IT IS TIME

that is what i want to do is end alzheimers
however i am realistic since i know that i can not in 2026 end alzheimers
but
i can slow it down
so
that is what i am trying to do is
slow it down some

for those who dont know my story i will review it

i was diagnosed with mild cognitive impairment that has turned out to be due to alzheimers disease

how do i know that

i had several neuropsychological memory tests that document that
the first was in 2010 when i applied to be in the adni 2 or alzheimers neuroimaging initiative 2
i was suppose to be in the normal control group but after my memory test i was placed in the mci or mild cognitive impairment group
i did this study for 5 years getting numerous memory tests lots of blood work a couple of spinal taps and several amyloid pet scans and several mris of the brain

during ths time i was being treated by a neurologists who placed me on aricept or donepezil did spinal fluid analysis for amyloid and tau and did genetic testing

my spinal fluid was compatible with alzheimers and my genetic testing showed i had the apoe4 apoe4 gene the late onset alzheimers gene

i maximized my health eating a mind diet controlling my cholesterol blood pressure weight and staying active mentally physically and social and all the other things should be doing

a quick review of what happens with alzheimers

amyloid plaque accumulates in the brain early on before symptoms start
this starts killing off brain cells
then
tau proteins in the damaged cells unwind and form tangles that kill more braincells
then
an inflammatory reaction occurs causing inflammation which kills more brain cells

this all starts 20 years before symptoms start

aduhelm and leqembi and kisunia remove the amyloid plaques
now
scientists are doing clinical trials which i dont qualify for to treat and remove the tau tangles and remove the inflammation

i have completed step one
my amyloid plaques are removed but they will return over time

after finishing the adni2 study i progressed to being in a clinical trial for aduhelm or aducanumab which removes the amyloid plaque from my brain
my initial amyloid pet scan was positive for amyloid

i received 18 monthly infusions of placebo then received a total of 34 monthly infusions of the drug aduhelm
at the end of the study my final amyloid pet scan was read as negative

my diagnosis is solidified by having abnormal memory tests a positive spinal fluid and a positive amyloid pet scan
yes i have alzheimers

now what do i do now
there are treatments in clinical trials that will remove amyloid and remove tau proteins and treat inflammation but those may not be available for me for 5-10 years
thats too late for me

so what do i do now

there are probably 2000 of us in the world that has this problem
we received aduhelm which removed our amyloid plaque
now
what do we do

what i want to do is to start leqembi monthly maintenance infusions to keep the amyloid from reaccumulating in my brain
this may slow down the disease some

if i do nothing it will reaccumulate over the next 5-10 years and i will be back to where i was 16 years ago

removing the amyloid plaque reduces memory loss by 30% but the longer its used and the more its used in patients the better it seems to be working
plus the earlier you start it in the disease process the better it works
i was treated real early

fingers crossed

so in 2 weeks i meet with a neurologist at Cedars Sinai in los angles to discuss all of this to see if i qualify to receive the leqembi infusions monthly
or
since leqembi is getting a self administered version you can use at home i would like to try eventually to get on this

i am at risk for brain bleeds since i am apoe4 apoe4 positive
our local neurologist wouldnt treat me because of this risk
she advised i go to a tertiary care center to get it done

i fear there will be a lot of red tape to go through to get this done
luckily the neurologist i am seeing was running the clinical trial on aduhelm that i was in

i really have no other options right now besides living healthy and taking my aricept

my last aduhelm infusion was almost 2 years ago
my last amyloid pet scan was almost 1 1/2 years ago

the good thing is my last moca memory test was normal in july 2025 as was my memory tests in 2024

it is time but i now have to wait to see whats next

MY STORY REVISITED #223-I HOPE THEY DID
this is a picture from my first walk to end alzheimers in texas around 2011
i think the child a former patient in that picture is now a college student

now 15 years later here i am still going strong mentally physically and socially

I wrote the blog below 11 years ago after someone reached out to me about their spouse asking for advice on what to do

my advice hasnt changed much since then except i would add we now have some hope we did not have when i wrote that blog
i was a couple of years away from starting the aduhelm infusions that eventually removed most of the amyloid plaque from my brain

now we have two medications given as infusions that if started early enough in the disease they can slow mental decline by 30%

i was not fully diagnosed yet but i would be a year later when i had my amyloid pet scan done for the biogen aduhelm or aducanumab study
it was positive for amyloid plaques which solidified my diagnosis
there are more drugs coming that also will help

