Persistent gout can destroy cartilage and bone, causing irreversible joint deformities and loss of motion. Survey results released in 2006 show that two-thirds of persons with gout consider the pain of attacks among the worst they&#8217;ve ever experienced. An estimated 75% of those surveyed said flare-ups made walking very difficult, and about 70% reported trouble putting on shoes or playing sports.</p><p>Tophi are firm chalky, gritty clumps of uric acid crystals that build up in tissue surrounding a joint. If gout is not treated, tophi can grow to the size of golf balls and can destroy bone and cartilage in the joints, similar to the process in rheumatoid arthritis. If they lodge in the spine, tophi can cause serious damage including compression, although this is very rare. In extreme cases, joint destruction results in complete disability.</p><h3>Kidney Conditions</h3><p><em>Kidney Stones.</em> Kidney stones occur in 10 &#8211; 40% of gout patients, and can occur at any time after the development of hyperuricemia. Although the stones are usually composed of uric acid, they may also be mixed with other materials.</p><div><div>Kidney stones result when urine becomes too concentrated, and substances in the urine crystallize to form stones. Symptoms occur when the stones begin to move down the ureter and cause intense pain. Kidney stones may form in the pelvis or calyces of the kidney or in the ureter.</div><p><img title="Nephrolithiasis" src="http://www.umm.edu/graphics/images/en/17091.jpg" alt="Gout Complications-Nephrolithiasis" border="0" /></p></div><p><em>Kidney Disease.</em> About 25% of patients with chronic hyperuricemia develop progressive kidney disease, which sometimes ends in kidney failure. It should be noted, however, that many experts believe that chronic hyperuricemia is unlikely to be a common cause of kidney disease. In most cases, the kidney disease comes first and causes high concentrations of uric acid.</p><h3>Gout and Heart Disease</h3><p>Gout is found in higher rates in people with high blood pressure, coronary artery disease, and heart failure. Hyperuricemia, in fact, has been associated with a higher risk of death from heart conditions. Studies also found an association between gout and having the metabolic syndrome &#8212; a collection of problems, such as abdominal obesity, high blood pressure, high triglycerides levels, and low &#8220;good&#8221; cholesterol levels. Gout and Hyperuricemia</p></div><p>Gout is a disease that results from an overload of uric acid in the body. This overload of uric acid leads to the formation of tiny crystals of urate that deposit in tissues of the body, especially the joints. When crystals form in the joints, it causes recurring attacks of joint inflammation (arthritis). Gout is considered a chronic and progressive disease. Chronic gout can also lead to deposits of hard lumps of uric acid in the tissues, particularly in and around the joints and may cause joint destruction, decreased kidney function, and kidney stones (nephrolithiasis).</p><p>Gout has the unique distinction of being one of the most frequently recorded medical illnesses throughout history. It is often related to an inherited abnormality in the body&#8217;s ability to process uric acid. Uric acid is a breakdown product of purines that are part of many foods we eat. An abnormality in handling uric acid can cause attacks of painful arthritis (gout attack), kidney stones, and blockage of the kidney-filtering tubules with uric acid crystals, leading to kidney failure. On the other hand, some people may only develop elevated blood uric acid levels (hyperuricemia) without having manifestations of gout, such as arthritis or kidney problems. The state of elevated levels of uric acid in the blood without symptoms is referred to as asymptomatic hyperuricemia. Asymptomatic hyperuricemia is considered a precursor state to the development of gout. The term <em>gout</em> refers the disease that is caused by an overload of uric acid in the body, resulting in painful arthritic attacks and deposits of lumps of uric acid crystals in body tissues.</p><p>Gouty arthritis is typically an extremely painful attack with a rapid onset of joint inflammation. The joint inflammation is precipitated by deposits of uric acid crystals in the joint fluid (synovial fluid) and joint lining (synovial lining). Intense joint inflammation occurs as the immune system reacts, causing white blood cells to engulf the uric acid crystals and chemical messengers of inflammation to be released, leading to pain, heat, and redness of the joint tissues. As gout progresses, the attacks of gouty arthritis typically occur more frequently and often in additional joints.</p><p><strong><a name="Whoisaffected"></a></strong></p><h3>Who is affected by gout?</h3><p>Approximately 5 million people in the United States suffer from gout. (Did you know that none other than Benjamin Franklin had terribly painful gouty arthritis?) Gout is nine times more common in men than in women. It predominantly attacks males after puberty, with a peak age of 75. In women, gout attacks usually occur after menopause.