Arc Flash Warning Label Information

Labels must be placed and sized so as to be visible to personnel before beginning maintenance or inspection.

Item A:- These durable UV and chemical resistant 3.2 mil laminated vinyl labels include a High-Tack adhesive and rounded corners to prevent peeling. The life span of these labels is 5 years in normal conditions and are capable of withstanding temperatures beyond 165 °F and -50 °F. Sufficient label durability is now a requirement as of the November 2011 NEC (National Electrical Code) update and ITU’s arc flash warning labels more than exceed this requirement.

Item B:- Flash Hazard Category: Represents the level of danger depending on the incident energy. Ratings range in number from 0-4. 0 represents little or no risk while 4 signifies greatest risk. For more information about the different arc flash hazard risk categories click here.

Item C:- Minimum Arc Rating: Identifies the arc performance of a material or system of materials (PPE) and are expressed in calories per centimeter squared.

Item D:- PPE: Identifies the Personal Protective Equipment that MUST be used according to the hazard category rating.

Item E:- Equipment ID: Identifies panel location within facility.

Item F:- Arc flash warning labels must have the word “WARNING” in black with an orange background and include the standard warning symbol (orange exclamation point with solid black triangle background).

Item G:- Flash Protection Boundary: This is the closest anyone can approach the equipment that has the potential for arc flash, without the use of PPE (Personal Protective Equipment).

Item H:- Limited Approach Boundary: This boundary may only be crossed by a “qualified” person who has been trained in the hazards of the component or equipment being serviced.

Item I:- Restricted Approach Boundary and Prohibited Approach Boundary: Only a qualified person using adequate shock prevention equip¬ment and techniques may cross these boundaries.

Item J:- Maximum Available Fault Current: This is one of the new NEC required features as of November 2011. All the required safety information provided on the label is based on the max. available fault current. If the system is altered in any way resulting in this value to change than the data must be re-evaluated. (NEC 2011 110.24)

Companies or utilities will often make changes in the electrical system to be more efficient or to increase capacity. When this is done the max. available fault current changes. As a result the PPE (Personal Protection Equipment) category and the approach boundary information becomes inaccurate and is no longer compliant. Inaccurate information could easily result in serious injury or even death in which the company would be held liable for.

Item K:- The date that the fault current calculation was performed should be on the label. The arc flash hazard analysis shall be updated when a major modification or renovation takes place. It shall be reviewed periodically, not to exceed 5 years, to account for changes in the electrical distribution system that could affect the results of the arc flash hazard analysis. (NFPA 70E section 130.5)

NFPA 70E 2009 requires that one of two specific pieces of information appear on arc flash labels: available incident energy or required level of PPE. These values are determined by an arc flash hazard analysis, and need to be calculated separately for each piece of equipment labeled. This is a new requirement. Previous versions of the code did not specify what information should appear on labels. NFPA 70E 2009 requires that all arc flash analysis be reviewed at least every five years to “account for changes in the electrical distribution system that could affect the results of the arc flash hazard analysis.” Although this does not require changes in arc flash labels, some consultants are recommending that the date of the arc flash analysis be included on the each label.

Kencing Tikus Atau Leptospirosis

LEPTOSPIROSIS atau jangkitan yang dibawa haiwan, termasuk daripada keluarga tikus dikategorikan sebagai jarang berlaku biarpun ia sebenarnya bukanlah asing kepada penduduk negara ini.

Pada Mei tahun 2009 misalnya, kita dikejutkan dengan jangkitan leptospirosis membabitkan enam tahanan warga Myanmar di Depoh Tahanan Imigresen, Juru Pulau Pinang.

Ia turut dikenali dengan pelbagai nama seperti Penyakit Weil, Demam Icterohemorrhage, Penyakit Swineherd’s, Demam Pesawah (Rice-field fever), Demam Pemotong tebu (Cane-cutter fever), Demam Swamp, Demam Lumpur, Jaundis Berdarah, Penyakit Stuttgart dan Demam Canicola.