ALZHEIMERS NEWS-MAXIMIZE TIME AND HEALTH

that is my advice to the person whose relative developed
memory issues
she contacted me for some advice of what to do
after the evaluation the final diagnosis
that dreaded a word
alzheimers disease

maximize time and health of your loved one
would be my advice

over the last 5 years ive been contacted a lot
about folks family members who have memory issues
some i know them
some i dont know

here is my advice to this person ill call she
and for the care of her loved one i call he

now that the diagnosis is made
first there is no treatment or cure for this disease
he will not get better
he will have a slow decline which can speed up later
he like me will probably miss the true benefits of the
treatment and cure that is in the works
but
at least it will be there for his kids and his grandkids
just like it will be for mine

he will be offered meds
there will be side effects
work through them since if he is on aricept(donepezil) they get better
over time
some folks have side effects to namenda (memantine) like my older brother
did that only got worse
when that happens it has to be stopped
dont let the doctor put him on the $200+ a month namenda xr
when the $15 a month generic namenda given twice a day
works just as well

remember these do not treat or cure alzheimers
they only slow down the symptoms

there are no other supplements or medications that will make
him better
remember if i have alzheimers im real early in the process
thats why i got better
im under no illusions that its gone and that it wont get worse
for the time being im enjoying the lull before the storm

thats what you have to do with him
enjoy the moment with him

now having undergone the overwhelming sense of doom you
get with this diagnosis with myself and my mother and brothers
i understand how you must feel

maximize his health
make sure a b12 level and a vitamin d3 level was drawn on him
if the levels are abnormal he needs to take supplements
you can get them otc
dont do this though without getting levels done first

make sure his health issues like blood pressure and cholesterol
are perfectly under control

watch for symptoms of depression which are quite common
with alzheimers
dont be afraid to have him treated if he does get depressed

make sure he is getting adequate sleep
remember sleep becomes an issue with alzheimers patients as they
progress
i am already experiencing this problem

make sure he exercises every day
no not a gym
have him go for a walk hike ride a bike
make sure he gets up and moves every day

make sure he does those things he enjoys doing

make sure he stays socially active
if he goes to church have him go as long as he can
if he has other activities he likes have him continue to do it
as long as they are safe to do

one thing my sisterinlaw did with my younger brother was he did
what they did church social gatherings eat out at restaurants vacations
hunting camp no he stopped hunting but the social part he took part in
i admire her a lot for this it takes a lot of courage and love to do this

make sure he stays mentally active
if he likes to read make sure he has access to what he likes to read
as long as possible

he is lucky that he is exposed to your energetic family
the youth exposure really is helpful in folks as the disease progresses

my mother use to light up when kids came around to interact with her

remember this affects kids a lot
dont be afraid to talk with them about his condition

be ready to deal with driving if he is doing so
there are places that will do a driving evaluation to tell if he should
stop driving
but when the time comes have him stop for his safety and for others

people with alzheimers can function in many situations for a long
time
let him do things as long as he can

make sure all the legal papers are done
now
this is a mistake a lot of people make
once the courts get involved if you dont it becomes a mess
will power of attorney power of attorney for health care living will
some do a no cpr form now so when things are worse it can be used

then
there are memories still there
short term memories are gone first
but
long term memories are still intact
this will be your last chance to pull out those memories of his
childhood and family and what he thinks of things
once its gone its gone
deep regrets on my part that i didnt do this with my affected family

then
become quite familiar with this website

my advice to them and to others havent changed much
i would recommend they consider the new antiamyloid drugs leqembi and kisunia if the loved one was in the early stages
it may slow things down some

i would also recommend they watch the clinical trials closely since several new drugs are in stage 2 and 3 studies

also several old drugs used for other things are being looked at
even lithium is being investigated

besides the www.alz.org website i would recommend you following beingpatient.com a great source of information that is kept up to date
you can search my name and see a 50 minute interview i did with them

start early
take care of you health as young as possible
get evaluated early in the disease

we want someone to be able to carry that white flower at the alzheimers event one day for the first one to be cured

MY STORY REVISITED #222-MY VIDEOS TELL MY STORY

about 4 years ago i did the above video for being patient
they had read the article in the new york times where i was interviewed about being in the aduhelm clinical trials

the video is about 50 minutes long
i had never gone back and watched it since i was interviewed

last week i was watching the video below about a patient similar to me and his spouse
after i finished watching it i thought i will watch my video above and compare what i said and he said
so i searched on the being patient website and found my video and watched it

that picture on the video is me giving a talk and a q and a session for a group in austin texas about 10 years ago
That video is also online and i went back and rewatched it also
it is below

in this video i talk about studies that have since failed
the solanezumab study the a4 study the colombia study the sniff study
none of these were successful

however
my study the aduhelm study i mentioned gave me 34 months of aduhelm infusions of an antibody that removed the measurable antibody plaque from my brain
then
not long after that the aduhelm cousin leqembi finished its trial
it is slightly more successful than aduhelm
biogen which marketed both of then dropped aduhelm and put all its effort into the leqembi treatments that the patient below took
aduhelm is off the market now