</p><p>While an elevated blood level of uric acid may indicate an increased risk of gout, the relationship between hyperuricemia and gout is unclear. Many patients with hyperuricemia do not develop gout (asymptomatic hyperuricemia), while some patients with repeated gout attacks have normal or low blood uric acid levels. In fact, the blood level of uric acid often lowers during an acute attack of gout. Among the male population in the United States, approximately 10% have hyperuricemia. However, only a small portion of those with hyperuricemia will actually develop gout. It is particularly indicated where the patient already has urate crystal deposition, including the presence of tophus. It is only available on prescription.</p><p>In the United States and Canada, febuxostat is branded as Uloric, Adenuric in the European Union (EU) and Feburic in Asia.</p><h2>How Does Febuxostat Work?</h2><p>Uric acid is naturally formed through the breakdown of purines in the body which normally expels excess acid via urine with a little through the stools. However, excess uric acid can be retained in the bloodstream when the body produces more acid than it can eliminate. High uric acid levels can eventually lead to the deposition of crystals of urate in the joints and surrounding tissue, causing the excruciating symptoms of gout.</p><p>Febuxostat is a xanthine oxidase inhibitor. Xanthine oxidase is an enzyme in the body that helps to convert purines into uric acid. Febuxostat can inhibit this activity and so can help to reduce the amount of uric acid produced.</p><h2>What to Tell Your Doctor Before Taking Febuxostat</h2><p>You should tell your doctor if you&#8230;</p><ul><li>are pregnant</li><li>are breastfeeding</li><li>are allergic to febuxostat or any of the ingredients in the tablets</li><li>have or have had heart problems</li><li>have thyroid problems</li><li>are taking any other medications</li></ul><h2>How to Take Febuxostat</h2><p>Your doctor will tell you how to take it. He or she will decide on the dose applicable to your condition. This is usually a 40 mg, 80 mg or a 120 mg tablet taken once per day with a full glass of water. It can be taken with or without food. Take the tablets exactly as your doctor prescribes. If you miss a dose do not double-up on the next dose. And do not stop taking the tablets unless your doctor tells you to.</p><p>Note that, after starting on a course, you may suffer one or more gout attacks. Do not stop taking the tablets. This happens naturally as uric acid is removed from the joints and tissue because of the action of the drug. Your doctor may prescribe NSAIDs (non-steroidal anti-inflammatory drugs) to help relieve the painful symptoms.</p><h2>Febuxostat Side Effects</h2><p>As with any drug there are possible negative side effects with the taking of febuxostat. But not everyone will suffer these side effects. In fact, most patients will tolerate it quite well. And where they do appear, they are usually fairly minor and either don&#8217;t need any treatment, or, only require some simple treatment by your physician.</p><p>Common side effects are:- headache, nausea, unexplained rash, diarrhea, liver function test abnormalities.</p><p>Uncommon are:- dizziness, fatigue, oedema, altered sense of taste, increase in blood amylase, decrease in platelet count, increase in blood creatinine, and arthralgia.</p><p>Rare side effects:- insomnia, nervousness, renal insufficiency, asthenia.</p><p>There are some rare, but serious, side effects that you should report urgently to your health-care provider. These are things such as:-</p><ul><li>chest pain</li><li>difficulty breathing</li><li>easy bruising or bleeding</li><li>wheezing</li><li>swelling of the mouth, lips, tongue, throat, etc.</li><li>unexplained rash</li><li>itching</li><li>hives</li><li>frequent / persistent infections</li><li>numbness in arms, legs, face</li><li>liver problems (yellowing of the whites of the eyes, dark colored urine)</li></ul><p>Note that the lists of side effects given above are not complete. It is a drug in the group called &#8220;non-steroidal anti-inflammatory drugs&#8221; (NSAIDs) and is widely used to reduce swelling, fever, stiffness, inflammation and pain, caused by arthritis, gout, bursitis, tendinitis, and ankylosing spondylitis.</p><p>It is probably the most commonly used drug-based medication to relieve gout symptoms. Brand names are Indocin and Indocin-SR. Indomethacin is available in capsule, slow-release capsule, oral suspension and suppository forms.</p><h2>How Does Indomethacin Reduce Gout Symptoms?</h2><p>Indomethacin works by helping to block or inhibit the production of prostaglandin molecules that are naturally produced by our bodies&#8217;cells when body tissue is damaged. This damage occurs in gout victims due to the presence of urate crystals in the joints and surrounding tissue.