Root cause analysis (RCA) is a tool to help investigate adverse events and

contributing factors, and develop recommendations. Procedures on how to conduct 
an RCA will be demonstrated through interactive exercises and group work. The 
facilitator will go through causal statements, actions, and outcome measurement 
used in RCA. Participants will also learn about diagramming and identification

<item><title>Continuing Education: Medication Safety-Patient Safety and Narcotic Administration- Lessons Learned from the Coroners Office .</title>

The Canadian Adverse Events Study, published in 2004, found that 7.5% of people 
admitted to hospitals in Canada experienced at least one adverse event and that 
almost 21% of such adverse events were fatal. The Office of the Chief Coroner 
for Ontario serves the living through high quality death investigations and 
inquests to ensure that no death will be overlooked, concealed or ignored. The 
findings are used to generate recommendations to help improve public safety and 
prevent deaths in similar circumstances. The Patient Safety Review Committee 
(PSRC) assists the Office of the Chief Coroner in the investigation, review and 
development of recommendations towards the prevention of deaths regarding 
health care-related cases where system-based errors appear to be a major 
factor. The PSRC also assists coroners in improving the investigation of deaths 
within, or arising from, the health care system in which system-based errors 
appear to have occurred.This webcast will feature the PSRC findings and 
recommendations on narcotic medication use in hospitals. Because of the 
frequency of their use, it is easy for providers to lose sight of the fact that 
these medicationscarry with them significant, and even fatal, side effects.This 
session will be case-based, and will focus on common sources of narcotic 
medication error, from prescribing to dispensing to administration to 
monitoring. Through exploration of these cases, the participant will gain an 
understanding of the underlying contributing actors and system-based strategies 
to prevent narcotic medication errors and serious side effects of narcotic 
administration.

<pubDate>26 Jan 2012 00:01 -0500</pubDate><description>Health Canada is advising Canadians not to use these foreign health products as 
they may pose a health risk. These products have been found by regulators in 
other countries to contain prescription, over-the-counter or unauthorized 
drugs, or heavy metals that are not declared on the product labels. 
Prescription drugs should only be taken under the supervision of a health care 
practitioner. These products are not authorized for sale in Canada and have not 
been found on the Canadian market, but they may have been brought into the 
country by travellers or purchased over the Internet. 

17 weight loss products (see Alert for a complete list), the U.S. Food and Drug 
Administration warned that these weight loss products contain unauthorized 
drugs (sibutramine, phenolphthalein)

Uprizing 2.0, the U.S. Food and Drug Administration warned that this body 
building product contains a controlled prescription drug (superdrol). 

Get Stiff, Maxi Mize, New Zealand&#x2019;s Medsafe warned that these sexual 
enhancement products contain prescription drugs (tadalafil, vardenafil, 
yohimbine) and/or unauthorized drugs (hydroxyhomosildenafil, 
hydroxythiohomosildenafil). 

Ying Da Wang tablets, the Australian Therapeutic Goods Administration warned 
that this sexual enhancement product contains a prescription drug (sildenafil). 

Slimina weightloss capsules (S-shape slim capsules), the Australian Therapeutic 
Goods Administration warned that these weight loss products contain 
prescription drugs (propranolol, nifedipine) and/or unauthorized drugs 
(sibutramine, fenfluramine, phenolphthalein). 

Athri-Eze, Sear Heang Tienchi Tu Chung Wan, Wiku Jahe Kencur (Akur Mujarab), 
Cap Wijaya Kusuma (An Ki It). The Singapore Health Sciences Authority warned 
that these Traditional Chinese or Traditional Malay Jamu products contain 
prescription and/or over-the-counter drugs (dexamethasone, prednisolone, 
furosemide, allopurinol, acetaminophen, chlorpheniramine), and/or an 
unauthorized drug (phenylbutazone). 

Cardiotium, Paidusu, SlimEasy Herbs Capsule, Tianran Zuanshi Xianweisu. The 
Hong Kong Department of Health warned that these health products contain 
prescription drugs (losartan, atorvastatin, thyroxine) and/or unauthorized 
drugs (sibutramine, phenolphthalein). Consult a health care practitioner 
immediately if you have taken &#x201c;Cardiotium&#x201d; while pregnant. The use of losartan 
during pregnancy can cause injury and even death to the fetus. 

[LuShenPai] Specific Hou Ton Qing, [AA] Pe Min Kan Wan - The Hong Kong 
Department of Health warned that these traditional Chinese health products 
contain excessive levels of heavy metals (arsenic or mercury).
