Private medicine is increasing in the UK and many doctors are now running their own clinics, private practices or medical businesses - and even more thinking of doing so.

Of course there are a plethora of "business advisers" all willing to "help" or provide services for a fee. Some of these might be helpful - many turn out not to be and end up being a waste of time or money.

Doctors in Business - Business support group for doctors in business
or those wanting to do business

Doctors in Business is an invitation only private club run by doctors for doctors. It has been set up by doctors who have been successful in setting up and running medical businesses who are often asked for advice by others.

Rather than charge for such advice, doctors in business is a mutually supportive networking club where doctors who are either in business (private practice, clinics etc) or aim to start their own businesses, can meet and talk to those who have already been successful in doing the same.

There are no membership fees and no formal advice is given. However it gives an opportunity for doctors interested in the business of healthcare to meet each other, swap advice and experience and network with others in the same or similar position.

If you would like more information or would like to be invited to an event, please contact doctors in business through the doctors in business website.
In the June edition of Vogue, the lovely Jo Ellison has produced a fabulously written article about her experiences getting her recurrent varicose veins treated by Mark Whiteley of The Whiteley Clinic.

"Mark Whiteley of The Whiteley Clinic whose name is synonymous with
medical excellence in vascular surgery" - Vogue, June 2013

In a two-page article entitled "Leg work", Jo explains how her veins had recurred after previous vein stripping surgery. She notes how Mark Whiteley assessed her recurrent varicose veins and then treated her with combinations of endovenous laser, phlebectomy and finally foam sclerotherapy.

She explains how the new key-hole endovenous "walk in, walk out" vein surgery that is performed under local anaesthetic, allowed her to get back to work immediately after treatment.

She also details how the compression for 2 weeks after the foam sclerotherapy was not fun - but how after all of her treatment, she is now happy to show her legs in fashionable skirts again.
With time, further microsclerotherapy and continuing exert care, her results will continue to improve and her confidence in her legs will grow.

As with our recent patient who was featured being treated for recurrent varicose veins at The Whiteley Clinic on Embarrassing Bodies, excellent results after our treatments are much quicker in simple first time veins in patients who come early.

The Whiteley Clinic specialises in patients with complex varicose veins, particularly those who have had "treatment " elsewhere and who have had recurrent varicose veins, We usually end up with excellent results and very happy patients - but the more complex the problem or the longer it has been left, the longer it will be and the more work is needed to get our usual excellent finished results.

Compare the patient satisfaction at The Whiteley Clinic over the years with any other vein clinic: http://www.thewhiteleyclinic.co.uk/patient_satisfaction.htm

Professor Mark Whiteley receiving the prize on behalf of Daniel Taylor for
Best Research Poster at
The Venous Forum, Royal Society of Medicine April 2013

Daniel Taylor, one of The Whiteley Clinic summer research fellows 2012, studied the 10 year outcomes of varicose vein surgery performed at The Whiteley Clinic.

As The Whiteley Clinic was the first clinic in the UK to use the new endovenous (or keyhole) techniques for varicose vein surgery, having started in March 1999, The Whiteley Clinic has a unique perspective on these new treatments.

Daniel Taylor and his fellow researchers followed up patients treated at The Whiteley Clinic between 10 and 14 years ago - and found over 93% success of the treatment - a success rate rarely found in such long term studies of varicose vein treatments.

The research project and the results were presented as a poster and were awarded prize for the best poster.
This week, Judy Holdstock from The Whiteley Clinic presented a research paper that showed a link between haemorrhoids ("piles") and varicose veins.

Judy Holdstock, Senior Vascular Technologist at The Whiteley Clinic, presenting research paper
about Haemorrhoids and Varicose Veins at The Venous Forum 25th april 2013

Presenting the paper:

" Pelvic Venous Reflux is Associated with Haemorrhoids in One Third of Cases - Should Haemorrhoids be Treated by Phlebologists?"

Judy showed The Whiteley Clinic research that shows that women who have pelvic varicose veins, can also have haemorrhoids from the same underlying cause.

This research, presented at The Venous Forum of The Royal Society of Medicine in London, may herald a completely new approach to understanding and treating haemorrhoids.

Who should treat varicose veins?

In the past, General Surgeons used to operate on them. Now with local anaesthetic "pin-hole" techniques, lots of doctors for other specialities are treating veins. In some countries, non-medically qualified people and purely commercial companies are jumping on the band-wagon and offering vein treatments.

Who Should You Trust to Treat Your Varicose Veins?

This wouldn't matter if everyone was doing the same thing and getting the same results. However, just like any practical skill, there are certain people who spend more time practising or researching a subject and who are likely to get better results than the "occasional dabbler".

Prof Mark Whiteley raises his head above the parapet with this controversial article in The Huffington Post.
Cosmetic News Magazine has published an article about Prof Mark Whiteley's research and treatments of veins.

On the Right Vein - Prof Mark Whiteley in Cosmetic News April 2013

The article explains why cosmetic and beauty clinics tend to treat thread veins and varicose veins incorrectly - by only treating the veins they can see. By treating these and ignoring the deeper veins that cause the veins on the surface, the risks of the veins coming back again - or causing skin blemishes - is higher.

Mark explains in the article which are the best ways to treat the veins - but points out a Doppler examination or Duplex examination is essential before any vein treatment is carried out.
At this morning's session at the Indovasc Symposium 2013, Mark Whiteley gave a talk entitled:

Superficial Venous Thrombosis - To Treat or Not?
Mark Whiteley

In the talk, Mark explained how the treatment of "Phlebitis" in the legs has completely changed over the last couple of years.

Until recently, "phlebitis" or superficial venous thrombosis of the leg, was considered to be a benign condition without any risk the patient. As such, doctors and nurses have been used to diagnosing this clinically and then treating with aspirin and graduated pressure stockings-and occasionally those who do not understand the process have used antibiotics.

However recommendations from the American College of Chest Physicians (ACCP) and the British Committee for Standards in Haematology in 2012 have produced new guidelines based upon randomised controlled studies that have been performed over the previous 2 to 3 years.

The key points are:

"Phlebitis" of the legs (superficial venous thrombophlebitis) is associated with deep vein thrombosis (DVT) and pulmonary embolism (PE) in a few patients

Treatment of superficial venous thrombophlebitis of the legs has shown to reduce the extension of clot, reducing deep vein thrombosis (DVT) and pulmonary embolism (PE)

Therefore it is now incorrect to diagnose "phlebitis" or superficial venous thrombophlebitis of the legs clinically and treat with aspirin, antibiotics and/or graduated pressure stockings as this will mean any underlying deep vein thrombosis (DVT) is missed and potential clot extension is more likely.

Hence the new recommendations are:

Anyone thought to have "phlebitis"or superficial venous thrombophlebitis needs to have a venous duplex ultrasound scan as soon as possible

If a DVT is identified, full anticoagulation needs to be started

If the clot is in the Great Saphenous Vein and close to the SaphenoFemoral Junction, full anticoagulation needs to be started

If the clot is less extensive but there are other factors then prophylactic low molecular weight heparin should be given

For more minor cases proven on venous duplex ultrasound scanning, non-steroidal anti-inflammatory tablets are sufficient

These guidelines show that "phlebitis" of the legs can no longer be treated as a minor condition as this may lead the patient to potentially dangerous deep vein thrombosis (DVT) or pulmonary embolism (PE).

Every patient with "phlebitis", or suspected to have "phlebitis", needs to have an urgent venous duplex ultrasound scan arranged and appropriate treatment prescribed upon the scan results.