one month after i gave the austin talk i got the results of my amyloid pet scan
it was positive for amyloid plaques
i was then accepted into the study where i received 18 months of placebo and 34 months of aduhelm resulting in the removal of those amyloid plaques

the plaques will eventually reaccumulate so you have to get on a maintenance dose to keep them from coming back
i am working on seeing a neurologist that will give me leqembi which is now available as an injection you can give at home

oh my how things changed in the alzheimers world

there are now thousands of us like me and the pattient below who have been treated

i consider us the lucky ones
so far

what i realized in watching first the couples video then my two videos is how far the evaluation and treatment of alzheimers has progressed in the last 15 years since i was diagnosed

we now say as i say in the video that some hope is better than no hope at all

our cases are similar
he has apoe4 apoe4 like i do
he took or is taking leqembi an antiamyloid antibody that removes amyloid plaque from the brain
i took aduhelm leqembis cousin that has removed all the measurable plaques from my brain

he is doing all the healthy things one should be doing to slow this down
things like staying active mentally socially and physically
eating a good diet like the mind diet
get adequate sleep
treat anxiety depression and stress
avoid meds or drugs that can make it worse
avoid excessive alcohol
dont smoke
control diabetes cholesterol and blood pressure
avoid obesity
do all of these starting when you are young

he is taking aricept or donepezil to slow down symptoms
it doesnt stop the disease

they are staying busy like my wife and i are
not letting the diagnosis slow you down

ALZHEIMERS NEWS-WHAT WOULD I SAY TO MY 40 YEAR OLD SELF

when i turned 40 years old i had just finished a 2 year stint doing full time emergency room doctoring and switched to a full family practice
it was at that time i did my first cholesterol check
it was sobering
knowing what i know now and what medicine knows now i should have been more aggressive in keeping those cholesterol numbers at a normal level

why
heart disease
stroke
alzheimers

those are the main reasons

my initial total cholesterol was over 300 and my ldl cholesterol was well over 250
normal is less than 200 and less then 100 respectively

it would be almost 20 years before i got on a statin to lower those levels
luckily i changed my diet increased my exercise and kept my ldl in the 130-150 range with diet changes

not good enough as we know now

i wander how much crude build up occurred in my arteries and in my brain
luckily studies of my carotids and heart have been normal so far
multiple brain mris have been normal
my brain well maybe not so good since i was diagnosed with mild cognitive impairment due to alzheimers disease at 60

would controlling my lipids for another 20 years made a difference

probably

i have been on statins now for 15 years and keep my ldl less than 100.

my blood pressure was always borderline
in 1990 when i first had my lipids checked the accepted level of blood pressure was 140/90
now it is 120/70

my blood pressure is now below that 120/80 level

what if it had been maintained at that level for those 20 extra years

what if

luckily for me i changed my diet exercised regularly playing sports running and riding bikes
i changed my diet

i usually got adequate sleep unless i was doing an overnighter in the emergency room or was on call and was up all night

the job was stressful at times but i think i handled it ok
i never was a heavy drinker and i quit smoking at 24

so some of what i did and changed when i turned 40 probably helped some
things might have been worse
a early stroke an early heart attack or alzheimers in my early 50s

so i would tell my 40 year old self
control your blood pressure and cholesterol with medications
keep doing all the healthy things you are doing

now 35 years later and looking at what i know today
what would i tell myself in 2026 if i just turned 40

get on statins and get your ldl below 100 if not lower
get on blood pressure meds to get your blood pressure to less than 120/80

get on the mind diet

get 7-8 hours of sleep a day

control stress

get regular exercise

check your apoe4 status
if its positive for apoe4 or apoe4/apoe4 then get really aggressive with all your prevention methods

the abnormal lipids ie cholesterol i have are related to my apoe4 gene
it controls lipid metabolism

they are looking at research that will change the apoe4 to apoe2 which be a game changer
however that is probably years away
those who are lucky enough to have apoe2 gene tend not to get alzheimers

i must add that checking your apoe4 can be sobering if it is positive
dont do it unless you really understand what it means
and if you cant handle the positive result

remember that about 25% of you will be positive for the apoe4 gene

the good thing is that a lifetime of good healthy living doing all the things one needs to do to slow down the disease can counter having the apoe4 gene

i just wished i had got started sooner like maybe when i was born
that is when your kids should start

if you share genetic dna with me you are at risk
start today even if you arent 40

avoid alcohol and tobacco

if you apoe4 is positive consider getting the new blood test done for alzheimers when it becomes available this year
if its positive seek out a new clinical trial for treating patients with positive tests and no symptoms
i really believe this is the point where treating patients will make a real difference
like i was at 40 and 50