Ketua Jabatan Perubatan, Hospital Sungai Buloh, Dr Christopher Lee berkata, leptospirois berpunca daripada pendedahan terhadap bakterium ‘leptospira interrogans.’

“Jangkitan terhadap manusia selalunya melalui air yang dicemari kuping kulit dan lendir hidung, mulut dan mata tikus. Bagaimanapun, leptospirosis tidak menyebabkan jangkitan daripada manusia kepada manusia,” katanya.

Jelas Lee, jangkitan ini berlaku di seluruh dunia terutama di negara beriklim tropika. Pusat Kawalan Penyakit dan Pencegahan Amerika Syarikat misalnya merekodkan 100 ke 200 kes leptospirosis setahun di negara maju itu dengan dengan 50 peratus daripadanya berlaku di Hawaii.

Kes leptospirosis biasanya meningkat apabila musim hujan atau banjir. Bagaimanapun, sehingga kini tiada rekod jumlah kes sebenar leptospirosis yang berlaku di seluruh dunia. Pertubuhan Kesihatan Sedunia (WHO) sebaliknya cuma memberi anggaran kes antara 0.1 atau 1 kes per 100 000 penduduk setahun di kawasan tropika.

Tanpa alasan yang kukuh, leptospirosis selalu terlepas pandang dan tidak banyak laporan mengenainya dibuat.

Kerap kali, mereka yang terbabit dengan aktiviti luar atau petani lebih berisiko terhadap leptospirosis.

Rekod WHO menunjukkan antara kes wabak leptospirosis yang meragut banyak nyawa adalah ketika Taufan Mitch pada 1995. Wabak leptospirosis yang dilaporkan di Nicaragua itu membabitkan komplikasi pendarahan di paru-paru. Pada 1998, wabak itu melanda kawasan tengah Amerika Syarikat. Tahun yang sama ia berlaku di Peru dan Ecuador disebabkan banjir besar.

Pada 21 September 2000, empat kematian direkodkan di Perancis membabitkan peserta merentas desa, Eco-Challenge. Tarikh yang sama di Kanada enam kes disyaki dijangkiti leptospirosis juga membabitkan peserta merentas desa Eco-Challenge. Sementara pada 17 Jun 2004, Kementerian Kesihatan Kenya melaporkan enam kematian dan 141 kes jangkitan leptospirosis dilaporkan di sekolah menengah di Bungoma, Kenya.

Komplikasi jangkitan ini termasuk jangkitan kuman di otak (meningitis), pendarahan yang teruk, kegagalan fungsi hati dan buah pinggang.

Di antara faktor risikonya adalah:

Pekerjaan yang terdedah kepada persekitaran – misalnya petani, peladang, tentera, pesawah, pekerja sistem pembetungan atau doktor haiwan.
Aktiviti rekreasi seperti berenang dalam air yang dicemari bakterium leptospira atau berkayak.
Terdedah kepada peralatan di rumah yang digunakan untuk penyembelihan, peliharaan anjing atau makanan yang digigit binatang seperti tikus, tupai dan arnab.

Gejala:

Leptospirosis tiada gejala yang khusus.
Umumnya masa pengeraman kuman itu di antara dua hingga 26 hari.
Pada peringkat awal ia akan membuat tubuh menggigil dan demam. Gejala ini membabitkan 75 – 100 peratus pesakit.
Batuk tidak berkahak ( 25 – 35 peratus pesakit)
Loya, muntah, cirit-birit, sakit sendi, sakit tulang, sakit tekak, sakit perut. Bagaimanapun ia gejala yang kurang berlaku
Radang selaput mata
Antara tujuh hingga 40 peratus pesakit mungkin mengalami sakit otot, limpa atau hati membengkak, bengkak kelenjar, otot mengeras dan ruam.