</p><p>White blood cells flood the damaged site in order to limit and help repair the damage. Prostaglandins are produced during this process resulting in pain, inflammation, swelling, stiffness, etc. This is the body&#8217;s natural inflammatory process. By inhibiting prostaglandins production, Indomethacin helps to reduce these symptoms.</p><h2>Dosage</h2><p>Capsules come in 25 mg and 50 mg. Sustained release capsules in 75 mg. Oral suspension 25 mg / ml, and suppositories 50 mg.</p><p>The recommended dose for adults is 50 to 200 mg per day. Never increase or decrease the dose or use it for less or longer than prescribed unless your doctor has told you to. Never go over the total daily dosage. And do not lie down for up to 30 minutes after taking a dose.</p><p>Your doctor will advise on the actual preparation and dosage depending on your particular case. He / she will normally start you at the lowest possible dosage for your case and over the shortest possible timescale in order to reduce the risk of side effects such as bleeding (see below). Take the medication with a meal or directly after a meal to help protect your stomach. Some doctors often prescribe certain drugs or antacids to help with this too.</p><h2>What Are the Side Effects of Indomethacin?</h2><p>The most common side effects are:-</p><ul><li>upset stomach</li><li>diarrhea</li><li>nausea</li><li>vomiting</li><li>drowsiness</li><li>tiredness</li><li>heartburn</li><li>headache</li><li>dizziness</li></ul><p>In the event that you get any of these side effects and they get worse, or even if they just persist, you should contact your physician.</p><p>Indomethacin also has some more serious side effects that you should be aware of. <span style="text-decoration: underline;">Contact your doctor</span> if you suffer any of the following:-</p><ul><li>unexplained / sudden weight gain</li><li>swelling of hands or feet</li><li>mood swings</li><li>difficulty swallowing</li><li>unexplained hearing changes (e.g. tinnitus)</li></ul><p>Indomethacin can increase your risk of heart attack and stroke as well as other circulatory problems. And it can also cause bleeding and even perforation of the stomach and intestines. So that if you suffer any of the following symptoms you must stop taking the medication and seek <span style="text-decoration: underline;">urgent medical attention</span>:-</p><ul><li>stiff neck</li><li>shortness of breath / trouble breathing</li><li>chest pain</li><li>slurred speech</li><li>weakness in any part of the body</li><li>extreme tiredness</li><li>vision problems</li><li>balance problems</li><li>change in urination flow</li><li>dark colored urine</li><li>yellowing of the skin</li><li>yellow eyes</li><li>blood in stools</li><li>severe stomach pain</li><li>rash</li><li>itching</li><li>swelling of the lips / mouth / tongue / throat</li></ul><p>The lists of possible side effects above are not complete. [...]]]></description> <content:encoded><![CDATA[<h2>What is Colchicine?</h2><p><em>Colchicine</em> is a medicine that has been used in the treatment of gout for many years. It is derived from the seeds of the autumn crocus plant, also known as meadow saffron. The plant&#8217;s use in the  treatment of rheumatism and inflammation was recorded in Egypt as far back as 1500 BC.</p><p>As a drug, colchicine was first isolated in 1820 by two French chemists, Pelletier and Caventon. And in 2009 the Food and Drug Administration (FDA) gave approval for its use as a standalone drug in the treatment of gout in the United States. Before 1982 it had only been approved in combination with probenecid. Colchicine can only be obtained on prescription.</p><h2>Why Colchicine And Not NSAIDs?</h2><p>Nowadays, non-steroidal anti-inflammatory drugs (NSAIDs) are usually the first choice medicines to relieve the pain and inflammation of gout. They are available over-the-counter as well as on prescription. And for very many people they can be very effective. However, there are some people who cannot take them, or at the very least should get clearance from their doctor before taking them. For example people having:-</p><ul><li>known kidney or liver disease</li><li>known allergies to certain medications</li><li>stomach ulcers</li><li>hypertension (high blood pressure)</li><li>being pregnant</li><li>certain heart conditions</li><li>diabetes</li><li>asthma</li></ul><p>In the cases where NSAIDs are inappropriate then colchicine is often prescribed. And, of course, there may be situations where the gout attack is so severe that NSAIDs prove to be ineffective for those who are clear to take them. Where this is the case then colchicine may be prescribed.</p><h2>Who Should Not Take Colchicine</h2><p>Clearly, you should not take colchicine if you&#8217;re allergic to it. But you must also inform your doctor if you suffer from any of the following:-</p><ul><li>kidney disease</li><li>liver disease</li><li>heart disease</li><li>intestinal disorder or bleeding</li><li>stomach ulcer</li><li>Crohn&#8217;s disease</li><li>ulcerative colitis</li></ul><p>Also, inform your doctor if you are pregnant, you intend to become pregnant during a proposed course of colchicine, or if you are breastfeeding. In addition to these issues there are certain drugs which can increase the toxicity of colchicine (see Dosage and Toxicity below).  