for me i think when i was accidently diagnosed at 60 i was in the early early stages of the disease process and this is why i am still doing ok well better than i was 15 years ago

starting aricept or donepezil and getting my amyloid plaques removed from my brain with aduhelm infusions and doing all the other healthy things
that's why im better

soon that is the way folks will be doing is treating themselves long before symptoms start with these new alzheimers medications

you can do that now by doing all these healthy things now to slow down the disease which can take years to cause symptoms

yep thats what i would say to myself then and now

I will go through a list of those that have been eliminated by rfkjr and not your friends or your kids friends

rotavirus
young families now dont have to worry about this one since their kids get immunized against it in early childhood
unless of course you dont immunize them
i have taken care of many a dehydrated infant from rotavirus diarrhea and gi symptoms
yes i have seen kids die from this

now parents who follow the rfkjr immunization schedule will need to worry about this
i think of it as the infant and toddlers norovirus

hepatitis a
we should never see a kid with this disease ever
but now we will
kids are a hands and mouth person
hepatitis a is easily transmitted this way
yes i have seen kids with this and they can get real sick

Hepatitis b
this is a bad one to miss
getting hepatitis b can lead to liver failure
most kids that get it catch it at birth
getting the shot at delivery protects the baby from catching it later in life also

influenza ie flu
i have treated a lot of kids with this infection every year when i was working
sometimes they would have to be admitted
if most of us get the flu shot it makes a difference in how much it spreads
yes kids and grandparents can die from the flu

Meningococcal meningitis
yes i have seen cases of this
many ended up in the morgue
if missed early on catastrophic things could happen like permanent brain damage death loss of limbs blindness and more

covid
granted its not as bad as it was when it first started
now it is attenuated and is usually like the flu
yes there are still deaths from covid just like with the flu

so what should a parent or patient do for correct information
some but not all state healthy departments will not follow rfkjr guidelines
eg the california public health department link above is a good one

for kids the american academy of pediatrics or the american academy of family physicans
for adults the american academy of family physicians or the american college of physicians

just like they did with covid the anitvaxxers will cause more deaths and harm to our health by encouraging americans to follow these new guidelines

trust your doctor organizations

our grandson mr h is immunocompromised from the meds he takes for his kidney transplant
luckily he was immunized against most diseases before he got his kidney transplant at 2 years old
some he got earlier than recommended
eg
the chickenpox or varicella he had both of them however he has no immunity against chickenpox so if he gets exposed to chickenpox or a close contact with someone with shingles he has to go to the hospital to receive a varicella or chickenpox antibody treatment
he also has reduced immunity to measles

he like all other transplant patients and immunocompromised people rely on herd immunity to prevent outbreaks
in other word the rest of us being immunized protects those in our community who are vulnerable to these infections
people like new babies pregnant women premature babies cancer patients elderly folks anyone on immunosuppressive drugs and of course transplant patients

we all should do our part to protect our communities
we all have someone in our family and friends that needs this protection

in our we have someone with rheumatoid arthritis ulcerative colitis congenital heart disease asthma heart disease diabetes obesity and cancer that need protection with immunizations and herd immunity

i tell folks that i dont want to die from something i could prevent
so should it be with our kids and our vulnerable family members

dont let them do it to us again

HAPPY 15TH ANNIVERSARY

i retired at 60 on december 22 2010 because i was diagnosed with mild cognitive impairment that is due to alzheimers proved over time with diagnostic tests and memory tests
i cried and was sad for a period of time
however
today on my 15th anniversary of having to retire because of my diagnosis and having to stop doing what i was lucky to do for my career
well
today 15 years later i am having a happy anniversary

why
i am still alive
my last several memory tests are normal
i see and feel no decline today minus what is expected of a 75 year old

15 years ago if you had asked me where would i be in 2025 i would say i would be dead now from alzheimers after spending some time in a long term care facility
but
i didnt and im alive

so
i am happy today on my anniversary

LAST DAY-THANKS
this my last day to work as a physician
always considered myself lucky to have been able to
do in life what i wanted to do since i was a middle schooler

in high school i was able to work in a local hospital in the
summer on weekends and during holidays
our family was poor so not only did i get financial benefit from
working but i also was exposed to a lot in the hospital setting
it made me know that i wanted to be a family physician

during college i worked as a emergency room technician
on weekends, weeknights, and holidays
this also gave me financial support but also exposed me to
different doctors and different emergency situations

i was accepted to medical school but never wavered on what
kind of doctor i wanted to be
a family physician

i was accepted to a family practice residency
after completing my residency i owed the federal government
three years to work at an approved federal underserved area