Langkah Pencegahan:

Leptospirosis adalah penyakit yang boleh dielakkan. Beberapa tip langkah penjagaan boleh diamalkan bagi mencegah pendedahan kepada bakteria Leptospira. Tutup luka dan lecetan kulit dengan balutan yang kalis air sebelum anda memulakan aktiviti/kerja di ladang/kebun atau tempat-tempat yang berpotensi untuk pendedahan kepada bakteria ini;
Pakai pakaian pelindung seperti sarung tangan, pelindung mata atau gogel, apron dan but apabila bekerja di ladang ataupun bekerja dengan haiwan-haiwan yang berpotensi untuk dijangkiti dengan Leptospira, terutamanya apabila anda berpotensi untuk terdedah pada urin haiwan tersebut;
Bersihkan diri dan tangan selepas membuat kerja-kerja di ladang dan kebun;
Pastikan anda tidak makan atau merokok semasa berada berhampiran dengan haiwan-haiwan yang mungkin dijangkiti ataupun semasa bekerja di kebun/ladang yang anda mungkin terdedah pada tanah yang telah dikontaminasi dengan bakteria tersebut;
Elakkan aktiviti rekreasi atau berenang di tempat-tempat yang berkemungkinan dicemari oleh urin haiwan;
Kawal kehadiran tikus di tempat anda dengan memastikan kebersihan yang optimum. Buang semua sampah sarap dan kemaskan buangan sisa makanan, bagi mengelak kehadiran tikus;
Sebelum minum atau membuka makanan di dalam tin, pastikan anda membasuh/membilas penutupnya terlebih dahulu bagi mengelak kemungkinan dijangkiti bakteria ini. Tin-tin ini mungkin terdedah pada urin haiwan pembawa Leptospira ketika di dalam stor;
Pastikan haiwan peliharaan anda mendapat vaksin dan rujuk kepada Klinik Veterinar bagi mengelak haiwan peliharaan anda daripada dijangkiti bakteria Leptospira.

In my view, Escalator is definitely not inspected and maintained regularly. I am sure there are no knowledgeable personnel to inspect machinery Escalator. In fact, the management also failed to properly inspect the machinery. Whereas, each machine must be inspected and properly maintained by a qualified or have extensive knowledge of the machinery Escalator. Results of the inspections shall be recorded in a record form. If there is any damage, the inspector shall inform the management to repair or replace the damaged equipment.

Things that are often adopted by management is when a machine such as the Escalator has been inspected by DOSH to obtain certificates of entitlement, they think do not have to be reinspected. In fact, although the machinery has been inspected by DOSH, the management shall appoint a person qualified to inspect and maintain equipment on a regular basis or once a month to ensure that the state of Escalator in safe manner.

Before this I was talking about the main causes of accidents at work. This time I want to share with you all about safety can not be achieved because of our failure in absorbing safety practices among employees and management.

Apart from poor management and lack of support from the management on the health and safety issues in the workplace, lack of communication with employees and management is also a factor to the failure to achieve high standards in safety and health management system at work.

For this reason, many HSE practitioners often having trouble absorb the safety and health practices at the workplace. Our failure as HSE practitioners is due to: -

The way we consult and motivate with employees too harsh so we disrespectful and looked down. Thus, the safety and health at the workplace can not be complied with fully because of reverence for HSE practitioners have been reduced.
The way we presenting during provide training to employees is very boring. The absence of elements of funny during delivery of the training program made causes are not encouraging. Treatment and a weak tone, slowly and apathetic during the presentation of the training is also a factor to the failure of the safety and health at the workplace.
In addition, a slide presentation created is not interesting because it contains related pictures or videos that can make a person does not feel sleepy while attending training. Funny video that has to do with the topic of training presentations can be included and shown to them.
Meeting organized boring because often talk about the same issues and problems without a solution. The issues discussed are also often find fault with other people so it will have a heart pain and thus does not respect and does not care about law enforcement that have been made. The meeting is a means to discuss the safety and health issues and not place blame or find fault with others.
The way we criticize with employees too aggressive and violent when we find there is something unsafe acts done by the employee. In this way, they (workers) will feel angry and upset with the comments given. Reduce Your High Blood Pressure By Drinking Tea

Scientists at The University of Western Australia and Unilever discovered that drinking three cups of tea a day lowers systolic and diastolic blood pressure.