Your doctor will then make a determination of the most appropriate treatment in your case.</p><h2>How Does Colchicine Work?</h2><p>Colchicine does not reduce the actual uric acid levels present in the blood. There are other drugs that are prescribed for this, such as probenecid and allopurinol. However, because a sudden change in uric acid levels can actually cause a gout attack or increase the severity of existing symptoms, colchicine may sometimes be prescribed at the same time as uric acid-reducing drugs.</p><p>Its main use is in treating the inflammation, swelling and associated pain of acute gout attacks and in the short-term prevention of gout. It does this by inhibiting the build-up of white blood cells in the areas of the body where uric acid crystals have formed. To protect the area, your body&#8217;s own defence mechanism tries to swamp the crystals with white blood cells and pro-inflammatory enzymes and lactic acid are produced as a result. It&#8217;s these that give rise to the inflammation, swelling and pain of gout. So by inhibiting this process you can reduce the pain, inflammation and swelling of a gout attack.</p><h2>Colchicine Side Effects</h2><p>As with any drug there are side effects. However, one has to weigh-up the severity of the pain and disability of the actual gout attack against the severity of the side effects. Common side effects are:-</p><ul><li>nausea</li><li>vomiting</li><li>diarrhea</li><li>bleeding in stomach</li><li>rash</li><li>anemia</li><li>muscle weakness</li><li>abdominal pain</li><li>less urination than normal or not urinating at all</li></ul><p>For a fuller list you need to talk to the pharmacist and carefully read the documentation that comes with the medicine. But please bear in mind that not everyone will suffer all of these. Some folks won&#8217;t suffer any. Others will suffer one or two maybe. Some may suffer more. Not only that, their severity will vary across individuals who have them.</p><p>That being said, if you experience any of the following when taking colchicine then call your doctor right away:-</p><ul><li>abdominal pain</li><li>diarrhea</li><li>unusual fatigue</li><li>tingling in fingers or toes</li><li>flu symptoms</li><li>bleeding or bruising which is unusual or unexplained</li><li>blood in stools</li><li>bleeding from anus</li><li>blood in urine</li></ul><p>And, if you have any of the following you need to contact emergency services at once, as these could be the signs of very serious complications:-</p><ul><li>fever</li><li>hives (red, itchy red welts on the skin)</li><li>burning in the mouth and throat</li><li>swelling on the face, neck, tongue, throat, lips</li></ul><h2>Colchicine Dosage and Toxicity</h2><p>As you&#8217;ve now seen above, colchicine is a very toxic drug which, if taken in too high doses, can cause serious complications, such as multiple organ failure and even death. So it is very important to follow the prescribed dosage very, very carefully. Generally speaking, this is between 2 and 4 tablets per day for most people. Tablets come in either 0.5mg and 0.6mg doses depending on which country you live in.</p><p>In addition, there are some drugs that may increase the toxicity of colchicine when taken at the same time. These are drugs such as:-</p><ul><li>immunosuppressants</li><li>statins</li><li>antibiotics</li><li>anti-fungals</li><li>antiretrovirals</li><li>antiarrhythmics</li><li>and even grapefruit juice!</li></ul><p>So, you need to tell your doctor which medicines, etc., you are taking if they are considering prescribing colchicine for your gout.</p><p>On a final note; do not buy <strong>colchicine</strong> online or from any other source which is not approved by your country&#8217;s drug administration, e.g. the FDA in the United States. First though, it&#8217;s important to understand just what hyperuricemia is&#8230;</p><h2>What is Hyperuricemia?</h2><p>Hyperuricemia is the name given to a condition in which a person has high levels of uric acid in their blood. This is normally considered as being more than 9 mg of uric acid per 100 ml of blood. The recommended level of uric acid is less than 6 mg per 100 ml. Someone in the range 6 mg to 9 mg per 100 ml is said to have raised or elevated uric acid levels.</p><p>It is a fact that you can have raised or even very high uric acid levels and not have gout. But that doesn&#8217;t mean that you won&#8217;t eventually get an attack of gout. You see, urate crystals, which cause gout symptoms, can take a long time to form out of uric acid, months or even years in some cases.</p><p>However, it is also a fact that many people with hyperuricemia can go through their lives without gout. On the other hand, there are also many people with normal levels of uric acid who get gout.