i was able to create with some help a site in the small
town near where i grew up i worked there for three years
although traumatic at times it was a learning experience that i
carried through out my career

after fulfilling my three year obligation and after sustaining a
large financial hit from my involvement with the local clinic
i was recruited to the area where i am working now
thanks to my medical school friend for helping me out in
my time of need

i have worked for 25 years as a family physician
i always wanted to be one

now it is time i have to quit not because i want to but
because a genetic health problem has risen its nasty head
and because of this will have to discontinue what i like to do

i do not ever dread going to work every day
feel like i have now just gotten good at what i do

realize that patients dont necessarily need all of the new
medications we have, the new tests we can do or need
the newest treatments available

they do better if you just listen to them, pat them on the
shoulder, be kind to them and be their advocate when
they need it

i have honed my skills in taking care of kids over my career so
that many of the kids have felt comfortable seeing me and look
forward to seeing me to get a sticker or sucker, hear my
duck voice, or have me joke with them
that part i will miss a lot

thanks to all my patients in the past for allowing me to fulfill
my dream of being a family physician
i hope i have done you a good job

this last week i have responded to two people who reached out about their memories

one was a younger person with memory issues

another was a 70 year old with early alzheimers who wanted to know more about early diagnosis and ask for my thoughts on her evaluation and treatment
i spent an hour and a half with her and her husband

one thing i did was tell them my story using it as vehicle to talk about alzheimers and its treatment and their future

when i got home after talking with the couple i thought a lot about my story

here is a short version

in 2019 i applied for long term care insurance and passed a 30 minute memory interview having answered all the nurses questions correct
i would say it was close to the moca memory test

i received my long term care policy
luckily i bought the best i could buy with a cost of living adjustment
if i had waited a year i would not have received my long term care insurance because of my diagnosis

when i turned 60 i volunteered for the adni 2 or alzheimers disease neuroimaging initiative 2 to be in the normal control group
i wanted to be monitored annually and if i developed problems i would stop practicing medicine

my mother and younger brother died from alzheimers and my father probably had it when he died
multiple grandparents and aunts and uncles and cousins had it

when i took the neuropsychological exam for memory it was found i had memory loss greater than it should be for my age and degree of education
i felt the abnormalities when i was taking the exam
i cried later

i saw a neurologist who put me on aricept or donepezil which i have taken for 15 years
i stopped practicing the day i saw the neurologist

my initial moca test score was a 25 which is in the mild cognitive impairment range
it now is in the normal range of 29-30 for the last several years

as part of my evaluation i had a spinal tap for amyloid and tau that was compatible with alzheimers
i later had one of several amyloid pet scans tor amyloid that were positive for amyloid
my last one a year ago after treatment was negative for measurable amyloid plague
my genetic test was positive for the apoe4 apoe4 gene
the late onset alzheimers gene
it puts you at a 10+ times increase for getting alzheimers
luckily you can overcome that if you control your lipids blood pressure sugar weight eat correctly the mind diet and do other healthy things such as stay active mentally physically and socially get adequate sleep control any mental health issues such as stress anxiety or depression avoid alcohol abuse and dont smoke

my vitamin b12 was below normal as well as my vitamin d3
i take supplements to correct these
my homocysteine level was elevated so i take supplements to correct this
my cholesterol and blood pressure was elevated so i take meds to control this
my moca memory test was low at 24 but gradually over a few years it returned to normal

why
the aricept and all the healthy things i was doing and maybe the aduhelm infusions mentioned below and some good luck

the adni study is the most important alzheimers study ever done
its all diagnostic studies
sadly you dont usually get the results since its a blind study

around 2017 i was accepted in the aduhelm clinical trial
i took 52 monthly infusions
the first 18 were placebo and the last 34 were the real drug aduhelm
aduhelm is a moncolonal antibody against amyloid
my initial amyloid pet scan was positive for amyloid plaque
i was given that result before i started the study
my final amyloid pet scan showed no measurable amyloid plaque

so the aduhelm did its job it removed the amyloid plaque

it was take off the market since its manufacturer also makes leqembi which does the same thing but better

i have had multiple amyloid pet scans and tau pet scans and the new alzheimers blood test
except for the first and the last amyloid pet scan i didnt get the results

i wish i had them since they would be usefully in planning my future care

It will take 5-10 years for the amyloid to reaccumulate so i will probably need to receive treatment with a drug like leqembi to keep the amyloid from reaccumulating
and
wait for new drugs that are in clinical trials now

my kids and grandkids and nephews and nieces will have access to these drugs probably before alzheimers starts up

we have come a long way in 15 years

another thing i did was i freely talk about my diagnosis with anyone that will listen to me