However, researchers based their findings on drinking black tea and the effect of drinking tea with milk is not known.

Tea is the world's second-most popular drink, after water. It is full of polyphenols, antioxidants that have been shown to stop cancer cells from growing. Another study from Harvard University found the drink could boost immune function. Now researchers have found more proof that it keeps the heart healthy.

Lead author Research Professor Jonathan Hodgson said: 'There is already mounting evidence that tea is good for your heart health, but this is an important discovery because it demonstrates a link between tea and a major risk factor for heart disease.'

Blood pressure measurement consists of two numbers. The first is the systolic and measures blood pressure when the heart beats, or contracts to push blood through the body. The second number is the diastolic and measures the amount of pressure in between beats, when the heart is at rest.

In the small study, 95 Australian participants aged between 35 and 75 were recruited to drink either three cups of black tea or a placebo with the same flavour and caffeine content, but not derived from tea.

After six months, the researchers found that compared with the placebo group, participants who drank black tea had a lower systolic and diastolic blood pressure of between 2 and 3 mmHg (millimetres of mercury). The authors believe a 2 to 3mmHg drop in blood pressure across the board would lead to a 10 per cent drop in the number of people with hypertension and heart disease.

Dr Hodgson wrote: 'A large proportion of the general population has blood pressure within the range included in this trial, making results of the trial applicable to individuals at increased risk of hypertension.' He added that more research is required to better understand how tea may reduce blood pressure, although earlier studies reported a link between tea drinking and the improved health of people's blood vessels.
The study is published this week in the Archives of Internal Medicine.

Tracy Parker, Heart Health Dietitian at the British Heart Foundation (BHF), told Mail Online: 'It is important to understand that a cuppa won’t cancel out a poor diet or lifestyle. There is evidence that antioxidant properties in tea could provide heart health benefits, but more research is required to better understand how tea may reduce blood pressure.

Crane topples at construction site near Block 108 at Simei Street 1 at around 11.30am, yesterday hitting the top level of a multi-storey carpark that was being upgraded.. It is understood that the crane was hauling the cement concrete from the ground to the third level during incident. It is understood that there is no injury during incident.

Possibility of Causes

myEOSH believed that the possibility causes of this incident due to overload and the boom was extended to the maximum length causing the crane topples. The crane operator should identify the distance of location for lifting activity so that the incident of crane topples could prevented.

Indonesia authorities had took corrective action to stop the illegal riders riding on the roof of train by installing the concrete balls. It is understood that at least 53 passengers died in accident after riding on the roof of train illegally.

Most of them were electrocuted by overhead power cable and others were fall off after the train is moving.

FIRE IN KL SENTRAL!!!!

This earlier morning 3.50 am, a fire was caught on the 348 Sentral Building, KL Sentral which is still under construction. It is understood that there is no injuries caused. It is also understood that the fire started at 30th and 31st floor of the unoccupied building.

The fire was spread quickly due to diesel oil coated plywood that was used as formwork structure.

During fighting the fire, the fire fighters have to used the uncompleted staircase to put on the fire at the higher floors.

There are 10 fire engine and 70 fire fighters were sent to the incident to put on the fire after receiving a distress call. The fire fighters had put on the fires after 3 hours.

Preventive Measures

The employer has to provide sufficient fire extinguisher at conspicuous location;
The flammable material shall be stored separately from any potential of fire;
The employer should consider to enforce smoking prohibition rules before entering the work site;
The employer should provide fire watcher during construction process;
The employer should consider to apply permit to work for night activity to control and monitor the site activities;
The supervisor should supervise and monitor the formwork activity and hot work activity as well at all times to prevent from the similar incident happened;
To ensure at least 2 emergency exit provided at each floor;
The employer shall ensure the emergency exit, access and egress are free from obstacles blocking so that the rescuing process could be carried out smoothly.