</p><p>There just doesn&#8217;t seem to be any single or clear reason why uric acid should build-up in a person&#8217;s bloodstream to the extent that it leads to hyperuricemia. But there are several factors that are known to help increase the risk of high uric acid. These are things such as&#8230;</p><ul><li>Genetics &#8211; certain inherited genes</li><li>A diet that is high in purines</li><li>Kidneys not functioning properly / kidney disease</li><li>Dehydration</li><li>Too much alcohol / binge drinking</li><li>Hypertension (high blood pressure)</li><li>Being overweight</li><li>Some medications (e.g. aspirin, niacin, diuretics such as thiazides, etc.)</li><li>Certain diseases (e.g. leukaemia, hypothyroidism, insulin resistance, psoriasis, glandular fever, etc.).</li></ul><h2>Hyperuricemia Symptoms</h2><p>Most people probably don&#8217;t discover that they have hyperuricemia until they have been tested for, and diagnosed with, gout. Because they had experienced the symptoms of gout &#8212; swollen, hot, red, stiff, inflamed and very painful joint &#8212; they had attended their physician who had ordered a blood test. So you could say that these gout symptoms are also hyperuricemia symptoms.</p><p>But, if you have hyperuricemia without gout being present, what other symptoms should you look for? Other symptoms are thing like; fatigue, kidney stones, urination problems, headaches, dizziness, fever, and, there are even some reports of loss of libido.</p><h2>Hyperuricemia Treatment Guidelines</h2><h3>Medical Treatment for Hyperuricemia</h3><p>The medical profession will usually prescribe certain drugs to help lower high uric acid blood levels. Typical of these are Allopurinol and Probenecid, with Allopurinol probably the most widely used of all the uric acid reducing drugs.</p><p>Some people will have negative reactions to these drugs, and unfortunately, they have to be taken over the longer term because they are only effective whilst being taken. Unless you factor-in some important lifestyle and dietary changes as well, there is nothing to prevent uric acid levels rising again once you come off the drugs. So talk to your physician about this.</p><h3>Dietary Changes for Hyperuricemia</h3><p>Uric acid is formed as a byproduct of the breakdown of purines in our bodies and in our foods. So that if your diet is rich in high purine foods there is an increased risk of having higher levels of uric acid in the blood. So folks with hyperuricemia need to change to a lower purine diet.</p><p>This essentially means cutting out &#8212; or at the very least seriously reducing &#8212; foods like organ meat, fatty red meat, game, poultry, fish, shellfish, and even some vegetables such as mushrooms, asparagus, cauliflower and spinach.</p><h3>Alcohol and Hyperuricemia</h3><p>There is a clear link between high alcohol consumption and high uric acid. This happens because of alcohol&#8217;s ability to both, increase the production of uric acid, and, to reduce the excretion of uric acid due to dehydration. So alcohol needs to be seriously reduced or stopped completely. Beer is especially bad because it also contributes purines to the equation.</p><h3>Link Between Hyperuricemia and Binge Eating and Drinking</h3><p>It&#8217;s amazing the number of times people go on holiday and end up with gout. My first gout attack happened on holiday in Greece! And the conclusion is clear: it&#8217;s all about binge eating and drinking. On holiday, you tend to over-eat and drink too much. If you already have raised uric acid levels, then this can lead to even more uric acid production and hyperuricemia. And it&#8217;s much the same at weekends. So be careful about binge eating and drinking then too.</p><p>Conversely, you need to be careful about reducing your diet too quickly as well. For example, if you go on a crash diet, the sudden change can actually increase your uric acid levels dramatically. So always take things easy with a new diet and always with the input and guidance of your doctor first.</p><h3>Stay Hydrated to Help Reduce Uric Acid Levels</h3><p>It is well known that a dehydrated body is more prone to higher uric acid levels in the blood than one which is well hydrated. So drink at least 3 litres of water everyday. This will help your kidneys do their job better and help flush uric acid out of your body.</p><h3>Having Hyperuricemia and Being Overweight</h3><p>Being overweight may be on of the most common causes of hyperuricemia. When you are overweight, not only do you have more purines in your body to produce uric acid, as well as putting additional strain on your joints, you are also much more likely to have high blood pressure, another cause of high uric acid. So, if you are overweight, you really must get back down to your healthy weight. Check with your doctor.</p><p>These are a few, but nevertheless, important <em>hyperuricemia treatment guidelines</em>. You can see that even if you use the medication route, you still need to incorporate dietary and lifestyle changes.