i have been interviewed by the new york times washington post the financial times bloomberg and local newspapers in the austin area and here in california

i have been on alzheimers panels at ucla usc texas state greater alzheimers los angles the local alzheimers association and numerous churches and memory centers
my favorite is to do q and a with the audiences

i seem to be able to give them answers they cant seem to get

GARDEN NEWS-MORE ONION CHIMES

these are what my onions looked like when we lived in texas after i harvested them and hung them up in our garage using panty hose

as we needed an onion we just cut off an onion
they would last months this way

i have tried it like this here in santa barbara but at harvest time the weather is overcast all the time and the humidity is high
when i did my onion chimes they would rot

so
now here i just harvest the onions when they are big like in the photo and process them in our food processor and freeze them in zip lock bags

we are still using onions we grew last season

when i first moved to santa barbara i was looking for onions to plant
i looked at a popular seed company from southern California
they advertised onion plants so i checked them out
when i read they would not be available for a few months i looked at the place they ordered onion plants

it was dixondale farms from south texas
the same place that many nurseries in texas get theirs
i bought mine in texas every year at a local nursery but they came originally from dixondale farms
they ship to most of the states

i order mine now direct from dixondale
its cheaper also

we order the texas early white and 1015 texas super sweet yellow onions and creole red onions
you can look at the dixondale website at some good pictures of them
they form a nice large bulb
that is if the weather is right here in the spring

some years we have to pick them early so they wont rot
in texas it never was a problem

we prepped this bed 3 months ago by adding 4 inches of compost plus a sprinkling of an organic fertilizer
this was all forked in with a broadfork
then the bed was raked out smooth
the 3 rows of drip tape was added
then the bed was covered with straw until we were ready to plant

the plants are shipped via fedex or usps and arrive in a couple of days
we always receive them the week after thanksgiving
they are short day varieties

to plant them we raked off the straw
and planted each plant 6 inches apart
fish emulsion with seaweed was used to water them in after they were all planted

we now have 300 onions planted in this one bed

i planted 70 in my garden in this 3 ft x 7 ft bed
that concrete block in the pathway sits on top of a 5 gallon bucket with holes in the sides and bottom
i put in my food scraps in the bucket and the worms enter the holes
i get 5 gallons of worm made compost
i have 7 of these in my garden
they fill up twice a year

doing the math thats 70 gallons of worm made compost i make each year
sorry landfill thats your loss

in late may or early june when the bulbs get large and the stalks start falling over we will harvest them to donate to those in need

we also harvest green onions every week during the growing season to the groups we donate vegetables to by picking every other green ones

in my garden i harvest every other one as we need green onions then harvest the remaining ones when they bulb up
we then process them and freeze them for soups stews and with beans

we planted them 5 across the bed and this is what they look like now

next in few weeks we will have leeks to plant
we will plant them the same way as we do the onions

in our garden we know have growing lettuce chard peas beets onions broccoli cauliflower cabbage celery cilantro parsley dino kale curly kale and soon potatoes
we always have a few collard trees growing all year round

i hear those onion chimes ringing now

we started our year ending fundraiser for the community garden which i manage and have my garden plot

it was cold that day and the walk was postponed but a short ceremony was held at the santa barbara mission
i was about 45 lbs lighter than i am now due to what they thought was covid affecting my appetite

i thought at the time that i was going to go from covid instead of alzheimers

We missed one walk to end alzheimers in 15 years

Dear California Central Coast Alzheimer's Association,

We’ve walked for 15 years - at least one walk per year and sometimes 2 or 3 in different towns.

My partner was a beloved family practice doc in Austin Texas. ALZ came for his beloved family: his preacher dad, Charles W; his nurse mom, Nell; his fireman & sheriff baby brother, Joe. Seeing the writing on the wall, my husband, Dr Nash signed up to be in an ALZ study in Dallas. “Sorry,” they said when he was tested, “You don’t qualify to be in the control group as planned… you already have Mild Cognitive Impairment.” On December 20, 2010, one of the last prescriptions Dr Nash wrote as a practicing physician? Aricept for one of his ALZ patients. 24 hours later in Dallas, his new neurologist was handing over a script for Aricept to my husband. To say our family was devastated was an understatement.

My Santa Barbara sister immediately shared a book, Zen Shorts, with us. The story within, The Farmer’s Luck. (See insert below.)

This diagnosis, an ALZ precursor? Such bad luck? Maybe.

a deliverer of Meals on Wheels,and a home builder with Habitat With Humanity. When we moved to Santa Barbara in 2017 to be close to family support, he dedicated his volunteer work as a community gardener. In addition to doing all the preventative things he could possibly do to buy us precious time, he also began clinical trials of Aduhelm at UCLA. State-of-the-art brain pet scans confirmed the naughty amyloid plaques had indeed deposited in his brain. Bad luck? Nope. Good luck that qualified him for the maybe magic bullet Aduhelm. After 34 monthly infusions, the amyloid plaques were gone. Gone! Gone! Gone!

