we also use kinesio tape. Kinesio tape  can be worn for three days, including in the shower, so while you are in between treatments,  you have joint support, without reduction of range of motion- in which is often the case when normal athletic tape is used.</p>
<p>Once the inflammation and edema has been reduced, your doctor will usually begin rehabilitation or physical therapy for strengthening, and to reduce your chances for re-injury. In the case of a sprain vs. a spasm, the treatment should vary in the rehab stage.  A sprained area should not be stretched. Stretching will lengthen the already stretched and lengthened area, which just makes the injury worse. However, a spasm should be stretched, as the muscle is in constant contraction. Stretching will help lengthen the muscle and break the spasm. A strain and a sprain should have therapy exercises, which will strengthen the muscle and joint to resolve any muscle imbalance and prevent the reoccurrence of future injuries.</p>
<p>After the initial pain and inflammation phase it is important to continue with rehab or therapy to decrease the buildup of scar tissue in the affected muscles, ligaments, or joints. If the injury is left to heal on its own, then the body will lay down collagen or scar tissue to prevent from further injury. The problem with scar tissue is that it will reduce the range of motion of the muscle or joint, which can lead to further injury, or increase chances of early degeneration. At Mountain View Pain Center, we remove the scar tissue with muscle release techniques, as well as ultrasound.</p>
<p>A prompt examination will help your doctor provide the best care for your injury. How your doctor will reaches a diagnosis is heavily dependent on the history you provide, so make sure to explain how the injury occurred, in as much detail is possible, whether you feel it is pertinent or not.</p>