The two mysterious bright lights looks like UFO were seen over Chatham on January 7, 2012. Then, less than a week later, there are four similar lights were seen over Essex, UK.

The first image was captured by Ernestas Griksas, 21, who was taking a picture of a cherry-picker outside his home in Chatham at around 1pm.

When he looked at the image afterwards he saw the bright disc-shaped objects. He told the Sun: 'There are two white discs I can't explain. I'm nowhere near a flightpath. One is slightly fainter as if it is further away or going at a different speed. 'The second sighting came last Friday at 7am when car salesman Josh Cummins spotted four bright objects in the sky as he drove to work through Loughton, in Essex.

Mr Cummins, 21, told the newspaper: 'I nearly crashed. I stopped to take this picture with my mobile. It was like the UFOs were surfing the clouds. They were there for 15 seconds then vanished. 'I wasn't a believer in UFOs but this made me think again. 'UFO fanatics will no doubt lay claim to the sightings as evidence to support their theories of alien life.

Expert Nick Pope said: 'Assuming the images are genuine, they're interesting, though the smaller objects weren't seen at the time, which raises the possibility of some glitch with the camera.

'As for the large one, I'm not sure. It might be some sort of atmospheric plasma phenomenon, but it's difficult to say. 'The South-East does seem to be a hotspot at present. I'm not sure why. 'One possibility is that it's a self-fulfilling prophesy, where one media report smokes out more from the same area.

'Another is that it's a consequence of population density as there are more potential witnesses if there's anything odd in the sky.'

There are 4 mysterious lights were seen in the sky over Essex.

Both sightings were about 75 miles, as the crow flies, from Rendlesham Forest, in Suffolk, which became known as the UK's Roswell after a group of servicemen went into the forest to investigate some mysterious lights and came out convinced they had seen seen an alien spacecraft.

Meanwhile, TV presenter Chris Evans reported an unexplained sighting yesterday. He tweeted: 'Approx 40 mins ago went out to walk the dog. Something passed overhead - alight, too low for a shooting star and then disappeared. Berkshire.' He added: 'Looked too fast for a Chinese Lantern. Hope it was something exciting.'

Tower Crane Mast Section with Crack Defect

Crack at tower mast segment no. 10

The mast sections of a topless tower crane in Johore Bahru were found to be defective and cracked. The cracks were located at segments no. 2, 3, and 10. They were found during routine monthly inspection. All the cracks were located at around similar locations (refer Figure 1)on the three mast sections.

The tower crane was erected in 2010 with safe lifting capacity of 1.8 tonne and capable of freestanding height of about 66 meters. The crane with 60m boom was made in 1995.

The cracks may have been initiated by some defects in the weldments. Then by repeated loadings, the crack propagated along the loading axis. This type of defect may not be detected visually during the early stages until the defect deteriorated further and propagated onto the surface.

Suggestions for improvement:-

Tower Crane

Tower crane erectors must ensure that the structures and components are properly and thoroughly inspected by the competent person on the ground and free of any defects before erection work for the crane can commence.
Routine and periodic inspections must be done regularly to pick up any irregularity on the crane and its components. Report shall be made to the employer on any defect that is found on the tower crane. All findings must be recorded for easy reference and further analysis. Such written records shall be forwarded for the Inspector’s attention when requested.
Operation of the tower crane with such defect must be stopped immediately and cannot be operated again until it has been certified safe for continual operation by the department.
Approval from the authority has to be obtained for repair works by welding involving any structural parts of the crane.
Any welding work that need to be carried out on the structural parts of the crane shall be undertaken by an approved welder. All such work shall conform to the crane’s manufacturer requirements or specifications.
Wall ties for the tower crane that need to be installed have to conform with the manufacturer’s specifications and requirements to prevent overstressing on the crane mast sections.