We, including my healthy hubby/perhaps survivor of ALZ, will walk Saturday Dec 6, 2025 in sunny Santa Barbara surrounded by donors, advocates, patients, care-givers, friends and family, including our FOUR precious grandchildren. Good luck, indeed.

that is me looking for an answer to slow down alzheimers disease
scientists are looking at all kinds of new treatments to slow it down or stop it when it starts
there are several new treatments in clinical trials that will help do that

now they are looking backwards
how can you slow it down or prevent it yourself
what prevention things have they found that may do that
the younger you start it the better
no age is it too late to start

here are their top 15 things to do

1 eat a plant based diet or at least the mind diet

2 exercise regularly
walk at least 3000 to 5000 steps more if you can
even that number can delay cognitive decline by 3 years
i use gardening as my exercise and i get 5000-10000 steps a day

3 get vaccinated
especially flu covid rsv pneumonia and shingles if you are older
these prevent inflammation that can occur in the brain from certain viral infections

4 take a daily multivitamin
this may slow down alzheimers some
i dont take a multivitamin since i take folate vitamin d vitamin b12 and iron since my levels are low if i dont take supplements
i have my levels checked yearly to make sure my levels are kept normal
most folks dont need to do that but may be helped by a daily vitamin supplement
dont take it without discussing with your doctor

5 avoid air pollutants
this is important if you live in a heavy smog area or near fires
air pollution contributes to dementia risk

6 protect your brain from trauma
i have had 3 concussions and at least one car wreck with head trauma
these didnt help me any

7 save your hearing
wear hearing aids if you need then
hearing loss increases your dementia risk

8 reduce loneliness feelings
control anxiety and depression

9 ensure you have normal blood pressure normal cholesterol and normal blood sugar
do this as early in your life as you can
heart kidneys and brain will be helped if you do

10 get a good nights sleep
deep sleep is when your brain repairs itself
avoid sleeping meds if you can
i struggle with this one since the donepezil interferes with my sleep
you will find me napping every afternoon from 1-3 so i can get my 7-8 hours of sleep

11 reduce or stop alcohol intake
bad things socially and physically happen if you drink too much
it exacerbates health issues like diabetes hypertension heart disease stroke and memory issues

12 dont smoke
it damages your brain heart and lungs

13 avoid sleep medications
many folks are masking other problems like sleep apnea that needs to be treated
they may influence your dementia risk

14 do crossword puzzles or other mentally stimulating activities like reading writing social interactions and play board games
keep your brain active

15 maintain your social connections
the pandemic was not good for this

the younger you start these things the better chance they will help to slow down or prevent you from getting dementia

we are getting close to the point where when you are younger say 30s or 40s you can get a dementia risk evaluation done if you want to know if you have it or will get it

the mayo clinic has a study that has done that

the testing is available now
the problem is there is no definite treatment in 2025 if you find you are at risk to get dementia

oh wait

yes there is

its those 15 things that i wrote about above

for me

I was diagnosed in 2010 with mild cognitive impairment that is due to alzheimers
over the last 15 years i have aggressively done those 15 things above

something all of you can easily do

i also added donepezil or aricept to my regimen
this slows down symptoms but does not slop the disease

i also took 34 monthly infusions of aduhelm or aducanumab which removed all of the measurable amyloid plaques from my brain

add all that up
here 15 years later my memory seems to be normal for a 75 year old male

i am waiting for what to do next
maybe i will get on a maintenance dose of one of the antiamyloid antibody meds like leqembi to keep the plaques from returning

i am also watching all the clinical trials that are ongoing some using medications that are already on the market that are used for the heart and for cancer treatment

when i do my moca memory test i have to draw a clock with a certain time on it
i bet if i was doing my memory test today i would not pass that part of the test

so at 700 when the sun usually comes up here in november then its really now 800 or is it 600
it says on ai that its really now 600
so
i dont leave now for the garden until 600 or should i wait till 700 now which is really 600
im confused again

so
ill just go by what my phone says
not by what my body says or the birds say

i always feel like i fell back and hit my head and i am in a daze for several days

my biorhythms and internal time clock will get all messed up

I know i will go at 700 the new not daylight savings time and see what the chickens are doing in the garden
i wonder if they will quit laying eggs

does this not affect them
they dont have a clock in their coop

the politicians were suppose to fix this
but
of course they did not fix this

i wonder if in march i will have to write again about the daylight not savings time change

i will not no longer vote for a politician who does not favor eliminating daylight not savings time

DONT BE AT A DISADVANTAGE

it is that time again to sign up or renew you medicare plans
as someone who worked in the medical field since i was 16 years old and someone who has been a physician for over 40 years
my advice to you is do not i repeat do not sign up for the medicare disadvantage plan

i am speaking as an insider and outsider

these plans are only a good choice for those who meet these criteria
you have no health issues and will not ever have any health issues

there is no politics in the video

its the facts you need to know to make a decision on what to do

sadly close to 50% of yall chose these medicare disadvantage plans
congress is pushing these plans on yall
why i ask
it costs medicare more if you are on this plan than if you are on regular medicare

all of what john oliver says in this video is true

all of it

these medicare disadvantage plans are an advantage to someone
its the insurance companies mainly

think of them as a really restrictive hmo insurance with copays and prior authorizations and out of network issues and more
none are good

this doesnt happen with regular medicare

you may pay less each month on the medicare disadvantage plan but may pay a lot more if you get sick

my wife she and i have regular medicare and a medicare supplement and a drug plan
we pay some for our meds probably about $150 a year

i have never paid for any extra for my care in the last 15 years
neither has my wife she

she just recently had heart valve surgery
our out of pocket was zero
if she had been on a medicare disadvantage plan she would have had to pay extra and would have had to go through referral and prior authorizations and probably some denials
there was none

dont let them take advantage of you

GARDEN NEWS-MY FALL GARDEN

last night the charlie brown pumpkin movie was shown in the pumpkin patch at our community garden
it was dark cozy and chilly but all there had a great time

a big ole pumpkin was there that weighed around 350 lbs for all to enjoy

our main garden is being transformed from the summer garden to the fall garden
i am also redoing my garden plot
it is about 400 square foot in diameter

i have 8 3ft x 7ft beds that are framed with wooden 2x4s
i also have 15 30 gallon grow bags
i also have a 3ft square bed that i grow a walking stick kale/collard or kale/collard tree
it produces big leaves that are like collards
they are tender and cook like regular collards

i also have a 3ft x 8ft bed that has concrete blocks around it
i grow strawberries in the holes in the blocks

i have a 4ft x 4 ft bed that i grew cherry tomatoes in the summer and this fall i have planted a vine snap pea there

after i removed my summer crops i added 4 inches of compost
either compost we buy for the garden or compost we make in our big compost bins or compost i make myself in my garden using buried 5 gallon buckets with holes on the sides and bottom

i add compost material from our kitchen in the buckets and worms come through the holes into the bucket to eat the compost materials
i produce about 30 gallons of this compost every 6 months
it is added to the beds when i prep them for the season

after i cover the beds with compost i add a scattering of an organic fertilizer
this is forked in with a garden fork
raked smooth
and
a soaking of a fish emulsion seaweed molasses mixture is added
the bed is then covered with a layer of pine straw
the pine straw holds in moisture
when i plant the bed i remove the pine straw

each of the beds are prepped this way
i never dig or till the beds

i do the same for the grow bags

on the back row is two beds that are 2ft x 7ft
they have a permanent 6 foot fence on t posts
these beds are used for peas in the winter and beans in the summer

in my grow bags this winter i am growing potatoes in 5 of them
lettuce carrots and garlic in the others
our soil here is clay so potatoes dont do well in the ground so i use the grow bags with good success

for the potatoes i get an organic red potato and leave it in our window until it makes sprouts from its eyes usually about 2 weeks
i then plant the whole potato in the bags

in my beds shown here i plant
dino kale and curly kale
cauliflower
cabbage
swiss chard
and
then on the back row i plant tall telephone shelling peas

in these beds shown here i plant
onions that i get from dixondale farms in texas called texas 1015 and early white onions
they make a nice bulb onion that you can store for several months
these we will harvest around next april or may
next is a bed for leeks
i get these from dixondale also
they will be harvested in april or may of next year
both the onions and leeks i will chop up and freeze for use in soups beans stews casseroles etc
next is a row of broccoli
next is a bed with celery with room for more veggies
i usually will plant a vegetable that we have extra in our greenhouse or maybe something i dont usually plant like red cabbage

at the back is a bed with a fence that i will plant magnolia snap peas a multicolored pea that tastes yummy off the vines
i eat a lot of my peas while im working in the garden

so
this is my fall winter garden that i will have fully planted soon
it will produce a lot of produce for us to use in our kitchen

the same thing is happening in our main garden
we are now prepping our beds there and will soon have it all planted for the winter also

we have an online fundraiser event that will start on november 15th and run to december 15th
it will be live starting november 15th
it is a garden go fund me like site
based on how much we raise we can also get a partial matching grant
all of the funds raised go to our garden
